It is a Schedule II controlled substance under the Controlled Substances Act and is only available with a valid prescription from a licensed healthcare provider. Adderall contains mixed amphetamine salts, which have a high potential for misuse and can lead to dependence. It is illegal to obtain Adderall without a valid prescription.

Products labeled as “natural Adderall” or OTC Adderall alternatives are dietary supplements, not FDA-approved ADHD medications. Although some supplements are marketed for focus or cognitive support, they are not approved by the Food and Drug Administration (FDA) to treat ADHD, and are not regulated the way that FDA-approved drugs are.

Why do people search for OTC Adderall?

People search for OTC Adderall for different reasons. Some may be looking for a way to improve focus or stay awake without a prescription, while others may be trying to overcome barriers to ADHD diagnosis and treatment.

According to Sanam Hafeez, PsyD, neuropsychologist and director of Comprehend the Mind, common barriers include:

Difficulty finding a specialist or long wait times for an evaluation

The cost of an ADHD assessment, medication management appointments, or prescription medication

Difficulty filling Adderall prescriptions because of ongoing shortages

Stigma surrounding ADHD diagnosis and treatment

These barriers can be frustrating, especially if ADHD symptoms affect your day-to-day life. Although there is no direct OTC substitute for Adderall, there are other options, including pursuing an ADHD evaluation and Adderall prescription or discussing alternative treatments with a healthcare provider.

OTC supplements marketed as Adderall alternatives

Supplements marketed as “natural Adderall” or OTC Adderall alternatives are widely available in stores and online. However, they are not FDA-approved to treat ADHD, and evidence for their safety and effectiveness varies by ingredient and is often limited or mixed.

“There are some supplements that might help a little with focus or cognitive performance, but the data we have on most of these is very limited, inconsistent, or just simply mixed when it comes to ADHD,” Dr. Hafeez says. “Additionally, people often have the idea that because something is natural, it must be safe, but this couldn’t be further from the truth. All supplements have the potential to cause side effects and interact with other medications.”

Dr. Hafeez explains that although supplements are unlikely to be as effective as ADHD medication, some may have a supportive role alongside evidence-based treatment, behavioral strategies, adequate sleep, exercise, and consistent routines. Commonly marketed options include:

Caffeine: Caffeine is a mild central nervous system stimulant that may support alertness and attention, but research on its role in ADHD is limited, and human studies have not shown clear benefits for ADHD symptoms. Telehealth has also expanded access to ADHD evaluation and treatment. People ages 17 and older must have at least five symptoms, while younger people must have at least six.

Because ADHD cannot be confirmed with a blood test or brain scan, Dr. Sawhne says the evaluation process is important.
