Here\u2019s what the experts say.\u00a0<\/span><\/p>\n<h2 id=\"glimepiride-vs-metformin-a-brief-overview\"><span style=\"font-weight: 400;\">Glimepiride vs. metformin: A brief overview<\/span><\/h2>\n<p><span style=\"font-weight: 400;\">Glimepiride and metformin are both approved to treat Type 2 diabetes, but there are some differences that make these drugs unique. <\/span><a href=\"https:\/\/www.singlecare.com\/prescription\/glimepiride\/what-is#uses\"><span style=\"font-weight: 400;\">Glimepiride<\/span><\/a><span style=\"font-weight: 400;\"> is a sulfonylurea drug that stimulates the pancreas to produce more insulin. This isn\u2019t always the case, but for glimepiride and metformin, it can be. \u201cMetformin and glimepiride can be combined for the management of Type 2 diabetes to help lower blood glucose levels,\u201d say <\/span><a href=\"https:\/\/www.ketchum.edu\/directory\/diana-roman\"><span style=\"font-weight: 400;\">Diana Roman<\/span><\/a><span style=\"font-weight: 400;\">, Pharm.D. and <\/span><a href=\"https:\/\/www.ketchum.edu\/directory\/miriam-morkos\"><span style=\"font-weight: 400;\">Miriam B. Morkos<\/span><\/a><span style=\"font-weight: 400;\">, Pharm.D. of <\/span><a href=\"https:\/\/www.ketchum.edu\/\"><span style=\"font-weight: 400;\">Marshall B. Ketchum University<\/span><\/a><span style=\"font-weight: 400;\">. In fact, <\/span><a href=\"https:\/\/www.sciencedirect.com\/science\/article\/abs\/pii\/S0168822718316814\"><span style=\"font-weight: 400;\">a 2019 study<\/span><\/a><span style=\"font-weight: 400;\"> demonstrated that this combination therapy is more effective than glimepiride alone, while <\/span><a href=\"https:\/\/pmc.ncbi.nlm.nih.gov\/articles\/PMC4234234\/\"><span style=\"font-weight: 400;\">a study from 2014<\/span><\/a><span style=\"font-weight: 400;\"> showed that it\u2019s more effective than metformin uptitration (the gradual increase of dosage).<\/span><\/p>\n<p><span style=\"font-weight: 400;\">That said, it\u2019s not recommended for every patient, specifically those looking to lose weight. \u201cMetformin and glimepiride should not be combined for [the purpose of] weight loss,\u201d Drs. Roman and Morkos say. \u201cAlthough metformin is considered weight-neutral with the potential for weight loss, glimepiride is associated with weight gain.\u201d<\/span><\/p>\n<p><span style=\"font-weight: 400;\">So, in patients whose high blood sugar hasn\u2019t responded to monotherapy, a glimepiride-metformin combination might be a good option\u2014as long as they\u2019re not expecting weight loss. That\u2019s because they work in different ways. <\/span><a href=\"https:\/\/medicine.yale.edu\/news-article\/how-a-widely-used-diabetes-medication-actually-works\/\"><span style=\"font-weight: 400;\">Metformin decreases the liver\u2019s glucose production<\/span><\/a><span style=\"font-weight: 400;\"> (gluconeogenesis), decreases glucose absorption in the intestines, and increases insulin sensitivity. Glimepiride, on the other hand, <\/span><a href=\"https:\/\/medlineplus.gov\/druginfo\/meds\/a696016.html\"><span style=\"font-weight: 400;\">works by increasing insulin production in the pancreas<\/span><\/a><span style=\"font-weight: 400;\">. Since they don\u2019t interfere with each other, these actions can work together to effectively manage blood sugar.<\/span><\/p>\n<h2 id=\"taking-glimepiride-and-metformin-together\"><span style=\"font-weight: 400;\">Taking glimepiride and metformin together<\/span><\/h2>\n<p><span style=\"font-weight: 400;\">It\u2019s important to know that you should only take glimepiride and metformin simultaneously if your healthcare provider prescribes them, and it\u2019s crucial to follow the dosage instructions exactly.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">The most common dosage for this combination is a <\/span><a href=\"https:\/\/dailymed.nlm.nih.gov\/dailymed\/fda\/fdaDrugXsl.cfm?setid=2b57dfa7-f2c2-45ca-e054-00144ff8d46c&amp;type=display\"><span style=\"font-weight: 400;\">1\u20132 mg dose of glimepiride<\/span><\/a><span style=\"font-weight: 400;\"> with <\/span><a href=\"https:\/\/www.singlecare.com\/prescription\/metformin-hcl\/dosage\"><span style=\"font-weight: 400;\">500\u20131,000 mg of metformin hydrochloride<\/span><\/a><span style=\"font-weight: 400;\">. And if you forget to take a dose, never make up for it by taking a double dose later; simply take the next scheduled dose as usual.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Your healthcare provider may want to test blood and urine glucose levels throughout glimepiride and metformin combination treatment. As with many diabetes medications, accompanying lifestyle changes\u2014like diet and exercise\u2014can also help manage blood sugar.<\/span><\/p>\n<h3 id=\"side-effects-of-glimepiride-with-metformin\"><span style=\"font-weight: 400;\">Side effects of glimepiride with metformin<\/span><\/h3>\n<p><span style=\"font-weight: 400;\">Glimepiride belongs to a drug class called sulfonylureas, which can increase the risk of hypoglycemia. \u201cSince glimepiride is a secretagogue that stimulates the release of insulin, it is associated with a risk of hypoglycemia, according to Drs. Roman and Morkos. The risk of hypoglycemia is increased when combined with other diabetes medications.\u201d Therefore, \u201cclinicians should inquire about the occurrence of any hypoglycemic events [in patients] while on glimepiride, and this medication should be used with caution in those with risk for hypoglycemia.\u201d<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Other than that, the side effects associated with taking these two together are primarily the same as the side effects of each one individually (see below). In some cases, these overlap and may cause an amplified version of that side effect. Of course, every patient is different, so not everyone experiences the same effects, and some might not experience any at all.<\/span><\/p>\n<h2 id=\"is-glimepiride-or-metformin-better-for-diabetes\"><span style=\"font-weight: 400;\">Is glimepiride or metformin better for diabetes?<\/span><\/h2>\n<p><span style=\"font-weight: 400;\">Both drugs are approved for the <\/span><a href=\"https:\/\/www.singlecare.com\/conditions\/type-2-diabetes-treatment-and-medications\"><span style=\"font-weight: 400;\">treatment of Type 2 diabetes mellitus<\/span><\/a><span style=\"font-weight: 400;\">, and both drugs have been proven effective. But which one is better? Metformin is recommended as a first-line treatment, while <\/span><a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/35185350\/\"><span style=\"font-weight: 400;\">glimepiride typically is not<\/span><\/a><span style=\"font-weight: 400;\">. But the answer can sometimes be more complicated than it might initially seem, since the best option for each person depends on various factors.<\/span><\/p>\n<h3 id=\"drug-class\"><span style=\"font-weight: 400;\">Drug class<\/span><\/h3>\n<p><span style=\"font-weight: 400;\">As we\u2019ve established, metformin and glimepiride are from two different drug classes. <\/span><a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/8829014\/\"><span style=\"font-weight: 400;\">Metformin is a biguanide<\/span><\/a><span style=\"font-weight: 400;\">, a drug class that decreases glucose production and absorption, while <\/span><a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/9793597\/\"><span style=\"font-weight: 400;\">glimepiride is a sulfonylurea<\/span><\/a><span style=\"font-weight: 400;\">, which <\/span><a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/12475777\/\"><span style=\"font-weight: 400;\">stimulates insulin production<\/span><\/a><span style=\"font-weight: 400;\">. Some people may respond better to one than the other or have preexisting conditions that prohibit the use of certain drug classes.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">On top of that, metformin is <\/span><a href=\"https:\/\/pmc.ncbi.nlm.nih.gov\/articles\/PMC5552844\/\"><span style=\"font-weight: 400;\">sometimes prescribed off-label to treat Type 1 diabetes<\/span><\/a><span style=\"font-weight: 400;\">, while glimepiride is only suitable for Type 2. Healthcare providers make the call on which type of diabetes medication is best for their specific patients.<\/span><\/p>\n<h3 id=\"side-effects\"><span style=\"font-weight: 400;\">Side effects<\/span><\/h3>\n<p><span style=\"font-weight: 400;\">Side effects might make one of these drugs unappealing or even unusable for certain people, so it\u2019s important to review them beforehand. You should check with your prescribing healthcare provider if you are unsure whether you have any of these conditions.<\/span><\/p>\n<h3 id=\"other-warnings-and-precautions\"><span style=\"font-weight: 400;\">Other warnings and precautions<\/span><\/h3>\n<p><span style=\"font-weight: 400;\">Some populations could be more susceptible to low blood sugar than others, and those individuals should use glimepiride with caution. People who have liver problems, a recent heart attack, severe infections, or other kidney issues should take metformin with caution.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Metformin is typically considered <\/span><a href=\"https:\/\/www.ncbi.nlm.nih.gov\/books\/NBK582828\/\"><span style=\"font-weight: 400;\">safe to take while pregnant or breastfeeding<\/span><\/a><span style=\"font-weight: 400;\">. Glimepiride is often considered safe for pregnancy but may not be safe with breastfeeding since there\u2019s very little information on how it affects breast milk and breastfed infants.<\/span><\/p>\n<h3 id=\"price\"><span style=\"font-weight: 400;\">Price<\/span><\/h3>\n<p><span style=\"font-weight: 400;\">Neither medication is particularly pricey, although glimepiride might be slightly less expensive. Without insurance coverage, the average out-of-pocket cost for a one-month supply of metformin is around $170, while glimepiride\u2019s average cash price is $22.
