They do so by preventing blood clots from forming initially, and keeping existing blood clots from getting any bigger. Blood clots are dangerous because they can move through your bloodstream, lodge somewhere\u2014like your heart, lungs, or brain\u2014and prevent essential blood flow. Blood thinners are part of the treatment plan for many cardiovascular conditions, from <\/span><a href=\"https:\/\/www.singlecare.com\/conditions\/atrial-fibrillation-treatment-and-medications\"><span style=\"font-weight: 400;\">atrial fibrillation<\/span><\/a><span style=\"font-weight: 400;\">, or irregular heart rhythm, to <\/span><a href=\"https:\/\/www.singlecare.com\/conditions\/deep-vein-thrombosis-treatment-and-medications\"><span style=\"font-weight: 400;\">deep vein thrombosis.<\/span><\/a><span style=\"font-weight: 400;\">\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Historically, there was one main oral blood thinner\u2014Coumadin (warfarin)\u2014that could prevent these life-threatening issues. But, it came with some drawbacks. If you\u2019ve watched any TV lately, you\u2019ve likely seen advertisements for the new blood thinners that were created to fix those issues, like <\/span><a href=\"https:\/\/www.singlecare.com\/blog\/eliquis-vs-xarelto\/\"><span style=\"font-weight: 400;\">Xarelto and Eliquis<\/span><\/a><span style=\"font-weight: 400;\">. Read on to learn everything you need to know about these alternatives to Coumadin.<\/span><\/p>\n<h2><span style=\"font-weight: 400;\">The problem with Coumadin<\/span><\/h2>\n<p><a href=\"https:\/\/www.singlecare.com\/prescription\/warfarin-sodium\"><span style=\"font-weight: 400;\">Coumadin (warfarin)<\/span><\/a><span style=\"font-weight: 400;\"> is one of the oldest cardiac medications, first popularized when President Dwight D. Eisenhower had a heart attack while in office. It\u2019s very effective at preventing clots by inhibiting vitamin K, a vitamin your body uses to create the proteins it needs to clot your blood. The drawback is that changes in certain diet and eating habits\u2014like consuming varying amounts of foods rich in vitamin K (rather than eating a consistent amount) or <\/span><a href=\"https:\/\/www.singlecare.com\/blog\/blood-thinners-and-alcohol\/\"><span style=\"font-weight: 400;\">drinking alcoholic beverages<\/span><\/a><span style=\"font-weight: 400;\">, can affect the blood\u2019s ability to clot. Excess vitamin K can make the blood more likely to clot. This is why healthcare providers recommend patients who take warfarin eat a consistent amount of foods rich in vitamin K. On the other hand, alcohol can thin the blood. People taking Coumadin need frequent blood tests to check their status and constant dose adjustments to achieve the perfect balance between bleeding too much or forming clots.\u00a0<\/span><\/p>\n<h2><span style=\"font-weight: 400;\">What are new blood thinners?<\/span><\/h2>\n<p><span style=\"font-weight: 400;\">Between 2010-2015 a series of new blood thinners hit the market,<\/span> <span style=\"font-weight: 400;\">each deemed either more or equally effective compared to warfarin for clot prevention by the U.S. Food and Drug Administration (FDA).\u00a0\u00a0<\/span><\/p>\n<ol>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><a href=\"https:\/\/www.singlecare.com\/prescription\/pradaxa\"><span style=\"font-weight: 400;\">Pradaxa (dabigatran)<\/span><\/a><span style=\"font-weight: 400;\">, initial approval 2010<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><a href=\"https:\/\/www.singlecare.com\/prescription\/xarelto\"><span style=\"font-weight: 400;\">Xarelto (rivaroxaban)<\/span><\/a><span style=\"font-weight: 400;\">, initial approval 2011<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><a href=\"https:\/\/www.singlecare.com\/prescription\/eliquis\"><span style=\"font-weight: 400;\">Eliquis (apixaban)<\/span><\/a><span style=\"font-weight: 400;\">, initial approval 2012<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><a href=\"https:\/\/www.singlecare.com\/prescription\/savaysa\"><span style=\"font-weight: 400;\">Savaysa (edoxaban)<\/span><\/a><span style=\"font-weight: 400;\">, initial approval 2015<\/span><\/li>\n<\/ol>\n<p><span style=\"font-weight: 400;\">These drugs were initially referred to as novel oral anticoagulants (NOACs). Now that they have been around for over five years, the <\/span><a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC6334188\/\" target=\"_blank\" rel=\"noopener\"><span style=\"font-weight: 400;\">preferred term<\/span><\/a><span style=\"font-weight: 400;\"> is direct-acting oral anticoagulants (DOACs). \u201cWith the first, Pradaxa, out since 2010, they\u2019re not so novel anymore,\u201d explains <\/span><a href=\"https:\/\/doctors.nyp.org\/frank-b-dorsa-md\/newyork-presbyterian-medical-group-hudson-valley-cardiology-and-vasculary-surgery\" target=\"_blank\" rel=\"noopener\"><span style=\"font-weight: 400;\">Frank Dorsa, MD<\/span><\/a><span style=\"font-weight: 400;\">, a cardiologist with the NewYork-Presbyterian Medical Group Hudson Valley in New York. Savaysa, the last to be approved, has never captured as large a market share as Eliquis or Xarelto as it did not offer any significant advantage over the other three DOACs.<\/span><\/p>\n<h2><span style=\"font-weight: 400;\">How do new blood thinners work?<\/span><\/h2>\n<p><span style=\"font-weight: 400;\">Blood thinners don\u2019t actually make the blood thin, explains <\/span><a href=\"https:\/\/www.linkedin.com\/in\/raman-mitra-03238232\/\" target=\"_blank\" rel=\"noopener\"><span style=\"font-weight: 400;\">Raman Mitra, MD<\/span><\/a><span style=\"font-weight: 400;\">, a cardiologist and director of the electrophysiology laboratory at <\/span><a href=\"https:\/\/nsuh.northwell.edu\/\" target=\"_blank\" rel=\"noopener\"><span style=\"font-weight: 400;\">North Shore University Hospital<\/span><\/a><span style=\"font-weight: 400;\"> in New York. There are a dozen or so proteins in the blood that collaborate in clotting, and if any one of those elements is impeded, blood takes longer to clot, and therefore the chance of a stroke is lessened. Instead of inhibiting vitamin K, each of the new blood thinners interferes with a key step in clot formation.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">The result is a more reliable clotting effect. Meaning, the dosage doesn\u2019t need to be constantly tinkered with, and your diet doesn\u2019t affect how well it works. It\u2019s less of an impediment to a person\u2019s life.<\/span><\/p>\n<h2><span style=\"font-weight: 400;\">What makes new blood thinners better than Coumadin?<\/span><\/h2>\n<p><span style=\"font-weight: 400;\">Warfarin has two clear strengths the DOACs don\u2019t have. First, it is the only option for those with a mechanical heart valve, says Dr. Mitra. Also, warfarin is by far the least expensive. However, warfarin alternatives are a better choice in almost every other way. According to Dr. Mitra and Dr. Dorsa, new blood thinners:\u00a0<\/span><\/p>\n<ul>\n<li><span style=\"font-weight: 400;\">Are superior in preventing strokes and death.<\/span><\/li>\n<li><span style=\"font-weight: 400;\">Are associated with less internal bleeding, particularly in the head.\u00a0<\/span><\/li>\n<li><span style=\"font-weight: 400;\">Have standard dosing, providing protection within a few hours of the first dose. Warfarin\u2019s dosing may take up to a week to fine tune.<\/span><\/li>\n<li><span style=\"font-weight: 400;\">Have no food restrictions, while warfarin\u2019s effectiveness depends on eating a consistent amount of vitamin K, found in leafy green vegetables.<\/span><\/li>\n<li><span style=\"font-weight: 400;\">Do not require testing unlike warfarin, which requires frequent testing.<\/span><\/li>\n<li><span style=\"font-weight: 400;\">Have fewer drug interactions, while warfarin\u2019s efficacy may be affected by common antibiotics among other drugs.<\/span><\/li>\n<\/ul>\n<h2><span style=\"font-weight: 400;\">What are the drawbacks to new blood thinners?<\/span><\/h2>\n<p><span style=\"font-weight: 400;\">While the newer blood thinners are most often the best choice, they do have some side effects and special considerations, just like any Rx. They include the following.<\/span><\/p>\n<ul>\n<li><b>Cost: <\/b><span style=\"font-weight: 400;\">The new blood thinners are much more expensive than Coumadin. \u201cI tell patients to find out which drug is covered best in their insurance plan, and we\u2019ll use that one,\u201d Dr. Mitra explains.<\/span><\/li>\n<li><b>Dosage: <\/b><a href=\"https:\/\/www.singlecare.com\/prescription\/xarelto\/dosage\"><span style=\"font-weight: 400;\">Xarelto<\/span><\/a><span style=\"font-weight: 400;\"> and Savaysa are taken once a day, while <\/span><a href=\"https:\/\/www.singlecare.com\/prescription\/eliquis\/dosage\"><span style=\"font-weight: 400;\">Eliquis<\/span><\/a><span style=\"font-weight: 400;\"> and Pradaxa are taken twice a day. Dr. Mitra says with patients who may not always remember the additional dose, he is more likely to prescribe Xarelto or Savaysa.\u00a0<\/span><\/li>\n<li><b>GI bleeding:<\/b><span style=\"font-weight: 400;\"> Xarelto, Savaysa, and Pradaxa have all been associated with <\/span><a href=\"https:\/\/www.singlecare.com\/blog\/xarelto-side-effects\/\"><span style=\"font-weight: 400;\">gastrointestinal (GI) bleeding<\/span><\/a><span style=\"font-weight: 400;\">, even comparing unfavorably with Warfarin. \u201cSo if someone is at risk for GI bleeding\u2014if they report heartburn or maybe they had a bleeding ulcer 10 years ago\u2014I\u2019m a little bit more likely to prescribe Eliquis,\u201d Dr. Dorsa says.\u00a0<\/span><\/li>\n<li><b>Kidney function:<\/b><span style=\"font-weight: 400;\"> \u201cIf someone has kidney disease, we\u2019re not likely to use Pradaxa because that drug is completely excreted by the kidneys,\u201d Dr. Mitra explains. \u201cFor someone with kidney disease, we\u2019re more likely to prescribe Eliquis, Xarelto, or Savaysa.\u201d\u00a0<\/span><\/li>\n<li><b>Reversal of blood thinner effects: <\/b><span style=\"font-weight: 400;\">Blood thinners<\/span><b> \u201c<\/b><span style=\"font-weight: 400;\">push the needle a little bit more toward fluidity and bleeding as opposed to clotting,\u201d Dr. Mitra says. \u201cIn the case of an injury\u2014say you cut yourself or bump yourself\u2014your blood will clot but it will just take a little longer, maybe three to five minutes.\u201d In a bleeding emergency, warfarin can be reversed in four to six hours with frozen plasma. With the DOACs, Eliquis and Xarelto will begin to reverse after cessation of the drug, within 24 to 48 hours. Reversal agents are available for all of the new blood thinners, says Dr. Mitra, but tend to be very expensive.<\/span><\/li>\n<\/ul>\n<h2><span style=\"font-weight: 400;\">Which is the best new blood thinner?<\/span><\/h2>\n<p><span style=\"font-weight: 400;\">The new blood thinners are comparable, but is there one that is best? Since there has been no study comparing the four new blood thinners to one another, there\u2019s no clear answer. Ultimately, finding the best medication should be what you and your healthcare provider decide upon. Your provider will look at your medical history and help you decide which drug you\u2019re best-suited for.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Often, the best approach is to try the medication that insurance will cover and has the lowest risk of side effects. If the treatment you\u2019re on isn\u2019t working for you, confer with your physician.
