For example, a person who is allergic to a common dye included in a prescription drug could have a compounded version of the drug made without that dye. More notably, compounded drugs are also made to counteract drug shortages. When there isn\u2019t enough supply of prescription medication to meet patient demand, <\/span><a href=\"https:\/\/www.singlecare.com\/blog\/compounding-pharmacy\/\"><span style=\"font-weight: 400;\">compounding pharmacies<\/span><\/a><span style=\"font-weight: 400;\"> can create similar versions, or copies, of the drug to fill the gap.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Before <\/span><a href=\"https:\/\/www.singlecare.com\/prescription\/ozempic\"><span style=\"font-weight: 400;\">Ozempic (semaglutide)<\/span><\/a><span style=\"font-weight: 400;\"> became a household name, most people probably didn\u2019t think much about compounded medications. Still, <\/span><a href=\"https:\/\/www.singlecare.com\/blog\/ozempic-shortage\/\"><span style=\"font-weight: 400;\">Ozempic\u2019s popularity has caused an ongoing shortage<\/span><\/a><span style=\"font-weight: 400;\"> of the drug\u2014and compounding pharmacies have ramped up their production of compounded semaglutide, an unofficial alternative that, in theory, has the same active ingredient and works just like the real thing.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">\u201cCompounded semaglutide has emerged as an alternative to Wegovy and Ozempic due to its lower cost and greater availability,\u201d says Paunel Vukasinov, MD, internist and obesity medicine specialist with Medical Offices of Manhattan.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">But despite its benefits\u2014which include helping patients save money and reducing the strain of supply shortages\u2014compounded semaglutide has raised ethical and safety concerns in the medical community, Dr. Vukasinov says. It has also raised concerns for Ozempic manufacturer Novo Nordisk, which <\/span><a href=\"https:\/\/ncpa.org\/newsroom\/qam\/2024\/10\/25\/novo-nordisk-asked-fda-block-semaglutide-compounding\"><span style=\"font-weight: 400;\">petitioned<\/span><\/a><span style=\"font-weight: 400;\"> the U.S. Food &amp; Drug Administration (FDA) in October 2024 to ban compounded formulations of semaglutide.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">In February 2025, the FDA declared that there is no longer an Ozempic shortage, and compounding pharmacies have 60 to 90 days to stop producing it, depending on the type of pharmacy or facility. Here\u2019s what to know, including why compounded semaglutide will be banned and what that means for patients.<\/span><\/p>\n<h2 id=\"concerns-with-compounded-versions-of-semaglutide\"><span style=\"font-weight: 400;\">Concerns with compounded versions of semaglutide<\/span><\/h2>\n<p><span style=\"font-weight: 400;\">Compounded medications <\/span><i><span style=\"font-weight: 400;\">can <\/span><\/i><span style=\"font-weight: 400;\">be made safely, but many <\/span><a href=\"https:\/\/www.singlecare.com\/blog\/is-compounded-semaglutide-safe\/\"><span style=\"font-weight: 400;\">concerns are being raised about compounded semaglutide<\/span><\/a><span style=\"font-weight: 400;\"> by healthcare providers and the FDA, which has published several statements warning patients about the <\/span><a href=\"https:\/\/www.fda.gov\/drugs\/postmarket-drug-safety-information-patients-and-providers\/fdas-concerns-unapproved-glp-1-drugs-used-weight-loss\"><span style=\"font-weight: 400;\">potential risks of using compounded semaglutide<\/span><\/a><span style=\"font-weight: 400;\">.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">The overarching issue is that compounded medications are not FDA-approved, which means their safety, efficacy, and quality are less predictable, says <\/span><a href=\"https:\/\/www.medicalofficesofmanhattan.com\/our-team\/paunel-vukasinov-md\/\"><span style=\"font-weight: 400;\">Dr. Vukasinov<\/span><\/a><span style=\"font-weight: 400;\">.<\/span><\/p>\n<ul>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><b>Dosing errors: <\/b><a href=\"https:\/\/www.fda.gov\/drugs\/human-drug-compounding\/fda-alerts-health-care-providers-compounders-and-patients-dosing-errors-associated-compounded\"><span style=\"font-weight: 400;\">Dosing errors have been reported<\/span><\/a><span style=\"font-weight: 400;\"> with the use of compounded semaglutide since some of these alternative forms require patients to measure out their own doses. In many cases, patients accidentally injected too much of the medication because of inexperience with multi-dose vials. In other cases, healthcare providers didn\u2019t correctly calculate or prescribe the right dose, causing patients to self-administer significantly higher amounts of the drug than recommended.<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><b>Different active ingredients: <\/b><span style=\"font-weight: 400;\">Some compounding pharmacies are using semaglutide salts in their formulations, which have not been proven safe or effective. In fact, the <\/span><a href=\"https:\/\/www.fda.gov\/drugs\/postmarket-drug-safety-information-patients-and-providers\/fdas-concerns-unapproved-glp-1-drugs-used-weight-loss\"><span style=\"font-weight: 400;\">FDA has told compounding pharmacies<\/span><\/a><span style=\"font-weight: 400;\"> not to use the salt forms of semaglutide in their formulations. Dr. Vukasinov says compounded products may also contain other additives or mixed ingredients that can alter the drug\u2019s efficacy or increase side effects.<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><b>Lack of regulations: <\/b><span style=\"font-weight: 400;\">Compounded semaglutide producers aren\u2019t required to follow the same quality assurance practices as official manufacturers, says <\/span><a href=\"https:\/\/www.thehcginstitute.com\/doctors\/danielle-kelvas-md\"><span style=\"font-weight: 400;\">Danielle Kelvas<\/span><\/a><span style=\"font-weight: 400;\">, MD, a primary care provider with The HCG Institute in Tennessee. Although compounded semaglutide <\/span><i><span style=\"font-weight: 400;\">can <\/span><\/i><span style=\"font-weight: 400;\">be as effective as Ozempic, the exact manufacturing process isn\u2019t the same, so there could be impurities in the medication. \u201cOzempic went through a rigorous FDA approval process to ensure its safety, efficacy, and quality,\u201d says Dr. Kelvas. \u201cWhen it&#8217;s compounded by a third party, they can skip many of those quality assurance stop signs.\u201d<\/span><\/li>\n<\/ul>\n<p><span style=\"font-weight: 400;\">According to the FDA, compounding pharmacies are allowed to make \u201ccopies\u201d of commercially available drugs when they are on the <\/span><a href=\"https:\/\/www.fda.gov\/drugs\/human-drug-compounding\/compounding-when-drugs-are-fdas-drug-shortages-list\"><span style=\"font-weight: 400;\">agency\u2019s drug shortages list<\/span><\/a><span style=\"font-weight: 400;\">. As of February 2025, <\/span><a href=\"https:\/\/dps.fda.gov\/drugshortages\/activeingredient\/semaglutide-injection\"><span style=\"font-weight: 400;\">semaglutide <\/span><\/a><span style=\"font-weight: 400;\">was removed from the list, and compounding pharmacies must stop producing their own versions of the drug. Meaning, people who were using compounded semaglutide to save money on the high cost of Ozempic will have to purchase the brand-name drug. But if price is a barrier, there are other ways to save, such as using SingleCare coupons.<\/span><\/p>\n<p><a class=\"cta-button\" href=\"https:\/\/www.singlecare.com\/prescription\/ozempic\"><span style=\"font-weight: 400;\">Ozempic coupons<\/span><\/a><span style=\"font-weight: 400;\">, <\/span><a class=\"cta-button\" href=\"https:\/\/www.singlecare.com\/prescription\/wegovy\"><span style=\"font-weight: 400;\">Wegovy coupons<\/span><\/a><span style=\"font-weight: 400;\">, <\/span><a class=\"cta-button\" href=\"https:\/\/www.singlecare.com\/prescription\/rybelsus\"><span style=\"font-weight: 400;\">Rybelsus coupons<\/span><\/a><\/p>\n<p><span style=\"font-weight: 400;\">Many of the concerns around compounded semaglutide relate to injectable formulations of the drug, but they also apply to compound versions of the semaglutide pill (<\/span><a href=\"https:\/\/www.singlecare.com\/prescription\/rybelsus\"><span style=\"font-weight: 400;\">Rybelsus<\/span><\/a><span style=\"font-weight: 400;\">). Per Dr. Kelvas, the semaglutide pill works well for <\/span><a href=\"https:\/\/www.singlecare.com\/blog\/rybelsus-vs-ozempic\/\"><span style=\"font-weight: 400;\">people with Type 2 diabetes who can\u2019t use injectable medications<\/span><\/a><span style=\"font-weight: 400;\">, but even compounded semaglutide in pill form has potential risks.<\/span><\/p>\n<h2 id=\"why-was-compounded-semaglutide-banned\"><span style=\"font-weight: 400;\">Why was compounded semaglutide banned?<\/span><\/h2>\n<p><span style=\"font-weight: 400;\">In October 2024, Novo Nordisk formally asked the FDA to ban the production of compounded semaglutide and requested that the agency move the drug to its <\/span><a href=\"https:\/\/www.fda.gov\/about-fda\/economic-impact-analyses-fda-regulations\/drug-products-or-categories-drug-products-present-demonstrable-difficulties-compounding-under\"><span style=\"font-weight: 400;\">Demonstrable Difficulties for Compounding<\/span><\/a><span style=\"font-weight: 400;\"> (DDC) list.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">In other words, Novo Nordisk is <\/span><a href=\"https:\/\/www.regulations.gov\/document\/FDA-2017-N-2562-0029\"><span style=\"font-weight: 400;\">claiming<\/span><\/a><span style=\"font-weight: 400;\"> that semaglutide is too complex of a drug to be safely compounded, citing patient safety and drug effectiveness as the main reasons why. However, revenue concerns are undoubtedly playing a role in the company\u2019s petition.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">\u201cOther companies are making billions off unsafely compounded semaglutide, so there&#8217;s good reason [for them to want it banned],\u201d says Dr. Kelvas.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Since Ozempic is no longer on the FDA\u2019s drug shortages list, manufacturers of compounded semaglutide must stop producing the medication within 60 to 90 days of the announcement, depending on the type of facility. However, according to FDA regulations, compounders are permitted to create modified versions of medications by adjusting dosages, incorporating additional ingredients, or altering the administration method to cater to the unique requirements of individual patients.<\/span><\/p>\n<h2 id=\"the-future-of-compounded-semaglutide\"><span style=\"font-weight: 400;\">The future of compounded semaglutide<\/span><\/h2>\n<p><span style=\"font-weight: 400;\">A similar <\/span><a href=\"https:\/\/www.fda.gov\/drugs\/drug-safety-and-availability\/fda-clarifies-policies-compounders-national-glp-1-supply-begins-stabilize\"><span style=\"font-weight: 400;\">recent FDA decision<\/span><\/a><span style=\"font-weight: 400;\"> to remove the GLP-1 drug tirzepatide (Mounjaro\/Zepbound) from the drug shortages list allows for up to 90 days from the calendar announcement to stop dispensing products. The ban on compounded semaglutide initiated the same response. This means it\u2019s not likely that anyone currently taking compounded semaglutide will be immediately affected by the ban.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">According to Dr. Vukasinov, the decision will have both positive and negative effects. On the plus side, he thinks it will give patients and providers more confidence in the safety, quality, and efficacy of semaglutide products while reinforcing the FDA\u2019s authority to regulate medications and prevent unsafe compounding practices. But it will also present significant challenges: \u201cPatients who rely on compounded versions due to cost or insurance coverage limitations may face difficulties accessing treatment, [and] lower-income patients may be disproportionately affected [because of the] high cost of branded medications.\u201d<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Overall, Dr. Vukasinov says the ban will likely protect patients from unsafe formulations but, at the same time, exacerbate issues of affordability and access unless the FDA takes steps to counteract those existing issues.<\/span><\/p>\n<h2 id=\"the-bottom-line\"><span style=\"font-weight: 400;\">The bottom line<\/span><\/h2>\n<p><a href=\"https:\/\/www.singlecare.com\/blog\/compounded-semaglutide-vs-ozempic\/\"><span style=\"font-weight: 400;\">Compounded semaglutide<\/span><\/a><span style=\"font-weight: 400;\"> allowed people to access a high-cost, in-demand drug that may not normally be accessible to them. However, compounded medications carry additional risks due to their lack of FDA approval. When a drug is extremely popular (like semaglutide), the potential for compounding pharmacies to take advantage of patients increases\u2014they may cut corners with quality or safety to keep up with competitors, and most patients would be unaware of these hidden dangers.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Because the FDA has a responsibility to ensure that people are receiving safe, legitimate medications, its concerns about compounded semaglutide are valid. However, the ban on compounded semaglutide will also limit access to a medication that many people rely on.
