Other corticosteroids are available that also treat inflammatory diseases. In patients with lupus without renal involvement, mycophenolate was shown to be superior to azathioprine when combined with steroids and thus may be a better option than azathioprine for reducing prednisone doses.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Mycophenolate can be used to reduce steroid use in many different inflammatory and immune diseases other than lupus. Mycophenolate has similar steroid-sparing effects as methotrexate when used for uveitis. In a <\/span><a href=\"https:\/\/acrabstracts.org\/abstract\/mycophenolate-mofetil-as-a-steroid-sparing-agent-in-polymyositis-and-dermatomyositis-a-systematic-review-of-the-literature\/\"><span style=\"font-weight: 400;\">systematic review<\/span><\/a><span style=\"font-weight: 400;\"> of 18 studies, 89% of patients with idiopathic inflammatory myopathies decreased their dose of steroids after starting mycophenolate, and 88% showed improvement in disease activity and experienced mild side effects. In a <\/span><a href=\"https:\/\/link.springer.com\/article\/10.1007\/s13555-019-00346-x\"><span style=\"font-weight: 400;\">head-to-head study<\/span><\/a><span style=\"font-weight: 400;\"> comparing mycophenolate and azathioprine for the treatment of pemphigus, patients taking mycophenolate required significantly lower steroid doses to achieve clinical remission compared to patients taking azathioprine.<\/span><\/p>\n<h3 id=\"4-mercaptopurine\"><span style=\"font-weight: 400;\">4. Mercaptopurine<\/span><\/h3>\n<p><span style=\"font-weight: 400;\">Mercaptopurine<\/span><span style=\"font-weight: 400;\">, which is brand-name Purinethol, may be a great option for reducing prednisone doses in patients with inflammatory bowel disease.\u00a0\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">According to the <\/span><a href=\"https:\/\/www.crohnscolitisfoundation.org\/emr\/emr-ibd-education-tool\/mercaptopurine-6-mp\"><span style=\"font-weight: 400;\">Crohn\u2019s &amp; Colitis Foundation<\/span><\/a><span style=\"font-weight: 400;\">, mercaptopurine can help maintain remission of Crohn\u2019s disease and reduce the long-term need for steroids. While steroids are effective at achieving remission of Crohn\u2019s disease, they should be used at the lowest effective dose and duration needed to induce remission, so that patients can avoid serious side effects of prednisone that can occur with long-term use. Azathioprine<\/span><\/h3>\n<p><span style=\"font-weight: 400;\">Azathioprine<\/span><span style=\"font-weight: 400;\"> is another DMARD that can reduce steroid doses in patients with inflammatory bowel disease. It is often used <\/span><a href=\"https:\/\/www.gastrojournal.org\/article\/S0016-5085(13)01526-6\/fulltext\"><span style=\"font-weight: 400;\">along with infliximab<\/span><\/a><span style=\"font-weight: 400;\"> for this purpose.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Azathioprine may also be used to reduce the use of steroids in patients with myasthenia gravis. A study comparing methotrexate and azathioprine in patients with myasthenia gravis demonstrated that both drugs had a <\/span><a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/21819556\/\"><span style=\"font-weight: 400;\">similar degree of steroid-sparing effects<\/span><\/a><span style=\"font-weight: 400;\">.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Azathioprine may also be effective at reducing the cumulative steroid dose in patients with giant cell arteritis, although data is mostly limited to case studies. Azathioprine may effectively lower the need for steroids in patients with <\/span><a href=\"https:\/\/academic.oup.com\/eurheartjsupp\/article\/23\/Supplement_E\/E83\/6386336\"><span style=\"font-weight: 400;\">recurrent pericarditis<\/span><\/a><span style=\"font-weight: 400;\">. In <\/span><a href=\"https:\/\/www.internationaljournalofcardiology.com\/article\/S0167-5273(11)00057-X\/fulltext\"><span style=\"font-weight: 400;\">one study<\/span><\/a><span style=\"font-weight: 400;\">, 84.7% of patients responded to treatment with azathioprine and were able to completely suspend steroid treatment after 4-12 months.<\/span><\/p>\n<h2 id=\"over-the-counter-nsaids\"><span style=\"font-weight: 400;\">Over-the-counter NSAIDs<\/span><\/h2>\n<p><span style=\"font-weight: 400;\">Patients with arthritis may be able to use <\/span><a href=\"https:\/\/www.singlecare.com\/drug-classes\/nsaids\"><span style=\"font-weight: 400;\">NSAIDs<\/span><\/a><span style=\"font-weight: 400;\"> instead of prednisone if their disease activity is not too severe. NSAIDs are not as effective as steroids for the treatment of arthritis, but if symptoms are adequately controlled with NSAIDs, patients may not need to take oral corticosteroids.
