On April 24, the U.S. Food and Drug Administration approved Pivya (pivmecillinam), a prescription medication for the treatment of uncomplicated UTIs in female adults. An uncomplicated UTI is one that affects a woman&#8217;s bladder without any structural abnormalities in the urinary tract. It provides a new antibiotic option since resistance to first-line treatments is growing.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">&ldquo;Uncomplicated UTIs are a very common condition impacting women and one of the most frequent reasons for antibiotic use,&rdquo; said Peter Kim, MD, director of the Division of Anti-Infectives for the FDA&rsquo;s Center for Drug Evaluation and Research in a <\/span><a href=\"https:\/\/www.fda.gov\/news-events\/press-announcements\/fda-approves-new-treatment-uncomplicated-urinary-tract-infections\" target=\"_blank\" rel=\"noopener\"><span style=\"font-weight: 400;\">statement<\/span><\/a><span style=\"font-weight: 400;\">. &ldquo;The FDA is committed to fostering new antibiotic availability when they prove to be safe and effective, and Pivya will provide an additional treatment option for uncomplicated UTIs.&rdquo;&nbsp;<\/span><\/p>\n<h2 id=\"what-is-pivya\"><span style=\"font-weight: 400;\">What is Pivya?<\/span><\/h2>\n<p><span style=\"font-weight: 400;\">Pivya is an oral antibiotic that has been shown to be <\/span><a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC6628187\/\" target=\"_blank\" rel=\"noopener\"><span style=\"font-weight: 400;\">very effective<\/span><\/a><span style=\"font-weight: 400;\"> in combating common causes of uncomplicated UTIs, such as<\/span><i><span style=\"font-weight: 400;\"> E. coli, Proteus mirabili<\/span><\/i><span style=\"font-weight: 400;\">s, and <\/span><i><span style=\"font-weight: 400;\">Staphylococcus saprophyticus<\/span><\/i><span style=\"font-weight: 400;\"> bacteria.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Pivmecillinam is not actually a new antibiotic. It&rsquo;s been <\/span><a href=\"https:\/\/www.cidrap.umn.edu\/antimicrobial-stewardship\/fda-approves-new-treatment-uncomplicated-urinary-tract-infections\" target=\"_blank\" rel=\"noopener\"><span style=\"font-weight: 400;\">used to treat bacterial infections in Europe for more than 40 years<\/span><\/a><span style=\"font-weight: 400;\">. With the recent approval, the U.K.-based biotechnology company Utility Therapeutics Ltd. is making this proven treatment available in the United States.&nbsp;<\/span><\/p>\n<h2 id=\"how-does-pivya-work\"><span style=\"font-weight: 400;\">How does Pivya work?<\/span><\/h2>\n<p><span style=\"font-weight: 400;\">The active antimicrobial agent of <\/span><a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC6628187\/\" target=\"_blank\" rel=\"noopener\"><span style=\"font-weight: 400;\">pivmecillinam<\/span><\/a><span style=\"font-weight: 400;\"> is mecillinam, which is categorized as an aminopenicillin, a specific class of beta-lactam antibiotics with a distinct mechanism of action (the way it works). Specifically, it <\/span><a href=\"https:\/\/www.biospace.com\/article\/releases\/utility-therapeutics-ltd-announces-financing-led-by-the-amr-action-fund-and-fda-acceptance-of-pivya-new-drug-application-with-priority-review-\/\" target=\"_blank\" rel=\"noopener\"><span style=\"font-weight: 400;\">targets a protein called the penicillin binding protein-2 (PBP-2)<\/span><\/a><span style=\"font-weight: 400;\"> in the cell wall of gram-negative bacteria.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Urinary tract infections occur when bacteria enter the urinary system, causing infection and inflammation. They&rsquo;re significantly more common in women than in men. <\/span><a href=\"https:\/\/www.singlecare.com\/conditions\/uti-symptoms\"><span style=\"font-weight: 400;\">Common symptoms<\/span><\/a><span style=\"font-weight: 400;\"> include pain and burning during urination, a frequent urge to urinate, and cloudy urine.&nbsp;<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Many people turn to home remedies, such as cranberry juice, water, and probiotics, to attempt to treat these painful infections or prevent them before they occur. However, most UTIs require <\/span><a href=\"https:\/\/www.singlecare.com\/blog\/how-long-does-a-uti-last\/\"><span style=\"font-weight: 400;\">treatment with antibiotics<\/span><\/a><span style=\"font-weight: 400;\">.&nbsp;<\/span><\/p>\n<p><span style=\"font-weight: 400;\">The <\/span><a href=\"https:\/\/www.mayoclinic.org\/diseases-conditions\/urinary-tract-infection\/diagnosis-treatment\/drc-20353453\" target=\"_blank\" rel=\"noopener\"><span style=\"font-weight: 400;\">most commonly prescribed antibiotics<\/span><\/a><span style=\"font-weight: 400;\"> for simple UTIs include:<\/span><\/p>\n<ul>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Bactrim, <\/span><a href=\"https:\/\/www.singlecare.com\/prescription\/bactrim-ds\"><span style=\"font-weight: 400;\">Bactrim DS<\/span><\/a><span style=\"font-weight: 400;\"> (trimethoprim and sulfamethoxazole)&nbsp;<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Monurol (<\/span><a href=\"https:\/\/www.singlecare.com\/prescription\/fosfomycin-tromethamine\"><span style=\"font-weight: 400;\">fosfomycin<\/span><\/a><span style=\"font-weight: 400;\">)&nbsp;<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Macrobid (<\/span><a href=\"https:\/\/www.singlecare.com\/prescription\/nitrofurantoin-monohyd-macro\"><span style=\"font-weight: 400;\">nitrofurantoin<\/span><\/a><span style=\"font-weight: 400;\">)<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Keflex (<\/span><a href=\"https:\/\/www.singlecare.com\/prescription\/cephalexin\"><span style=\"font-weight: 400;\">cephalexin<\/span><\/a><span style=\"font-weight: 400;\">)<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><a href=\"https:\/\/www.singlecare.com\/prescription\/ceftriaxone-sodium\"><span style=\"font-weight: 400;\">Ceftriaxone<\/span><\/a><\/li>\n<\/ul>\n<p><span style=\"font-weight: 400;\">Sometimes, people who get frequent UTIs are prescribed a daily, low-dose antibiotic to ward off future infections. However, concern over antibiotic resistance has continued to grow in recent years. The FDA calls it a &ldquo;<\/span><a href=\"https:\/\/www.fda.gov\/emergency-preparedness-and-response\/mcm-issues\/antimicrobial-resistance-information-fda\" target=\"_blank\" rel=\"noopener\"><span style=\"font-weight: 400;\">serious, complex, and costly public health problem<\/span><\/a><span style=\"font-weight: 400;\">.&rdquo; <\/span><a href=\"https:\/\/www.cedars-sinai.org\/health-library\/diseases-and-conditions\/a\/antibiotic-resistance.html\" target=\"_blank\" rel=\"noopener\"><span style=\"font-weight: 400;\">Contributing factors include<\/span><\/a><span style=\"font-weight: 400;\"> the overprescription of antibiotics or the inappropriate use of antibiotics to treat viral infections (antibiotics do not work against viruses).<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Pivya will support decreased antibiotic resistance while still curing painful UTIs. According to Utility Therapeutics Ltd., <\/span><a href=\"https:\/\/utilitytherapeutics.com\/our-focus\/\" target=\"_blank\" rel=\"noopener\"><span style=\"font-weight: 400;\">oral pivmecillinam has long been a key treatment for UTIs<\/span><\/a><span style=\"font-weight: 400;\"> in Denmark since the 1970s, and despite its widespread use, the resistance rates have remained low.&nbsp;<\/span><\/p>\n<h2 id=\"what-are-the-possible-side-effects-of-pivya\"><span style=\"font-weight: 400;\">What are the possible side effects of Pivya?<\/span><\/h2>\n<p><span style=\"font-weight: 400;\">The most common side effects associated with taking Pivya are nausea and diarrhea.&nbsp;<\/span><\/p>\n<p><span style=\"font-weight: 400;\">However, like any medication, other side effects and complications are possible. Pivya comes with warnings about severe allergic and skin reactions, carnitine depletion, and <\/span><a href=\"https:\/\/www.singlecare.com\/blog\/antibiotics-that-cause-c-diff\/\"><i><span style=\"font-weight: 400;\">C. diff<\/span><\/i><\/a><span style=\"font-weight: 400;\"> colitis. The medication may also interfere with a newborn screening test for a rare metabolic disorder called isovaleric acidemia.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Some people should not take Pivya, including people who are sensitive to <\/span><a href=\"https:\/\/www.ncbi.nlm.nih.gov\/books\/NBK545311\/\" target=\"_blank\" rel=\"noopener\"><span style=\"font-weight: 400;\">beta-lactam antibiotics<\/span><\/a><span style=\"font-weight: 400;\"> (like penicillin), have a carnitine deficiency, or have <\/span><a href=\"https:\/\/my.clevelandclinic.org\/health\/diseases\/17760-porphyria\" target=\"_blank\" rel=\"noopener\"><span style=\"font-weight: 400;\">porphyria<\/span><\/a><span style=\"font-weight: 400;\">, a rare disorder that affects the skin and nervous system.&nbsp;<\/span><\/p>\n<h2 id=\"when-will-pivya-be-available\"><span style=\"font-weight: 400;\">When will Pivya be available?<\/span><\/h2>\n<p><span style=\"font-weight: 400;\">Pivya will be available in tablet form. However, Utility Therapeutics has not announced when the medication will hit U.S. shelves.
