But some of these \u201cwonder drugs,\u201d as they were once called, can also leave the body vulnerable to an infectious disease called C.Difficile.\u00a0<\/span><\/p>\n<h2><b>What is C.Diff?<\/b><\/h2>\n<p><i><span style=\"font-weight: 400;\">Clostridioides difficile<\/span><\/i><span style=\"font-weight: 400;\">,<\/span><a href=\"https:\/\/www.healio.com\/infectious-disease\/gastrointestinal-infections\/news\/online\/%7B7e40ee44-6c11-4c46-9b8d-aeae8a9ee43f%7D\/clostridioides-difficile--whats-in-a-name\" target=\"_blank\" rel=\"noopener noreferrer\"> <span style=\"font-weight: 400;\">formerly known as <\/span><i><span style=\"font-weight: 400;\">Clostridium difficile<\/span><\/i><\/a><span style=\"font-weight: 400;\">, or C.Diff, is a<\/span><a href=\"https:\/\/www.cdc.gov\/cdiff\/what-is.html\" target=\"_blank\" rel=\"noopener noreferrer\"> <span style=\"font-weight: 400;\">contagious bacteria<\/span><\/a><span style=\"font-weight: 400;\"> that can cause severe diarrhea. Other common C.Diff symptoms include fever, nausea, loss of appetite, and abdominal pain.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">It&#8217;s estimated to cause<\/span><a href=\"https:\/\/www.cdc.gov\/cdiff\/what-is.html\" target=\"_blank\" rel=\"noopener noreferrer\"> <span style=\"font-weight: 400;\">almost 500,000<\/span><\/a><span style=\"font-weight: 400;\"> illnesses in the United States and<\/span><a href=\"https:\/\/www.cdc.gov\/drugresistance\/biggest_threats.html\" target=\"_blank\" rel=\"noopener noreferrer\"> <span style=\"font-weight: 400;\">15,000 deaths<\/span><\/a><span style=\"font-weight: 400;\"> each year, according to the<\/span><a href=\"https:\/\/www.cdc.gov\/\" target=\"_blank\" rel=\"noopener noreferrer\"> <span style=\"font-weight: 400;\">Centers for Disease Control and Prevention<\/span><\/a><span style=\"font-weight: 400;\">. It is currently classified by the CDC as an \u201c<\/span><a href=\"https:\/\/www.cdc.gov\/drugresistance\/biggest_threats.html\" target=\"_blank\" rel=\"noopener noreferrer\"><span style=\"font-weight: 400;\">Urgent Threat<\/span><\/a><span style=\"font-weight: 400;\">\u201d\u2014the Centers&#8217; highest threat level\u2014to human health from an infectious pathogen in the U.S. (The CDC plans to release an<\/span><a href=\"https:\/\/www.cdc.gov\/drugresistance\/biggest_threats.html\" target=\"_blank\" rel=\"noopener noreferrer\"> <span style=\"font-weight: 400;\">updated report<\/span><\/a><span style=\"font-weight: 400;\"> regarding this data in fall 2019.)\u00a0<\/span><\/p>\n<h2><b>Which antibiotics cause C.Diff?<\/b><\/h2>\n<p><span style=\"font-weight: 400;\">So how exactly does antibiotic usage make one susceptible to a life-threatening disease like C.Diff? When you take an antibiotic, it works to kill a bacterial infection in your body. In the process, these drugs can also destroy the healthy bacteria that keep invaders like C.Diff in check. It&#8217;s important to note, not all antibiotics cause C.Diff, and not everyone has the same risk. Although almost any antibiotic can cause it, the worst culprit, in this case, is often broad-spectrum antibiotics. Those most at risk are patients 65 or older who have been in a healthcare setting, such as a hospital or nursing home.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">\u201cBroad-spectrum antibiotics have activity against a [large] range of bacteria that reside in the gut,\u201d explains<\/span><a href=\"https:\/\/www.gwumc.edu\/smhs\/facultydirectory\/profile.cfm?empName=Hana%20Akselrod&amp;FacID=2069179770&amp;show=1\" target=\"_blank\" rel=\"noopener noreferrer\"> <span style=\"font-weight: 400;\">Dr. Hana Akselrod<\/span><\/a><span style=\"font-weight: 400;\">, assistant professor of medicine in the Division of Infectious Disease at the<\/span><a href=\"https:\/\/smhs.gwu.edu\/\" target=\"_blank\" rel=\"noopener noreferrer\"> <span style=\"font-weight: 400;\">George Washington School of Medicine and Health Sciences<\/span><\/a><span style=\"font-weight: 400;\">. \u201cIt\u2019s very important to have a healthy and diverse population of these bacteria.\u201d When people are given broad-spectrum antibiotics, \u201ctheir gut bacteria are depleted,\u201d thereby allowing for a \u201cpathogenic species,\u201d such as C.Diff, to \u201cessentially create overgrowth of aggressive bacteria that produce toxins that damage the bowel and create very severe illness.\u201d<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Erika Prouty, Pharm.D., former adjunct professor at<\/span><a href=\"https:\/\/www1.wne.edu\/pharmacy-and-health-sciences\/\" target=\"_blank\" rel=\"noopener noreferrer\"> <span style=\"font-weight: 400;\">Western New England University College of Pharmacy<\/span><\/a><span style=\"font-weight: 400;\"> in Springfield, Massachusetts, breaks it down further: Broad-spectrum antibiotics can be potentially threatening for patients because they \u201cnot only target the bad bacteria we\u2019re trying to eradicate, but they also kill a lot of the good bacteria that\u2019s in our digestive system.\u201d\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Both Dr. Akselrod and Dr. Prouty identify clindamycin and fluoroquinolones as some of the worst offenders. The list of antibiotics that <\/span><a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC3632900\/\" target=\"_blank\" rel=\"noopener noreferrer\"><span style=\"font-weight: 400;\">could cause C.Diff<\/span><\/a><span style=\"font-weight: 400;\"> includes:<\/span><\/p>\n<ul>\n<li style=\"font-weight: 400;\"><span style=\"font-weight: 400;\">cephalosporins<\/span><\/li>\n<li style=\"font-weight: 400;\"><a href=\"https:\/\/www.singlecare.com\/prescription\/clindamycin-hcl\" target=\"_blank\" rel=\"noopener noreferrer\"><span style=\"font-weight: 400;\">clindamycin<\/span><\/a><span style=\"font-weight: 400;\"> (<\/span><a href=\"https:\/\/www.singlecare.com\/prescription\/cleocin\" target=\"_blank\" rel=\"noopener noreferrer\"><span style=\"font-weight: 400;\">Cleocin<\/span><\/a><span style=\"font-weight: 400;\">)<\/span><\/li>\n<li style=\"font-weight: 400;\"><a href=\"https:\/\/www.singlecare.com\/prescription\/ciprofloxacin\" target=\"_blank\" rel=\"noopener noreferrer\"><span style=\"font-weight: 400;\">ciprofloxacin<\/span><\/a><span style=\"font-weight: 400;\"> (<\/span><a href=\"https:\/\/www.singlecare.com\/prescription\/cipro?q=Cipro\" target=\"_blank\" rel=\"noopener noreferrer\"><span style=\"font-weight: 400;\">Cipro<\/span><\/a><span style=\"font-weight: 400;\">)<\/span><\/li>\n<li style=\"font-weight: 400;\"><a href=\"https:\/\/www.singlecare.com\/prescription\/levofloxacin\" target=\"_blank\" rel=\"noopener noreferrer\"><span style=\"font-weight: 400;\">levofloxacin<\/span><\/a><span style=\"font-weight: 400;\"> (<\/span><a href=\"https:\/\/www.singlecare.com\/blog\/what-is-levaquin\/\" target=\"_blank\" rel=\"noopener noreferrer\"><span style=\"font-weight: 400;\">Levaquin<\/span><\/a><span style=\"font-weight: 400;\">)<\/span><\/li>\n<li style=\"font-weight: 400;\"><a href=\"https:\/\/www.singlecare.com\/prescription\/moxifloxacin-hcl\" target=\"_blank\" rel=\"noopener noreferrer\"><span style=\"font-weight: 400;\">moxifloxacin<\/span><\/a><span style=\"font-weight: 400;\"> (<\/span><a href=\"https:\/\/www.singlecare.com\/prescription\/avelox\" target=\"_blank\" rel=\"noopener noreferrer\"><span style=\"font-weight: 400;\">Avalox<\/span><\/a><span style=\"font-weight: 400;\">, <\/span><a href=\"https:\/\/www.singlecare.com\/prescription\/moxifloxacin-hcl\" target=\"_blank\" rel=\"noopener noreferrer\"><span style=\"font-weight: 400;\">Vigamox<\/span><\/a><span style=\"font-weight: 400;\">)<\/span><\/li>\n<li style=\"font-weight: 400;\"><a href=\"https:\/\/www.singlecare.com\/prescription\/amoxicillin\" target=\"_blank\" rel=\"noopener noreferrer\"><span style=\"font-weight: 400;\">amoxicillin<\/span><\/a><span style=\"font-weight: 400;\"> (Amoxil)<\/span><\/li>\n<\/ul>\n<p><span style=\"font-weight: 400;\">\u201cIt\u2019s unfortunate,\u201d says Dr. Akselrod, \u201cbecause those antibiotics are in wide use for everything from pneumonia to urinary tract infections.\u201d The other issue is the necessity of certain IV antibiotics regularly used by physicians in hospital emergency rooms, which also fall under the \u201cbroad-spectrum\u201d umbrella. These drugs include:<\/span><\/p>\n<ul>\n<li style=\"font-weight: 400;\"><a href=\"https:\/\/www.singlecare.com\/prescription\/piperacillin-sod-tazobactam-so\" target=\"_blank\" rel=\"noopener noreferrer\"><span style=\"font-weight: 400;\">piperacillin\/tazobactam<\/span><\/a><span style=\"font-weight: 400;\"> (Zosyn)<\/span><\/li>\n<li style=\"font-weight: 400;\"><a href=\"https:\/\/www.singlecare.com\/prescription\/meropenem\" target=\"_blank\" rel=\"noopener noreferrer\"><span style=\"font-weight: 400;\">meropenem<\/span><\/a><span style=\"font-weight: 400;\"> (Merrem)<\/span><\/li>\n<\/ul>\n<p><span style=\"font-weight: 400;\">IV antibiotics are routine in an ER setting because they\u2019re used as a \u201cfirst-line therapy when patients come in sick and they\u2019re not sure what\u2019s causing the infection,\u201d explains Dr. Akselrod. But as a medical professional immersed in this field, she is well aware of the fine line physicians must walk in terms of both treating the illness at hand, while not introducing additional infection. \u201cOver time we\u2019ve become cognizant of the risks versus the benefits of this kind of broad-standard therapy,\u201d she says. \u201cIt can be a tough call whether or not to start IV antibiotics. What helps us make the right decision is a judicious approach and trying to have a specific reason for giving antibiotics.\u201d<\/span><\/p>\n<h2><strong>Which antibiotics are less likely to cause C.Diff?<\/strong><\/h2>\n<p><span style=\"font-weight: 400;\">If you are at high risk for C.Diff, it\u2019s worth talking to your healthcare provider about choosing a lower-risk treatment. The antibiotics that are <\/span><a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC3632900\/\" target=\"_blank\" rel=\"noopener noreferrer\"><span style=\"font-weight: 400;\">less likely to cause C.Diff<\/span><\/a><span style=\"font-weight: 400;\"> include:<\/span><\/p>\n<ul>\n<li style=\"font-weight: 400;\"><a href=\"https:\/\/www.singlecare.com\/prescription\/azithromycin\" target=\"_blank\" rel=\"noopener noreferrer\"><span style=\"font-weight: 400;\">azithromycin<\/span><\/a><span style=\"font-weight: 400;\"> (<\/span><a href=\"https:\/\/www.singlecare.com\/prescription\/azithromycin\" target=\"_blank\" rel=\"noopener noreferrer\"><span style=\"font-weight: 400;\">Zithromax<\/span><\/a><span style=\"font-weight: 400;\">, <\/span><a href=\"https:\/\/www.singlecare.com\/prescription\/zithromax-z-pak\" target=\"_blank\" rel=\"noopener noreferrer\"><span style=\"font-weight: 400;\">Z-Pak<\/span><\/a><span style=\"font-weight: 400;\">)<\/span><\/li>\n<li style=\"font-weight: 400;\"><a href=\"https:\/\/www.singlecare.com\/prescription\/clarithromycin\" target=\"_blank\" rel=\"noopener noreferrer\"><span style=\"font-weight: 400;\">clarithromycin<\/span><\/a><span style=\"font-weight: 400;\"> (Biaxin)<\/span><\/li>\n<li style=\"font-weight: 400;\"><a href=\"https:\/\/www.ncbi.nlm.nih.gov\/m\/pubmed\/22563022\/\" target=\"_blank\" rel=\"noopener noreferrer\"><span style=\"font-weight: 400;\">doxycycline<\/span><\/a><span style=\"font-weight: 400;\"> (<\/span><a href=\"https:\/\/www.singlecare.com\/prescription\/oracea\" target=\"_blank\" rel=\"noopener noreferrer\"><span style=\"font-weight: 400;\">Oracea<\/span><\/a><span style=\"font-weight: 400;\">, <\/span><a href=\"https:\/\/www.singlecare.com\/prescription\/vibramycin\" target=\"_blank\" rel=\"noopener noreferrer\"><span style=\"font-weight: 400;\">Vibramycin<\/span><\/a><span style=\"font-weight: 400;\">)<\/span><\/li>\n<li style=\"font-weight: 400;\"><a href=\"https:\/\/www.singlecare.com\/prescription\/erythromycin\" target=\"_blank\" rel=\"noopener noreferrer\"><span style=\"font-weight: 400;\">erythromycin<\/span><\/a><span style=\"font-weight: 400;\"> (<\/span><a href=\"https:\/\/www.singlecare.com\/prescription\/eryped-400\" target=\"_blank\" rel=\"noopener noreferrer\"><span style=\"font-weight: 400;\">Eryped<\/span><\/a><span style=\"font-weight: 400;\">)<\/span><\/li>\n<li style=\"font-weight: 400;\"><span style=\"font-weight: 400;\">fidaxomicin (<\/span><a href=\"https:\/\/www.singlecare.com\/prescription\/dificid\" target=\"_blank\" rel=\"noopener noreferrer\"><span style=\"font-weight: 400;\">Dificid<\/span><\/a><span style=\"font-weight: 400;\">)<\/span><\/li>\n<li style=\"font-weight: 400;\"><a href=\"https:\/\/www.singlecare.com\/prescription\/minocycline-hcl\" target=\"_blank\" rel=\"noopener noreferrer\"><span style=\"font-weight: 400;\">minocycline<\/span><\/a><span style=\"font-weight: 400;\"> (Minocin, <\/span><a href=\"https:\/\/www.singlecare.com\/prescription\/solodyn\" target=\"_blank\" rel=\"noopener noreferrer\"><span style=\"font-weight: 400;\">Solodyn<\/span><\/a><span style=\"font-weight: 400;\">)<\/span><\/li>\n<li style=\"font-weight: 400;\"><span style=\"font-weight: 400;\"><a href=\"https:\/\/www.singlecare.com\/prescription\/metronidazole\">metronidazole<\/a> (<a href=\"https:\/\/www.singlecare.com\/prescription\/flagyl\">Flagyl<\/a>)<\/span><\/li>\n<\/ul>\n<h2><b>What antibiotics treat C.Diff?<\/b><\/h2>\n<p><span style=\"font-weight: 400;\">Although broad-spectrum meds are at the top of the list of C.Diff causes, there are only a few types of C.Diff antibiotics capable of curing this particular infection. <\/span><a href=\"https:\/\/www.singlecare.com\/prescription\/vancomycin-hcl\" target=\"_blank\" rel=\"noopener noreferrer\"><span style=\"font-weight: 400;\">Vancomycin<\/span><\/a><span style=\"font-weight: 400;\"> is the most frequently used antibiotic for C.Diff, says Dr. Prouty, citing the importance of oral, as opposed to IV, treatment: \u201cThe IV doesn\u2019t actually penetrate the gastro-intestinal system, so it\u2019s pretty much useless.\u201d And since all infections are caused by different microbes (a bacterium that causes disease), \u201cnot all antibiotics are going to target those microbes and kill them,\u201d she says. Therefore,specific drugs must be used in C.Diff treatment.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">It has been suggested that<\/span> <span style=\"font-weight: 400;\"><a href=\"https:\/\/www.healio.com\/news\/infectious-disease\/20220912\/c-difficile-infections-fell-during-pandemic-but-patients-had-higher-mortality-costs\" target=\"_blank\" rel=\"noopener\">rates of C.diff<\/a> have fallen<\/span><span style=\"font-weight: 400;\"> in recent years at least partially due to<\/span><a href=\"https:\/\/www.cdc.gov\/antibiotic-use\/healthcare\/implementation\/core-elements.html\" target=\"_blank\" rel=\"noopener noreferrer\"> <span style=\"font-weight: 400;\">antibiotic stewardship programs<\/span><\/a><span style=\"font-weight: 400;\"> in hospitals, which seek to reduce the prescribing of unnecessary antibiotics. But when it comes to patients being proactive over C.Diff prevention, Dr. Akselrod says the number one thing they can do is have a frank conversation with their medical provider about whether or not they \u201creally need that antibiotic, and to minimize the time spent on it.\u201d\u00a0<\/span><\/p>\n","protected":false},"excerpt":{"rendered":"<p>The introduction of antibiotics to cure infections undoubtedly revolutionized medicine over the past century.
