Common brand-name drugs that contain this ingredient include <\/span><a href=\"https:\/\/www.singlecare.com\/prescription\/ozempic\"><span style=\"font-weight: 400;\">Ozempic<\/span><\/a><span style=\"font-weight: 400;\">, <\/span><a href=\"https:\/\/www.singlecare.com\/prescription\/wegovy\"><span style=\"font-weight: 400;\">Wegovy<\/span><\/a><span style=\"font-weight: 400;\">, and <\/span><a href=\"https:\/\/www.singlecare.com\/prescription\/rybelsus\"><span style=\"font-weight: 400;\">Rybelsus<\/span><\/a><span style=\"font-weight: 400;\">\u2014all manufactured by Novo Nordisk, a Danish pharmaceutical company. Ozempic and Rybelsus are prescribed to control <\/span><a href=\"https:\/\/www.singlecare.com\/blog\/normal-blood-glucose-levels\/\"><span style=\"font-weight: 400;\">blood sugar levels<\/span><\/a><span style=\"font-weight: 400;\"> in adults with Type 2 diabetes, and Wegovy is prescribed for weight management in adults with obesity (or those who are overweight with one or more weight-associated comorbidities). Ozempic and Wegovy are also used to reduce the risk of major cardiovascular events.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Due to the rising demand for weight loss drugs and the resulting <\/span><a href=\"https:\/\/www.singlecare.com\/blog\/ozempic-shortage\/\"><span style=\"font-weight: 400;\">drug shortages<\/span><\/a><span style=\"font-weight: 400;\">, some specialized compounding pharmacies were allowed to make compounded semaglutide injections. Compounded versions of semaglutide contain varying ingredients, and they\u2019re not approved by the U.S. Food and Drug Administration (FDA).\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Although obtaining compounded semaglutide products was often easier and cheaper than brand-name medications, pharmacists and healthcare professionals warn that these products can come with risks. In February 2025, the FDA ordered compounding pharmacies to stop production of compounded semaglutide by May.<\/span><\/p>\n<h2 id=\"why-and-when-semaglutide-is-compounded\"><span style=\"font-weight: 400;\">Why and when semaglutide is compounded<\/span><\/h2>\n<p><span style=\"font-weight: 400;\">According to the FDA, compounded drugs are made by licensed pharmacists (or under the supervision of pharmacists in outsourcing facilities) who combine, mix, or alter ingredients of a drug. Their purpose is to meet an individual patient\u2019s specific needs when an FDA-approved drug doesn\u2019t.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Why would an individual patient need a custom version of semaglutide when brand-name versions are available? There are a few times when compounding is necessary. A patient may be allergic to one of the ingredients in an FDA-approved medication or may require a different form, such as a liquid, per the <\/span><a href=\"https:\/\/www.fda.gov\/drugs\/human-drug-compounding\/compounding-and-fda-questions-and-answers\"><span style=\"font-weight: 400;\">FDA<\/span><\/a><span style=\"font-weight: 400;\">.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Compounded drugs are also made during drug shortages. The <\/span><a href=\"https:\/\/dps.fda.gov\/drugshortages\"><span style=\"font-weight: 400;\">FDA<\/span><\/a><span style=\"font-weight: 400;\"> has a drug shortage list, and injectable semaglutide was in low supply for three years, from early 2022 to February 2025. During drug shortages, the FDA recognizes that patients may turn to compounded formulations of the medications they need.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Some patients who were unable to get brand-name semaglutide turned their attention to compounded versions. \u201cThe growing demand for <\/span><a href=\"https:\/\/www.singlecare.com\/drug-classes\/glp-1-agonists\"><span style=\"font-weight: 400;\">GLP-1 receptor agonists<\/span><\/a><span style=\"font-weight: 400;\"> has driven a surge in compounded semaglutide products,\u201d says <\/span><a href=\"https:\/\/www.drugwatch.com\/contributors\/dazhi-liu-pharm-d-bcop\/\"><span style=\"font-weight: 400;\">Dazhi Liu<\/span><\/a><span style=\"font-weight: 400;\">, Pharm.D., a clinical pharmacy specialist in West New York, New Jersey. This is because compounded drugs are formulated with a custom blend of ingredients.<\/span><\/p>\n<p><strong>RELATED: <a href=\"https:\/\/www.singlecare.com\/blog\/generic-glp-1\/\">Are there generic GLP-1 drugs?<\/a><\/strong><\/p>\n<p><span style=\"font-weight: 400;\">\u201cCompounded semaglutide is a medicine that\u2019s custom-made for the patient by a compounding pharmacy,\u201d explains <\/span><a href=\"https:\/\/www.digestivereset.com\/\"><span style=\"font-weight: 400;\">Inna Melamed<\/span><\/a><span style=\"font-weight: 400;\">, Pharm.D., a functional medicine practitioner and author of <\/span><i><span style=\"font-weight: 400;\">Digestive Reset<\/span><\/i><span style=\"font-weight: 400;\">. \u201cIt\u2019s made of the same active ingredient, semaglutide, in brands like Wegovy and Ozempic.\u201d<\/span><\/p>\n<p><span style=\"font-weight: 400;\">However, it\u2019s not possible to know exactly what compounded semaglutide is made of because there isn\u2019t a universal formula or standard. \u201cCompounding pharmacies mix the ingredients, so it can vary,\u201d says <\/span><a href=\"https:\/\/www.gutsygirlmd.com\/about\"><span style=\"font-weight: 400;\">Supriya Rao<\/span><\/a><span style=\"font-weight: 400;\">, MD, a board-certified obesity medicine specialist practicing in the Boston area. \u201cCompounded semaglutide could possibly be made of semaglutide, but it can also contain vitamins or other ingredients.\u201d <\/span><a href=\"https:\/\/www.singlecare.com\/prescription\/b-12\/what-is\"><span style=\"font-weight: 400;\">Vitamin B12<\/span><\/a><span style=\"font-weight: 400;\"> may be added to compounded semaglutide, for example.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">It\u2019s also possible for these compounded versions to be made with salt forms of semaglutide rather than the pure ingredient in the FDA-approved drugs. \u201cSome compounded semaglutide might not even contain semaglutide,\u201d Dr. Rao explains. \u201cSometimes a salt alternative that is chemically similar that may achieve the same result will be used.\u201d The <\/span><a href=\"https:\/\/www.fda.gov\/drugs\/postmarket-drug-safety-information-patients-and-providers\/fdas-concerns-unapproved-glp-1-drugs-used-weight-loss\"><span style=\"font-weight: 400;\">FDA<\/span><\/a><span style=\"font-weight: 400;\"> warns that these salt forms, including semaglutide sodium and semaglutide acetate, should not be used.<\/span><\/p>\n<h2 id=\"are-all-compounded-semaglutides-the-same\"><span style=\"font-weight: 400;\">Are all compounded semaglutides the same?<\/span><\/h2>\n<p><span style=\"font-weight: 400;\">Compounded drugs containing the same active ingredient aren\u2019t always the same because two patients may have different needs, resulting in various formulations. They\u2019re not the same as name-brand medications either. \u201cCompounded semaglutide is <\/span><a href=\"https:\/\/www.singlecare.com\/blog\/compounded-semaglutide-vs-ozempic\/\"><span style=\"font-weight: 400;\">not identical to Ozempic<\/span><\/a><span style=\"font-weight: 400;\"> or Wegovy,\u201d Dr. Rao states. \u201cWith compounded semaglutide, there\u2019s no way to be sure of what you are taking.\u201d<\/span><\/p>\n<p><span style=\"font-weight: 400;\">People wishing to access weight-loss medications may turn to an online pharmacy for easily accessible and more affordable compounded semaglutide. However, healthcare professionals warn against this because these formulations aren\u2019t standardized. \u201cCompounding pharmacies may use different ingredients or processes, so the quality, formulation, and effectiveness will vary,\u201d Dr. Rao explains. \u201cSince compounded semaglutide is not an FDA-approved drug, the quality will vary greatly due to the lack of oversight.\u201d<\/span><\/p>\n<p><span style=\"font-weight: 400;\">In addition to the lack of quality standards, there are concerns about the potential risks and side effects of compounded semaglutide products. Is <\/span><a href=\"https:\/\/www.singlecare.com\/blog\/is-compounded-semaglutide-safe\/\"><span style=\"font-weight: 400;\">compounded semaglutide safe<\/span><\/a><span style=\"font-weight: 400;\">? \u201cDifferences in the purity of the semaglutide source, formulation methods, and quality controls can lead to significant variability in effectiveness and safety,\u201d Dr. Liu explains. \u201cCompounded products lack bioequivalence testing and can result in dosing errors, reduced efficacy, and adverse events.\u201d<\/span><\/p>\n<p><span style=\"font-weight: 400;\">The popularity of compounded semaglutide products raises significant safety concerns, Dr. Liu says. \u201cMy colleagues and I have noted troubling prescribing practices, including widespread use without adequate patient screening or integration with lifestyle modifications,\u201d she adds. The American Diabetes Association issued a <\/span><a href=\"https:\/\/diabetesjournals.org\/care\/article\/doi\/10.2337\/dci24-0091\/157478\/\"><span style=\"font-weight: 400;\">guidance statement<\/span><\/a><span style=\"font-weight: 400;\"> on this topic, recommending against the use of non-FDA-approved compounded GLP-1 receptor agonist formulations like semaglutide out of concern related to content, safety, quality, and effectiveness.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Now that the FDA declared the semaglutide shortage over, and given pharmacies a deadline to stop producing it, compounded semaglutide many not be widely available after May 2025.<\/span><\/p>\n<h2 id=\"sourcing-semaglutide-for-compounding\"><span style=\"font-weight: 400;\">Sourcing semaglutide for compounding<\/span><\/h2>\n<p><span style=\"font-weight: 400;\">Patients must get a prescription from a healthcare provider for compounded semaglutide, which specialized pharmacies can fill. But where do compounding pharmacies get their raw materials, and how do they ensure safety and sterility?<\/span><\/p>\n<p><span style=\"font-weight: 400;\">\u201cCompounding pharmacies obtain semaglutide as a raw active pharmaceutical ingredient (API) from bulk suppliers,\u201d Dr. Liu says. \u201cWhile some suppliers meet acceptable standards, compounded products are not FDA-approved, and the final formulations may vary in purity, potency, and safety.\u201d<\/span><\/p>\n<p><span style=\"font-weight: 400;\">FDA-approved drugs undergo a lengthy process, including research, rigorous testing, and clinical trials. The approval process determines whether a drug\u2019s benefits outweigh its risks and side effects. It ultimately concludes whether a drug is safe to use in humans. Since compounded semaglutide doesn\u2019t undergo this process, there are risks like dosing errors and counterfeit products, Dr. Liu says. There are also risks associated with contamination and sterility.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">To mitigate the risks, the <\/span><a href=\"https:\/\/www.fda.gov\/drugs\/postmarket-drug-safety-information-patients-and-providers\/fdas-concerns-unapproved-glp-1-drugs-used-weight-loss\"><span style=\"font-weight: 400;\">FDA<\/span><\/a><span style=\"font-weight: 400;\"> recommends talking to your healthcare provider and filling prescriptions at state-licensed pharmacies. While compounded drugs aren\u2019t FDA approved, this doesn\u2019t mean accredited compounding pharmacies go unregulated. Some outsourcing facilities are FDA registered, which means they\u2019re inspected by the FDA and expected to follow the same current good manufacturing practice regulations as drug manufacturers. There are also requirements set by each state\u2019s board of pharmacy, which oversees pharmacy compounding.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">From a safety perspective, brand-name medications are always a better option. And now that the FDA has removed semaglutide from its drug shortages list compounded semaglutide won\u2019t be available.<\/span><\/p>\n<h2 id=\"how-do-i-know-if-my-semaglutide-is-compounded\"><span style=\"font-weight: 400;\">How do I know if my semaglutide is compounded?<\/span><\/h2>\n<p><a href=\"https:\/\/www.singlecare.com\/blog\/when-will-ozempic-go-generic\/\"><span style=\"font-weight: 400;\">Generic semaglutide<\/span><\/a><span style=\"font-weight: 400;\"> doesn\u2019t exist, so that leaves just two options\u2014brand-name and compounded. Both can share the same active ingredient, semaglutide, Dr. Melamed says. However, that doesn\u2019t make them the same, and it\u2019s important to do your due diligence, notes Dr. Liu. \u201cWhile compounding has its place, not all compounding pharmacies are equal, and the risks of using unapproved medications should not be overlooked,\u201d Dr. Liu says. \u201cPatients and providers must prioritize safety, monitor for adverse events, and report concerns to the FDA.\u201d<\/span><\/p>\n<p><span style=\"font-weight: 400;\">So how do you know which one you have? If you go the traditional route\u2014consult a healthcare professional and pick up your prescription at your local pharmacy\u2014you likely have brand-name semaglutide. Brand-name semaglutide comes in two forms\u2014pre-filled injectable pens (Ozempic and Wegovy) and oral tablets (Rybelsus). You\u2019ll know your semaglutide isn\u2019t compounded if you spot the brand name on the packaging and information leaflet.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Compounded semaglutide can vary in appearance. For starters, it won\u2019t have any brand names printed on it. It\u2019s also packaged differently, which can be an easy identifier. Brand-name injectable semaglutide pens are pre-loaded, but compounded semaglutide doesn\u2019t come in pre-filled injection pens. Compounded semaglutide comes in small glass vials and is administered by the patient using syringes.
