It is also used <\/span><a href=\"https:\/\/www.singlecare.com\/blog\/off-label-prescription-drugs\/\"><span style=\"font-weight: 400;\">off-label<\/span><\/a><span style=\"font-weight: 400;\"> for other forms of <\/span><a href=\"https:\/\/www.singlecare.com\/conditions\/dementia-treatments-and-medications\"><span style=\"font-weight: 400;\">dementia<\/span><\/a><span style=\"font-weight: 400;\">. Donepezil is available as a tablet, an orally disintegrating tablet, and a weekly patch. It is a <\/span><a href=\"https:\/\/www.singlecare.com\/drug-classes\/cholinesterase-inhibitors\"><span style=\"font-weight: 400;\">central acetylcholinesterase inhibitor<\/span><\/a><span style=\"font-weight: 400;\">. Acetylcholinesterase is an enzyme responsible for the breakdown of acetylcholine. A deficiency of acetylcholine characterizes forms of dementia that donepezil manages, and donepezil appears to increase available concentrations of this molecule. However, donepezil use comes with a risk of <\/span><a href=\"https:\/\/www.singlecare.com\/blog\/donepezil-side-effects\/\"><span style=\"font-weight: 400;\">side effects<\/span><\/a><span style=\"font-weight: 400;\"> including cardiac effects, loss of appetite resulting in weight loss, and the development of muscle cramps. The patch may cause a local reaction at the application site, but this shouldn\u2019t be mistaken for an allergic reaction to the medication. Donepezil interacts with anticholinergic agents, antipsychotic agents, medications that slow the heart rate, and other medications that impact cardiac conduction. It\u2019s important to be aware of donepezil\u2019s drug interactions as other drugs, supplements, and foods can affect its effectiveness or worsen side effects.<\/span><\/p>\n<h2 id=\"donepezil-drug-interactions\"><span style=\"font-weight: 400;\">Donepezil drug interactions<\/span><\/h2>\n<p><span style=\"font-weight: 400;\">Donepezil interacts with several classes of medications, including many that impact the central nervous system (CNS) and the heart. In some instances, combinations may increase side effects like the development of stomach ulcers. Below is a review of the more common and serious drug-drug interactions.<\/span><\/p>\n<h3 id=\"anticholinergic-and-cholinergic-drugs\"><span style=\"font-weight: 400;\">Anticholinergic and cholinergic drugs<\/span><\/h3>\n<p><span style=\"font-weight: 400;\">Donepezil and acetylcholinesterase inhibitors may decrease the effect of anticholinergic agents, and likewise, anticholinergic agents may decrease the effect of acetylcholinesterase inhibitors. Anticholinergic drugs and acetylcholinesterase inhibitors combined work against each other\u2014anticholinergics block the effects of acetylcholine, while acetylcholinesterase inhibitors prevent its breakdown. The overall result is a diminished therapeutic effect, which may worsen symptoms of Alzheimer\u2019s Disease. <\/span><a href=\"https:\/\/www.accessdata.fda.gov\/drugsatfda_docs\/label\/2013\/020690s037,021720s010,022568s007lbl.pdf\"><span style=\"font-weight: 400;\">Product labels<\/span><\/a><span style=\"font-weight: 400;\"> acknowledge the opposing mechanisms and recommend avoidance of the combination whenever possible.<\/span><\/p>\n<p><b>Examples of <\/b><a href=\"https:\/\/www.singlecare.com\/drug-classes\/antiemetics\"><b>anticholinergic medications<\/b><\/a><b> include:<\/b><\/p>\n<ul>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Elavil<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Atropine<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Cetirizine<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Diphenhydramine<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Loratadine<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Pamelor<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Quinidine<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Scopolamine<\/span><\/li>\n<\/ul>\n<p><span style=\"font-weight: 400;\">Acetylcholinesterase inhibitors, like donepezil, may impact the metabolism of cholinergic medications, which are also metabolized by cholinesterases. This will increase those <\/span><a href=\"https:\/\/www.ncbi.nlm.nih.gov\/books\/NBK538163\/\"><span style=\"font-weight: 400;\">cholinergic medication<\/span><\/a><span style=\"font-weight: 400;\"> therapeutic levels, possibly resulting in <\/span><a href=\"https:\/\/www.ncbi.nlm.nih.gov\/books\/NBK539783\/\" target=\"_blank\" rel=\"noopener\"><span style=\"font-weight: 400;\">enhanced cholinergic effects<\/span><\/a><span style=\"font-weight: 400;\">, including increased salivary and bronchial secretions and urinary bladder stimulation.\u00a0<\/span><\/p>\n<p><b>Examples of <\/b><a href=\"https:\/\/www.singlecare.com\/drug-classes\/cholinergic-agonists\"><b>cholinergic medications<\/b><\/a><b> include:<\/b><\/p>\n<ul>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Methacholine<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Bethanechol<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Pilocarpine<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Rivastigmine<\/span><\/li>\n<\/ul>\n<h3 id=\"antipsychotic-medications\"><span style=\"font-weight: 400;\">Antipsychotic medications<\/span><\/h3>\n<p><span style=\"font-weight: 400;\">Donepezil may interact with certain <\/span><a href=\"https:\/\/www.singlecare.com\/drug-classes\/antipsychotics\"><span style=\"font-weight: 400;\">antipsychotic agents<\/span><\/a><span style=\"font-weight: 400;\">, like aripiprazole, clozapine, haloperidol, lurasidone, olanzapine, quetiapine, and ziprasidone by enhancing the neurotoxic effects of these medications. The result is the development of <\/span><a href=\"https:\/\/www.ncbi.nlm.nih.gov\/books\/NBK534115\/\" target=\"_blank\" rel=\"noopener\"><span style=\"font-weight: 400;\">extrapyramidal symptoms<\/span><\/a><span style=\"font-weight: 400;\">, including muscle rigidity, shuffling gait, and uncontrolled facial movements. The mechanism behind this interaction is due to the pharmacology of each medication\u2014increased acetylcholine in the CNS coupled with dopamine receptor blockade. The combination should be avoided as some <\/span><a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/12109969\/\" target=\"_blank\" rel=\"noopener\"><span style=\"font-weight: 400;\">case reports<\/span><\/a><span style=\"font-weight: 400;\"> have outlined an onset of severe extrapyramidal symptoms within days of initiating specific antipsychotics with donepezil.\u00a0<\/span><\/p>\n<h3 id=\"neuromuscular-blockers\"><span style=\"font-weight: 400;\">Neuromuscular blockers<\/span><\/h3>\n<p><span style=\"font-weight: 400;\">Donepezil may enhance the neuromuscular-blocking effect of succinylcholine. Individuals requiring neuromuscular blockade and receiving donepezil should be monitored for <\/span><a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/12552219\/\" target=\"_blank\" rel=\"noopener\"><span style=\"font-weight: 400;\">prolonged effects of the blockade<\/span><\/a><span style=\"font-weight: 400;\">. The mechanism of this interaction is likely decreased metabolism of succinylcholine due to the inhibition of acetylcholinesterase brought on by donepezil. Fortunately, neuromuscular blockers are only employed in controlled medical settings where patients are closely monitored.\u00a0<\/span><\/p>\n<h3 id=\"heart-rate-lowering-medications\"><span style=\"font-weight: 400;\">Heart rate-lowering medications<\/span><\/h3>\n<p><span style=\"font-weight: 400;\">Donepezil may slow down the heart rate. Thus, when combined with other medications, which also intentionally or unintentionally <\/span><a href=\"https:\/\/www.singlecare.com\/blog\/normal-heart-rate\/\"><span style=\"font-weight: 400;\">slow the heart rate<\/span><\/a><span style=\"font-weight: 400;\">, additive bradycardia may occur. Some classes of medications, such as <\/span><a href=\"https:\/\/www.singlecare.com\/drug-classes\/beta-blockers\"><span style=\"font-weight: 400;\">beta blockers<\/span><\/a><span style=\"font-weight: 400;\"> and certain <\/span><a href=\"https:\/\/www.singlecare.com\/drug-classes\/calcium-channel-blockers\"><span style=\"font-weight: 400;\">calcium channel blockers<\/span><\/a><span style=\"font-weight: 400;\">, are taken for their heart rate-lowering effects. Other medications like <\/span><a href=\"https:\/\/onlinelibrary.wiley.com\/doi\/full\/10.1111\/j.1528-1167.2011.03212.x\" target=\"_blank\" rel=\"noopener\"><span style=\"font-weight: 400;\">lacosamide<\/span><\/a><span style=\"font-weight: 400;\">, which is an antiepileptic, have blocking effects on the heart&#8217;s atrioventricular (AV) node. This results in partial or complete disruption of impulse transmission from the atria to the ventricles, thus <\/span><a href=\"https:\/\/www.ncbi.nlm.nih.gov\/books\/NBK545199\/\" target=\"_blank\" rel=\"noopener\"><span style=\"font-weight: 400;\">slowing the heart rate<\/span><\/a><span style=\"font-weight: 400;\">. It is important to be aware of <\/span><a href=\"https:\/\/www.mayoclinic.org\/diseases-conditions\/bradycardia\/symptoms-causes\/syc-20355474\" target=\"_blank\" rel=\"noopener\"><span style=\"font-weight: 400;\">symptoms of slowed heart rate<\/span><\/a><span style=\"font-weight: 400;\">, which manifest due to the lack of oxygen perfusing major organs and systems, including chest pain, confusion, dizziness, fatigue, and syncope. The onset of any of these symptoms warrants emergency medical services.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Medications implicated to cause <\/span><a href=\"https:\/\/www.singlecare.com\/conditions\/low-heart-rate-causes\"><span style=\"font-weight: 400;\">bradycardia<\/span><\/a><span style=\"font-weight: 400;\"> include:<\/span><\/p>\n<ul>\n<li><span style=\"font-weight: 400;\">Adenosine<\/span><\/li>\n<li><span style=\"font-weight: 400;\">Amiodarone<\/span><\/li>\n<li><span style=\"font-weight: 400;\">Atenolol<\/span><\/li>\n<li><span style=\"font-weight: 400;\">Bisoprolol<\/span><\/li>\n<li><span style=\"font-weight: 400;\">Carvedilol<\/span><\/li>\n<li><span style=\"font-weight: 400;\">Clonidine<\/span><\/li>\n<li><span style=\"font-weight: 400;\">Digoxin<\/span><\/li>\n<li><span style=\"font-weight: 400;\">Diltiazem<\/span><\/li>\n<li><span style=\"font-weight: 400;\">Labetalol<\/span><\/li>\n<li><span style=\"font-weight: 400;\">Metoprolol<\/span><\/li>\n<li><span style=\"font-weight: 400;\">Nadolol<\/span><\/li>\n<li><span style=\"font-weight: 400;\">Propranolol\u00a0<\/span><\/li>\n<li><span style=\"font-weight: 400;\">Rivastigmine<\/span><\/li>\n<li><span style=\"font-weight: 400;\">Sotalol<\/span><\/li>\n<li><span style=\"font-weight: 400;\">Verapamil<\/span><\/li>\n<\/ul>\n<h3 id=\"qtc-prolonging-medications\"><span style=\"font-weight: 400;\">QTc-prolonging medications<\/span><\/h3>\n<p><span style=\"font-weight: 400;\">Donepezil may enhance the QTc-prolonging effect of other medications with just a single dose, which can result in life-threatening heart rhythm patterns like <\/span><a href=\"https:\/\/my.clevelandclinic.org\/health\/diseases\/21915-torsades-de-pointes\" target=\"_blank\" rel=\"noopener\"><span style=\"font-weight: 400;\">torsades de pointes<\/span><\/a><span style=\"font-weight: 400;\">. The QT interval is the part of an electrocardiogram (EKG) report that represents the time it takes the heart to contract and recover. <\/span><a href=\"https:\/\/my.clevelandclinic.org\/health\/diseases\/17183-long-q-t-syndrome-lqts\" target=\"_blank\" rel=\"noopener\"><span style=\"font-weight: 400;\">Long QT syndromes <\/span><\/a><span style=\"font-weight: 400;\">can be inherited and are the result of an issue with the heart\u2019s electrical conduction such that it takes longer than normal to recharge between heartbeats, which is due to an issue with ion channels or the flow of ions (sodium, calcium, potassium, and chloride) in and out of heart muscle cells to produce electrical activity. Medications can cause acquired long QT syndrome. Some approved indications include <\/span><a href=\"https:\/\/www.singlecare.com\/blog\/osteoarthritis-vs-rheumatoid-arthritis\/\"><span style=\"font-weight: 400;\">osteoarthritis, rheumatoid arthritis<\/span><\/a><span style=\"font-weight: 400;\">, and <\/span><a href=\"https:\/\/www.singlecare.com\/blog\/juvenile-arthritis\/\"><span style=\"font-weight: 400;\">juvenile rheumatoid arthritis<\/span><\/a><span style=\"font-weight: 400;\">. NSAIDs are often consumed outside of these indications for their ability to improve symptoms such as pain, stiffness, swelling, and tenderness of the joints. Many NSAIDs are available over-the-counter (OTC) as non-prescription medications, so they are readily accessible to individuals. <\/span><a href=\"https:\/\/www.singlecare.com\/blog\/prescription-savings-card-otc-meds\/\"><span style=\"font-weight: 400;\">OTC medications<\/span><\/a><span style=\"font-weight: 400;\"> can go under-recognized as contributing to significant drug interactions and adverse effects. One <\/span><a href=\"https:\/\/agsjournals.onlinelibrary.wiley.com\/doi\/full\/10.1111\/jgs.18647\" target=\"_blank\" rel=\"noopener\"><span style=\"font-weight: 400;\">study<\/span><\/a><span style=\"font-weight: 400;\"> found an increased risk of peptic ulcers with the coadministration of NSAIDs and cholinesterase inhibitor donepezil, beyond the risk with NSAID use alone.\u00a0\u00a0<\/span><\/p>\n<p><a href=\"https:\/\/www.singlecare.com\/drug-classes\/nsaids\"><b>NSAIDs<\/b><\/a><b> include the following medications:<\/b><\/p>\n<ul>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Celecoxib<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Diclofenac<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Etodolac<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Ibuprofen<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Indomethacin<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Ketorolac<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Meloxicam<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Naproxen<\/span><\/li>\n<\/ul>\n<h3 id=\"anticonvulsants\"><span style=\"font-weight: 400;\">Anticonvulsants<\/span><\/h3>\n<p><span style=\"font-weight: 400;\">Some <\/span><a href=\"https:\/\/www.singlecare.com\/drug-classes\/anticonvulsants\"><span style=\"font-weight: 400;\">anticonvulsants<\/span><\/a><span style=\"font-weight: 400;\"> may interact with donepezil. This becomes of greatest concern in older adult patients with an Alzheimer\u2019s diagnosis and a concurrent epilepsy diagnosis. Anticonvulsants<\/span><a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/17433928\/\" target=\"_blank\" rel=\"noopener\"><span style=\"font-weight: 400;\"> with enzyme-inducing properties<\/span><\/a><span style=\"font-weight: 400;\"> like carbamazepine, phenytoin, and <a href=\"https:\/\/www.singlecare.com\/prescription\/phenobarbital\/dosage\">phenobarbital<\/a> may decrease blood levels of donepezil. Individuals receiving one of these anticonvulsants may be better managed with alternative agents than donepezil and vice versa. If the combination cannot be avoided, individuals and their caregivers should be educated on the risk of increased symptoms of Alzheimer\u2019s disease while receiving the combination.\u00a0<\/span><\/p>\n<h3 id=\"corticosteroids\"><span style=\"font-weight: 400;\">Corticosteroids<\/span><\/h3>\n<p><span style=\"font-weight: 400;\">Concomitantly taking corticosteroids with donepezil may enhance their adverse effects, including increased risk of muscle weakness. Patients taking these two types of medications together should be monitored for an exacerbation of muscle weakness, with a dose reduction or tapering donepezil considered in the setting of a need for high-dose steroids. Huperzia moss may be ingested as a tea. It is reported to work by preventing the breakdown of acetylcholine, which is a similar mechanism to donepezil. While significant data on this interaction is lacking, it is best to avoid the combination given the similar mechanisms (presumed or known).\u00a0<\/span><\/p>\n<h3 id=\"donepezil-and-st-john-s-wort\"><span style=\"font-weight: 400;\">Donepezil and St John\u2019s wort<\/span><\/h3>\n<p><a href=\"https:\/\/www.singlecare.com\/blog\/st-johns-wort\/\"><span style=\"font-weight: 400;\">St. John\u2019s wort<\/span><\/a><span style=\"font-weight: 400;\"> is a supplement used for effects on depression but can cause interactions with many medications.
