Here is an overview of Medicare Part D drug formularies, including the drugs covered, common plan rules and restrictions, and how to find the best plan for your list of medications.<\/span><\/p>\n<h2><span style=\"font-weight: 400;\">What is a Medicare drug formulary?<\/span><\/h2>\n<p><span style=\"font-weight: 400;\">A <\/span><a href=\"https:\/\/www.singlecare.com\/blog\/what-is-a-formulary\/\"><span style=\"font-weight: 400;\">drug formulary<\/span><\/a><span style=\"font-weight: 400;\"> is a health plan\u2019s list of covered drugs. Most plans with drug coverage, including Medicare and non-Medicare plans, have their own drug formulary. These formularies are created by Pharmacy and Therapeutics (P&amp;T) committees for each individual health insurance company.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">The <\/span><a href=\"https:\/\/www.cms.gov\/Medicare\/Prescription-Drug-Coverage\/PrescriptionDrugCovContra\/RxContracting_FormularyGuidance\" target=\"_blank\" rel=\"noopener\"><span style=\"font-weight: 400;\">Centers for Medicare and Medicaid Services (CMS)<\/span><\/a><span style=\"font-weight: 400;\"> then reviews the formularies to ensure that plans offer comprehensive drug benefits that provide beneficiaries with cost-effective access to necessary medications.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">\u201cMedicare formularies include generic and brand-name prescription drugs,\u201d says<\/span> <a href=\"https:\/\/www.linkedin.com\/in\/ashley-woodcox-6395a459\/\" target=\"_blank\" rel=\"noopener\"><span style=\"font-weight: 400;\">Ashley Woodcox<\/span><\/a><span style=\"font-weight: 400;\">, BSHCA, executive director of <\/span><a href=\"https:\/\/www.brookdale.com\/en\/communities\/brookdale-carmel.html\" target=\"_blank\" rel=\"noopener\"><span style=\"font-weight: 400;\">Brookdale Senior Living<\/span><\/a><span style=\"font-weight: 400;\">. \u201cMany plans divide the covered drugs into tiers, or levels, based on cost. Drugs in a lower tier will generally have a lower copayment than non-preferred drugs in a higher tier.\u201d<\/span><\/p>\n<p><span style=\"font-weight: 400;\">While each plan can divide its tiers how they choose, an example of a drug plan\u2019s tiers may include:\u00a0<\/span><\/p>\n<ul>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><b>Tier 1:<\/b><span style=\"font-weight: 400;\"> Mostly generic prescription drugs<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><b>Tier 2:<\/b><span style=\"font-weight: 400;\"> Preferred, brand-name prescription drugs<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><b>Tier 3:<\/b><span style=\"font-weight: 400;\"> Non-preferred, brand-name prescription drugs<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><b>Tier 4:<\/b><span style=\"font-weight: 400;\"> Specialty, brand-name prescription drugs<\/span><\/li>\n<\/ul>\n<h2><span style=\"font-weight: 400;\">What drugs are covered by Medicare Part D?<\/span><\/h2>\n<p><a href=\"https:\/\/www.singlecare.com\/blog\/medicare-part-d-plans\/\"><span style=\"font-weight: 400;\">Medicare Part D<\/span><\/a><span style=\"font-weight: 400;\"> covers prescription drugs that are regularly taken at home. All Medicare drug plans are required to cover at least two drugs in most prescription drug categories. However, it\u2019s up to the individual drug plans to make coverage determinations that include which specific drugs they cover within those categories.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Medicare prescription drug plans are also required to cover nearly all drugs in <\/span><a href=\"https:\/\/www.cms.gov\/newsroom\/fact-sheets\/medicare-advantage-and-part-d-drug-pricing-final-rule-cms-4180-f\" target=\"_blank\" rel=\"noopener\"><span style=\"font-weight: 400;\">Medicare\u2019s Six Protected Classes<\/span><\/a><span style=\"font-weight: 400;\">, which include:\u00a0<\/span><\/p>\n<ol>\n<li><b>Antipsychotics: <\/b><span style=\"font-weight: 400;\">Drugs used to help treat forms of psychosis.<\/span><\/li>\n<li><b>Antidepressants: <\/b><span style=\"font-weight: 400;\">Drugs used to help treat depression and anxiety disorders.<\/span><\/li>\n<li><b>Anticonvulsants: <\/b><span style=\"font-weight: 400;\">Drugs used to treat seizure disorders.<\/span><\/li>\n<li><b>Antineoplastics: <\/b><span style=\"font-weight: 400;\">Drugs used to treat cancer, also known as anticancer drugs.<\/span><\/li>\n<li><b>Antiretrovirals: <\/b><span style=\"font-weight: 400;\">Drugs commonly used to treat HIV\/AIDs.<\/span><\/li>\n<li><b>Immunosuppressants<\/b><span style=\"font-weight: 400;\">: Commonly used to reduce the strength of the body\u2019s immune system to prevent organ rejection after a transplant.<\/span><\/li>\n<\/ol>\n<h2><span style=\"font-weight: 400;\">What drugs aren\u2019t covered by Medicare Part D?<\/span><\/h2>\n<p><span style=\"font-weight: 400;\">Although Medicare Part D plans cover a wide variety of drugs, there are certain prescription drugs that aren\u2019t covered.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Examples of <\/span><a href=\"https:\/\/www.cms.gov\/files\/document\/excluded-drug-reference-file-faq.pdf\" target=\"_blank\" rel=\"noopener\"><span style=\"font-weight: 400;\">drugs excluded from Part D coverage<\/span><\/a><span style=\"font-weight: 400;\"> include:\u00a0<\/span><\/p>\n<ul>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Brand-name drugs when a generic version containing the same active ingredients is available<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Drugs used to treat eating disorders<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Medications for weight loss or gain<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Over-the-counter medications<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Cosmetic drugs, such as those for hair growth<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Prescription vitamins and mineral products, except prenatal vitamins\u00a0<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Drugs used to treat sexual or erectile dysfunction<\/span><\/li>\n<\/ul>\n<p><span style=\"font-weight: 400;\">\u201cIt&#8217;s important to understand that each drug formulary is different, and your plan may not include the specific medication your doctor has prescribed,\u201d says Woodcox. \u201cIn this case, a similar drug should be available.\u201d However, when this is not the case, you or your prescriber can request a <\/span><a href=\"https:\/\/www.cms.gov\/Medicare\/Appeals-and-Grievances\/MedPrescriptDrugApplGriev\/Exceptions\" target=\"_blank\" rel=\"noopener\"><span style=\"font-weight: 400;\">formulary exception<\/span><\/a><span style=\"font-weight: 400;\"> or look into changing plans during the next enrollment period.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">You can enroll in Part D coverage through a stand-alone Part D plan or a <\/span><a href=\"https:\/\/www.singlecare.com\/blog\/what-is-medicare-advantage\/\"><span style=\"font-weight: 400;\">Medicare Advantage plan<\/span><\/a><span style=\"font-weight: 400;\"> that includes Part D benefits.<\/span><\/p>\n<h2><span style=\"font-weight: 400;\">Network pharmacies<\/span><\/h2>\n<p><span style=\"font-weight: 400;\">Medicare Part D plans often have agreements with pharmacies within their network to provide services and supplies to consumers for a discounted price. Plans may not cover prescription drugs from a pharmacy <\/span><a href=\"https:\/\/www.singlecare.com\/blog\/healthcare-defined-out-of-network\/\"><span style=\"font-weight: 400;\">outside of its network<\/span><\/a><span style=\"font-weight: 400;\">.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">In addition to network pharmacies, plans may offer other options for getting prescription medications, including preferred pharmacies, mail-order programs, and two- or three-month retail pharmacy programs. Regardless of the pharmacy, it\u2019s important to have your Medicare ID card on hand when filling your prescription.<\/span><\/p>\n<h3><span style=\"font-weight: 400;\">Preferred pharmacies<\/span><\/h3>\n<p><span style=\"font-weight: 400;\">Some Part D plans may have preferred pharmacies to help consumers save on out-of-pocket prescription drug costs. A preferred pharmacy has an agreement with the drug plan to charge less for prescription drugs purchased through them. As a result, consumers will have a lower copay for covered drugs than if they purchased them through a non-preferred pharmacy.\u00a0<\/span><\/p>\n<h3><span style=\"font-weight: 400;\">Mail-order programs<\/span><\/h3>\n<p><span style=\"font-weight: 400;\">Some drug plans offer a <\/span><a href=\"https:\/\/www.singlecare.com\/blog\/mail-order-pharmacy\/\"><span style=\"font-weight: 400;\">mail-order option<\/span><\/a><span style=\"font-weight: 400;\"> for home delivery of prescriptions. Not only do these programs offer the convenience of skipping a visit to the pharmacy every time a refill is needed, but they may also offer additional savings when purchasing a 90-day supply.<\/span><\/p>\n<h3><span style=\"font-weight: 400;\">Two- or three-month retail pharmacy programs<\/span><\/h3>\n<p><span style=\"font-weight: 400;\">Some retail pharmacies offer the option to purchase a two- or three-month supply of the covered prescription drugs rather than a 30-day supply. This may be a cost-effective option for those wanting to save time with fewer visits to the pharmacy.<\/span><\/p>\n<h2><span style=\"font-weight: 400;\">Coverage rules and restrictions<\/span><\/h2>\n<p><span style=\"font-weight: 400;\">To ensure that certain drugs are used correctly and only when necessary, plans may enforce coverage rules and restrictions, <\/span><a href=\"https:\/\/www.medicare.gov\/drug-coverage-part-d\/what-medicare-part-d-drug-plans-cover\/drug-plan-coverage-rules\" target=\"_blank\" rel=\"noopener\"><span style=\"font-weight: 400;\">according to Medicare<\/span><\/a><span style=\"font-weight: 400;\">. This includes prior authorization, step therapy, quantity limits, and opioid pain medication safety checks.\u00a0<\/span><\/p>\n<h3><span style=\"font-weight: 400;\">Prior authorization<\/span><\/h3>\n<p><span style=\"font-weight: 400;\">Some plans may require <\/span><a href=\"https:\/\/www.singlecare.com\/blog\/prior-authorization\"><span style=\"font-weight: 400;\">prior authorization<\/span><\/a><span style=\"font-weight: 400;\"> for certain drugs. This means that before a plan will cover the cost of a drug, the plan must give pre-approval based on prior authorization criteria set by the plan. To do so, your doctor must submit the proper documentation to show that a drug is medically necessary.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">In some cases, a plan may use the prior authorization process when it only covers a drug when used to treat certain medical conditions but will not cover the drug when used to treat other medical conditions.<\/span><\/p>\n<h3><span style=\"font-weight: 400;\">Step therapy<\/span><\/h3>\n<p><a href=\"https:\/\/www.singlecare.com\/blog\/step-therapy\/\"><span style=\"font-weight: 400;\">Step therapy<\/span><\/a><span style=\"font-weight: 400;\"> requires that you try an alternative drug on a lower tier before the plan covers the higher cost of a drug on a higher tier.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">If your doctor believes there will be negative side effects caused by the less expensive drug, it won\u2019t be as effective, or that the more expensive drug is medically necessary, they may request an exception. If approved, the plan will cover the cost of the more expensive drug without requiring that you try other medications first.<\/span><\/p>\n<h3><span style=\"font-weight: 400;\">Quantity limits\u00a0<\/span><\/h3>\n<p><span style=\"font-weight: 400;\">A <\/span><a href=\"https:\/\/www.singlecare.com\/blog\/quantity-limits\/\"><span style=\"font-weight: 400;\">quantity limit<\/span><\/a><span style=\"font-weight: 400;\"> is a restriction on the amount of a certain drug \u200cthe plan will cover during a specific period\u200c. These limits are based on several factors, such as pharmaceutical guidelines, Food and Drug Administration (FDA) labeling, and dosing recommendations.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">This helps reduce costs and ensure \u200cthe drugs are being used as prescribed. For example, for a once-daily medication, they may only cover 30 pills every 30 days.<\/span><\/p>\n<h3><span style=\"font-weight: 400;\">Opioid pain medication safety checks<\/span><\/h3>\n<p><span style=\"font-weight: 400;\">Opioid medications can pose serious addiction and health risks if not used properly. For this reason, drug plans and pharmacies conduct safety reviews to ensure that commonly abused prescription drugs are being used safely and that the prescription is appropriate for your needs.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">A plan may also limit the initial covered supply of opioids \u200cto decrease the risk of addiction or serious health issues from long-term use.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Opioid safety reviews rarely apply to consumers with cancer or sickle cell disease, palliative or end-of-life care, hospice, or those living in a long-term care facility.\u00a0<\/span><\/p>\n<h2><span style=\"font-weight: 400;\">How often do Medicare formularies change?<\/span><\/h2>\n<p><span style=\"font-weight: 400;\">Formulary changes may occur throughout the year for several reasons, such as the release of new drugs or new generics, new medical information, or changes to drug therapies. In most cases, plans must inform you of changes through written notice\u2014at least 30 days prior to the date the change becomes effective or at the time of refill.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">If the FDA removes the medication from the market or finds it unsafe, your plan may remove the drug from their formulary immediately. Plans can also replace brand-name drugs with generic drugs under certain requirements. In these cases, they should inform you of the changes made after they go into effect.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">This may cause changes to your medication coverage throughout the year. In these cases, the plan should provide the consumer with an annual notice of change. If a medication you\u2019re taking is removed from the formulary, you\u2019ll have to pay full price or ask for an <\/span><a href=\"https:\/\/www.cms.gov\/Medicare\/Appeals-and-Grievances\/MedPrescriptDrugApplGriev\/Exceptions\" target=\"_blank\" rel=\"noopener\"><span style=\"font-weight: 400;\">exception<\/span><\/a><span style=\"font-weight: 400;\">. During the next enrollment period, it\u2019s important to review your formulary for the coming year and make changes accordingly.<\/span><\/p>\n<h2><span style=\"font-weight: 400;\">How to find your Medicare formulary<\/span><\/h2>\n<p><span style=\"font-weight: 400;\">It\u2019s important for you to review and understand your Medicare plan\u2019s drug formulary. Contact your plan or visit their website for more information.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">You can also find out which plans cover your medications by comparing Part D drug plans at <\/span><a href=\"https:\/\/www.medicare.gov\/plan-compare\/#\/?lang=en&amp;year=2022\" target=\"_blank\" rel=\"noopener\"><span style=\"font-weight: 400;\">medicare.gov<\/span><\/a><span style=\"font-weight: 400;\">. You should also receive an \u201cEvidence of Coverage\u201d (EOC) from Medicare each year, which will explain what the plan covers and how much you\u2019ll need to pay.\u00a0<\/span><\/p>\n<h2><span style=\"font-weight: 400;\">How to find the best Medicare formulary<\/span><\/h2>\n<p><span style=\"font-weight: 400;\">All Medicare Part D plans are different, so it\u2019s important to research and find the best plan to meet your specific needs. Start by compiling a list of your current prescription drugs and their dosages.
