Metronidazole<\/span><\/a>, the generic version of <\/span>Flagyl<\/span><\/a>, is a nitroimidazole antibiotic that is FDA-approved to treat a wide variety of parasitic, bacterial, and sexually transmitted infections\u2014such as amebiasis, peritonitis, <\/span>trichomoniasis<\/span><\/a>, and pelvic-inflammatory disease. Though it may be effective in limited cases, metronidazole is not the first-line treatment for urinary tract infections (UTIs). Here\u2019s what you need to know about metronidazole for UTIs, plus more effective treatment options.<\/span><\/p>\n

Can metronidazole treat UTI?\u00a0<\/span><\/h2>\n

Metronidazole <\/span>can treat UTIs on rare occasions, but it’s usually not the best choice, <\/span>says <\/span>Greg Marchand<\/span><\/a>, MD, a board-certified OB-GYN at the Marchand Institute in Mesa, Arizona<\/span>. Metronidazole works against infections caused by <\/span>anaerobic bacteria<\/span><\/a>, such as appendicitis, liver abscesses, sinusitis, and tetanus. However, <\/span>research<\/span><\/a> indicates that anaerobic infections are rarely the cause of UTIs.<\/span><\/p>\n

In fact, <\/span>85%-95% of UTIs are caused by <\/span>aerobic bacteria<\/span><\/a>, like E. coli (80%) or Staphylococcus saprophyticus (5% to 15%). Metronidazole is ineffective against<\/span> these bacteria. <\/span>In the rare case that<\/span> a UTI is found to be caused by anaerobic bacteria, metronidazole might be prescribed, says <\/span>Sazan Sylejmani, Pharm.D.<\/span><\/a>, owner and pharmacy manager at <\/span>Westmont Pharmacy<\/span><\/a> in Illinois<\/span>.<\/span><\/p>\n

One example of when this medication could be used to treat UTI is in women with <\/span>bacterial vaginosis<\/span><\/a> (BV) caused by <\/span>the bacteria Gardnerella vaginalis. BV <\/span>increases the risk<\/span><\/a> of developing a UTI. Metronidazole is often prescribed to treat BV and is <\/span>92% to 96%<\/span><\/a> effective<\/span><\/a> against UTIs caused by G. vaginalis.<\/span><\/p>\n

How long does it take for metronidazole to work?<\/span><\/h2>\n

The time it takes for an antibiotic to work depends on the severity of the infection, the type of bacteria, and the patient\u2019s overall health. \u201cGenerally, UTI symptoms improve significantly within 48 to 72 hours with appropriate antibiotic treatment,\u201d Dr. Sylejmani says.<\/span><\/p>\n

Dr. Marchand adds that if the bacteria are anaerobic and susceptible to metronidazole, a typical antibiotic course is 500 mg of metronidazole tablets taken twice daily for seven days. To eliminate the infection, you must complete the full course of treatment.<\/span><\/p>\n

\u201cIf metronidazole is working, you should see relief from your symptoms, including dysuria (pain, burning, or stinging during urination), frequent urination, and increased urgency to urinate within one week,\u201d says Dr. Marchand. In some cases, your prescribing healthcare provider may want to send a urine culture to be sure the bacteria are gone, he adds.<\/span><\/p>\n\t\t

Potential limitations of metronidazole for UTI<\/span><\/h2>\n

\u201cThe efficacy of metronidazole for a UTI would be limited to cases where anaerobic bacteria are involved, which is rare in uncomplicated UTI scenarios,\u201d says Dr. Sylejmani. Moreover, metronidazole is associated with various side effects and contraindications, which might limit its use.<\/span><\/p>\n
