Attention deficit hyperactivity disorder<\/span> (<\/span>ADHD<\/span><\/a>) is a common condition in the U.S.\u20147 million children between the ages of 3 and 17 years have been diagnosed with <\/span>ADHD<\/span>, and about 4.4% of adults have <\/span>ADHD<\/span>.\u00a0<\/span><\/p>\n

Concerta<\/span><\/a> (<\/span>methylphenidate<\/span> hydrochloride) <\/span>and <\/span>Vyvanse<\/span><\/a> (lisdexamfetamine dimesylate) are two stimulant <\/span>ADHD medications<\/span> approved by the Food and Drug Administration (<\/span>FDA<\/span>) to treat <\/span>ADHD<\/span>. <\/span>Vyvanse<\/span> can also treat moderate to severe <\/span>binge eating disorder<\/span><\/a> (BED) in adults.<\/span><\/p>\n

What are the main differences between <\/span>Concerta<\/span> and <\/span>Vyvanse<\/span>?<\/span><\/h2>\n

Concerta<\/span><\/a> and <\/span>Vyvanse<\/span><\/a> are both <\/span>stimulant medications<\/span>. Both are classified as <\/span>Schedule II<\/span> drugs<\/span><\/a> and <\/span>controlled substances<\/span><\/a> by the U.S. Drug Enforcement Administration (DEA) because of the potential for abuse and dependence. As stimulants, these medications work by affecting neurotransmitters (chemical messengers) in the brain called dopamine and norepinephrine. This helps improve symptoms of ADHD, such as increasing focus and decreasing impulsivity. The actions on neurotransmitters also help improve BED.<\/span><\/p>\n

Concerta<\/span> is used to treat <\/span>ADHD<\/span> in adults and children. <\/span>It<\/span> is available in both <\/span>brand-name<\/span> and generic formulations. The <\/span>active ingredient<\/span> of <\/span>Concerta<\/span> is <\/span>methylphenidate<\/span><\/a> (extended-release). <\/span>Concerta<\/span> is typically dosed once per day.<\/span><\/p>\n

Vyvanse<\/span><\/a> is used for the <\/span>treatment of ADHD<\/span> in adults and children. It can also be used for moderate to severe <\/span>binge eating disorders<\/span> in adults. The active ingredient in <\/span>Vyvanse<\/span> is called <\/span>lisdexamfetamine<\/span>, which is known as a <\/span>prodrug<\/span>. When you take Vyvanse, it starts in its inactive form, <\/span>lisdexamfetamine. When it travels through the gastrointestinal tract and liver, it<\/span> is converted to its active form, <\/span>dextroamphetamine<\/span>. This mechanism of action lowers <\/span>Vyvanse<\/span>\u2019s abuse potential. <\/span>Vyvanse<\/span> is available in both <\/span>brand-name<\/span> and generic form. <\/span>Vyvanse<\/span> is dosed once daily.\u00a0<\/span><\/p>\n

The length of treatment with <\/span>Concerta<\/span> or <\/span>Vyvanse<\/span> varies. Is <\/span>Concerta<\/span> or <\/span>Vyvanse<\/span> more effective?<\/span><\/h2>\n

Although both medications undergo clinical trials for safety and efficacy in order to be approved by the <\/span>FDA<\/span>, there is little data that compares the two drugs to each other. In 2014, <\/span>the manufacturer of Vyvanse<\/span> announced <\/span>results from two studies<\/span><\/a> that compared the efficacy of <\/span>Concerta<\/span> to <\/span>Vyvanse<\/span> in patients ages 13\u201317 years old with <\/span>ADHD<\/span>. Each patient took <\/span>Concerta<\/span>, <\/span>Vyvanse<\/span>, or a <\/span>placebo (inactive medicine)<\/span>.\u00a0<\/span><\/p>\n

In the first study, the results looked at the change from <\/span>baseline<\/span> at week six in the <\/span>Attention Deficit Hyperactivity Disorder<\/span> Rating Scale-IV (<\/span>ADHD<\/span>-RS-IV) total score. At the end of the study, patients who took <\/span>Vyvanse<\/span> had a slightly larger reduction in the score (25.4 point reduction) compared to patients who took <\/span>Concerta<\/span> (22.1-point reduction) or <\/span>placebo<\/span> (17-point reduction). In this study, <\/span>Vyvanse<\/span> was found to be \u201cstatistically superior\u201d to <\/span>Concerta<\/span>.\u00a0<\/span><\/p>\n

In the second study, patients were treated for eight weeks. In this study, patients who took <\/span>Vyvanse<\/span> had a 25.6-point reduction in <\/span>ADHD<\/span>-RS-IV total score. Patients who took <\/span>Concerta<\/span> had a 23.5-point reduction, and patients who took a <\/span>placebo<\/span> had a 13.4-point reduction. The researchers found this to not be statistically significant but stated that <\/span>Vyvanse<\/span> \u201cwas found to have a larger mean reduction (improvement) of 2.1 points compared to <\/span>Concerta<\/span> on the primary efficacy analysis.\u201d\u00a0<\/span><\/p>\n

Although <\/span>Vyvanse<\/span> was slightly more effective in this study, the medications were only studied in patients ages 13\u201317 years old. Also, there are many factors to take into consideration when choosing a medication that is best for the patient, including <\/span>ADHD symptoms<\/span>, the potential for abuse, potential <\/span>side effects<\/span> and drug interactions, and cost, among other factors.\u00a0<\/span><\/p>\n

Before taking <\/span>Concerta<\/span> or <\/span>Vyvanse<\/span>, tell your healthcare provider about your symptoms, medical history, and other medical conditions you have, as well as all of the medications you take. If you have a history of abuse or dependence (this can include alcohol, <\/span>prescription drugs<\/span>, or street drugs), tell your healthcare provider. There are also non-stimulant drug options, such as Strattera (atomoxetine) or Intuniv (guanfacine extended-release), to consider. Brand-name<\/span> versions of <\/span>Concerta<\/span> or Vyvanse are not typically covered by insurance plans. If they are covered, they may have higher out-of-pocket costs. Medicare prescription plans do not typically cover brand-name Concerta or Vyvanse, either. Instead, insurance and Medicare prescription plans are more likely to cover generic forms of Concerta or Vyvanse, although coverage will vary by plan. Drug interactions of <\/span>Concerta<\/span> vs. <\/span>Vyvanse<\/span><\/h2>\n

Concerta<\/span> or <\/span>Vyvanse<\/span> should not be taken with monoamine oxidase inhibitors (<\/span>MAOIs<\/span><\/a>) because the combination could cause a hypertensive (high <\/span>blood pressure<\/span>) crisis. <\/span>Concerta<\/span> or <\/span>Vyvanse<\/span> should be separated from an MAOI by at least 14 days.\u00a0<\/span><\/p>\n

Concerta<\/span> or <\/span>Vyvanse<\/span> should not be used with other medications that increase serotonin, such as <\/span>antidepressants<\/span>, triptans for migraine, or the cough suppressant dextromethorphan. The combination of drugs can increase the risk of <\/span>serotonin syndrome<\/span><\/a>, a life-threatening condition caused by the buildup of serotonin.\u00a0\u00a0<\/span><\/p>\n

Warnings of <\/span>Concerta<\/span> and <\/span>Vyvanse<\/span><\/h2>\n

Every time you fill a prescription for <\/span>Concerta<\/span> or <\/span>Vyvanse<\/span>, you will receive a medication guide. This guide contains information about the drug, such as <\/span>side effects<\/span> and warnings. <\/span>Concerta<\/span> and <\/span>Vyvanse<\/span> both have a <\/span>black box warning<\/span><\/a>. Also called a boxed warning, this is the strongest warning required by the <\/span>FDA<\/span>. Certain patients should not take <\/span>Concerta<\/span> or <\/span>Vyvanse<\/span>. When a medication should not be used in certain instances, this is called a contraindication. <\/span>Concerta<\/span> and <\/span>Vyvanse<\/span> are contraindicated in patients who have an allergy to the medication or any ingredient in the medicine. Also, <\/span>Concerta<\/span> or <\/span>Vyvanse<\/span> should not be used within two weeks of a monoamine oxidase inhibitor (MAOI). People with certain medical conditions (such as seizures, glaucoma, or high blood pressure) should only take Concerta or Vyvanse if the healthcare provider determines the benefits outweigh the risks, so be sure to tell your healthcare provider about all medical conditions and medical history.<\/span><\/p>\n
