The <\/span><a href=\"https:\/\/www.singlecare.com\/drug-classes\/glp-1-agonists\"><span style=\"font-weight: 400;\">glucagon-like peptide-1 (GLP-1) receptor agonist<\/span><\/a><span style=\"font-weight: 400;\"> helps the pancreas release insulin and lowers blood sugar. It also mimics the GLP-1 hormone that tells your brain you\u2019re full. As a result, weight loss may occur.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">The effectiveness of semaglutide in assisting weight loss efforts has created intense demand for the product, often leaving it <\/span><a href=\"https:\/\/www.singlecare.com\/blog\/ozempic-shortage\/\"><span style=\"font-weight: 400;\">in short supply<\/span><\/a><span style=\"font-weight: 400;\">. The high demand status and off-label use may lead to questioning of whether Ozempic is a controlled substance or drug that can be abused. Fortunately, it is not a controlled substance, but it is wise to know why and understand the restrictions on Ozempic\u2019s use.\u00a0<\/span><\/p>\n<h2 id=\"what-does-the-fda-consider-a-controlled-substance\"><span style=\"font-weight: 400;\">What does the FDA consider a controlled substance?<\/span><\/h2>\n<p><span style=\"font-weight: 400;\">The Food and Drug Administration (FDA) and Drug Enforcement Administration (DEA) are the two <\/span><a href=\"https:\/\/www.ncbi.nlm.nih.gov\/books\/NBK582130\/#:~:text=With%20these%20schedules%20in%20place,%C2%A7%20812%5D.\"><span style=\"font-weight: 400;\">agencies at the heart of controlled substance governance<\/span><\/a><span style=\"font-weight: 400;\">. The FDA approves prescription drugs for use and indicates what health conditions the drugs help. If a drug demonstrates the possibility of causing mental or physical dependence and abuse, it is labeled as a controlled substance.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Since not all controlled substances have an equal risk of dependence and abuse, the DEA assigns each to a <\/span><a href=\"https:\/\/www.dea.gov\/drug-information\/drug-scheduling\"><span style=\"font-weight: 400;\">Schedule from I to V<\/span><\/a><span style=\"font-weight: 400;\">, with drugs on Schedule I having the highest abuse potential and Schedule V the least among controlled substances.\u00a0<\/span><\/p>\n<ul>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><b>Schedule I:<\/b><span style=\"font-weight: 400;\"> These include drugs with no medically accepted use and a high risk of abuse, like ecstasy and heroin.<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><b>Schedule II:<\/b><span style=\"font-weight: 400;\"> These include drugs with a high potential for abuse and severe dependence, including most <\/span><a href=\"https:\/\/www.singlecare.com\/drug-classes\/opioids\"><span style=\"font-weight: 400;\">opioid pain-relievers<\/span><\/a><span style=\"font-weight: 400;\"> and stimulants for attention deficit hyperactivity disorder (ADHD). Cocaine, <\/span><a href=\"https:\/\/www.singlecare.com\/prescription\/oxycodone-hcl\/what-is\"><span style=\"font-weight: 400;\">oxycodone<\/span><\/a><span style=\"font-weight: 400;\">, and <\/span><a href=\"https:\/\/www.singlecare.com\/prescription\/adderall\/what-is\"><span style=\"font-weight: 400;\">Adderall<\/span><\/a><span style=\"font-weight: 400;\"> (dextroamphetamine and amphetamine) are some of the members of this group.<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><b>Schedule III:<\/b><span style=\"font-weight: 400;\"> These are drugs with a moderate to low potential for physical and psychological dependence. Examples include <\/span><a href=\"https:\/\/www.singlecare.com\/prescription\/ketamine-hcl\/what-is\"><span style=\"font-weight: 400;\">ketamine<\/span><\/a><span style=\"font-weight: 400;\"> and <\/span><a href=\"https:\/\/www.singlecare.com\/prescription\/testosterone-cypionate\/what-is\"><span style=\"font-weight: 400;\">testosterone<\/span><\/a><span style=\"font-weight: 400;\">.<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><b>Schedule IV:<\/b><span style=\"font-weight: 400;\"> These are drugs with a low potential for abuse and a low risk of dependence. Examples include <\/span><a href=\"https:\/\/www.singlecare.com\/drug-classes\/benzodiazepines\"><span style=\"font-weight: 400;\">benzodiazepines<\/span><\/a><span style=\"font-weight: 400;\"> like <\/span><a href=\"https:\/\/www.singlecare.com\/prescription\/xanax\/what-is\"><span style=\"font-weight: 400;\">Xanax<\/span><\/a><span style=\"font-weight: 400;\"> (alprazolam) and <\/span><a href=\"https:\/\/www.singlecare.com\/prescription\/ativan\/what-is\"><span style=\"font-weight: 400;\">Ativan<\/span><\/a><span style=\"font-weight: 400;\"> (lorazepam), as well as sedative-hypnotics like <\/span><a href=\"https:\/\/www.singlecare.com\/prescription\/ambien\/what-is\"><span style=\"font-weight: 400;\">Ambien<\/span><\/a><span style=\"font-weight: 400;\"> (zolpidem).\u00a0<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><b>Schedule V:<\/b><span style=\"font-weight: 400;\"> These drugs have a lower potential for abuse than Schedule <\/span>IV drugs. Examples include the antidiarrheal <a href=\"https:\/\/www.singlecare.com\/prescription\/lomotil\/what-is\">Lomotil<\/a> (diphenoxylate and atropine) and nerve pain medication <a href=\"https:\/\/www.singlecare.com\/prescription\/lyrica\/what-is\">Lyrica<\/a> (pregabalin).<\/li>\n<\/ul>\n<h2 id=\"is-ozempic-a-controlled-substance\"><span style=\"font-weight: 400;\">Is Ozempic a controlled substance?<\/span><\/h2>\n<p><span style=\"font-weight: 400;\">Ozempic is not a controlled substance. Abuse potential and physical or psychological dependence have not been demonstrated with Ozempic. Although it\u2019s not labeled as a controlled substance or assigned to a schedule by the DEA, that doesn\u2019t mean that there are no concerns of risk with its use. For instance, Ozempic is to be avoided by anyone with a history of pancreatitis or a personal or family history of medullary thyroid cancer or multiple endocrine neoplasia type 2 syndrome (MEN2). The latter is actually the subject of a boxed warning on Ozempic\u2019s label.<\/span><\/p>\n<h2 id=\"weight-loss-drugs-that-are-controlled-substances\"><span style=\"font-weight: 400;\">Weight loss drugs that are controlled substances<\/span><\/h2>\n<p><span style=\"font-weight: 400;\">Unlike Ozempic, there are weight loss drugs listed by the DEA as having the potential for abuse. Specifically, <\/span><a href=\"https:\/\/www.singlecare.com\/drug-classes\/cns-stimulants\"><span style=\"font-weight: 400;\">central nervous system (CNS) stimulants<\/span><\/a><span style=\"font-weight: 400;\"> (also called sympathomimetics) are used for weight management and are similar to amphetamines. Most are more affordable than <\/span><a href=\"https:\/\/www.singlecare.com\/prescription\/ozempic\"><span style=\"font-weight: 400;\">Ozempic<\/span><\/a><span style=\"font-weight: 400;\">, and this has kept them in use.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">CNS stimulants suppress appetite and are approved for short-term use up to 12 weeks to assist with obesity treatment. In addition, their stimulation of the sympathetic nervous system can lead to increases in heart rate and blood pressure, which is why those with heart disease, a history of heart attack, or uncontrolled hypertension do not use these options. Controlled substance weight loss drugs include the following:<\/span><\/p>\n<ul>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><a href=\"https:\/\/www.singlecare.com\/prescription\/adipex-p\"><b>Adipex-P<\/b><\/a><b> (phentermine):<\/b><span style=\"font-weight: 400;\"> Listed as a Schedule IV drug by the DEA, phentermine remains a commonly prescribed weight loss treatment, but it is only approved for three months of use and has contraindications to its use that center around its abuse potential and cardiovascular risk.<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><a href=\"https:\/\/www.singlecare.com\/prescription\/diethylpropion-hcl\"><b>Diethylpropion<\/b><\/a><b>:<\/b><span style=\"font-weight: 400;\"> The DEA also puts diethylpropion on Schedule IV, indicating some abuse potential, and the same contraindications as Adipex-P apply.<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><a href=\"https:\/\/www.singlecare.com\/prescription\/benzphetamine-hcl\"><b>Benzphetamine<\/b><\/a><b>:<\/b><span style=\"font-weight: 400;\"> Having a higher abuse potential than the others, benzphetamine is classified as a Schedule III drug. It is labeled with similar warnings to Adipex-P and diethylpropion.<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><a href=\"https:\/\/www.singlecare.com\/prescription\/phendimetrazine-tartrate\"><b>Phendimetrazine<\/b><\/a><b>:<\/b><span style=\"font-weight: 400;\"> Another Schedule III drug, phendimetrazine has the same contraindications and time constraints as the others listed above.<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><a href=\"https:\/\/www.singlecare.com\/prescription\/qsymia\"><b>Qsymia<\/b><\/a><b> (phentermine and topiramate):<\/b><span style=\"font-weight: 400;\"> Combining the anticonvulsant topiramate, which has an appetite-suppressing effect, and the stimulant phentermine, Qsymia is FDA approved for chronic weight management. It lands on Schedule IV due to its phentermine component and has warnings about use in the setting of cardiovascular disease, in addition to concerns about topiramate side effects that include drowsiness, changes in sensation, and kidney stones.<\/span><\/li>\n<\/ul>\n<h2 id=\"is-there-any-risk-for-semaglutide-misuse\"><span style=\"font-weight: 400;\">Is there any risk for semaglutide misuse?<\/span><\/h2>\n<p><span style=\"font-weight: 400;\">Semaglutide, under its brand names <\/span><a href=\"https:\/\/www.singlecare.com\/blog\/wegovy-vs-ozempic\/\"><span style=\"font-weight: 400;\">Ozempic and Wegovy<\/span><\/a><span style=\"font-weight: 400;\">, is not a controlled substance. Nonetheless, it can be misused. The desire to lose weight can drive people to use GLP-1 receptor agonists in an unapproved fashion, such as when they do not meet the FDA\u2019s indications for use. For example, the FDA indication for <\/span><a href=\"https:\/\/www.singlecare.com\/prescription\/wegovy\"><span style=\"font-weight: 400;\">Wegovy<\/span><\/a><span style=\"font-weight: 400;\"> use for weight management is a body mass index (BMI) of at least 30 or a BMI of at least 27 with at least one weight-related comorbidity, such as hypertension or high cholesterol.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">A <\/span><a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC10384093\/#B28-pharmaceuticals-16-00994\"><span style=\"font-weight: 400;\">study looking at markers of misuse<\/span><\/a><span style=\"font-weight: 400;\"> found that semaglutide seems prone to it. Compounded versions of semaglutide are also <\/span><a href=\"https:\/\/www.fda.gov\/drugs\/postmarket-drug-safety-information-patients-and-providers\/fdas-concerns-unapproved-glp-1-drugs-used-weight-loss\"><span style=\"font-weight: 400;\">of concern to the FDA<\/span><\/a><span style=\"font-weight: 400;\"> for both legal and safety reasons. Compound versions are typically only permitted by law when there are <\/span><a href=\"https:\/\/www.fda.gov\/drugs\/drug-safety-and-availability\/drug-shortages\"><span style=\"font-weight: 400;\">official drug shortages<\/span><\/a><span style=\"font-weight: 400;\">. Otherwise, it is considered patent infringement. Besides legality concerns, compound medication also entails the risk of improper manufacturing by whichever entity is making the drug.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Cases of individuals getting <\/span><a href=\"https:\/\/www.fda.gov\/drugs\/drug-safety-and-availability\/fda-warns-consumers-not-use-counterfeit-ozempic-semaglutide-found-us-drug-supply-chain\"><span style=\"font-weight: 400;\">counterfeit semaglutide<\/span><\/a><span style=\"font-weight: 400;\"> without a prescription have also been reported. Without medically supervised health care, <\/span><a href=\"https:\/\/dailymed.nlm.nih.gov\/dailymed\/drugInfo.cfm?setid=adec4fd2-6858-4c99-91d4-531f5f2a2d79\"><span style=\"font-weight: 400;\">the drug\u2019s dangers<\/span><\/a><span style=\"font-weight: 400;\"> could escalate.
