Anyone can get a UTI, but they are <\/span><a href=\"https:\/\/www.cdc.gov\/antibiotic-use\/uti.html?CDC_AA_refVal=https%3A%2F%2Fwww.cdc.gov%2Fantibiotic-use%2Fcommunity%2Ffor-patients%2Fcommon-illnesses%2Futi.html\" target=\"_blank\" rel=\"noopener\"><span style=\"font-weight: 400;\">more common in females<\/span><\/a><span style=\"font-weight: 400;\">. The <\/span><a href=\"https:\/\/www.singlecare.com\/conditions\/uti-symptoms\"><span style=\"font-weight: 400;\">symptoms<\/span><\/a><span style=\"font-weight: 400;\"> include a burning sensation while urinating, increased frequency of urination, and feeling an urgent need to urinate\u2014even with an empty bladder. If left untreated, UTIs can move into the kidneys, causing back pain, vomiting, and fever\u2014sometimes requiring hospitalization. Fortunately, many antibiotics work to treat UTIs, and often people begin to feel better within a day or so. Macrobid (nitrofurantoin) is one of the more <a href=\"https:\/\/www.singlecare.com\/resources\/antibiotics\">commonly prescribed antibiotics<\/a> for UTIs in females. However, choosing the best antibiotic for UTIs in females depends on a few factors.\u00a0<\/span><\/p>\n<h2 id=\"first-line-antibiotics-for-utis-in-females\"><span style=\"font-weight: 400;\">First-line antibiotics for UTIs in females<\/span><\/h2>\n<p><span style=\"font-weight: 400;\">The initial antibiotic chosen to treat a UTI may differ among medical professionals, partly because bacterial resistance to an antibiotic may vary from region to region.\u201cEach region might be a little bit different in terms of resistance. We try to stick with older antibiotics that have been around for a long time and find the one that we think is going to have the least resistance,\u201d says <\/span><a href=\"https:\/\/www.memorialcare.org\/providers\/g-thomas-ruiz\" target=\"_blank\" rel=\"noopener\"><span style=\"font-weight: 400;\">G. Thomas Ruiz, MD,<\/span><\/a> <span style=\"font-weight: 400;\">OB\/GYN Lead at MemorialCare Orange Coast Medical Center in Fountain Valley, California.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Healthcare providers in the United States tend to prescribe one of three <\/span><a href=\"https:\/\/www.ncbi.nlm.nih.gov\/books\/NBK557479\/\" target=\"_blank\" rel=\"noopener\"><span style=\"font-weight: 400;\">first-line antibiotics for simple UTIs in females<\/span><\/a><span style=\"font-weight: 400;\">.<\/span><\/p>\n<h3 id=\"macrobid-nitrofurantoin\"><span style=\"font-weight: 400;\">Macrobid (nitrofurantoin)<\/span><\/h3>\n<p><a href=\"https:\/\/www.singlecare.com\/prescription\/macrobid\"><span style=\"font-weight: 400;\">Macrobid<\/span><\/a><span style=\"font-weight: 400;\"> (nitrofurantoin mono-macro)<\/span><span style=\"font-weight: 400;\"> is commonly prescribed first for UTI symptoms. \u201cMacrobid is a really good antibiotic for UTIs,\u201d says Dr. Ruiz. For an uncomplicated UTI, the dose is twice a day for five days, which can make it more likely for patients to take the full course correctly in comparison to a seven-day antibiotic. However, Macrobid is not prescribed when there is concern about infection in the kidneys.<\/span><\/p>\n<h3 id=\"septra-or-bactrim-sulfamethoxazole-trimethoprim\"><span style=\"font-weight: 400;\">Septra or Bactrim (sulfamethoxazole-trimethoprim)<\/span><\/h3>\n<p><a href=\"https:\/\/www.singlecare.com\/prescription\/bactrim\"><span style=\"font-weight: 400;\">Bactrim<\/span><\/a><span style=\"font-weight: 400;\"> and Septra are <\/span><a href=\"https:\/\/www.singlecare.com\/drug-classes\/sulfonamides\"><span style=\"font-weight: 400;\">sulfa-based antibiotics<\/span><\/a><span style=\"font-weight: 400;\"> that are usually prescribed twice a day for a week. These have long been considered the <\/span><a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC1071654\/#:~:text=Trimethoprim%2Dsulfamethoxazole,-Trimethoprim%2Dsulfamethoxazole\" target=\"_blank\" rel=\"noopener\"><span style=\"font-weight: 400;\">standard treatment for acute and recurrent UTIs<\/span><\/a><span style=\"font-weight: 400;\"> for their effectiveness, low cost, and tolerability. However, there is more bacterial resistance to these antibiotics in some regions.<\/span><\/p>\n<h3 id=\"monurol-fosfomycin\"><span style=\"font-weight: 400;\">Monurol (fosfomycin)\u00a0<\/span><\/h3>\n<p><a href=\"https:\/\/www.singlecare.com\/prescription\/monurol\"><span style=\"font-weight: 400;\">Monurol<\/span><\/a><span style=\"font-weight: 400;\"> (fosfomycin)<\/span><span style=\"font-weight: 400;\"> is specifically for bladder infections and comes in a powder instead of a pill. It can be more expensive than other antibiotics, so it is not always people\u2019s first choice, though you can use a <\/span><a href=\"https:\/\/www.singlecare.com\/prescription-discount-card\"><span style=\"font-weight: 400;\">SingleCare discount card<\/span><\/a><span style=\"font-weight: 400;\"> to reduce the price.\u00a0<\/span><\/p>\n<h2 id=\"other-commonly-prescribed-antibiotics-for-utis-in-females\"><span style=\"font-weight: 400;\">Other commonly prescribed antibiotics for UTIs in females<\/span><\/h2>\n<p><span style=\"font-weight: 400;\">If those antibiotics can\u2019t be prescribed, the next options are oral beta-lactams, which are used for different types of bacterial infections and are one of the <\/span><a href=\"https:\/\/www.ncbi.nlm.nih.gov\/books\/NBK545311\/\" target=\"_blank\" rel=\"noopener\"><span style=\"font-weight: 400;\">most commonly prescribed drug classes<\/span><\/a><span style=\"font-weight: 400;\">.\u00a0<\/span><\/p>\n<h3 id=\"amoxicillin\"><span style=\"font-weight: 400;\">Amoxicillin\u00a0<\/span><\/h3>\n<p><a href=\"https:\/\/www.singlecare.com\/prescription\/amoxicillin\"><span style=\"font-weight: 400;\">Amoxicillin<\/span><\/a><span style=\"font-weight: 400;\"> is in the <\/span><a href=\"https:\/\/www.singlecare.com\/drug-classes\/penicillins\"><span style=\"font-weight: 400;\">penicillin family<\/span><\/a><span style=\"font-weight: 400;\"> and works on many types of infections. But since <\/span><a href=\"https:\/\/www.cdc.gov\/antibiotic-use\/community\/pdfs\/penicillin-factsheet.pdf\" target=\"_blank\" rel=\"noopener\"><span style=\"font-weight: 400;\">10% of U.S. patients report having had an allergy to penicillin<\/span><\/a><span style=\"font-weight: 400;\">, it\u2019s not always the first choice. And since it\u2019s used for various kinds of infections, bacteria in the body can become resistant over time.<\/span><\/p>\n<h3 id=\"ampicillin\"><span style=\"font-weight: 400;\">Ampicillin\u00a0<\/span><\/h3>\n<p><a href=\"https:\/\/www.singlecare.com\/prescription\/ampicillin\"><span style=\"font-weight: 400;\">Ampicillin<\/span><\/a><span style=\"font-weight: 400;\">, which is <\/span><a href=\"https:\/\/www.singlecare.com\/blog\/ampicillin-vs-amoxicillin\/\"><span style=\"font-weight: 400;\">very similar to amoxicillin<\/span><\/a><span style=\"font-weight: 400;\">, isn&#8217;t usually the first choice, either. That\u2019s because the <\/span><a href=\"https:\/\/www.singlecare.com\/prescription\/ampicillin\/dosage\"><span style=\"font-weight: 400;\">dosage of ampicillin<\/span><\/a><span style=\"font-weight: 400;\"> is usually taken every six hours for at least seven days. \u201cThe longer you take an antibiotic, the lower the compliance and the less likelihood that you\u2019ll finish the treatment,\u201d says Dr. Ruiz, since not finishing a course of antibiotics promotes bacterial resistance.<\/span><\/p>\n<h3 id=\"keflex-cephalexin\"><span style=\"font-weight: 400;\">Keflex (cephalexin)<\/span><\/h3>\n<p><a href=\"https:\/\/www.singlecare.com\/prescription\/Keflex\"><span style=\"font-weight: 400;\">Keflex<\/span><\/a><span style=\"font-weight: 400;\"> (cephalexin) is another secondary antibiotic for UTI treatment. It is often one that doctors switch patients to if a urine culture indicates the strain of bacteria is resistant to the antibiotic prescribed as a first line of treatment.\u00a0<\/span><\/p>\n<h3 id=\"rocephin-ceftriaxone\"><span style=\"font-weight: 400;\">Rocephin (ceftriaxone)<\/span><\/h3>\n<p><span style=\"font-weight: 400;\">Rocephin (<\/span><a href=\"https:\/\/www.singlecare.com\/prescription\/ceftriaxone-sodium?q=Ceftriaxone%20Sodium\"><span style=\"font-weight: 400;\">ceftriaxone<\/span><\/a><span style=\"font-weight: 400;\">) is another <\/span><a href=\"https:\/\/www.singlecare.com\/drug-classes\/cephalosporins\"><span style=\"font-weight: 400;\">cephalosporin-class drug<\/span><\/a><span style=\"font-weight: 400;\">, a type of medication that\u2019s similar to penicillin and used for many types of bacterial infections. It is injectable, so it\u2019s not used as a first choice since a nurse or healthcare provider has to administer it. And because it treats a wide range of bacteria, it may lead to bacterial resistance over time.\u00a0<\/span><\/p>\n<h2 id=\"antibiotics-for-complicated-utis-in-females\"><span style=\"font-weight: 400;\">Antibiotics for complicated UTIs in females<\/span><\/h2>\n<p><span style=\"font-weight: 400;\">A <\/span><a href=\"https:\/\/www.ncbi.nlm.nih.gov\/books\/NBK436013\/\" target=\"_blank\" rel=\"noopener\"><span style=\"font-weight: 400;\">complicated UTI<\/span><\/a><span style=\"font-weight: 400;\"> is one that occurs in immunocompromised patients, involves the kidneys, or is associated with sepsis, fevers, stones, catheters, or urinary obstruction. Pregnancy is considered another high-risk condition because UTIs are associated with preterm labor. However, a healthcare provider will choose an antibiotic that is safe to use in pregnancy. In general, complicated UTIs are associated with a higher risk of treatment failure and require longer antibiotic courses. Some antibiotics that are generally reserved for complicated UTIs include <\/span><a href=\"https:\/\/www.singlecare.com\/prescription\/cipro\"><span style=\"font-weight: 400;\">Cipro<\/span><\/a><span style=\"font-weight: 400;\"> (ciprofloxacin) and <\/span><a href=\"https:\/\/www.singlecare.com\/prescription\/levofloxacin\"><span style=\"font-weight: 400;\">Levaquin<\/span><\/a><span style=\"font-weight: 400;\"> (levofloxacin).<\/span><\/p>\n<h3 id=\"cipro-ciprofloxacin\"><span style=\"font-weight: 400;\">Cipro (ciprofloxacin)<\/span><\/h3>\n<p><span style=\"font-weight: 400;\">Cipro<\/span><span style=\"font-weight: 400;\"> is the most popular <\/span><a href=\"https:\/\/www.singlecare.com\/drug-classes\/quinolones\"><span style=\"font-weight: 400;\">fluoroquinolone-classified antibiotic<\/span><\/a><span style=\"font-weight: 400;\">, which Dr. Ruiz says should be reserved for use on \u201ccomplicated UTIs that are resistant to everything else.\u201d The reason they use this one last is that \u201cyou don\u2019t want someone to get a resistance to an antibiotic and then nothing else works.\u201d\u00a0<\/span><\/p>\n<h3 id=\"levaquin-levofloxacin\"><span style=\"font-weight: 400;\">Levaquin <\/span><span style=\"font-weight: 400;\">(levofloxacin)<\/span><\/h3>\n<p><span style=\"font-weight: 400;\">Levaquin<\/span><span style=\"font-weight: 400;\"> is used for many types of infections, including UTIs. Like Cipro, it\u2019s also a fluoroquinolone antibiotic, so it\u2019s not commonly used as a first choice in order to avoid the development of bacterial resistance.\u00a0<\/span><\/p>\n<h2 id=\"side-effects-of-common-uti-antibiotics\"><span style=\"font-weight: 400;\">Side effects of common UTI antibiotics<\/span><\/h2>\n<p><span style=\"font-weight: 400;\">Antibiotics generally work well for UTIs. But as with any medication, there may be side effects. Some of the more common effects that may occur with UTI antibiotics include:<\/span><\/p>\n<ul>\n<li><span style=\"font-weight: 400;\">Upset stomach, nausea, vomiting\u00a0<\/span><\/li>\n<li><span style=\"font-weight: 400;\">Diarrhea\u00a0<\/span><\/li>\n<li><span style=\"font-weight: 400;\">Photosensitivity<\/span><\/li>\n<li><span style=\"font-weight: 400;\">Vaginal yeast infection\u00a0<\/span><\/li>\n<li><span style=\"font-weight: 400;\">Change in the color of the urine<\/span><\/li>\n<\/ul>\n<p><span style=\"font-weight: 400;\">Serious side effects include severe diarrhea or signs that you are allergic to the medication, such as rash, trouble breathing, or swelling. Contact your healthcare provider immediately if you experience these side effects.\u00a0<\/span><\/p>\n<h2 id=\"how-do-antibiotics-treat-utis\"><span style=\"font-weight: 400;\">How do antibiotics treat UTIs?\u00a0<\/span><\/h2>\n<p><span style=\"font-weight: 400;\">UTIs occur when bacteria enter the urethra and infect the urinary tract. Females get UTIs more frequently than males because the urethra is shorter and closer to the rectum, according to the Centers for Disease Control and Prevention.\u00a0<\/span><\/p>\n<p><a href=\"https:\/\/www.singlecare.com\/conditions\/uti-causes\"><span style=\"font-weight: 400;\">Factors that contribute to UTIs in females<\/span><\/a><span style=\"font-weight: 400;\"> include:<\/span><\/p>\n<ul>\n<li><span style=\"font-weight: 400;\">Sexual activity<\/span><\/li>\n<li><span style=\"font-weight: 400;\">Not urinating after sexual intercourse<\/span><\/li>\n<li><span style=\"font-weight: 400;\">Using diaphragms, vaginal rings, or spermicide<\/span><\/li>\n<li><span style=\"font-weight: 400;\">Overwashing or douching<\/span><\/li>\n<li><span style=\"font-weight: 400;\">Wiping from back to front<\/span><\/li>\n<li><span style=\"font-weight: 400;\">Pregnancy<\/span><\/li>\n<li><span style=\"font-weight: 400;\">Wearing tight clothing, which can trap heat and bacteria<\/span><\/li>\n<li><span style=\"font-weight: 400;\">Dehydration<\/span><\/li>\n<li><span style=\"font-weight: 400;\">Infrequent urination<\/span><\/li>\n<li><span style=\"font-weight: 400;\">Anatomic variation, such as a short urethra<\/span><\/li>\n<li><span style=\"font-weight: 400;\">Abnormalities in the urinary tract<\/span><\/li>\n<li><span style=\"font-weight: 400;\">Medical conditions like diabetes, obesity, and sickle cell anemia<\/span><\/li>\n<li><span style=\"font-weight: 400;\">Using a catheter\u00a0<\/span><\/li>\n<li><span style=\"font-weight: 400;\">Bacterial resistance to medication<\/span><\/li>\n<li><span style=\"font-weight: 400;\">Menopause<\/span><\/li>\n<li><span style=\"font-weight: 400;\">Uterine prolapse, often caused by childbirth\u00a0<\/span><\/li>\n<\/ul>\n<p><span style=\"font-weight: 400;\">When prescribed an antibiotic, it goes to work fairly quickly. \u201cAntibiotics work to treat UTIs by killing the bacteria or stopping the bacteria from growing,\u201d says <\/span><a href=\"https:\/\/www.linkedin.com\/in\/monte-swarup-10b17352\/\" target=\"_blank\" rel=\"noopener\"><span style=\"font-weight: 400;\">Monte Swarup, MD<\/span><\/a><span style=\"font-weight: 400;\">, a board-certified OB\/GYN in Chandler, Arizona, and founder of <\/span><a href=\"https:\/\/hpvhub.com\/doctor\/monte-swarup-m-d-facog\/\" target=\"_blank\" rel=\"noopener\"><span style=\"font-weight: 400;\">HPV HUB<\/span><\/a><span style=\"font-weight: 400;\">.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">More specifically, \u201cAntibiotics work by blocking bacterial growth processes, like DNA and protein synthesis. Therefore, the bacteria cannot continue to grow and will die,\u201d says <\/span><a href=\"https:\/\/umg.rwjms.rutgers.edu\/doctor_page.php?phys_id=2094\" target=\"_blank\" rel=\"noopener\"><span style=\"font-weight: 400;\">Juana Hutchinson-Colas, MD<\/span><\/a><span style=\"font-weight: 400;\">, Professor of Obstetrics, Gynecology, and Reproductive Sciences at Rutgers Robert Wood Johnson Medical School. Healthcare providers will consider the type of bacteria, likelihood of resistance, and other health factors when choosing an antibiotic to treat a UTI. The <\/span><a href=\"https:\/\/www.nature.com\/articles\/s41467-019-11571-5\" target=\"_blank\" rel=\"noopener\"><span style=\"font-weight: 400;\">most common UTI bacteria<\/span><\/a><span style=\"font-weight: 400;\"> is E.coli.<\/span><\/p>\n<h2 id=\"how-long-do-antibiotics-take-to-treat-a-uti\"><span style=\"font-weight: 400;\">How long do antibiotics take to treat a UTI?\u00a0<\/span><\/h2>\n<p><span style=\"font-weight: 400;\">Once you start treatment, most people feel better quickly. \u201cAntibiotics are very effective, and most of the time, symptoms will take one to two days to go away,\u201d says Dr. Swarup. However, you still need to take the entire course of antibiotics, which may be several pills over the course of five to ten days. Failure to complete the dose could mean a return of symptoms or for your infection to become resistant to antibiotics, making it harder to treat.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\"><a href=\"https:\/\/www.singlecare.com\/prescription\/phenazopyridine-hcl\">Phenazopyridine<\/a>, like the OTC drug Azo, may help relieve pain and discomfort of UTIs, but it is not an antibiotic and will not cure the infection.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">If your UTI has been fully treated, you\u2019ll notice a complete absence of symptoms, and symptoms should not come back. A urine culture can determine whether or not someone still has a UTI, but if they\u2019ve taken the entire course of antibiotics and are feeling totally better, it is likely gone. If you have a UTI and antibiotics haven\u2019t improved your symptoms, see your healthcare provider. Antibiotic prophylaxis can help prevent recurrent UTIs, but it is preferable to use alternative means first in order to avoid developing resistance.<\/span><\/p>\n<h2 id=\"choosing-the-best-antibiotic-for-female-utis\"><span style=\"font-weight: 400;\">Choosing the best antibiotic for female UTIs<\/span><\/h2>\n<p><span style=\"font-weight: 400;\">There are a few ways doctors choose the best antibiotic when treating a UTI. Before prescribing an antibiotic, a healthcare provider will likely order a urine culture. \u201cThe original treatment is an \u2018educated guess\u2019 on common types of bacteria that cause UTIs in women,\u201d says Dr. Swarup, who adds, \u201cA urine culture and antibiotic sensitivity test should be sent out when the UTI is treated.\u201d The <\/span><a href=\"https:\/\/my.clevelandclinic.org\/health\/diagnostics\/22126-urine-culture\" target=\"_blank\" rel=\"noopener\"><span style=\"font-weight: 400;\">urine culture<\/span><\/a><span style=\"font-weight: 400;\"> identifies the type of bacteria causing the infection and which antibiotics the bacteria are sensitive to (i.e., the antibiotics most likely to kill the bacteria). Healthcare providers then use this information to select the most effective antibiotic.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Dr. Ruiz stresses the importance of getting a culture before treating a UTI, \u201cBefore we start on antibiotics, we want a urine culture in case the bacteria is resistant to the antibiotic we start them on.\u201d Since cultures typically take three days to come back with enough information to determine what bacteria are present, doctors usually prescribe an initial antibiotic that is most likely to work best for that patient. If the culture shows that the bacterium is one that doesn\u2019t respond well to that antibiotic, your healthcare provider will switch the medication and you will complete that course.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Vaginosis and yeast infections are other conditions that can cause burning on urination similar to a UTI. But the treatment is different, so talking with a healthcare provider and getting the right testing is important.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Doctors use other information to determine which antibiotic to prescribe. \u201cA prescriber must take into consideration the medical history, especially medication allergies,\u201d says Dr. Hutchinson-Colas. <\/span><a href=\"https:\/\/www.singlecare.com\/blog\/recurrent-uti\/\"><span style=\"font-weight: 400;\">Recurrence of UTIs<\/span><\/a><span style=\"font-weight: 400;\"> is one factor that may influence their choice.
