It\u2019s available under the brand name <\/span><a href=\"https:\/\/www.singlecare.com\/prescription\/inderal-la\"><span style=\"font-weight: 400;\">Inderal<\/span><\/a><span style=\"font-weight: 400;\"> and is typically prescribed to treat high blood pressure, heart disease, and essential tremor. It\u2019s also approved to prevent migraine headaches. Although it\u2019s effective for various health conditions, it can also cause serious issues when it\u2019s not taken as instructed.<\/span><\/p>\n<h2 id=\"can-you-overdose-on-propranolol\"><span style=\"font-weight: 400;\">Can you overdose on propranolol?<\/span><\/h2>\n<p><span style=\"font-weight: 400;\">Yes, it\u2019s possible to overdose on propranolol. A plasma concentration higher than 2 micrograms per milliliter (\u03bcg\/mL) is considered toxic, and concentrations higher than 3 \u03bcg\/mL are fatal. Overdose is not particularly common, but still possible when propranolol is taken in ways other than how it\u2019s prescribed.\u00a0<\/span><\/p>\n<h2 id=\"how-beta-blocker-overdose-happens\"><span style=\"font-weight: 400;\">How beta blocker overdose happens<\/span><\/h2>\n<p><span style=\"font-weight: 400;\">Propranolol is a <\/span><a href=\"https:\/\/www.singlecare.com\/drug-classes\/beta-blockers\"><span style=\"font-weight: 400;\">beta blocker<\/span><\/a><span style=\"font-weight: 400;\">, a class of medications that <\/span><a href=\"https:\/\/www.health.harvard.edu\/heart-health\/the-many-roles-of-beta-blockers\"><span style=\"font-weight: 400;\">work by preventing beta receptors<\/span><\/a><span style=\"font-weight: 400;\"> from being stimulated by the stress hormones epinephrine and noradrenaline. As a result, the stress hormones are limited in increasing blood pressure and heart rate. But ingesting too much can cause serious consequences. The following are some ways that a beta blocker overdose from drugs like propranolol could occur.<\/span><\/p>\n<h3 id=\"not-taking-it-as-prescribed\"><span style=\"font-weight: 400;\">Not taking it as prescribed<\/span><\/h3>\n<p><span style=\"font-weight: 400;\">Many times, beta blocker self-poisoning is accidental, a result of failing to follow the healthcare provider and pharmacist\u2019s instructions. \u201cAs propranolol is often dosed two to three times a day, it can be hard to keep track of and remember whether one already took their dose or not. It happens to the best of us! This however can result in patients accidentally doubling up on a dose that had already been taken,\u201d says <\/span><a href=\"https:\/\/tourette.org\/professionals\/dr-cooper-stone-do\/\"><span style=\"font-weight: 400;\">Cooper Stone<\/span><\/a><span style=\"font-weight: 400;\">, DO, a psychiatrist in Philadelphia, Pennsylvania<\/span><span style=\"font-weight: 400;\">. \u201cSimilarly, it can be difficult to take one\u2019s medications perfectly timed evenly throughout the day. Therefore sometimes doses may be less spaced than otherwise advised.\u201d<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Although someone who forgets a dose might also try to take two doses to make up for it, this can be dangerous since it means ingesting twice as much as necessary. If you forget to take a dose of propranolol or another beta blocker, simply take the next dose at the normal time instead.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">\u201cThe risk of toxicity will depend in large part on the dose that is being prescribed and how well acclimated the patient is to the medication,\u201d Dr. Stone says. \u201cAccidentally doubling up on two 10 mg pills is less worrisome than doing the same for two 80 mg pills.\u201d<\/span><\/p>\n<h3 id=\"drug-interactions\"><span style=\"font-weight: 400;\">Drug interactions<\/span><\/h3>\n<p><span style=\"font-weight: 400;\">Propranolol may also <\/span><a href=\"https:\/\/www.singlecare.com\/blog\/propranolol-interactions\/\"><span style=\"font-weight: 400;\">interact with certain drugs<\/span><\/a><span style=\"font-weight: 400;\">, making overdose or serious side effects more likely. Dr. Stone says that accidental overdose can occur if a patient starts taking a medication that significantly reduces the metabolism of propranolol\u2014particularly if the dose of propranolol they were taking was already high.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Some medication classes that can increase the level or effect of propranolol include:<\/span><\/p>\n<ul>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Antiarrhythmics<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Antihypertensive drugs<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Antidepressants<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Certain antibiotics, antifungals, and antivirals<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Migraine drugs<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Theophylline<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Warfarin<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Alcohol<\/span><\/li>\n<\/ul>\n<p><span style=\"font-weight: 400;\">Not all of these interactions will cause severe symptoms. But some could put you at greater risk of complications. Other interactions can make propranolol less effective. Let your healthcare provider know about all prescription and over-the-counter (OTC) medications you take, including vitamins, herbal products, and supplements.<\/span><\/p>\n<h3 id=\"misreading-labels\"><span style=\"font-weight: 400;\">Misreading labels<\/span><\/h3>\n<p><span style=\"font-weight: 400;\">Every prescription drug\u2019s label contains important information on how to take it. Always know the dosage of each pill, how often to take it, and what time of day to take it. This is especially important for anyone who is using propranolol tablets together with other medications that affect the heart\u2014like <\/span><a href=\"https:\/\/www.singlecare.com\/drug-classes\/calcium-channel-blockers\"><span style=\"font-weight: 400;\">calcium channel blockers<\/span><\/a><span style=\"font-weight: 400;\"> or <\/span><a href=\"https:\/\/www.singlecare.com\/drug-classes\/tricyclic-antidepressants\"><span style=\"font-weight: 400;\">tricyclic antidepressants<\/span><\/a><span style=\"font-weight: 400;\">\u2014since they may increase the risk of serious side effects.<\/span><\/p>\n<h3 id=\"diet\"><span style=\"font-weight: 400;\">Diet<\/span><\/h3>\n<p><span style=\"font-weight: 400;\">While it\u2019s not necessarily a serious overdose risk factor, diet can also influence how someone responds to a drug. For example, Dr. Stone says high-protein meals can increase the medication\u2019s absorption. \u201cThis can be an issue if one is on a higher dose and normally takes this medication consistently without food but then decides to take it with a meal,\u201d he says. \u201cThis is not to say you should not take propranolol with food; rather, one should just be consistent with taking it with or without food.\u201d\u00a0<\/span><\/p>\n<h2 id=\"propranolol-overdose-symptoms\"><span style=\"font-weight: 400;\">Propranolol overdose symptoms<\/span><\/h2>\n<p><span style=\"font-weight: 400;\">Recognizing the <\/span><a href=\"https:\/\/www.mountsinai.org\/health-library\/poison\/beta-blockers-overdose\"><span style=\"font-weight: 400;\">symptoms of propranolol overdose<\/span><\/a><span style=\"font-weight: 400;\"> is important, so you know when to seek immediate treatment. Symptoms include:<\/span><\/p>\n<ul>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Lightheadedness<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Low blood pressure, which could lead to shock if too low<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Irregular, fast, or slow heartbeat<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Shortness of breath and swelling of the legs, indicating heart failure<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Trouble breathing<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Double or blurred vision<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Weakness<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Nervousness<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Excessive sweating<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Drowsiness<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Confusion<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Seizures<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Fever<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Coma\u00a0<\/span><\/li>\n<\/ul>\n<p><span style=\"font-weight: 400;\">\u201cPropranolol is a lipophilic beta blocker, which may easily cross the blood-brain barrier and may cause seizures in overdose situations,\u201d according to <\/span><a href=\"https:\/\/manhattancardiology.com\/team\/dr-randy-gould\/\"><span style=\"font-weight: 400;\">Randy Gould, MD<\/span><\/a><span style=\"font-weight: 400;\">, a cardiologist at Manhattan Cardiology and contributor to<\/span><a href=\"http:\/\/www.labfinder.com\/\"> <span style=\"font-weight: 400;\">www.labfinder.com<\/span><\/a><span style=\"font-weight: 400;\">. Other symptoms of propranolol poisoning may include weakness, fatigue, cough, lightheadedness, blurred or double vision, difficulty breathing, fainting, or even cardiac arrest.<\/span><\/p>\n<h2 id=\"propranolol-overdose-treatment\"><span style=\"font-weight: 400;\">Propranolol overdose treatment<\/span><\/h2>\n<p><span style=\"font-weight: 400;\">At the hospital emergency department, doctors have a couple of different ways of treating beta blocker poisoning. \u201cTreatment of propranolol overdose is primarily supportive and includes measures to accelerate the removal of the drug (e.g., gastric lavage, activated charcoal, and magnesium citrate), as well as treatment to reverse the drug\u2019s severe hemodynamic effects,\u201d Dr. Gould says.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Essentially, the doctors want to get the propranolol out of the person\u2019s system as quickly as possible and get the cardiovascular system functioning normally again. Activated charcoal, for example, <\/span><a href=\"https:\/\/www.poison.org\/articles\/activated-charcoal\"><span style=\"font-weight: 400;\">binds to substances in the stomach<\/span><\/a><span style=\"font-weight: 400;\"> so they can\u2019t enter the rest of the body. And gastric lavage\u2014commonly known as stomach pumping\u2014physically removes the drug from the body through a tube.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Clinicians may also administer certain emergency medicines. According to Dr. Gould, \u201cepinephrine, atropine, isoproterenol, dopamine, and glucagon have all been recommended as therapeutic interventions.\u201d Glucagon, for example, is <\/span><a href=\"https:\/\/my.clevelandclinic.org\/health\/articles\/22283-glucagon\"><span style=\"font-weight: 400;\">a hormone that regulates blood sugar<\/span><\/a><span style=\"font-weight: 400;\">, and also increases the heart rate and blood pressure\u2014so it may work against the low blood pressure and slow heart rate caused by beta blocker toxicity. Isoproterenol is used for a variety of heart conditions and can increase the heart\u2019s strength in contractions.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">In fact, <\/span><a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/30916882\/\"><span style=\"font-weight: 400;\">a 2019 study on 2,967 cases<\/span><\/a><span style=\"font-weight: 400;\"> of beta blocker toxicity showed that treatments like activated charcoal, gastric lavage, temporary pacemakers, glucagon, intubation for respiratory insufficiency, and cardiopulmonary resuscitation prevented a fatality in every case.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Additionally, an older <\/span><a href=\"https:\/\/www.sciencedirect.com\/science\/article\/pii\/0002934386908417\"><span style=\"font-weight: 400;\">case report from <\/span><i><span style=\"font-weight: 400;\">The American Journal of Medicine<\/span><\/i><\/a><span style=\"font-weight: 400;\"> showed that high-dose glucagon and isoproterenol treatment can work for severe propranolol overdose. And, <\/span><a href=\"https:\/\/pmc.ncbi.nlm.nih.gov\/articles\/PMC9511288\/\"><span style=\"font-weight: 400;\">a 2022 case report from <\/span><i><span style=\"font-weight: 400;\">Clinical Medicine Insights<\/span><\/i><\/a> <span style=\"font-weight: 400;\">concluded that healthcare providers should administer glucagon right away for overdose patients who are resistant to standard therapy.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Regardless, it\u2019s important to get someone who might be overdosing medical help as soon as possible, so healthcare professionals can determine the best course of action.<\/span><\/p>\n<h3 id=\"if-you-think-someone-is-overdosing\"><span style=\"font-weight: 400;\">If you think someone is overdosing<\/span><\/h3>\n<p><span style=\"font-weight: 400;\">If you think someone is overdosing, call 911 immediately. It will help if you have an idea of the weight and age of the person who is overdosing as well as an idea of when the person last took the medication and how much they took. Try to keep them awake if possible, lay them on their side, and wait for paramedics to arrive.
