Or maybe you\u2019re experiencing nausea, bloating, or constipation on a regular basis. If so, you may have a condition called diverticulitis. The common digestive system disorder occurs when small pouches called diverticula form in your colon and become inflamed and infected. The presence of diverticula is called diverticulosis. Here, experts explain the difference between diverticulosis and diverticulitis, plus treatment and prevention strategies.\u00a0<\/span><\/p>\n<h2><span style=\"font-weight: 400;\">What is diverticulitis and diverticulosis?<\/span><\/h2>\n<p><span style=\"font-weight: 400;\">The condition <\/span><a href=\"https:\/\/www.niddk.nih.gov\/health-information\/digestive-diseases\/diverticulosis-diverticulitis\" target=\"_blank\" rel=\"noopener\"><span style=\"font-weight: 400;\">diverticulosis<\/span><\/a><span style=\"font-weight: 400;\"> occurs when diverticula are present. These are small, sac-like, bulging pouches that push through weak spots of the colon\/large intestine. Diverticulosis is very common, occurring in about <\/span><a href=\"https:\/\/medlineplus.gov\/diverticulosisanddiverticulitis.html\" target=\"_blank\" rel=\"noopener\"><span style=\"font-weight: 400;\">half<\/span><\/a><span style=\"font-weight: 400;\"> of individuals older than 60; almost everyone will have it by the <\/span><a href=\"https:\/\/www.asge.org\/home\/for-patients\/patient-information\/understanding-diverticulosis\" target=\"_blank\" rel=\"noopener\"><span style=\"font-weight: 400;\">age of 80<\/span><\/a><span style=\"font-weight: 400;\">.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">What causes diverticulosis to develop in the first place? According to <\/span><a href=\"https:\/\/www.linkedin.com\/in\/drjosemorey\/\" target=\"_blank\" rel=\"noopener\"><span style=\"font-weight: 400;\">Jos\u00e9 Morey, MD<\/span><\/a><span style=\"font-weight: 400;\">, a musculoskeletal radiologist at <\/span><a href=\"https:\/\/www.mcrrads.com\/jose-morey-md.html\" target=\"_blank\" rel=\"noopener\"><span style=\"font-weight: 400;\">Medical Center Radiologists<\/span><\/a><span style=\"font-weight: 400;\"> in Norfolk, Virginia, diverticulosis tends to develop as a result of consuming a Western diet, which is low in fiber. Low-fiber diets cause constipation, which increases pressure inside the digestive tract and causes you to strain during bowel movements. Over time, these factors can cause the development of diverticula. \u201c<\/span><span style=\"font-weight: 400;\">Eating less red meat and more fiber can help prevent their development,\u201d Dr. Morey says. There also appears to be a hereditary component to their development.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Diverticulosis usually doesn\u2019t cause symptoms, but can cause lower abdominal cramping in some individuals. Diverticulosis is also permanent, says Dr. Morey. That means once you develop diverticula, the pouches won&#8217;t go away. In fact, the condition can worsen. When diverticula become inflamed and infected, the condition is called diverticulitis.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">\u201cThe term \u2018itis\u2019 is Latin for inflammation\/infection,\u201d\u00a0 Dr. Morey says.\u00a0 So any time you see it added to the end of something, it means the inflammation of the prefix. For example, hepatitis is inflammation of the liver, and pancreatitis is inflammation of the pancreas. \u201cHence, diverticulitis is inflammation or infection of the diverticula,\u201d he explains.<\/span><\/p>\n<h2><span style=\"font-weight: 400;\">What is the difference between diverticulitis and diverticulosis?<\/span><\/h2>\n<p><span style=\"font-weight: 400;\">The National Institute of Diabetes and Digestive and Kidney Diseases defines diverticulosis as a condition in which small sacs called diverticula push through the wall of the colon. Typically, these sacs form in the sigmoid colon, which is the left lower part of the colon. But Dr. Morey says the diverticula can develop in other locations along the alimentary canal, the internal passageway from the mouth to anus, including the ascending colon and even the small intestine.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">The presence of diverticula (diverticulosis) usually occurs without any symptoms, says <\/span><a href=\"https:\/\/www.linkedin.com\/in\/elena-a-ivanina-do-mph-09309378\/\" target=\"_blank\" rel=\"noopener\"><span style=\"font-weight: 400;\">Elena Ivanina, DO<\/span><\/a><span style=\"font-weight: 400;\">, a board-certified gastroenterologist and director of neuro-integrative gastroenterology at Lenox Hill Hospital\u2019s <\/span><a href=\"https:\/\/www.northwell.edu\/find-care\/find-a-doctor\/gastroenterology\/dr-elena-a-ivanina-do-11372774\" target=\"_blank\" rel=\"noopener\"><span style=\"font-weight: 400;\">Northwell Health<\/span><\/a><span style=\"font-weight: 400;\"> in New York<\/span><span style=\"font-weight: 400;\">.<\/span><span style=\"font-weight: 400;\"> But it is possible to experience lower<\/span> <span style=\"font-weight: 400;\">abdominal discomfort and changes in bowel habits like constipation or diarrhea with diverticulosis, \u00a0says <a href=\"https:\/\/www.instagram.com\/gutsygirlmd\/\">Supriya Rao, MD<\/a>, a board-certified gastroenterologist at <a href=\"https:\/\/www.integratedgic.com\/blog\/supriya-rao\">Integrated Gastroenterology Consultants<\/a> in Chelmsford, MA.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Diverticulitis is a more serious condition than diverticulosis. It occurs when diverticula become inflamed and\/or infected. This can happen quickly, causing complications of diverticulitis like \u201csevere abdominal pain that is unrelenting, blood and mucus in the stool, fever, and changes in bowel habits,\u201d Dr. Rao says. The good news? Most of the time, diverticulosis doesn\u2019t turn into diverticulitis\u2014in fact, <\/span><a href=\"https:\/\/www.niddk.nih.gov\/health-information\/digestive-diseases\/diverticulosis-diverticulitis\/definition-facts\" target=\"_blank\" rel=\"noopener\"><span style=\"font-weight: 400;\">most people<\/span><\/a><span style=\"font-weight: 400;\"> won\u2019t experience any symptoms or issues from diverticulosis.<\/span><\/p>\n<h2><span style=\"font-weight: 400;\">What causes diverticulitis and diverticulosis?<\/span><\/h2>\n<p><span style=\"font-weight: 400;\">In the United States, diverticulosis is very common. <\/span><a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC3174080\" target=\"_blank\" rel=\"noopener\"><span style=\"font-weight: 400;\">Research<\/span><\/a><span style=\"font-weight: 400;\"> estimates its prevalence at less than 5% in people younger than 40. But by the age of 60, more than 50% of people will develop diverticulosis. About <\/span><span style=\"font-weight: 400;\"><a href=\"https:\/\/www.nature.com\/articles\/s41572-020-0153-5\" target=\"_blank\" rel=\"noopener\">25%<\/a> of those with diverticulosis will develop symptomatic diverticulosis, with usual symptoms including left lower abdominal cramping and irregular bowels.<\/span><span style=\"font-weight: 400;\"> The rate of diverticulosis increases with age, affecting almost everyone older than 80. In contrast, the rates of diverticul<\/span><i><span style=\"font-weight: 400;\">itis<\/span><\/i><span style=\"font-weight: 400;\"> vary by study, with <\/span><a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/23856358\/\" target=\"_blank\" rel=\"noopener\"><span style=\"font-weight: 400;\">one study<\/span><\/a><span style=\"font-weight: 400;\"> finding only 4% of patients with diverticulosis ever developing diverticulitis.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">It is widely believed that <\/span><a href=\"https:\/\/www.ncbi.nlm.nih.gov\/books\/NBK430771\/\" target=\"_blank\" rel=\"noopener\"><span style=\"font-weight: 400;\">too little fiber<\/span><\/a><span style=\"font-weight: 400;\"> contributes to the development of diverticulosis. That\u2019s because not eating enough fibrous fruits, vegetables, beans, legumes, nuts, and whole grains can lead to chronic <\/span><a href=\"https:\/\/www.singlecare.com\/conditions\/constipation-treatment-and-medications\"><span style=\"font-weight: 400;\">constipation<\/span><\/a><span style=\"font-weight: 400;\">, which causes a buildup of pressure in the colon\u2014this may allow diverticula to form.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">More research into the cause of diverticulitis is needed, but one theory is that diverticula may become infected when bacteria from stool gets pushed into the tiny sacs. The good news is that these conditions are <\/span><a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/21790681\/\" target=\"_blank\" rel=\"noopener\"><span style=\"font-weight: 400;\">not linked<\/span><\/a><span style=\"font-weight: 400;\"> with an increase in colon cancer in the long term.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">The <\/span><a href=\"https:\/\/my.clevelandclinic.org\/health\/diseases\/10352-diverticular-disease\" target=\"_blank\" rel=\"noopener\"><span style=\"font-weight: 400;\">following factors<\/span><\/a><span style=\"font-weight: 400;\"> increase your risk of diverticulosis or diverticulitis<\/span><span style=\"font-weight: 400;\">:<\/span><\/p>\n<ul>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">You are older than 40.<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">You are female and over the age of 70.<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">You are overweight or obese.<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">You eat too many low-fiber foods.<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">You eat a diet that is high in fat and red meat.<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">You do not engage in regular exercise<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">You regularly take <\/span><a href=\"https:\/\/my.clevelandclinic.org\/health\/drugs\/11086-non-steroidal-anti-inflammatory-medicines-nsaids\" target=\"_blank\" rel=\"noopener\"><span style=\"font-weight: 400;\">nonsteroidal anti-inflammatory drugs (NSAIDs)<\/span><\/a><span style=\"font-weight: 400;\">, like <\/span><a href=\"https:\/\/www.singlecare.com\/prescription\/aspirin-low-dose\"><span style=\"font-weight: 400;\">aspirin<\/span><\/a><span style=\"font-weight: 400;\">, <\/span><a href=\"https:\/\/www.singlecare.com\/prescription\/ibuprofen\"><span style=\"font-weight: 400;\">ibuprofen<\/span><\/a><span style=\"font-weight: 400;\">, and <\/span><a href=\"https:\/\/www.singlecare.com\/prescription\/naproxen-sodium\"><span style=\"font-weight: 400;\">naproxen<\/span><\/a><span style=\"font-weight: 400;\">.<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">You are a smoker.<\/span><\/li>\n<\/ul>\n<h2><span style=\"font-weight: 400;\">Symptoms of diverticulosis vs. diverticulitis\u00a0<\/span><\/h2>\n<p><span style=\"font-weight: 400;\">Often, diverticulosis does not cause symptoms. However, you may experience the following:<\/span><\/p>\n<ul>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Abdominal cramping or pain (usually in the lower left side)<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Abdominal tenderness<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Bloating\/swelling<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Constipation<\/span><\/li>\n<\/ul>\n<p><span style=\"font-weight: 400;\">The most common symptoms of diverticulitis, which are present at greater intensity during a flare-up, include:\u00a0<\/span><\/p>\n<ul>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Pain and tenderness in the left side of the lower abdomen<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Fever or chills<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Nausea and vomiting<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Abdominal cramps<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Rectal bleeding<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Constipation<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Diarrhea<\/span><\/li>\n<\/ul>\n<p><span style=\"font-weight: 400;\">It\u2019s important to remember that some of these symptoms may be associated with a general gastrointestinal disorder such as irritable bowel syndrome (IBS), inflammatory bowel disease (IBD), celiac disease, appendicitis, gallstones, ulcers, and other digestive diseases. It\u2019s important to receive a diagnosis from a medical professional to differentiate between these conditions and diverticular disease.<\/span><\/p>\n<p><b>RELATED: <\/b><a href=\"https:\/\/www.singlecare.com\/blog\/ibd-vs-ibs\/\"><b>IBD vs. IBS<\/b><\/a><\/p>\n<p><span style=\"font-weight: 400;\">To <\/span><a href=\"https:\/\/www.niddk.nih.gov\/health-information\/digestive-diseases\/diverticulosis-diverticulitis\/diagnosis\" target=\"_blank\" rel=\"noopener\"><span style=\"font-weight: 400;\">diagnose<\/span><\/a><span style=\"font-weight: 400;\"> diverticular disease, your provider will perform a physical exam to check your vital signs and palpate your abdomen for tender areas or masses. This may involve a digital rectal exam. They will also thoroughly review your medical history, including your bowel movement history, diet, medical conditions, and current medications. One or more of the following tests may be ordered:<\/span><\/p>\n<ul>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Blood tests<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Stool test<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Imaging tests: barium enema imaging, computed tomography (CT)<\/span><span style=\"font-weight: 400;\">, <\/span><span style=\"font-weight: 400;\">ultrasound, or <\/span><a href=\"https:\/\/medlineplus.gov\/mriscans.html\" target=\"_blank\" rel=\"noopener\"><span style=\"font-weight: 400;\">magnetic resonance imaging (MRI)<\/span><\/a><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Colonoscopy<\/span><\/li>\n<\/ul>\n<p><span style=\"font-weight: 400;\">According to Dr. Rao, pockets that can form in the lining of the colon due to weaknesses in the wall, or diverticulosis, can be seen during colonoscopy. Dr. Ivanina explains that \u201cif there is pus or inflammatory exudate seen in a diverticulum,\u201d diverticulitis is suspected.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">\u201cOn a CT scan, a pouch (diverticulum) that appears very thick with surrounding inflammatory changes is likely infected and would mean that you have diverticulitis,\u201d says Dr. Ivanina. A CT scan can also help to determine the severity of your diverticulitis.<\/span><\/p>\n<h2><span style=\"font-weight: 400;\">What is the treatment for diverticulitis and diverticulosis?\u00a0<\/span><\/h2>\n<p><span style=\"font-weight: 400;\">Diverticulosis cannot be reversed, says Dr. Rao. \u201cThe best treatment is a high-fiber diet, adequate water intake (to avoid constipation), and stool softeners as needed to avoid constipation,\u201d she says. And although no specific foods can heal diverticulosis, Dr. Ivanina says that increasing dietary fiber can help to prevent the development of new diverticula, diverticulitis, or diverticular bleeding.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Diverticulitis treatment is very nuanced, says Dr. Ivanina. \u201cImmunocompetent, relatively healthy patients with uncomplicated diverticulitis who have no evidence of systemic inflammation can potentially be managed as outpatients,\u201d she says.\u00a0\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Most attacks of diverticulitis can be treated by bowel rest (a clear liquid diet) and a course of antibiotics, Dr. Morey says; these are the most common treatments. Commonly prescribed antibiotics for diverticulitis include:<\/span><\/p>\n<ul>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><a href=\"https:\/\/www.singlecare.com\/prescription\/flagyl\"><span style=\"font-weight: 400;\">Flagyl<\/span><\/a><span style=\"font-weight: 400;\"> (metronidazole)<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><a href=\"https:\/\/www.singlecare.com\/prescription\/bactrim\"><span style=\"font-weight: 400;\">Bactrim<\/span><\/a><span style=\"font-weight: 400;\"> (trimethoprim-sulfamethoxazole)<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><a href=\"https:\/\/www.singlecare.com\/prescription\/cipro\"><span style=\"font-weight: 400;\">Cipro<\/span><\/a><span style=\"font-weight: 400;\"> (ciprofloxacin)<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><a href=\"https:\/\/www.singlecare.com\/prescription\/augmentin\"><span style=\"font-weight: 400;\">Augmentin<\/span><\/a><span style=\"font-weight: 400;\"> (amoxicillin and clavulanic acid)<\/span><\/li>\n<\/ul>\n<p><span style=\"font-weight: 400;\">To save on these and other drugs, you can use a <\/span><a href=\"https:\/\/www.singlecare.com\/\"><span style=\"font-weight: 400;\">SingleCare discount card<\/span><\/a><span style=\"font-weight: 400;\">.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Surgery is considered when there is perforation of the diverticula due to severe active inflammation, says Dr. Morey, adding that if you develop an abscess, it needs to be evaluated.\u00a0 Surgery may also be necessary If multiple, uncomplicated flares of diverticulitis occur, adds Dr. Rao\u2014in that case, your surgeon may discuss resection (removal) of the affected area, says Dr. Rao. Dr. Ivanina emphasizes that surgery is an individualized decision that must be made between a patient and their colorectal or general surgeon.<\/span><span style=\"font-weight: 400;\">\u00a0<\/span><\/p>\n<h2><span style=\"font-weight: 400;\">Diverticulosis vs. diverticulitis prevention<\/span><\/h2>\n<p><span style=\"font-weight: 400;\">Diverticulosis is a common condition in older adults, with prevalence increasing over time, says Dr. Rao. And there are many modifiable risk factors that contribute to the development of diverticular disease, says Dr. Ivanina. These include being overweight or obese, low physical activity, smoking, and NSAID use.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Diet, lifestyle, and genetic factors all influence the development of diverticular disease, says Dr. Rao. \u201cA high-fiber diet that is plant based, adequate water intake, and daily activity will help with gut motility (the movement of food through the digestive tract),\u201d she says.
