Mycophenolate<\/span><\/h3>\n<p><span style=\"font-weight: 400;\">Mycophenolate is an important treatment for <\/span><a href=\"https:\/\/jamanetwork.com\/journals\/jamanetworkopen\/fullarticle\/2823522\"><span style=\"font-weight: 400;\">systemic lupus<\/span><\/a><span style=\"font-weight: 400;\">. <\/span><a href=\"https:\/\/www.frontiersin.org\/articles\/10.3389\/fmed.2021.622225\/full\"><span style=\"font-weight: 400;\">Two studies<\/span><\/a><span style=\"font-weight: 400;\"> demonstrated that people with lupus taking mycophenolate and voclosporin (an immunosuppressant) could achieve clinical response while using much lower doses of prednisone.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Mycophenolate can also reduce steroid use in other inflammatory and immune diseases. <\/span><a href=\"https:\/\/jamanetwork.com\/journals\/jama\/fullarticle\/2749597\"><span style=\"font-weight: 400;\">A 2023 study<\/span><\/a><span style=\"font-weight: 400;\"> found it has similar steroid-sparing effects as methotrexate for uveitis. In a <\/span><a href=\"https:\/\/acrabstracts.org\/abstract\/mycophenolate-mofetil-as-a-steroid-sparing-agent-in-polymyositis-and-dermatomyositis-a-systematic-review-of-the-literature\/\"><span style=\"font-weight: 400;\">systematic review of 18 studies<\/span><\/a><span style=\"font-weight: 400;\">, 89% of patients with idiopathic inflammatory myopathies (inflammatory muscle diseases) decreased their steroid dose after starting mycophenolate, and 88% showed improvement in disease activity. In a <\/span><a href=\"https:\/\/link.springer.com\/article\/10.1007\/s13555-019-00346-x\"><span style=\"font-weight: 400;\">head-to-head study<\/span><\/a><span style=\"font-weight: 400;\"> comparing mycophenolate and azathioprine for pemphigus (a rare autoimmune skin condition), patients taking mycophenolate required significantly lower steroid doses to achieve remission.<\/span><\/p>\n<h3 id=\"4-mercaptopurine\"><span style=\"font-weight: 400;\">4. Mercaptopurine<\/span><\/h3>\n<p><span style=\"font-weight: 400;\">Mercaptopurine may help reduce prednisone doses in people with inflammatory bowel disease (IBD). It is also an immunosuppressant that can reduce steroid doses in people with inflammatory bowel disease, often used <\/span><a href=\"https:\/\/www.gastrojournal.org\/article\/S0016-5085(13)01526-6\/fulltext\"><span style=\"font-weight: 400;\">alongside infliximab<\/span><\/a><span style=\"font-weight: 400;\"> for this purpose.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Azathioprine may also help reduce steroid use in people with myasthenia gravis. A <\/span><a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/21819556\/\"><span style=\"font-weight: 400;\">study comparing methotrexate and azathioprine<\/span><\/a><span style=\"font-weight: 400;\"> in people with myasthenia gravis found that both drugs had a similar degree of steroid-sparing effects.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Azathioprine may also be effective at reducing steroid doses in people with giant cell arteritis, though data are mostly limited to case studies. It may also lower the need for steroids in people with recurrent pericarditis (recurring inflammation around the heart). In one <\/span><a href=\"https:\/\/academic.oup.com\/eurheartjsupp\/article\/23\/Supplement_E\/E83\/6386336\"><span style=\"font-weight: 400;\">study<\/span><\/a><span style=\"font-weight: 400;\">, 84.7% of people responded to azathioprine treatment and were able to stop steroid treatment entirely after 4 to 12 months.<\/span><\/p>\n<h2 id=\"over-the-counter-nsaids\"><span style=\"font-weight: 400;\">Over-the-counter NSAIDs<\/span><\/h2>\n<p><span style=\"font-weight: 400;\">People with arthritis may be able to use <\/span><a href=\"https:\/\/www.singlecare.com\/drug-classes\/nsaids\"><span style=\"font-weight: 400;\">NSAIDs<\/span><\/a><span style=\"font-weight: 400;\"> instead of prednisone if their disease activity is not too severe. NSAIDs are not as effective as steroids for arthritis, but if symptoms are adequately controlled, oral corticosteroids may not be needed.
