Some<\/span> <a href=\"https:\/\/www.heart.org\/en\/health-topics\/cholesterol\/prevention-and-treatment-of-high-cholesterol-hyperlipidemia\/cholesterol-medications\"><span style=\"font-weight: 400;\">options include<\/span><\/a><span style=\"font-weight: 400;\">:<\/span><\/p>\n<p><a href=\"https:\/\/www.singlecare.com\/prescription\/repatha\"><span style=\"font-weight: 400;\">Repatha<\/span><\/a><span style=\"font-weight: 400;\"> (evolocumab) is a brand-name medication that is FDA approved to lower cholesterol and reduce the risk of heart attacks and strokes, by helping to restore blood flow to the heart in individuals with heart disease. Similar to statins, Repatha is a <\/span><a href=\"https:\/\/www.singlecare.com\/conditions\/high-cholesterol-treatment-and-medications\"><span style=\"font-weight: 400;\">cholesterol-lowering<\/span><\/a><span style=\"font-weight: 400;\"> prescription drug. However, these medications work differently and belong to different drug classes. Statins are the first-line treatment for high cholesterol. If statins alone don\u2019t lower your low-density lipoprotein (LDL) cholesterol levels, or if they cause unpleasant side effects, your provider may recommend Repatha as a treatment option. You can take Repatha alongside statins, such as<\/span> <a href=\"https:\/\/www.singlecare.com\/prescription\/lipitor\"><span style=\"font-weight: 400;\">Lipitor<\/span><\/a><span style=\"font-weight: 400;\"> (atorvastatin) or <\/span><a href=\"https:\/\/www.singlecare.com\/prescription\/crestor\"><span style=\"font-weight: 400;\">Crestor<\/span><\/a><span style=\"font-weight: 400;\"> (rosuvastatin), or on its own because it works differently.<\/span><\/p>\n<h2 id=\"is-repatha-a-statin\"><span style=\"font-weight: 400;\">Is Repatha a statin?<\/span><\/h2>\n<p><span style=\"font-weight: 400;\">No, Repatha is not a statin. Statins work by blocking an enzyme called HMG-CoA reductase, which is involved in cholesterol production in the liver. Repatha works by enhancing the removal of LDL cholesterol from circulation. \u201cRepatha is a <\/span><a href=\"https:\/\/www.singlecare.com\/drug-classes\/pcsk9-inhibitors\"><span style=\"font-weight: 400;\">PCSK9 inhibitor<\/span><\/a><span style=\"font-weight: 400;\">. PCSK9 is a protein that recycles receptors for the bad cholesterol. When we block this protein, we increase the receptors for the bad cholesterol. When we increase those, more cholesterol can leave the blood,\u201d explains <\/span><a href=\"https:\/\/www.providence.org\/doctors\/cardiology\/ca\/santa-monica\/rigved-tadwalkar-1679885883\"><span style=\"font-weight: 400;\">Rigved Tadwalkar, MD<\/span><\/a><span style=\"font-weight: 400;\">, a board-certified cardiologist and medical director of the Cardiac Rehabilitation Center at Providence Saint John\u2019s Health Center in Santa Monica, California.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Because Repatha is in a completely different drug class than statins, it works for people who can\u2019t tolerate statins. \u201cThis is my go-to medication when a statin doesn\u2019t work for the patient,\u201d explains <\/span><a href=\"https:\/\/www.memorialcare.org\/providers\/yu-ming-ni\"><span style=\"font-weight: 400;\">Yu-Ming Ni, MD<\/span><\/a><span style=\"font-weight: 400;\">, board-certified cardiologist and lipidologist at MemorialCare Heart and Vascular Institute at Orange Coast Medical Center in Fountain Valley, California. \u201cWhile I typically prescribe Repatha with a statin, there are some patients\u2014around 9%\u2014that cannot tolerate statins because they have <\/span><a href=\"https:\/\/www.singlecare.com\/blog\/statin-side-effects\/\"><span style=\"font-weight: 400;\">muscle side effects<\/span><\/a><span style=\"font-weight: 400;\">. Repatha works quite well and is one of the most effective cholesterol-reducing medications.\u201d That\u2019s not to say Repatha and other PCSKP inhibitors like <\/span><a href=\"https:\/\/www.singlecare.com\/prescription\/praluent\"><span style=\"font-weight: 400;\">Praluent<\/span><\/a><span style=\"font-weight: 400;\"> (alirocumab) are better than statins; they just work differently.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Additionally, Repatha can help people whose cholesterol is still high when taking statins. \u201cRepatha is a really important medication because it allows people to lower their dosages of the statins but still lowers their cholesterol,\u201d Dr. Tadwalkar explains. Taking a lower dosage may help to lower your risk of side effects from statins.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">While Repatha may be more tolerable than statins in some individuals, the medication is not without side effects and risks.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">According to the <\/span><a href=\"https:\/\/www.repatha.com\/repatha-side-effects\"><span style=\"font-weight: 400;\">manufacturer\u2019s prescribing information<\/span><\/a><span style=\"font-weight: 400;\">, common side effects of Repatha include:<\/span><\/p>\n<ul>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Runny nose, sore throat, and other common cold symptoms<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Flu or flu-like symptoms<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Back pain<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">High blood sugar levels<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Redness, pain, or bruising at the injection site<\/span><\/li>\n<\/ul>\n<p><span style=\"font-weight: 400;\">Serious side effects of Repatha that may require hospitalization caused by allergic reactions include:<\/span><\/p>\n<ul>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Trouble breathing or swallowing<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Raised bumps (hives)<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Rash or itching<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Swelling of the face, lips, tongue, throat or arms<\/span><\/li>\n<\/ul>\n<p><span style=\"font-weight: 400;\">Pregnant or breastfeeding women should consult their healthcare provider before taking Repatha. All patients should let their provider know about any over-the-counter or prescription medications and dietary supplements they are taking to avoid possible interactions.<\/span><\/p>\n<h2 id=\"repatha-vs-statins\"><span style=\"font-weight: 400;\">Repatha vs. statins<\/span><\/h2>\n<p><span style=\"font-weight: 400;\">Repatha and statins are both cholesterol-lowering medications, but there are some key differences:<\/span><\/p>\n<ul>\n<li><b>History of use: <\/b><span style=\"font-weight: 400;\">Repatha is a relatively new medication. It first gained FDA approval in 2015. Statins first gained FDA approval for use in<\/span> <a href=\"https:\/\/jamanetwork.com\/journals\/jamacardiology\/fullarticle\/2583424\"><span style=\"font-weight: 400;\">1987<\/span><\/a><span style=\"font-weight: 400;\">, so they have a much longer history of use.<\/span><\/li>\n<li><b>Class of medication: <\/b><span style=\"font-weight: 400;\">Repatha is a type of monoclonal antibody created in a lab to bind to proprotein convertase subtilisin\/kexin type 9 (PCSK9). <\/span><a href=\"https:\/\/www.singlecare.com\/drug-classes\/statins\"><span style=\"font-weight: 400;\">Statins<\/span><\/a><span style=\"font-weight: 400;\"> are a class of medication called HMG-CoA reductase inhibitors that block the synthesis of cholesterol.<\/span><\/li>\n<li><b>Reason prescribed: <\/b><span style=\"font-weight: 400;\">Repatha is generally prescribed when a person has side effects like joint and muscle pain from statins. Your healthcare provider may also prescribe Repatha when high doses of statins are not effective, you have a high risk of cardiovascular disease, or you cannot tolerate statins. What\u2019s more, <\/span><a href=\"https:\/\/www.sciencedirect.com\/science\/article\/pii\/S1933287422002914\"><span style=\"font-weight: 400;\">research<\/span><\/a><span style=\"font-weight: 400;\"> indicates that Repatha can also lower triglycerides (a type of lipid, or fat, found in your blood). However, statins are a first-line treatment, meaning doctors often start with them.<\/span><\/li>\n<li><b>Medication form: <\/b><span style=\"font-weight: 400;\">Repatha is an injectable medication that comes in a prefilled syringe. It is given as a subcutaneous injection, typically in the upper arm, upper thigh, or stomach. Statins are an oral medication, in pill form.<\/span><\/li>\n<li><b>Side effects: <\/b><span style=\"font-weight: 400;\">Repatha\u2019s most common side effects include injection site reactions and flu-like symptoms for a few days following the injection. Statins are often well-tolerated, but they can cause several potential side effects, including muscle weakness and pain, nausea, diarrhea, fatigue, and headaches.<\/span><\/li>\n<li><b>Effectiveness:<\/b><span style=\"font-weight: 400;\"> The<\/span> <a href=\"https:\/\/www.heart.org\/en\/health-topics\/cholesterol\/prevention-and-treatment-of-high-cholesterol-hyperlipidemia\/cholesterol-medications\"><span style=\"font-weight: 400;\">American Heart Association<\/span><\/a><span style=\"font-weight: 400;\"> states that statins are the most recommended medication for people with high cholesterol because they are so effective. Repatha is also effective, especially when combined with a statin. <\/span><a href=\"https:\/\/www.ahajournals.org\/doi\/10.1161\/CIRCULATIONAHA.122.061620\"><span style=\"font-weight: 400;\">Clinical trials <\/span><\/a><span style=\"font-weight: 400;\">showed a 15% to 20% lower risk of major adverse cardiovascular events (heart attack, stroke) and a 23% lower risk of cardiovascular death in those who took Repatha compared to those who took the placebo over a longer time frame. In<\/span> <a href=\"https:\/\/www.nejm.org\/doi\/full\/10.1056\/NEJMoa1615664\"><span style=\"font-weight: 400;\">one study<\/span><\/a><span style=\"font-weight: 400;\">, researchers found that the use of evolocumab along with statins helped to lower LDL (\u201cbad\u201d) cholesterol by an average of 30 mg per deciliter. This helped to reduce the risk of cardiovascular events, such as heart attack or stroke.<\/span><\/li>\n<li><b>Reputation: <\/b><span style=\"font-weight: 400;\">Repatha is a new medication, so it is not as well known. Both Drs. Ni and Dr. Tadwalker generally praise its effectiveness. Statins have a good reputation among healthcare professionals. However, many patients fear muscle and joint pain, even though the side effect only happens to a small percentage of those taking it. In fact, <\/span><a href=\"https:\/\/www.ahajournals.org\/doi\/10.1161\/JAHA.118.011765\"><span style=\"font-weight: 400;\">2019 study<\/span><\/a><span style=\"font-weight: 400;\"> found that a leading cause of people not taking statins\u2014even when recommended by a doctor\u2014is concerns over possible side effects.<\/span><b><\/b><\/li>\n<\/ul>\n<p><span style=\"font-weight: 400;\">\t\t<div class=\"singlecare-dynamic-newsletter-wrapper sin-newsletter-wrapper\">\n\t\t\t<span class=\"heading\">Get our heart health newsletter<\/span>\n\t\t\t<form class=\"form-wrapper\" data-newsletter-form=\"1\" data-subsource=\"Heart Health Newsletter\">\n\t\t\t\t<div class=\"input-wrapper\">\n\t\t\t\t\t<label for=\"dynamic-email\">Email<\/label>\n\t\t\t\t\t<input type=\"email\" id=\"dynamic-email\" class=\"email\" required>\n\t\t\t\t<\/div>\n\t\t\t\t<div class=\"input-wrapper\">\n\t\t\t\t\t<input class=\"submit-form-btn\" type=\"submit\" value=\"Click to sign up\">\n\t\t\t\t<\/div>\n\t\t\t<\/form>\n\t\t\t<p class=\"success-message\" aria-live=\"polite\"><\/p>\n\t\t<\/div>\n\t<\/span><\/p>\n<h2 id=\"repatha-and-statins-together\"><span style=\"font-weight: 400;\">Repatha and statins together<\/span><\/h2>\n<p><span style=\"font-weight: 400;\">Repatha and statins are frequently prescribed together either for people at high risk for cardiovascular events or in those whose cholesterol levels do not lower with statins alone.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Dr. Tadwalkar says \u201cWe use them together because they are complementary and can be remarkably effective in working together since they have different mechanisms.\u201d Indeed, one<\/span> <a href=\"https:\/\/www.nejm.org\/doi\/full\/10.1056\/NEJMoa1615664\"><span style=\"font-weight: 400;\">study<\/span><\/a><span style=\"font-weight: 400;\"> noted that using both medications together worked very well in reducing cholesterol.<\/span><\/p>\n<h2 id=\"discontinuation-concerns-and-alternatives\"><span style=\"font-weight: 400;\">Discontinuation concerns and alternatives<\/span><\/h2>\n<p><span style=\"font-weight: 400;\">On June 30th, 2024, Amgen (the manufacturer of Repatha) discontinued the Repatha Pushtronex\u00ae System. However, the company continues to provide the Repatha\u00ae SureClick\u00ae Autoinjector. The decision had nothing to do with the effectiveness or safety of Repatha.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">This may have spurred rumors that Amgen would stop making Repatha. However, there is no publicly available information about the potential discontinuation of the medication.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Dr. Ni hopes these rumors that Repatha may be discontinued are just rumors. \u201cIt would be a tragedy if it does get discontinued. Repatha is a fantastic medication that has been used for a long time, and it would change things for the way I practice.\u201d<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Dr. Tadwalkar also thinks it is unlikely that Repatha will be discontinued, saying, \u201cPCSK9 inhibitors, like Repatha, have a critical role in managing cholesterol, especially in high-risk populations.
