Just when it\u2019s time to sit down and start homework, the daily dose of <\/span><a href=\"https:\/\/www.singlecare.com\/blog\/children-adhd-medication-summer-break\/\" target=\"_blank\" rel=\"noopener noreferrer\"><span style=\"font-weight: 400;\">ADHD medication wears off<\/span><\/a><span style=\"font-weight: 400;\">. Epic meltdowns ensue\u2014after which it can take hours to get back on track.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">What many parents don\u2019t realize is: This doesn\u2019t have to be your daily nightmare.\u00a0<\/span><\/p>\n<h2><span style=\"font-weight: 400;\">What is ADHD medication rebound?<\/span><\/h2>\n<p><span style=\"font-weight: 400;\">The flare of symptoms at the tail end of a medication\u2019s effectiveness are called <\/span><a href=\"https:\/\/www.singlecare.com\/blog\/children-adhd-medication-summer-break\/\" target=\"_blank\" rel=\"noopener noreferrer\"><span style=\"font-weight: 400;\">ADHD medication rebound<\/span><\/a><span style=\"font-weight: 400;\">, or sometimes just <\/span><a href=\"https:\/\/www.understood.org\/en\/learning-attention-issues\/treatments-approaches\/medications\/adhd-medication-rebound-what-you-need-to-know\" target=\"_blank\" rel=\"noopener noreferrer\"><span style=\"font-weight: 400;\">ADHD rebound<\/span><\/a><span style=\"font-weight: 400;\">. The intense behavior change that you see in your child for approximately 60 minutes at the end of a dose is the brain\u2019s reaction to stimulant medication leaving the body. It\u2019s most commonly seen with short-acting ADHD medications\u2014such as <\/span><a href=\"https:\/\/www.singlecare.com\/prescription\/ritalin\" target=\"_blank\" rel=\"noopener noreferrer\"><span style=\"font-weight: 400;\">Ritalin<\/span><\/a><span style=\"font-weight: 400;\">, <\/span><a href=\"https:\/\/www.singlecare.com\/prescription\/amphetamine-dextroamphetamine\" target=\"_blank\" rel=\"noopener noreferrer\"><span style=\"font-weight: 400;\">Adderall<\/span><\/a><span style=\"font-weight: 400;\">, or <\/span><a href=\"https:\/\/www.singlecare.com\/prescription\/focalin\" target=\"_blank\" rel=\"noopener noreferrer\"><span style=\"font-weight: 400;\">Focalin<\/span><\/a><span style=\"font-weight: 400;\">. But, it can occur with long-acting medications, too.<\/span><\/p>\n<h2><span style=\"font-weight: 400;\">How to manage ADHD medication rebound<\/span><\/h2>\n<p><span style=\"font-weight: 400;\">These strategies can help alleviate the rebound effect, and help parents manage the after-school witching hour.<\/span><\/p>\n<h3><span style=\"font-weight: 400;\">1. Address the cause of ADHD rebound.<\/span><\/h3>\n<p><span style=\"font-weight: 400;\">ADHD rebound happens when your child\u2019s body processes\u2014or metabolizes\u2014stimulant medication too quickly. If your child has a fast metabolism, a drug that is designed to last for eight to 12 hours may only last for six. The beneficial effects wear off before the next planned dose, and the symptoms that were previously controlled come back suddenly, often even more extreme.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Long-acting ADHD medications\u2014like <\/span><a href=\"https:\/\/www.singlecare.com\/prescription\/adderall-xr\" target=\"_blank\" rel=\"noopener noreferrer\"><span style=\"font-weight: 400;\">Adderall XR<\/span><\/a><span style=\"font-weight: 400;\">, <\/span><a href=\"https:\/\/www.singlecare.com\/prescription\/concerta\" target=\"_blank\" rel=\"noopener noreferrer\"><span style=\"font-weight: 400;\">Concerta<\/span><\/a><span style=\"font-weight: 400;\">, and <\/span><a href=\"https:\/\/www.singlecare.com\/prescription\/vyvanse\" target=\"_blank\" rel=\"noopener noreferrer\"><span style=\"font-weight: 400;\">Vyvanse<\/span><\/a><span style=\"font-weight: 400;\">\u2014are designed to enter and leave the bloodstream gradually. If your child is taking a short-acting stimulant, the <\/span><a href=\"https:\/\/www.aacap.org\/AACAP\/Press\/Press_Releases\/2013\/AACAP_Announces_New_Parents_Medication_Guide_on_ADHD.aspx\" target=\"_blank\" rel=\"noopener noreferrer\"><span style=\"font-weight: 400;\">American Academy of Child and Adolescent Psychiatry<\/span><\/a><span style=\"font-weight: 400;\"> (AACAP) recommends working with your physician to switch to a different delivery system that may prevent the steep drop-off in medication levels that triggers after-school explosions. Or, work with the school nurse to give a second \u201cbooster\u201d dose of medication midday so coverage extends through the evening hours when school is over, but kids still need to concentrate on take-home assignments or extracurricular activities.<\/span><\/p>\n<h3><span style=\"font-weight: 400;\">2. Add a natural remedy for ADHD symptoms in the evening.<\/span><\/h3>\n<h4><b>Put physical activity first on your child\u2019s after-school to-do list<\/b><span style=\"font-weight: 400;\">.\u00a0<\/span><\/h4>\n<p><span style=\"font-weight: 400;\">\u201cThere is now significant evidence that exercise is beneficial for optimal brain function, and especially valuable for ADHD,\u201d says <\/span><a href=\"https:\/\/osher.ucsf.edu\/patient-care\/patient-care-team\/sanford-newmark\" target=\"_blank\" rel=\"noopener noreferrer\"><span style=\"font-weight: 400;\">Sanford Newmark, MD<\/span><\/a><span style=\"font-weight: 400;\">, director of clinical programs at the UCSF Osher Center for Integrative Medicine and author of <\/span><a href=\"https:\/\/www.amazon.com\/ADHD-Without-Drugs-Integrative-Pediatricians\/dp\/0982671407\/ref=sr_1_1?ie=UTF8&amp;qid=1321493079&amp;sr=8-1\" target=\"_blank\" rel=\"noopener noreferrer\"><i><span style=\"font-weight: 400;\">ADHD Without Drugs<\/span><\/i><\/a><span style=\"font-weight: 400;\">. \u201cThe parents of my patients with ADHD often tell me that getting daily exercise is an absolute key to how their child functions at school and at home.\u201d <\/span><a href=\"https:\/\/journals.sagepub.com\/doi\/10.1177\/1087054712454192\" target=\"_blank\" rel=\"noopener noreferrer\"><span style=\"font-weight: 400;\">Research indicates<\/span><\/a><span style=\"font-weight: 400;\"> that exercise sparks the same dopamine and noradrenergic systems that stimulant medications activate.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">\u201cExercise is one of the best gifts you can give your ADHD brain,\u201d <\/span><a href=\"https:\/\/www.drhallowell.com\/\" target=\"_blank\" rel=\"noopener noreferrer\"><span style=\"font-weight: 400;\">Edward Hallowell, MD<\/span><\/a><span style=\"font-weight: 400;\">, author of <\/span><a href=\"https:\/\/www.drhallowell.com\/books\/driven-to-distraction\/\" target=\"_blank\" rel=\"noopener noreferrer\"><i><span style=\"font-weight: 400;\">Driven to Distraction<\/span><\/i><\/a><span style=\"font-weight: 400;\"> agrees. Biking home from school, or sending kids into the backyard to jump on the trampoline can <\/span><a href=\"https:\/\/doi.org\/10.1016\/j.jshs.2014.11.004\" target=\"_blank\" rel=\"noopener noreferrer\"><span style=\"font-weight: 400;\">boost concentration<\/span><\/a><span style=\"font-weight: 400;\"> and cognitive processing just enough to focus on homework. At the very least, it can help your child meet the <\/span><a href=\"https:\/\/www.who.int\/dietphysicalactivity\/factsheet_young_people\/en\/\" target=\"_blank\" rel=\"noopener noreferrer\"><span style=\"font-weight: 400;\">WHO<\/span><\/a><span style=\"font-weight: 400;\"> recommendation for 60 minutes of aerobic activity each day.<\/span><\/p>\n<h4><b>Offer your child a high-protein afternoon snack<\/b><span style=\"font-weight: 400;\">.\u00a0<\/span><\/h4>\n<p><span style=\"font-weight: 400;\">Dietary choices alone often aren\u2019t enough to control ADHD symptoms, but <\/span><a href=\"http:\/\/doi.org\/10.1186\/1744-9081-7-40\" target=\"_blank\" rel=\"noopener noreferrer\"><span style=\"font-weight: 400;\">some research<\/span><\/a><span style=\"font-weight: 400;\"> indicate that kids with ADHD are low in tryptophan, which helps produce the dopamine, noradrenaline, and serotonin needed for learning and controlling behavior.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">\u201cMost children eat a breakfast and lunch with far too many processed carbohydrates, which are quickly digested and result in unstable blood sugar levels,\u201d explains Dr. Newmark. \u201cAdding protein at each meal will stabilize blood sugar and result in more even attention and performance.\u201d\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">In other words, having an after-school snack that\u2019s packed with protein, like yogurt, cottage cheese, or peanut butter spread on apple slices could help control ADHD symptoms.<\/span><\/p>\n<h4><b>Try a mindfulness activity before starting homework.<\/b><span style=\"font-weight: 400;\">\u00a0<\/span><\/h4>\n<p><span style=\"font-weight: 400;\">Spending a few moments using an app like <\/span><a href=\"https:\/\/www.calm.com\/\" target=\"_blank\" rel=\"noopener noreferrer\"><span style=\"font-weight: 400;\">Calm<\/span><\/a><span style=\"font-weight: 400;\"> or coloring a mandala can mellow out challenging emotions and boost concentration skills. <\/span><span style=\"font-weight: 400;\">&#8220;More and more research is finding exciting benefits to boosting executive function and attention after even short mindfulness trainings,&#8221; says <\/span><a href=\"http:\/\/drchristopherwillard.com\/\" target=\"_blank\" rel=\"noopener noreferrer\"><span style=\"font-weight: 400;\">Christopher Willard, Psy.D.<\/span><\/a><span style=\"font-weight: 400;\">, a Harvard Medical School psychologist and author of <\/span><a href=\"https:\/\/www.amazon.com\/Raising-Resilience-Families-Thriving-Children\/dp\/1622038673\" target=\"_blank\" rel=\"noopener noreferrer\"><i><span style=\"font-weight: 400;\">Raising Resilience<\/span><\/i><\/a><i><span style=\"font-weight: 400;\">.<\/span><\/i><\/p>\n<p><strong>RELATED<\/strong>: <a href=\"https:\/\/www.singlecare.com\/blog\/can-you-make-vyvanse-last-longer\/\">Can you make Vyvanse last longer?<\/a><\/p>\n<h3><span style=\"font-weight: 400;\">3. Change your evening schedule.<\/span><\/h3>\n<p><span style=\"font-weight: 400;\">If you notice medication wears off around 7 p.m., when you usually start homework, try flipping the order of your after-school routine. Prioritize high-concentration tasks that can trigger big feelings while medication is still in effect. Later in the evening, schedule fun or engaging activities that aren\u2019t affected by a resurgence of hyperactivity, impulsivity, or inattention\u2014like eating dinner together as a family, or playing a competitive game of Wii Sports. Whenever possible, create a soothing environment for when medication is processing out of your child\u2019s system.<\/span><\/p>\n<h3><span style=\"font-weight: 400;\">4. Try a different type of medication.<\/span><\/h3>\n<p><span style=\"font-weight: 400;\">Stimulants medications are often the first-prescribed to treat ADHD symptoms because they work for <\/span><a href=\"https:\/\/doi.org\/10.3389\/fnins.2013.00082\" target=\"_blank\" rel=\"noopener noreferrer\"><span style=\"font-weight: 400;\">70-80% of children<\/span><\/a><span style=\"font-weight: 400;\">. But they aren\u2019t the only option. If, after working with your physician to adjust dosage and timing, your child is experiencing extreme ADHD rebound it might be time to try a different type of drug.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">There are three non-stimulant medications that are FDA-approved to treat ADHD: <\/span><a href=\"https:\/\/www.singlecare.com\/prescription\/atomoxetine-hcl\" target=\"_blank\" rel=\"noopener noreferrer\"><span style=\"font-weight: 400;\">Strattera<\/span><\/a><span style=\"font-weight: 400;\">, <\/span><a href=\"https:\/\/www.singlecare.com\/prescription\/intuniv\" target=\"_blank\" rel=\"noopener noreferrer\"><span style=\"font-weight: 400;\">Intuniv<\/span><\/a><span style=\"font-weight: 400;\">, and <\/span><a href=\"https:\/\/www.singlecare.com\/prescription\/kapvay\" target=\"_blank\" rel=\"noopener noreferrer\"><span style=\"font-weight: 400;\">Kapvay<\/span><\/a><span style=\"font-weight: 400;\">. Though, the <\/span><a href=\"https:\/\/www.aacap.org\/AACAP\/Press\/Press_Releases\/2013\/AACAP_Announces_New_Parents_Medication_Guide_on_ADHD.aspx\" target=\"_blank\" rel=\"noopener noreferrer\"><span style=\"font-weight: 400;\">AACAP notes<\/span><\/a><span style=\"font-weight: 400;\">, they are less effective at reducing symptoms and should only be tried when stimulants are not effective or negative effects are intolerable.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">It\u2019s hard when you hear reports from teachers that your child acted like an angel all day, but then transforms into a little monster as soon as he or she gets home. Remember, ADHD rebound isn\u2019t just bad behavior. It\u2019s a reaction to the end of a dose of medication that makes symptoms more intense.
