However, prior authorization and documentation of medical necessity from a healthcare provider are required.\u201d\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">As for Medicaid coverage, guidelines vary by state.\u201cMost states offer some level of coverage for TRT under their prescription drug benefit,\u201d Dr. Shusterman says.
