PDE-4 inhibitors are a promising class of newer therapeutic agents used for a variety of inflammatory conditions. Otezla has been approved by the Food and Drug Administration (FDA) in the treatment of adults with active psoriatic arthritis, individuals with moderate to severe plaque psoriasis who are candidates for systemic or phototherapy, and adults with oral ulcers associated with Behcet\u2019s disease.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Otezla is not an over-the-counter medication. It\u2019s available by prescription only, in 14-day or 28-day convenience packs of 10-20-30 mg tablets or 30 mg tablets alone. After an initial five-day period where the dosage is slowly increased, Otezla is typically taken in 30 mg tablets once or twice per day.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">All prescription drugs, including Otezla, have possible side effects. If any of these side effects become bothersome, notify your healthcare provider.\u00a0<\/span><\/p>\n<ul>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Gastrointestinal problems, including:\u00a0<\/span>\n<ul>\n<li style=\"font-weight: 400;\" aria-level=\"2\"><span style=\"font-weight: 400;\">Nausea<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"2\"><span style=\"font-weight: 400;\">Vomiting<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"2\"><span style=\"font-weight: 400;\">Diarrhea<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"2\"><span style=\"font-weight: 400;\">Abdominal pain\u00a0<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"2\"><span style=\"font-weight: 400;\">Dyspepsia, also known as indigestion\u00a0<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"2\"><span style=\"font-weight: 400;\">Increased frequency and volume of bowel movements\u00a0<\/span><\/li>\n<\/ul>\n<\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Feelings of malaise, including:<\/span>\n<ul>\n<li style=\"font-weight: 400;\" aria-level=\"2\"><span style=\"font-weight: 400;\">Fatigue<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"2\"><span style=\"font-weight: 400;\">Insomnia<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"2\"><span style=\"font-weight: 400;\">Back pain<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"2\"><span style=\"font-weight: 400;\">Arthralgias, also known as joint pain<\/span><\/li>\n<\/ul>\n<\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Tension and migraine headaches\u00a0<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Nasal congestion, sore throat, and upper respiratory tract infections\u00a0<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Decreased appetite and weight loss\u00a0<\/span><\/li>\n<\/ul>\n<h3 id=\"weight-loss\"><span style=\"font-weight: 400;\">Weight loss<\/span><\/h3>\n<p><span style=\"font-weight: 400;\">During initial <\/span><a href=\"https:\/\/www.accessdata.fda.gov\/drugsatfda_docs\/label\/2017\/205437s006lbl.pdf\" target=\"_blank\" rel=\"noopener\"><span style=\"font-weight: 400;\">clinical trials and phase 3 studies<\/span><\/a><span style=\"font-weight: 400;\">, a weight decrease of 5% to 10% was observed in 5% to 12% of people taking Otezla. A decrease in body weight was observed in 3% to 5% of patients taking a placebo.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">In <\/span><a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/34097014\/\" target=\"_blank\" rel=\"noopener\"><span style=\"font-weight: 400;\">more recent clinical studies<\/span><\/a><span style=\"font-weight: 400;\">, apremilast was associated with a mean weight loss of 4.85 pounds after six months of treatment. In these cases, the weight loss was considered unplanned or undesirable.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">It\u2019s possible that severe diarrhea, nausea, and vomiting contributed to this unintended weight loss. The exact mechanism is unknown and needs further investigation. Unexpected or unplanned weight loss (more than 10 pounds) should be evaluated by a healthcare professional.\u00a0<\/span><\/p>\n<h3 id=\"diarrhea\"><span style=\"font-weight: 400;\">Diarrhea<\/span><\/h3>\n<p><span style=\"font-weight: 400;\">Otezla use is associated with severe diarrhea, nausea, and vomiting in some people. It usually resolves within the first few weeks of treatment. The exact incidence of Otezla-associated diarrhea has not been reported, but it is considered a common side effect.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">While diarrhea is usually mild and self-limited, sometimes hospitalization, lowering the dose, or stopping the medication may be required.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Complications from gastrointestinal symptoms are more likely to occur in people over 65 years of age or individuals taking medications that could lead to dehydration or hypotension (low blood pressure). If any of the following signs and symptoms occur, seek medical attention right away:<\/span><\/p>\n<ul>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Diarrhea lasting more than 48 hours\u00a0<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Blood or pus in the stool<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Stool with a black or \u201ctarry\u201d appearance\u00a0<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Extremely foul smelling stool<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Severe abdominal or rectal pain, a fever over 101 degrees Fahrenheit, or signs of dehydration\u00a0<\/span><\/li>\n<\/ul>\n<h2 id=\"serious-side-effects\"><span style=\"font-weight: 400;\">Serious side effects of Otezla<\/span><\/h2>\n<p><span style=\"font-weight: 400;\">Otezla use is associated with several severe adverse reactions. If they occur, seek emergency medical treatment right away. Adverse events can also be reported directly to the FDA using <\/span><a href=\"https:\/\/www.accessdata.fda.gov\/scripts\/medwatch\/index.cfm\" target=\"_blank\" rel=\"noopener\"><span style=\"font-weight: 400;\">MedWatch<\/span><\/a><span style=\"font-weight: 400;\">, the FDA\u2019s safety information and adverse event reporting program.\u00a0<\/span><\/p>\n<ul>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Severe diarrhea or vomiting<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Severe hypersensitivity or allergic reactions\u00a0<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Psychological disturbances and mood changes, including:<\/span>\n<ul>\n<li style=\"font-weight: 400;\" aria-level=\"2\"><span style=\"font-weight: 400;\">Depression\u00a0<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"2\"><span style=\"font-weight: 400;\">Suicidal ideation (thinking about or planning suicide)\u00a0<\/span><\/li>\n<\/ul>\n<\/li>\n<\/ul>\n<h3 id=\"depression\"><span style=\"font-weight: 400;\">Depression<\/span><\/h3>\n<p><span style=\"font-weight: 400;\">Treatment with Otezla is associated with an increased incidence of depression. During the 16-week placebo-controlled clinical trials, 1% of patients reported depression or depressed mood.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Individuals with a history of depression and\/or suicidal thoughts should talk to a healthcare provider about risks and benefits of treatment before starting Otezla.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Patients, caregivers, and family members should be aware of this side effect and contact their healthcare provider if new-onset or worsening depression occurs while taking this medication.\u00a0<\/span><\/p>\n<p><b>RELATED: <\/b><a href=\"https:\/\/www.singlecare.com\/blog\/anxiety-vs-depression\/\"><b>Anxiety vs. depression<\/b><\/a><\/p>\n<h2 id=\"otezla-and-cancer\"><span style=\"font-weight: 400;\">Can Otezla cause cancer?<\/span><\/h2>\n<p><span style=\"font-weight: 400;\">Otezla is not associated with an increased risk of cancer. Otezla has been used safely for the treatment of psoriatic conditions in <\/span><a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC6830556\/\" target=\"_blank\" rel=\"noopener\"><span style=\"font-weight: 400;\">case reports<\/span><\/a><span style=\"font-weight: 400;\"> of individuals with active malignancy.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Some autoimmune and inflammatory conditions that are treated by Otezla, such as <\/span><a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/32074260\/\" target=\"_blank\" rel=\"noopener\"><span style=\"font-weight: 400;\">psoriatic conditions<\/span><\/a><span style=\"font-weight: 400;\"> and <\/span><a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/32383138\/\" target=\"_blank\" rel=\"noopener\"><span style=\"font-weight: 400;\">Bechet\u2019s disease<\/span><\/a><span style=\"font-weight: 400;\">, are associated with an increased risk of cancer. More research is needed in this area.\u00a0<\/span><\/p>\n<h2 id=\"how-long-do-side-effects-last\"><span style=\"font-weight: 400;\">How long do Otezla side effects last?<\/span><\/h2>\n<p><span style=\"font-weight: 400;\">Most of the side effects (gastrointestinal problems and feelings of malaise) associated with Otezla use go away after several weeks of treatment. Immediate attention should be given to more serious side effects (severe allergic reaction, depression, and suicidal thoughts), as they may result in death.<\/span><\/p>\n<h2 id=\"warnings\"><span style=\"font-weight: 400;\">Otezla contraindications and warnings<\/span><\/h2>\n<p><span style=\"font-weight: 400;\">Otezla is not habit-forming or addictive. While there are no withdrawal symptoms associated with stopping this medication, there may be a return of the symptoms this medication was being used to treat.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">In healthy adults, the <\/span><a href=\"https:\/\/online.epocrates.com\/drugs\/688001\/Otezla\/Adult-Dosing\" target=\"_blank\" rel=\"noopener\"><span style=\"font-weight: 400;\">maximum dosage of Otezla<\/span><\/a><span style=\"font-weight: 400;\"> is 60 mg per day. For individuals with kidney disease, no more than 30 mg of Otezla should be taken per day. While it is unlikely that an overdose will result in imminent death, if too much medication is administered, seek medical care or contact the Poison Control Centers at 1-800-222-1222.\u00a0<\/span><\/p>\n<h3><span style=\"font-weight: 400;\">Restrictions<\/span><\/h3>\n<p><span style=\"font-weight: 400;\">Otezla is <\/span><a href=\"https:\/\/online.epocrates.com\/drugs\/688003\/Otezla\/Contradindications-Cautions\" target=\"_blank\" rel=\"noopener\"><span style=\"font-weight: 400;\">contraindicated<\/span><\/a><span style=\"font-weight: 400;\"> for use in individuals with a known allergy to this medication or to components of the medication. Otezla should be used with caution in the following groups of people:<\/span><\/p>\n<h4><span style=\"font-weight: 400;\">Individuals with severe renal impairment\u00a0<\/span><\/h4>\n<p><span style=\"font-weight: 400;\">Severely decreased kidney function means that too much medication will build up in the blood. The dose should be decreased for individuals with severe renal impairment.<\/span><\/p>\n<h4><span style=\"font-weight: 400;\">Pregnant women and breastfeeding mothers<\/span><\/h4>\n<p><span style=\"font-weight: 400;\">There have been <\/span><a href=\"https:\/\/online.epocrates.com\/drugs\/688013\/Otezla\/Pregnancy-Lactation\" target=\"_blank\" rel=\"noopener\"><span style=\"font-weight: 400;\">no adequately performed studies<\/span><\/a><span style=\"font-weight: 400;\"> on the effects of Otezla on a developing fetus in pregnant women. In animal studies, Otezla showed an increase in miscarriage, fetal death, and skeletal abnormalities Women who may become pregnant should discuss these risks with a healthcare professional before starting treatment and consider using effective contraception during treatment.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">There is a registry that monitors outcomes in pregnant women exposed to apremilast. Information about this registry can be obtained <\/span><a href=\"http:\/\/mothertobaby.org\/ongoing-study\/otezla\" target=\"_blank\" rel=\"noopener\"><span style=\"font-weight: 400;\">here<\/span><\/a> <span style=\"font-weight: 400;\">or by calling 1-877-311-8972.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Apremilast was detected in the milk of lactating animals. Usually, when a drug is found in animal milk, it is also present in human breast milk. If breastfeeding, discuss the risks and benefits of treatment with a healthcare provider.<\/span><\/p>\n<h2 id=\"interactions\"><span style=\"font-weight: 400;\">Otezla interactions<\/span><\/h2>\n<p><span style=\"font-weight: 400;\">Otezla is not contraindicated for use with any medications, but there may be <\/span><a href=\"https:\/\/online.epocrates.com\/drugs\/688004\/Otezla\/Drug-Interactions\" target=\"_blank\" rel=\"noopener\"><span style=\"font-weight: 400;\">drug interactions<\/span><\/a><span style=\"font-weight: 400;\"> if Otezla is taken with certain medications.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Taking Otezla with the following medications may result in Otezla being less effective:\u00a0<\/span><\/p>\n<ul>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Barbiturates\u00a0<\/span>\n<ul>\n<li style=\"font-weight: 400;\" aria-level=\"2\"><span style=\"font-weight: 400;\">Phenobarbital<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"2\"><span style=\"font-weight: 400;\">Thiopental<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"2\"><span style=\"font-weight: 400;\">Secobarbital<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"2\"><span style=\"font-weight: 400;\">Pentobarbital<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"2\"><span style=\"font-weight: 400;\">Mephobarbital<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"2\"><span style=\"font-weight: 400;\">Methohexital<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"2\"><span style=\"font-weight: 400;\">Butalbital<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"2\"><span style=\"font-weight: 400;\">Amobarbital<\/span><\/li>\n<\/ul>\n<\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Carbamazepine<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Phenytoin<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Rifampin\u00a0<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">St. John\u2019s Wort\u00a0<\/span><\/li>\n<\/ul>\n<p><span style=\"font-weight: 400;\">Tell your healthcare provider about all prescription and over-the-counter medications you are taking, including vitamins and supplements.<\/span><\/p>\n<h2 id=\"how-to-avoid-side-effects\"><span style=\"font-weight: 400;\">How to avoid Otezla side effects<\/span><\/h2>\n<h3><span style=\"font-weight: 400;\">1. It is important to follow medical advice while taking this medication.\u00a0<\/span><\/h3>\n<p><span style=\"font-weight: 400;\">Take only the dose prescribed by a healthcare professional. Do not take more medication and do not take less.\u00a0<\/span><\/p>\n<h3><span style=\"font-weight: 400;\">2. Take all of the doses how and when they are prescribed.<\/span><\/h3>\n<p><span style=\"font-weight: 400;\">Otezla is often started slowly to reduce gastrointestinal side effects. It is important to follow this schedule. If there is a missed dose, take the next dose as soon as you remember. Patients should continue taking the medication as prescribed even if they feel well after the first few doses.<\/span><\/p>\n<h3><span style=\"font-weight: 400;\">3. Do not stop this medication without seeking medical advice.<\/span><\/h3>\n<p><span style=\"font-weight: 400;\">You should not stop taking this medication unless you and your healthcare provider have decided that is the right decision.\u00a0<\/span><\/p>\n<h3><span style=\"font-weight: 400;\">4. Patients should disclose a full medication list, all medical conditions, and any allergies before taking Otezla.<\/span><\/h3>\n<p><span style=\"font-weight: 400;\">This includes all medications and supplements, medical conditions, and allergic reactions to medications in the past. It is important to share any previous history of depression or suicidal thoughts, as people have competed suicide while taking this medication.<\/span><\/p>\n<h3><span style=\"font-weight: 400;\">5. Have a strong support system in place.<\/span><\/h3>\n<p><span style=\"font-weight: 400;\">This medication is associated with an increased risk of depression and suicide. It is important to have a strong emotional support network of friends and family in place.6. Store this medication correctly and out of the reach of children.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Otezla should be stored at room temperature (68 to 77 degrees Fahrenheit). This medication should be stored away from children\u2019s medications and out of the reach of children.<\/span><\/p>\n<h3><span style=\"font-weight: 400;\">7. Swallow the tablet whole.<\/span><\/h3>\n<p><span style=\"font-weight: 400;\">Do not crush, break or chew the tablet. This medication may be taken with or without food.\u00a0<\/span><\/p>\n<h3><span style=\"font-weight: 400;\">8. Individuals with severe renal impairment should take this medication in the morning.\u00a0<\/span><\/h3>\n<p><span style=\"font-weight: 400;\">Following a morning schedule allows you to watch for side effects from too much medication circulating in the blood.\u00a0<\/span><\/p>\n<h3><span style=\"font-weight: 400;\">9. Consider effective methods of contraception.<\/span><\/h3>\n<p><span style=\"font-weight: 400;\">Use of this medication is associated with miscarriage and fetal death. Women of childbearing age should consider using appropriate contraception while taking this medication.\u00a0<\/span><\/p>\n<h3><span style=\"font-weight: 400;\">10. Monitor your weight.\u00a0<\/span><\/h3>\n<p><span style=\"font-weight: 400;\">This medication may be associated with unwanted weight loss. Check your weight regularly while taking Otezla.
