Eosinophilic asthma is the most common subtype of severe asthma\u2014a respiratory disease characterized by high levels of eosinophils, a type of white blood cell or immune cell. Never heard of it? Here\u2019s what you need to know about this subtype of asthma and how it\u2019s treated.<\/span><\/p>\n<h2 id=\"what-is-eosinophilic-asthma\">What is eosinophilic asthma?<\/h2>\n<p><span style=\"font-weight: 400;\">Eosinophilic asthma is a chronic lung disease that is diagnosed when tests find high levels of eosinophils, a particular type of white blood cell, in your blood, lung tissue, and sputum\u2014despite <\/span><a href=\"https:\/\/openres.ersjournals.com\/content\/1\/1\/00024-2015\" target=\"_blank\" rel=\"noopener\"><span style=\"font-weight: 400;\">use of inhaled corticosteroids<\/span><\/a><span style=\"font-weight: 400;\"> even at high doses. At normal levels, eosinophils are a crucial part of your immune system. They fight bacteria and ward off infections. When eosinophil levels are elevated (over <\/span><a href=\"https:\/\/www.thelancet.com\/journals\/lanres\/article\/PIIS2213-2600(15)00367-7\/fulltext\" target=\"_blank\" rel=\"noopener\"><span style=\"font-weight: 400;\">300 or 400 cells per \u03bcL<\/span><\/a><span style=\"font-weight: 400;\">), they are associated with more asthma complications, likely from induction of swelling and inflammation in your airways.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">\u201c[Eosinophils] are protective\u2014as well as destructive; they are foe and friend,\u201d says <\/span><a href=\"https:\/\/medicine.tulane.edu\/departments\/pulmonary-diseases-critical-care-environmental-medicine-research-tulane-cancer-center-0\" target=\"_blank\" rel=\"noopener\"><span style=\"font-weight: 400;\">Anil Mishra<\/span><\/a><span style=\"font-weight: 400;\">, Ph.D., professor of medicine and director of the Eosinophilic Disorder Center at the Tulane University School of Medicine. Higher eosinophil counts tend to be associated with more severe eosinophilic asthma disease, says <\/span><a href=\"https:\/\/www.linkedin.com\/in\/jon-romeo-9b6498b5\/\" target=\"_blank\" rel=\"noopener\"><span style=\"font-weight: 400;\">Jon Romeo, DO<\/span><\/a><span style=\"font-weight: 400;\">, chair of the <\/span><a href=\"https:\/\/acaai.org\/\" target=\"_blank\" rel=\"noopener\"><span style=\"font-weight: 400;\">American College of Allergy, Asthma and Immunology<\/span><\/a><span style=\"font-weight: 400;\"> (ACAAI) Asthma Committee.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Luckily, the condition is fairly rare. The exact prevalence of eosinophilic asthma is unknown, according to the <\/span><a href=\"https:\/\/apfed.org\/about-ead\/eosinophilic-asthma\/\" target=\"_blank\" rel=\"noopener\"><span style=\"font-weight: 400;\">American Partnership for Eosinophilic Disorders<\/span><\/a><span style=\"font-weight: 400;\">. Some <\/span><a href=\"https:\/\/openres.ersjournals.com\/content\/1\/1\/00024-2015\" target=\"_blank\" rel=\"noopener\"><span style=\"font-weight: 400;\">studies<\/span><\/a><span style=\"font-weight: 400;\"> estimate it affects 5% of adults with asthma. Other <\/span><a href=\"https:\/\/journal.chestnet.org\/article\/S0012-3692(21)00700-5\/pdf\" target=\"_blank\" rel=\"noopener\"><span style=\"font-weight: 400;\">research<\/span><\/a><span style=\"font-weight: 400;\"> suggests that severe eosinophilic asthma may be more prevalent than experts have thought in the past.<\/span><\/p>\n<h2 id=\"symptoms\">Symptoms<\/h2>\n<p><span style=\"font-weight: 400;\">The symptoms of eosinophilic asthma are similar to other types of asthma, but with a few added indicators. Common eosinophilic asthma symptoms can include:<\/span><\/p>\n<ul>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Coughing<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Wheezing<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Shortness of breath<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Chest tightness<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Inflamed nasal passages<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Nasal polyps<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Chronic sinus problems<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Lung function abnormalities<\/span><\/li>\n<\/ul>\n<h2 id=\"causes\">Causes<\/h2>\n<p><span style=\"font-weight: 400;\">The specific cause of eosinophilic asthma is unknown. Other types of asthma (like allergic asthma) are triggered by environmental factors, including allergens such as dust mites, mold, animal fur, or pet dander. That does not appear to be the case for this type. However, it is possible that you could have allergies and eosinophilic asthma at the same time, says Dr. Romeo.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">No genetic link has been pinpointed, either, although experts do note that asthma often runs in families. It\u2019s most common in those with adult-onset asthma, rather than those who have the condition from childhood. In fact, it\u2019s most commonly diagnosed in adults between the ages of 35 and 50 years of age. But it can occur in children, young adults, and older adults, too.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">\u201cThere is no known way to prevent asthma, outside of reducing risk\u2014i.e., limit maternal history of smoking, living in cities, etc.,\u201d Dr. Romeo says. \u201cThere are numerous genetic and environmental factors at play and so it is hard to know in advance who will progress to asthma and who won&#8217;t.\u201d<\/span><\/p>\n<h2 id=\"diagnosis\">How is eosinophilic asthma diagnosed?<\/h2>\n<p><span style=\"font-weight: 400;\">If your asthma is severe\u2014meaning it\u2019s not well-controlled with inhalers and oral medications\u2014your primary care provider will likely refer you to a pulmonologist, allergist, or immunologist for testing and diagnosis. Your healthcare provider will look for specific markers of this form of asthma, such as swollen sinuses and growths in your nasal passages. To diagnose eosinophilic asthma, there are four main tests:<\/span><\/p>\n<ol>\n<li><b>Blood test: <\/b><span style=\"font-weight: 400;\">A blood draw can indicate high eosinophil levels\u2014but blood eosinophil counts are not as precise for diagnosing asthma as the other tests.<\/span><\/li>\n<li><b>Sputum test:<\/b><span style=\"font-weight: 400;\"> You cough up a sputum sample that your provider sends for analysis or examines under a microscope. High sputum eosinophil levels indicate a positive diagnosis.<\/span><\/li>\n<li><b>Bronchial biopsy: <\/b><span style=\"font-weight: 400;\">Your physician puts you to sleep then inserts a scope into your lungs to extract a tissue or fluid sample. It\u2019s the most accurate, but the most invasive.<\/span><\/li>\n<li><b>Breath test: <\/b><span style=\"font-weight: 400;\">This measures the presence of an elevated fractional exhaled nitric oxide (FENO) concentration.<\/span><\/li>\n<\/ol>\n<p><span style=\"font-weight: 400;\">If you don\u2019t want to undergo invasive testing, your provider can also use your response to treatment to confirm a diagnosis. However, this type of asthma is often misdiagnosed as chronic obstructive pulmonary disease (COPD). Testing can help to rule out that diagnosis.<\/span><\/p>\n<h2 id=\"treatment\">Treatment<\/h2>\n<p><span style=\"font-weight: 400;\">The goal of any <\/span><a href=\"https:\/\/www.singlecare.com\/conditions\/asthma-treatment-and-medications\"><span style=\"font-weight: 400;\">asthma treatment<\/span><\/a><span style=\"font-weight: 400;\"> is to improve your symptoms and quality of life with as few side effects as possible. For eosinophilic asthma, that means reducing the eosinophils causing inflammation in your airways.<\/span><\/p>\n<h3><span style=\"font-weight: 400;\">Medications<\/span><\/h3>\n<p><span style=\"font-weight: 400;\">Some people with this type of asthma respond to traditional therapies, so your provider will likely start with:<\/span><\/p>\n<ul>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><a href=\"https:\/\/www.singlecare.com\/drug-classes\/inhaled-corticosteroids\"><b>Inhaled corticosteroids<\/b><\/a><b>: <\/b><span style=\"font-weight: 400;\">These are inhalers such as <\/span><a href=\"https:\/\/www.singlecare.com\/prescription\/flovent-hfa\"><span style=\"font-weight: 400;\">Flovent HFA<\/span><\/a><span style=\"font-weight: 400;\"> (fluticasone) and <\/span><a href=\"https:\/\/www.singlecare.com\/prescription\/asmanex-60-metered-doses\"><span style=\"font-weight: 400;\">Asmanex<\/span><\/a><span style=\"font-weight: 400;\"> (mometasone) that are meant to control asthma symptoms and prevent exacerbations. Their effectiveness is based on reducing airway inflammation. If needed, they are available in combination with long-acting bronchodilators, with two examples being <\/span><a href=\"https:\/\/www.singlecare.com\/prescription\/advair-diskus\"><span style=\"font-weight: 400;\">Advair Diskus<\/span><\/a><span style=\"font-weight: 400;\"> (fluticasone-salmeterol) and <\/span><a href=\"https:\/\/www.singlecare.com\/prescription\/symbicort\"><span style=\"font-weight: 400;\">Symbicort<\/span><\/a><span style=\"font-weight: 400;\"> (budesonide-formoterol).<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><a href=\"https:\/\/www.singlecare.com\/drug-classes\/leukotriene-modifiers\"><b>Oral medications<\/b><\/a><b>: <\/b><span style=\"font-weight: 400;\">These are leukotriene modifiers that suppress the inflammatory response caused by leukotrienes in your body. Examples include Singular (montelukast sodium), Zyflo (zileuton), and Accolate (zafirlukast).<\/span><\/li>\n<\/ul>\n<p><a href=\"https:\/\/www.singlecare.com\/drug-classes\/corticosteroids\"><span style=\"font-weight: 400;\">Oral corticosteroids<\/span><\/a><span style=\"font-weight: 400;\"> may be used short-term to reduce severe airway inflammation, but are not considered a long-term therapy for most people because they have significant side effects, according to the Asthma and Allergy Foundation of America. Your provider will assess how well your symptoms are controlled. If you\u2019re still having persistent asthma symptoms or asthma exacerbations (aka asthma attacks), it\u2019s time to try another treatment.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Just as Singulair targets leukotrienes to suppress the inflammation they cause, there are several biologic medications that target and thwart eosinophilic inflammation. These medications are administered via an injection or IV drip.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">According to the American Lung Association, there are four appropriate options for this condition:<\/span><\/p>\n<ul>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><b>Fasenra (benralizumab)<\/b><span style=\"font-weight: 400;\">, a monoclonal antibody that binds to the interleukin-5 receptor (IL-5 receptor) on eosinophil cells and prevents their activation. It\u2019s also known as an anti-interleukin-5 agent, or anti-IL-5 agent, since it blocks the action of interleukin-5.<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><b>Nucala (mepolizumab), <\/b><span style=\"font-weight: 400;\">a monoclonal antibody blocks interleukin-5, this medication is typically used in combination with other asthma treatments for asthmatic patients 6 and older.<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><b>Cinqair (reslizumab),<\/b> <b>\u00a0<\/b><span style=\"font-weight: 400;\">a monoclonal antibody blocks interleukin-5, this medication is an add-on treatment for patients 18 and older.<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><b>Dupixent (dupilumab), <\/b><span style=\"font-weight: 400;\">a monoclonal antibody that<\/span> <span style=\"font-weight: 400;\">is an interleukin-4 (IL-4) and interleukin-13 (IL-13) inhibitor also approved for chronic sinusitis with nasal polyps and for atopic dermatitis\u2014making it a good treatment for people with sinus problems and eczema.<\/span><\/li>\n<\/ul>\n<p><span style=\"font-weight: 400;\">These types of medications \u201crapidly reduce eosinophil counts in peripheral blood in humans,\u201d says a <\/span><a href=\"https:\/\/www.frontiersin.org\/articles\/10.3389\/fmed.2018.00049\/full\" target=\"_blank\" rel=\"noopener\"><i><span style=\"font-weight: 400;\">Frontiers in Medicine<\/span><\/i><span style=\"font-weight: 400;\"> review<\/span><\/a><span style=\"font-weight: 400;\">.\u00a0\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Additionally, there\u2019s a new medication, Tezspire (tezepelumab-ekko) that was approved in December 2021. According to the manufacturer, it\u2019s \u201cthe <\/span><a href=\"https:\/\/www.amgen.com\/newsroom\/press-releases\/2022\/01\/tezspire-tezepelumabekko-now-available-in-the-united-states-for-the-treatment-of-severe-asthma\" target=\"_blank\" rel=\"noopener\"><span style=\"font-weight: 400;\">first and only biologic for severe asthma that does not have a phenotype<\/span><\/a><span style=\"font-weight: 400;\">\u2014eosinophilic or allergic\u2014or biomarker limitation within its approved label.\u201d It\u2019s a\u00a0 thymic stromal lymphopoietin (TSLP) antagonist that targets the action of a cytokine that contributes to inflammation. It is taken by injection, once every four weeks.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Additionally, Xolair (omalizumab), a biologic originally approved for allergy-induced asthma may be useful for some people with eosinophilic asthma. <\/span><a href=\"https:\/\/www.jacionline.org\/article\/S0091-6749(18)32032-3\/fulltext#\" target=\"_blank\" rel=\"noopener\"><span style=\"font-weight: 400;\">Research<\/span><\/a><span style=\"font-weight: 400;\"> has shown that it can reduce blood eosinophil levels.\u00a0\u00a0<\/span><\/p>\n<table class=\" For example, while IL-5 is well-known for its association with eosinophilia, another cytokine may be a target for future treatments, says Dr. Mishra. According to a <\/span><a href=\"https:\/\/onlinelibrary.wiley.com\/doi\/abs\/10.1111\/all.15180\" target=\"_blank\" rel=\"noopener\"><span style=\"font-weight: 400;\">paper that he co-authored for the journal <\/span><i><span style=\"font-weight: 400;\">Allergy<\/span><\/i><\/a><span style=\"font-weight: 400;\"> in 2021, animal studies have shown potential benefits in targeting the IL-18 pathway.
