Since Prozac (fluoxetine) was approved by the FDA in 1987, <\/span><a href=\"https:\/\/www.singlecare.com\/drug-classes\/ssris\"><span style=\"font-weight: 400;\">selective serotonin reuptake inhibitors (SSRIs)<\/span><\/a><span style=\"font-weight: 400;\"> have been the <\/span><span style=\"font-weight: 400;\">most commonly prescribed<\/span><span style=\"font-weight: 400;\"> antidepressants on the market. <\/span><\/p>\n<p><span style=\"font-weight: 400;\">In addition to depression, healthcare providers also prescribe SSRIs for other mental health conditions. Examples include <\/span><a href=\"https:\/\/www.singlecare.com\/conditions\/anxiety-treatment-and-medications\"><span style=\"font-weight: 400;\">generalized anxiety disorder (GAD)<\/span><\/a><span style=\"font-weight: 400;\">, <\/span><a href=\"https:\/\/www.singlecare.com\/blog\/news\/ocd-statistics\/\"><span style=\"font-weight: 400;\">obsessive-compulsive disorder (OCD)<\/span><\/a><span style=\"font-weight: 400;\">, panic disorder (PD), and <\/span><a href=\"https:\/\/www.singlecare.com\/blog\/what-is-ptsd\/\"><span style=\"font-weight: 400;\">post-traumatic stress disorder (PTSD)<\/span><\/a><span style=\"font-weight: 400;\">. In fact, <\/span><span style=\"font-weight: 400;\">some SSRIs<\/span><span style=\"font-weight: 400;\"> are approved to treat premenstrual syndrome and <\/span><a href=\"https:\/\/www.singlecare.com\/conditions\/pmdd-symptoms\"><span style=\"font-weight: 400;\">premenstrual dysphoric disorder (PMDD)<\/span><\/a><span style=\"font-weight: 400;\">.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">SSRIs are popular treatment options for a wide range of conditions due to their <\/span><span style=\"font-weight: 400;\">safety, tolerability, and efficacy profile<\/span><span style=\"font-weight: 400;\">. While many of these conditions are approved by the FDA, there is evidence that SSRIs can be useful for various conditions outside of their approved indications. This is called <\/span><a href=\"https:\/\/www.singlecare.com\/blog\/off-label-prescription-drugs\/\"><span style=\"font-weight: 400;\">off-label prescribing<\/span><\/a><span style=\"font-weight: 400;\">, and SSRIs rank among the top use cases for <\/span><a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC9878961\/\" target=\"_blank\" rel=\"noopener\"><span style=\"font-weight: 400;\">off-label use<\/span><\/a><span style=\"font-weight: 400;\">. Pharmacists, in particular, should be familiar with off-label uses of SSRIs, as you may come across off-label prescriptions in everyday practice. Continue reading to learn more about off-label uses for SSRIs and how pharmacists can address off-label uses with their patients.<\/span><\/p>\n<h2 id=\"how-ssris-work\"><span style=\"font-weight: 400;\">How SSRIs work<\/span><\/h2>\n<p><span style=\"font-weight: 400;\">Serotonin is a neurotransmitter, or chemical messenger, that transmits messages throughout the brain. It circulates between neurons and then absorbs into the bloodstream. SSRIs work by blocking the reabsorption of serotonin into your blood, leaving a higher amount of serotonin in your brain. Higher serotonin levels are thought to have many effects, including improving mood, emotions, and sleep.<\/span><\/p>\n<h2 id=\"8-common-off-label-uses-for-ssris\"><span style=\"font-weight: 400;\">8 common off-label uses for SSRIs<\/span><\/h2>\n<p><span style=\"font-weight: 400;\">Due to its favorable safety profile, healthcare providers commonly prescribe SSRIs for off-label uses. This means they prescribe the drug for a condition outside of its approved indications. This is common and legal in the United States, and many patients aren\u2019t even aware that they are taking the drug off-label. These are some of the common off-label uses for SSRIs.<\/span><\/p>\n<h3 id=\"1-migraine-prevention\"><span style=\"font-weight: 400;\">1. Migraine prevention<\/span><\/h3>\n<p><span style=\"font-weight: 400;\">There is some <\/span><a href=\"https:\/\/www.neurology.org\/doi\/10.1212\/01.wnl.0000176949.34080.64\" target=\"_blank\" rel=\"noopener\"><span style=\"font-weight: 400;\">research<\/span><\/a><span style=\"font-weight: 400;\"> showing that low levels of serotonin may increase a patient\u2019s risk of developing a migraine. For this reason, doctors may prescribe SSRIs for migraine prevention, according to the <\/span><a href=\"https:\/\/www.aafp.org\/pubs\/afp\/issues\/2003\/0801\/p498.html\" target=\"_blank\" rel=\"noopener\"><span style=\"font-weight: 400;\">American Academy of Family Physicians<\/span><\/a><span style=\"font-weight: 400;\">. The most commonly prescribed SSRI for this use is fluoxetine 20\u201340 mg per day; however, <\/span><a href=\"https:\/\/www.aafp.org\/pubs\/afp\/issues\/2019\/0101\/p17.html\" target=\"_blank\" rel=\"noopener\"><span style=\"font-weight: 400;\">studies of its efficacy<\/span><\/a><span style=\"font-weight: 400;\"> have had inconsistent results. Therefore, its use isn\u2019t supported or discouraged by the American Academy of Neurology and American Headache Society\u2019s <\/span><a href=\"https:\/\/www.neurology.org\/doi\/10.1212\/wnl.0b013e3182535d20\" target=\"_blank\" rel=\"noopener\"><span style=\"font-weight: 400;\">clinical guidelines<\/span><\/a><span style=\"font-weight: 400;\"> for migraine prevention. Instead, the guidelines recommend certain <\/span><a href=\"https:\/\/www.singlecare.com\/drug-classes\/anticonvulsants\"><span style=\"font-weight: 400;\">antiepileptic drugs<\/span><\/a><span style=\"font-weight: 400;\">, <\/span><a href=\"https:\/\/www.singlecare.com\/drug-classes\/beta-blockers\"><span style=\"font-weight: 400;\">beta blockers<\/span><\/a><span style=\"font-weight: 400;\">, or <\/span><a href=\"https:\/\/www.singlecare.com\/drug-classes\/triptans\"><span style=\"font-weight: 400;\">triptans<\/span><\/a><span style=\"font-weight: 400;\"> for migraine prevention.<\/span><\/p>\n<h3 id=\"2-body-dysmorphic-disorder-bdd\"><span style=\"font-weight: 400;\">2. Body dysmorphic disorder (BDD)<\/span><\/h3>\n<p><a href=\"https:\/\/www.aafp.org\/pubs\/afp\/issues\/1999\/1015\/p1738.html\" target=\"_blank\" rel=\"noopener\"><span style=\"font-weight: 400;\">Body dysmorphic disorder (BDD)<\/span><\/a><span style=\"font-weight: 400;\"> is a mental health condition where a person is hyperfocused on one or more flaws in their appearance. This negatively impacts their social, academic, and occupational functioning. BDD is typically treated with cognitive behavioral therapy (CBT) and medications, such as SSRIs. However, according to the American Academy of Family Physicians, dosages of SSRIs may need to be higher than those typically used for eating disorders. SSRIs pharmacists may see prescribed for BDD include:<\/span><\/p>\n<ul>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Luvox (fluvoxamine) 200-250 mg per day<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Prozac (fluoxetine) 40-80 mg per day<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Paxil (paroxetine) 40-60 mg per day<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Zoloft (sertraline) 100-200 mg per day<\/span><\/li>\n<\/ul>\n<h3 id=\"3-impulse-control-disorder-icd\"><span style=\"font-weight: 400;\">3. Impulse-control disorder (ICD)<\/span><\/h3>\n<p><a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC3089999\/\" target=\"_blank\" rel=\"noopener\"><span style=\"font-weight: 400;\">Impulse-control disorder (ICD)<\/span><\/a><span style=\"font-weight: 400;\"> is another mental health condition characterized by the lack of ability to exercise self-control. People may experience sudden urges to do something that is outside societal norms. This can lead to social or occupational impairment and cause legal or financial troubles. <\/span><a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC9878961\/\" target=\"_blank\" rel=\"noopener\"><span style=\"font-weight: 400;\">Research<\/span><\/a><span style=\"font-weight: 400;\"> shows serotonin may play a role in managing impulsive behavior, and fluoxetine at a dosage of 20-60 mg per day is one of the most commonly prescribed regimens for this condition. Irritable bowel syndrome (IBS)<\/span><\/h3>\n<p><a href=\"https:\/\/www.singlecare.com\/conditions\/ibs-treatment-and-medications\"><span style=\"font-weight: 400;\">Irritable bowel disorder (IBS)<\/span><\/a><span style=\"font-weight: 400;\"> is a gastrointestinal disorder that causes a wide range of symptoms, including abdominal pain, cramping, discomfort, bloating, diarrhea, or constipation. The underlying cause of IBS is unknown, and management is focused on relieving symptoms and minimizing triggers. One strategy for combating symptoms of IBS is managing underlying depression or stress. This is where antidepressants can come in. While <\/span><a href=\"https:\/\/www.singlecare.com\/drug-classes\/tricyclic-antidepressants\"><span style=\"font-weight: 400;\">tricyclic antidepressants (TCAs)<\/span><\/a><span style=\"font-weight: 400;\"> are the most effective antidepressants, the data supporting the use of SSRIs for IBS is encouraging. SSRI agents used for IBS may include paroxetine 5-20 mg per day and fluoxetine 20 mg per day.<\/span><\/p>\n<h3 id=\"5-diabetic-neuropathy\"><span style=\"font-weight: 400;\">5. Diabetic neuropathy<\/span><\/h3>\n<p><a href=\"https:\/\/www.singlecare.com\/conditions\/peripheral-diabetic-neuropathy-treatment-and-medications\"><span style=\"font-weight: 400;\">Diabetic neuropathy<\/span><\/a><span style=\"font-weight: 400;\">\u2014a type of nerve damage that develops over time\u2014is a common complication of <\/span><a href=\"https:\/\/www.singlecare.com\/conditions\/diabetes-treatment-and-medications\"><span style=\"font-weight: 400;\">diabetes<\/span><\/a><span style=\"font-weight: 400;\">. It can affect the feet, legs, arms, and hands. Treatment focuses on managing the symptoms, and first-line treatments include Lyrica (pregabalin), Neurontin (gabapentin), Cymbalta (duloxetine), and Elavil (amitriptyline). If first-line treatments aren\u2019t effective, other drugs, including SSRIs, may be considered. Examples include paroxetine at a dosage of 40 mg per day.<\/span><\/p>\n<h3 id=\"6-premature-ejaculation\"><span style=\"font-weight: 400;\">6. Premature ejaculation<\/span><\/h3>\n<p><span style=\"font-weight: 400;\">Premature ejaculation is one of the <\/span><a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/16205491\/\" target=\"_blank\" rel=\"noopener\"><span style=\"font-weight: 400;\">most common<\/span><\/a><span style=\"font-weight: 400;\"> sexual problems. Serotonin may play an important role in the ejaculatory reflex, and higher levels of serotonin can delay ejaculation. Therefore, certain SSRIs may be beneficial for premature ejaculation. These include paroxetine, fluoxetine, sertraline, fluvoxamine, citalopram, and escitalopram.<\/span><\/p>\n<h3 id=\"7-paraphilias-and-hypersexuality\"><span style=\"font-weight: 400;\">7. Paraphilias and hypersexuality<\/span><\/h3>\n<p><span style=\"font-weight: 400;\">According to Current Neuropharmacology, there is important evidence supporting the use of SSRIs in the treatment of paraphilia and hypersexuality. <\/span><a href=\"https:\/\/www.ncbi.nlm.nih.gov\/books\/NBK554425\/\" target=\"_blank\" rel=\"noopener\"><span style=\"font-weight: 400;\">Paraphilias<\/span><\/a><span style=\"font-weight: 400;\"> are intense, recurrent sexual urges or fantasies involving atypical activities, whereas <\/span><a href=\"https:\/\/my.clevelandclinic.org\/health\/diseases\/22690-sex-addiction-hypersexuality-and-compulsive-sexual-behavior\" target=\"_blank\" rel=\"noopener\"><span style=\"font-weight: 400;\">hypersexuality<\/span><\/a><span style=\"font-weight: 400;\"> is sexual addiction. The utility of SSRIs in these conditions may be explained by their ability to weaken sexual drive and compulsive behaviors. Fluoxetine has shown the strongest evidence in both these conditions, but healthcare professionals may also prescribe sertraline in certain cases.<\/span><\/p>\n<h3 id=\"8-hot-flashes-caused-by-menopause\"><span style=\"font-weight: 400;\">8. Hot flashes caused by menopause<\/span><\/h3>\n<p><a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC3889979\/\" target=\"_blank\" rel=\"noopener\"><span style=\"font-weight: 400;\">Hot flashes<\/span><\/a><span style=\"font-weight: 400;\"> are the most common symptom of <\/span><a href=\"https:\/\/www.singlecare.com\/conditions\/menopause-treatment-and-medications\"><span style=\"font-weight: 400;\">menopause<\/span><\/a><span style=\"font-weight: 400;\">. While they are typically mild, hot flashes can negatively impact some women\u2019s quality of life. Luckily, there is evidence supporting the use of SSRIs in the treatment of hot flashes in menopause. In one <\/span><a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC3889979\/\" target=\"_blank\" rel=\"noopener\"><span style=\"font-weight: 400;\">study<\/span><\/a><span style=\"font-weight: 400;\">, SSRIs showed modest improvement in the severity and frequency of hot flashes, with Lexapro (escitalopram) showing the greatest efficacy.<\/span><\/p>\n<p><a class=\"cta-button\" href=\"https:\/\/www.singlecare.com\/resources\/antidepressants\">Save on common antidepressants<\/a><\/p>\n<h2 id=\"counseling-points-for-patients-taking-an-ssri\"><span style=\"font-weight: 400;\">Counseling points for patients taking an SSRI<\/span><\/h2>\n<p><span style=\"font-weight: 400;\">Regardless of whether an SSRI is being prescribed on- or off-label, pharmacists are in a key position to ensure the safety of patients taking SSRIs. Here are some important counseling points for SSRIs:<\/span><\/p>\n<ul>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><b>Risk of suicidal thoughts and behaviors. <\/b><span style=\"font-weight: 400;\">SSRIs have a <\/span><a href=\"https:\/\/www.singlecare.com\/blog\/black-box-warning\/\"><span style=\"font-weight: 400;\">boxed warning<\/span><\/a><span style=\"font-weight: 400;\"> for the risk of suicidality. They can increase the risk of suicidal thoughts and behaviors in children and young adults. Any worsening in mood, thoughts, or behaviors should be reported to a healthcare provider as soon as possible.<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><b>Stopping treatment.<\/b><span style=\"font-weight: 400;\"> Although SSRIs aren\u2019t addictive, they can cause withdrawal symptoms if they\u2019re stopped abruptly or if their dose is lowered too quickly. Patients should speak with their healthcare provider before stopping any SSRIs. Together, they can come up with a plan to lower their dosage gradually to avoid withdrawal symptoms.<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><b>Take exactly as prescribed. <\/b><span style=\"font-weight: 400;\">SSRIs can take some time to work. Even if patients don\u2019t see results right away, they must continue taking the medication exactly as prescribed by their provider.<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><b>Drug interactions. <\/b><span style=\"font-weight: 400;\">SSRIs have many potential drug interactions, and these interactions can vary depending on the specific SSRI. Some interactions can increase the risk of serious side effects, while others can make drugs less effective. Before starting an SSRI, evaluate the patient\u2019s full medication list for potential drug interactions.<\/span><\/li>\n<\/ul>\n","protected":false},"excerpt":{"rendered":"<p>It\u2019s common to need a prescription for depression.
