First, the provider will perform a medical evaluation to assess symptoms and medical history. If low testosterone is suspected, blood tests are performed to provide confirmation, he explains.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Based on this evaluation and lab results, the provider will prescribe an appropriate form of testosterone. Dr. Schweig emphasizes the importance of avoiding online prescriptions that do not require medical supervision, &ldquo;Proper monitoring by a healthcare provider is essential to ensure safe and effective treatment, adjust dosages, and monitor potential side effects.&rdquo;<\/span><\/p>\n<h2 id=\"types-of-testosterone-replacement-therapy-trt\"><span style=\"font-weight: 400;\">Types of testosterone replacement therapy (TRT )<\/span><\/h2>\n<p><span style=\"font-weight: 400;\">Different <\/span><a href=\"https:\/\/www.singlecare.com\/conditions\/low-testosterone-treatment-and-medications\"><span style=\"font-weight: 400;\">testosterone medications<\/span><\/a><span style=\"font-weight: 400;\"> approved for TRT are classified based on their route of administration. Typically, a testosterone treatment plan may involve about <\/span><a href=\"https:\/\/www.ncbi.nlm.nih.gov\/books\/NBK534853\/\" target=\"_blank\" rel=\"noopener\"><span style=\"font-weight: 400;\">six months of therapy<\/span><\/a><span style=\"font-weight: 400;\"> per prescription. Patients&rsquo; satisfaction with these methods <\/span><a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC3946859\/\" target=\"_blank\" rel=\"noopener\"><span style=\"font-weight: 400;\">will vary depending on factors<\/span><\/a><span style=\"font-weight: 400;\"> like ease of use, cost, symptom improvement, and side effects.<\/span><\/p>\n<h3 id=\"injectable-testosterone\"><span style=\"font-weight: 400;\">Injectable testosterone<\/span><\/h3>\n<p><span style=\"font-weight: 400;\">Testosterone comes in injectable forms, such as<\/span> <a href=\"https:\/\/www.singlecare.com\/prescription\/testosterone-cypionate\"><span style=\"font-weight: 400;\">testosterone cypionate<\/span><\/a><span style=\"font-weight: 400;\"> and<\/span> <a href=\"https:\/\/www.singlecare.com\/prescription\/testosterone-enanthate\"><span style=\"font-weight: 400;\">testosterone enanthate<\/span><\/a><span style=\"font-weight: 400;\">. The standard route for administering these testosterone shots was intramuscular. Still, recent studies have shown that the<\/span> <a href=\"https:\/\/academic.oup.com\/jcem\/article\/102\/7\/2349\/3098651\" target=\"_blank\" rel=\"noopener\"><span style=\"font-weight: 400;\">subcutaneous route offers as much effectiveness<\/span><\/a><span style=\"font-weight: 400;\"> and safety as intramuscular testosterone injections and is an accepted alternative.<\/span><span style=\"font-weight: 400;\"> A typical dose for testosterone injections is <\/span><a href=\"https:\/\/www.npjournal.org\/article\/S1555-4155(16)30716-4\/pdf\" target=\"_blank\" rel=\"noopener\"><span style=\"font-weight: 400;\">200 mg\/mL, administered every two weeks<\/span><\/a><span style=\"font-weight: 400;\">. The long-acting testosterone undecanoate, which the FDA approved in 2014, is typically dosed at 750 mg once, another 750 mg four weeks later, and subsequently 750 mg every 10 weeks. Testosterone injections are often performed by the patient themselves and are usually the least expensive way to receive testosterone replacement.&nbsp;<\/span><\/p>\n<h3 id=\"transdermal-testosterone-topical-gels-patches-and-solutions\"><span style=\"font-weight: 400;\">Transdermal testosterone (topical gels, patches, and solutions)<\/span><\/h3>\n<p><span style=\"font-weight: 400;\">Transdermal TRT<\/span> <a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC5080989\/\" target=\"_blank\" rel=\"noopener\"><span style=\"font-weight: 400;\">involves applying<\/span><\/a><span style=\"font-weight: 400;\"> gels containing testosterone (5&ndash;10 mg daily of 1% gel containing 50&ndash;100 mg testosterone) or testosterone patches, such as <\/span><a href=\"https:\/\/www.singlecare.com\/prescription\/androderm\/what-is\"><span style=\"font-weight: 400;\">Androderm<\/span><\/a><span style=\"font-weight: 400;\"> (2 or 4 mg every night), to the skin of the upper arms, back or thighs. Axiron is a 60 mg topical testosterone solution. The recommended dose is two pumps applied to the underarms once daily. Providers can adjust the dose based on serum testosterone levels <\/span><a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC5182226\/\" target=\"_blank\" rel=\"noopener\"><span style=\"font-weight: 400;\">measured 14 days<\/span><\/a><span style=\"font-weight: 400;\"> after the start of the therapy. Gels are commonly recommended for convenience, cost-effectiveness, and minimal hormonal level variability. <\/span><span style=\"font-weight: 400;\">Alcohol-based gels are flammable. Avoid fire, flames, or smoking until the gel is dry. Testosterone may be transferred to another person when there&rsquo;s skin-to-skin contact with the application site. The patient should thoroughly wash hands after application and cover the application site with clothing once dried.&nbsp;<\/span><\/p>\n<h3 id=\"subcutaneous-testosterone-pellet\"><span style=\"font-weight: 400;\">Subcutaneous testosterone pellet<\/span><\/h3>\n<p><a href=\"https:\/\/my.clevelandclinic.org\/health\/drugs\/18781-testosterone-subcutaneous-pellets\" target=\"_blank\" rel=\"noopener\"><span style=\"font-weight: 400;\">Testosterone pellets<\/span><\/a><span style=\"font-weight: 400;\"> are grain-sized dissolvable implants (600-1200 mg, every 4-6 months) inserted under the skin to release hormones gradually over time. However, pellets are only sometimes used due to insufficient data on how to maintain testosterone concentrations during treatment and the need for repeated implantation. <\/span><span style=\"font-weight: 400;\">Side effects include pellet expulsion, infection, and fibrosis of the surrounding skin. Surgery is needed to insert them, and there&rsquo;s limited data on the serum testosterone concentrations during treatment. Most physicians do not routinely recommend testosterone in pellet preparation<\/span><\/p>\n<h3 id=\"oral-testosterone\"><span style=\"font-weight: 400;\">Oral testosterone&nbsp;<\/span><\/h3>\n<p><span style=\"font-weight: 400;\">The oral agent <\/span><a href=\"https:\/\/www.singlecare.com\/prescription\/jatenzo\"><span style=\"font-weight: 400;\">Jatenzo (testosterone undecanoate)<\/span><\/a><span style=\"font-weight: 400;\"> received FDA approval in 2019. It is a good option for treating low testosterone levels <\/span><a href=\"https:\/\/www.fda.gov\/news-events\/press-announcements\/fda-approves-new-oral-testosterone-capsule-treatment-men-certain-forms-hypogonadism\" target=\"_blank\" rel=\"noopener\"><span style=\"font-weight: 400;\">due to specific medical conditions<\/span><\/a><span style=\"font-weight: 400;\"> like damage to the pituitary gland and Klinefelter syndrome. The recommended starting dose is <\/span><a href=\"https:\/\/www.jatenzo.com\/dosing\" target=\"_blank\" rel=\"noopener\"><span style=\"font-weight: 400;\">237 mg twice per day<\/span><\/a><span style=\"font-weight: 400;\">. <\/span><a href=\"https:\/\/www.singlecare.com\/prescription\/kyzatrex\"><span style=\"font-weight: 400;\">Kyzatrex<\/span><\/a><span style=\"font-weight: 400;\">, which received approval in 2022, is formulated to <\/span><a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC9577662\/\" target=\"_blank\" rel=\"noopener\"><span style=\"font-weight: 400;\">bypass the liver<\/span><\/a><span style=\"font-weight: 400;\">, as the older oral agent methyltestosterone presents a significant liver toxicity risk. Available in 100, 150, and 100 mg doses, Kyzatrex is taken twice a day.<\/span><\/p>\n<h3 id=\"nasal-testosterone-gel\"><span style=\"font-weight: 400;\">Nasal testosterone gel<\/span><\/h3>\n<p><a href=\"https:\/\/www.singlecare.com\/prescription\/natesto\/what-is\"><span style=\"font-weight: 400;\">Natesto<\/span><\/a><span style=\"font-weight: 400;\"> is a nasal testosterone gel formulation designed to be dispensed into the nose with a recommended dose of two pumps (5.5 mg per pump and one actuation per nostril) three times a day. This formulation allows patients to self-administer testosterone, is non-invasive, and has a low total daily dose. However, frequent dosing may be inconvenient for some people, and some may experience side effects like headaches, nose bleeding, and nasal discomfort.<\/span><\/p>\n<h3 id=\"gum-and-cheek-buccal-testosterone\"><span style=\"font-weight: 400;\">Gum and cheek (buccal) testosterone<\/span><\/h3>\n<p><a href=\"https:\/\/www.mayoclinic.org\/drugs-supplements\/testosterone-buccal-route\/proper-use\/drg-20066308\" target=\"_blank\" rel=\"noopener\"><span style=\"font-weight: 400;\">Buccal testosterone medications<\/span><\/a><span style=\"font-weight: 400;\"> are administered in the space to the left or right of the upper gums and cheek, called the buccal cavity. While the medicine looks like a tablet, it works like a patch, sticking to the gum for 12 hours. The dosage for adults is typically 30 mg, applied twice daily.<\/span><\/p>\n<h2 id=\"how-much-does-trt-cost\"><span style=\"font-weight: 400;\">How much does TRT cost?<\/span><\/h2>\n<p><span style=\"font-weight: 400;\">&ldquo;Depending on which type of testosterone is considered (injection, cream, patch, or pill), insurance may or may not cover it,&rdquo; says Dr. Tandon.
