Many conditions and medications affect the absorption of levothyroxine, and problems can arise if the body has insufficient thyroid hormone.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">One variable that impacts levothyroxine absorption is stomach acidity. If there isn\u2019t enough acid in the stomach, orally administered thyroid hormone may not be absorbed as well. The fear is that blood levels of thyroxine could then be inadequate. An <\/span><a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/25040647\/\"><span style=\"font-weight: 400;\">observational study<\/span><\/a><span style=\"font-weight: 400;\"> and a <\/span><a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/17669709\/\"><span style=\"font-weight: 400;\">retrospective review<\/span><\/a><span style=\"font-weight: 400;\"> both found that <\/span><a href=\"https:\/\/www.singlecare.com\/blog\/tsh-test\/\"><span style=\"font-weight: 400;\">thyroid-stimulating hormone (TSH)<\/span><\/a><span style=\"font-weight: 400;\"> levels were higher in patients taking a PPI with their levothyroxine compared to those taking levothyroxine alone. This finding indicates that thyroid hormone levels were lower since they are <\/span><a href=\"https:\/\/www.singlecare.com\/blog\/levothyroxine-tsh-high\/\"><span style=\"font-weight: 400;\">inversely correlated with TSH<\/span><\/a><span style=\"font-weight: 400;\">.<\/span><\/p>\n<h2 id=\"taking-levothyroxine-and-omeprazole-together\"><span style=\"font-weight: 400;\">Taking levothyroxine and omeprazole together<\/span><\/h2>\n<p><span style=\"font-weight: 400;\">In essence, the two drugs have a negative interaction, but it\u2019s not significant enough to forbid their concurrent use in most cases. One <\/span><a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/25372582\/\"><span style=\"font-weight: 400;\">randomized study<\/span><\/a><span style=\"font-weight: 400;\">, albeit small, concluded that omeprazole at a dose of 20\u201340 mg daily did not meaningfully affect levothyroxine absorption. Nonetheless, it is important to be carefully monitored by a healthcare provider when taking both drugs.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Ensuring proper thyroid hormone levels means checking TSH levels regularly. When levothyroxine therapy is begun or changed, this usually amounts to a TSH blood test every four to six weeks until the effective dose is determined. Likewise, healthcare providers may recommend checking TSH levels every four to six weeks when starting, increasing, or stopping omeprazole. Always let the healthcare provider managing your hypothyroidism know if you start taking omeprazole, whether you get it over the counter or are prescribed it by another healthcare provider.\u00a0<\/span><\/p>\n<h3 id=\"how-far-apart-should-levothyroxine-and-omeprazole-be-taken\"><span style=\"font-weight: 400;\">How far apart should levothyroxine and omeprazole be taken?\u00a0<\/span><\/h3>\n<p><span style=\"font-weight: 400;\">The general recommendation is to take levothyroxine on an empty stomach and at least 30\u201360 minutes before breakfast since many foods can negatively impact its absorption. Supplements and other drugs can, too. Calcium, magnesium, iron, and antacids are common examples of pills that should not be taken at the same time as levothyroxine and may best be separated by four hours.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Omeprazole and other PPIs, including <\/span><a href=\"https:\/\/www.singlecare.com\/prescription\/nexium\"><span style=\"font-weight: 400;\">Nexium<\/span><\/a><span style=\"font-weight: 400;\"> (esomeprazole), <\/span><a href=\"https:\/\/www.singlecare.com\/prescription\/prevacid\"><span style=\"font-weight: 400;\">Prevacid<\/span><\/a><span style=\"font-weight: 400;\"> (lansoprazole), and <\/span><a href=\"https:\/\/www.singlecare.com\/prescription\/protonix\"><span style=\"font-weight: 400;\">Protonix<\/span><\/a><span style=\"font-weight: 400;\"> (pantoprazole), reduce stomach acid for 24 hours. In a study looking at 30 patients taking the PPI pantoprazole, the <\/span><a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/25372582\/\"><span style=\"font-weight: 400;\">timing of the PPI dose<\/span><\/a><span style=\"font-weight: 400;\"> did not change how much of an impact it had on levothyroxine therapy. As such, it\u2019s not likely to matter whether someone takes omeprazole at the same time of day as levothyroxine or not.\u00a0<\/span><\/p>\n<h2 id=\"omeprazole-and-levothyroxine-comparison\"><span style=\"font-weight: 400;\">Omeprazole and levothyroxine comparison<\/span><\/h2>\n<p><span style=\"font-weight: 400;\">In regard to their drug classes, their uses, and their <\/span><a href=\"https:\/\/www.singlecare.com\/blog\/omeprazole-side-effects\/\"><span style=\"font-weight: 400;\">side effects<\/span><\/a><span style=\"font-weight: 400;\">, omeprazole and levothyroxine are very different. Omeprazole reduces stomach acid, which helps with GERD and peptic ulcers, but less gastric acidity may increase the risk of infection or nutrient malabsorption. Levothyroxine provides thyroid hormone to correct hypothyroidism, but if it is not dosed and monitored appropriately, excess or insufficient thyroid hormone could result and cause <\/span><a href=\"https:\/\/www.singlecare.com\/blog\/hypothyroidism-vs-hyperthyroidism\/\"><span style=\"font-weight: 400;\">hyperthyroidism or hypothyroidism<\/span><\/a><span style=\"font-weight: 400;\"> symptoms.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Both drugs are generic, but brand-name formulations are available.
