But before you get too excited about them, serious concerns about these OTC inhalers are reason enough to leave them on the store shelf in most cases. We&rsquo;ll explain why and offer our thoughts on OTC inhalers for asthma.<\/span><\/p>\n<h2 id=\"otc-inhalers-for-asthma\"><span style=\"font-weight: 400;\">OTC inhalers for asthma<\/span><\/h2>\n<p><span style=\"font-weight: 400;\">Epinephrine is the active ingredient in OTC inhaled medications for <\/span><a href=\"https:\/\/www.singlecare.com\/blog\/early-signs-of-asthma\/\"><span style=\"font-weight: 400;\">asthma<\/span><\/a><span style=\"font-weight: 400;\">. Although it&rsquo;s available as a solution for use in <\/span><a href=\"https:\/\/www.singlecare.com\/blog\/what-is-a-nebulizer-machine\/\"><span style=\"font-weight: 400;\">nebulizer machines<\/span><\/a><span style=\"font-weight: 400;\"> under the brand names <\/span><a href=\"https:\/\/www.singlecare.com\/prescription\/asthmanefrin-refill\/what-is\"><span style=\"font-weight: 400;\">Asthmanefrin<\/span><\/a><span style=\"font-weight: 400;\"> and <\/span><a href=\"https:\/\/www.singlecare.com\/prescription\/s2-racepinephrine\/what-is\"><span style=\"font-weight: 400;\">S2<\/span><\/a><span style=\"font-weight: 400;\">, it&rsquo;s also available as an inhaler. Primatene Mist is the only handheld OTC inhaler approved in the United States for temporary relief of asthma symptoms.<\/span><\/p>\n<h3 id=\"primatene-mist\"><span style=\"font-weight: 400;\">Primatene Mist<\/span><\/h3>\n<p><span style=\"font-weight: 400;\">You may have heard of epinephrine for its use in <\/span><a href=\"https:\/\/www.singlecare.com\/blog\/how-to-get-an-epipen\/\"><span style=\"font-weight: 400;\">pen injectors<\/span><\/a><span style=\"font-weight: 400;\"> taken for severe allergic reactions, such as a bee sting allergy. It&rsquo;s available in an inhaled form as Primatene Mist, a brand-name product with no generic alternative. Primatene Mist is the only OTC inhaler for asthma. Like all other inhalers in the United States, Primatene Mist is produced using <\/span><a href=\"https:\/\/www.fda.gov\/drugs\/frequently-asked-questions-popular-topics\/transition-cfc-propelled-albuterol-inhalers-hfa-propelled-albuterol-inhalers-questions-and-answers\" target=\"_blank\" rel=\"noopener\"><span style=\"font-weight: 400;\">hydrofluoroalkane (HFA) as the propellant<\/span><\/a><span style=\"font-weight: 400;\"> (the spray that moves the medicine out of the inhaler) rather than chlorofluorocarbons (CFC), which are harmful to the ozone layer.&nbsp;<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Primatene Mist was actually taken off the market in 2011 due to CFC use and not reintroduced until 2018 in the HFA form. It is approved and <\/span><a href=\"https:\/\/www.accessdata.fda.gov\/drugsatfda_docs\/label\/2018\/205920Orig1s000lbl.pdf\" target=\"_blank\" rel=\"noopener\"><span style=\"font-weight: 400;\">labeled<\/span><\/a><span style=\"font-weight: 400;\"> for use in patients 12 and older who have been previously diagnosed with asthma. In other words, you should not try Primatene Mist for any symptoms unless a healthcare professional has diagnosed you with <\/span><a href=\"https:\/\/www.singlecare.com\/blog\/asthma-statistics\/\"><span style=\"font-weight: 400;\">asthma<\/span><\/a><span style=\"font-weight: 400;\">.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">The manufacturer of Primatene Mist, Amphastar Pharmaceuticals, points out a couple of nuances of the device within its <\/span><a href=\"https:\/\/www.primatene.com\/spray-indicator.aspx\" target=\"_blank\" rel=\"noopener\"><span style=\"font-weight: 400;\">detailed instructions<\/span><\/a><span style=\"font-weight: 400;\">. It recommends that the device be shaken and then sprayed into the air once prior to using it for an actual dose. The inhaler comes with a spray indicator on the device that counts down in groups of 20 doses, starting with 160 sprays when new to zero when it is empty. This feature is akin to a dose counter on many asthma counter inhalers available with a prescription.<\/span><\/p>\n<h4 id=\"primatene-mist-dosing\"><strong>Primatene Mist dosing<\/strong><\/h4>\n<p><span style=\"font-weight: 400;\">Dosage instructions for Primatene Mist are as follows:<\/span><\/p>\n<ul>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Take one puff from the inhaler and wait one minute. Healthcare providers occasionally administer these products <\/span><a href=\"https:\/\/www.singlecare.com\/blog\/off-label-prescription-drugs\/\"><span style=\"font-weight: 400;\">off-label<\/span><\/a><span style=\"font-weight: 400;\"> for croup at even younger ages.&nbsp;<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Asthmanefrin and S2 have similar side effects, drug interactions, and contraindications as Primatene Mist.&nbsp;<\/span><\/p>\n<p><span style=\"font-weight: 400;\">The average retail price of <\/span><a href=\"https:\/\/www.singlecare.com\/prescription\/asthmanefrin-refill\"><span style=\"font-weight: 400;\">Asthmanefrin<\/span><\/a><span style=\"font-weight: 400;\"> is about $42 for 30 vials, and the average retail price of <\/span><a href=\"https:\/\/www.singlecare.com\/prescription\/s2-racepinephrine\"><span style=\"font-weight: 400;\">S2<\/span><\/a><span style=\"font-weight: 400;\"> is about $76 for 30 vials.<\/span><\/p>\n<h2 id=\"what-you-should-know-before-buying-an-otc-asthma-inhaler\"><span style=\"font-weight: 400;\">What you should know before buying an OTC asthma inhaler<\/span><\/h2>\n<p><span style=\"font-weight: 400;\">All of the products mentioned above rely on epinephrine as the active ingredient. Epinephrine stimulates adrenergic receptors in the body in a nonselective manner, meaning it activates alpha, beta-1, and beta-2 receptors. The desired effect is to stimulate the beta-2 receptors, relaxing the spasms in the smooth muscles surrounding the bronchial tubes and opening the airways. However, when used as a bronchodilator, epinephrine also stimulates alpha and beta-1 receptors, leading to potential adverse effects like tachycardia and elevated blood pressure.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">The side effects are just part of the story as to why many asthma experts feel that OTC epinephrine inhalers are unsafe. Other concerns include overuse of the products when the patient should instead be using a prescription asthma controller medication under the care of a healthcare provider. Another fear is that it could lead to a misdiagnosis of heart disease, which can manifest as shortness of breath and mimic asthma. If someone uses an OTC epinephrine inhaler in this scenario, the untreated heart problem could worsen as a result.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Cumulatively, these concerns led the American Thoracic Society and the National Asthma Education and Prevention Program to <\/span><a href=\"https:\/\/www.atsjournals.org\/doi\/10.1513\/AnnalsATS.201902-164HP\" target=\"_blank\" rel=\"noopener\"><span style=\"font-weight: 400;\">recommend against OTC epinephrine inhalers<\/span><\/a><span style=\"font-weight: 400;\">. Despite the controversy, the FDA explained in a <\/span><a href=\"https:\/\/www.fda.gov\/news-events\/press-announcements\/fda-statement-approval-otc-primatene-mist-treat-mild-asthma\" target=\"_blank\" rel=\"noopener\"><span style=\"font-weight: 400;\">2018 statement<\/span><\/a><span style=\"font-weight: 400;\"> that patient access to OTC medications was a compelling reason for approving Primatene Mist for asthma relief. Taking the product only under the supervision of a healthcare provider and as part of an asthma management plan were cautionary notes in the FDA&rsquo;s statement.<\/span><\/p>\n<h2 id=\"alternatives-to-otc-inhalers-for-asthma\"><span style=\"font-weight: 400;\">Alternatives to OTC inhalers for asthma<\/span><\/h2>\n<p><span style=\"font-weight: 400;\">Fortunately, albuterol is an effective and safe alternative to OTC epinephrine inhalers for asthma. It is available in inhaler and nebulizer solution forms as a bronchodilator for people with <\/span><a href=\"https:\/\/www.singlecare.com\/blog\/copd-vs-asthma\/\"><span style=\"font-weight: 400;\">asthma or chronic obstructive pulmonary disease (COPD)<\/span><\/a><span style=\"font-weight: 400;\">. <\/span><a href=\"https:\/\/www.singlecare.com\/prescription\/albuterol-sulfate-hfa\/what-is\"><span style=\"font-weight: 400;\">Albuterol Sulfate Hfa<\/span><\/a><span style=\"font-weight: 400;\"> inhalers come with a counter in them that displays how many doses are left.
