Hundreds of clinical trials are currently testing GLP-1 treatments for various conditions. With a growing demand for GLP-1 agonists, pharmacists can help guide healthcare providers and patients about proper use, potential side effects, and what to expect.<\/span><\/p>\n<h2><span style=\"font-weight: 400;\">Current approved uses of GLP-1 agonists<\/span><\/h2>\n<p><span style=\"font-weight: 400;\">GLP-1 receptor agonists have gained FDA approval for several medical conditions. They work by mimicking the GLP-1 hormone, which helps improve insulin secretion, suppress glucagon release, and slow gastric emptying.\u00a0<\/span><\/p>\n<h3><span style=\"font-weight: 400;\">GLP-1 medications for Type 2 diabetes<\/span><\/h3>\n<p><span style=\"font-weight: 400;\">Most GLP-1 agonists are approved primarily for the management of <\/span><a href=\"https:\/\/www.singlecare.com\/blog\/diabetes-support\/\"><span style=\"font-weight: 400;\">Type 2 diabetes<\/span><\/a><span style=\"font-weight: 400;\">. Most patients see a significant drop in their A1C levels within <\/span><a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/41019499\/\"><span style=\"font-weight: 400;\">three to six months<\/span><\/a><span style=\"font-weight: 400;\"> of starting treatment, with many <\/span><a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/30896359\/\"><span style=\"font-weight: 400;\">staying adherent<\/span><\/a><span style=\"font-weight: 400;\"> after an early response.\u00a0<\/span><\/p>\n<p><a href=\"https:\/\/www.singlecare.com\/prescription\/mounjaro\"><span style=\"font-weight: 400;\">Mounjaro (tirzepatide)<\/span><\/a><span style=\"font-weight: 400;\"> is a similar option for Type 2 diabetes that is classified as a dual GIP\/GLP-1 receptor agonist. It mimics two gut hormones and has been shown to be more effective than semaglutide in managing blood sugar control. Still, many healthcare providers might not prescribe tirzepatide as a first-line treatment for Type 2 diabetes due to costs and available alternatives.\u00a0<\/span><\/p>\n<p><b>RELATED:<\/b><a href=\"https:\/\/www.singlecare.com\/blog\/generic-glp-1\/\"> <b>Are there generic GLP-1 drugs?<\/b><\/a><\/p>\n<h3><span style=\"font-weight: 400;\">GLP-1 medications for weight loss<\/span><\/h3>\n<p><a href=\"https:\/\/www.singlecare.com\/prescription\/wegovy\"><span style=\"font-weight: 400;\">Wegovy (semaglutide)<\/span><\/a><span style=\"font-weight: 400;\"> is<\/span><a href=\"https:\/\/www.singlecare.com\/blog\/news\/wegovy-semaglutide-fda-approval\/\"> <span style=\"font-weight: 400;\">approved<\/span><\/a><span style=\"font-weight: 400;\"> for chronic weight management, as it helps patients feel full longer and manage appetite. <\/span><a href=\"https:\/\/www.singlecare.com\/prescription\/zepbound\"><span style=\"font-weight: 400;\">Zepbound (tirzepatide)<\/span><\/a><span style=\"font-weight: 400;\"> is another similar option to support weight loss in patients with obesity. Clinical trials have shown that patients can lose <\/span><a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/36321278\/\"><span style=\"font-weight: 400;\">15% of their body weight on semaglutide and up to 20% on tirzepatide<\/span><\/a><span style=\"font-weight: 400;\">. To qualify for these medications, per the FDA, patients must have:<\/span><\/p>\n<ul>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">BMI \u2265 30, or<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">BMI \u2265 27 with at least one weight-related condition, such as high cholesterol, high blood pressure, or Type 2 diabetes<\/span><\/li>\n<\/ul>\n<p><span style=\"font-weight: 400;\">Wegovy has also been approved as an oral tablet for patients who prefer not to use an injection. Foundayo (orforglipron) is another oral tablet approved by the FDA for weight loss. Unlike oral semaglutide, it can be taken at any time of day, <\/span><a href=\"https:\/\/www.accessdata.fda.gov\/drugsatfda_docs\/label\/2026\/220934Orig1s000lbl.pdf\"><span style=\"font-weight: 400;\">with or without food<\/span><\/a><span style=\"font-weight: 400;\">.\u00a0<\/span><\/p>\n<h3><span style=\"font-weight: 400;\">GLP-1 medications for cardiovascular risks<\/span><\/h3>\n<p><span style=\"font-weight: 400;\">Some GLP-1 agonists are approved to lower the risk of heart attacks, strokes, and cardiovascular death, with the list of GLP-1s for this use continuing to grow. Victoza was the first GLP-1 approved to lower <\/span><a href=\"https:\/\/www.singlecare.com\/blog\/counseling-patients-on-heart-health\/\"><span style=\"font-weight: 400;\">cardiovascular risks<\/span><\/a><span style=\"font-weight: 400;\"> in adults with Type 2 diabetes and heart disease. Trulicity, Wegovy, and Ozempic have gained similar approvals, with Wegovy standing out for its use in adults with cardiovascular disease who have obesity.\u00a0<\/span><\/p>\n<h3><span style=\"font-weight: 400;\">GLP-1 medications for sleep apnea<\/span><\/h3>\n<p><a href=\"https:\/\/www.fda.gov\/news-events\/press-announcements\/fda-approves-first-medication-obstructive-sleep-apnea\"><span style=\"font-weight: 400;\">Zepbound is the first medication approved for obstructive sleep apnea (OSA)<\/span><\/a><span style=\"font-weight: 400;\">. More specifically, it is approved for moderate to severe OSA in adults with obesity, with the SURMOUNT-OSA trial showing that tirzepatide directly lowered the apnea-hypopnea index. Other GLP-1 agonists are also being studied for OSA and could see FDA approvals for this use in the future.\u00a0<\/span><\/p>\n<h3><span style=\"font-weight: 400;\">GLP-1 medications for chronic kidney disease<\/span><\/h3>\n<p><span style=\"font-weight: 400;\">Ozempic is approved for reducing the risk of worsening kidney disease and cardiovascular death in adults with <\/span><a href=\"https:\/\/www.singlecare.com\/blog\/news\/ozempic-kidney-disease-fda-approval\/\"><span style=\"font-weight: 400;\">chronic kidney disease<\/span><\/a><span style=\"font-weight: 400;\"> and Type 2 diabetes. The <\/span><a href=\"https:\/\/www.nejm.org\/doi\/abs\/10.1056\/NEJMoa2403347\"><span style=\"font-weight: 400;\">FLOW trial<\/span><\/a><span style=\"font-weight: 400;\"> found that the risk of major kidney disease events and kidney-related death was 24% lower with semaglutide 1 mg than with placebo. Other GLP-1 medications could follow suit for approvals, with a meta-analysis showing that GLP-1 agonists significantly reduced the <\/span><a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/39608381\/\"><span style=\"font-weight: 400;\">risk of serious kidney complications<\/span><\/a><span style=\"font-weight: 400;\"> in people with Type 2 diabetes.<\/span><\/p>\n<h3><span style=\"font-weight: 400;\">GLP-1 medications for liver disease<\/span><\/h3>\n<p><span style=\"font-weight: 400;\">The Wegovy injection is approved for adults with noncirrhotic metabolic dysfunction-associated steatohepatitis (MASH) with moderate to advanced liver fibrosis. It\u2019s the first GLP-1 medication approved for this form of liver disease. In the <\/span><a href=\"https:\/\/www.nejm.org\/doi\/full\/10.1056\/NEJMoa2413258\"><span style=\"font-weight: 400;\">Phase 3 ESSENCE trial<\/span><\/a><span style=\"font-weight: 400;\">, MASH resolved without any worsening fibrosis in around 63% of patients on semaglutide versus 34% on placebo. Tirzepatide has shown <\/span><a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/38856224\/\"><span style=\"font-weight: 400;\">similar results<\/span><\/a><span style=\"font-weight: 400;\"> in other trials, although it\u2019s not currently approved for this purpose.\u00a0<\/span><\/p>\n<p><a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/38847460\/\"><span style=\"font-weight: 400;\">Some studies<\/span><\/a><span style=\"font-weight: 400;\"> are also exploring other dual and triple hormonal agonists to improve patient outcomes. Researchers are finding that these medications could help with heart failure, alcohol use disorder, and other conditions.\u00a0<\/span><\/p>\n<h3><span style=\"font-weight: 400;\">Heart failure<\/span><\/h3>\n<p><span style=\"font-weight: 400;\">The metabolic effects of GLP-1 medications may support improved cardiovascular outcomes through both direct and indirect effects. Recent studies show that GLP-1 agonists benefit patients who have heart failure with preserved ejection fraction (HFpEF). The <\/span><a href=\"https:\/\/www.nejm.org\/doi\/10.1056\/NEJMoa2306963\"><span style=\"font-weight: 400;\">STEP-HFpEF trial<\/span><\/a><span style=\"font-weight: 400;\">, in particular, found that semaglutide improved heart failure symptoms, walking distance, and overall quality of life in patients with HFpEF and obesity. In addition, patients on tirzepatide in the <\/span><a href=\"https:\/\/www.nejm.org\/doi\/10.1056\/NEJMoa2410027\"><span style=\"font-weight: 400;\">SUMMIT trial<\/span><\/a><span style=\"font-weight: 400;\"> experienced improved symptoms and a reduced risk of cardiovascular death or worsening heart failure. Still, further research can help confirm these results, as GLP-1s are not approved for this use yet.<\/span><\/p>\n<h3><span style=\"font-weight: 400;\">Peripheral artery disease (PAD)<\/span><\/h3>\n<p><span style=\"font-weight: 400;\">Patients who struggle with PAD can look forward to potentially expanded uses of GLP1s for improving symptoms. With known benefits on metabolic health and possible direct effects\u00a0 on vascular function, GLP-1 agonists could help manage walking limitations in patients with PAD. The <\/span><a href=\"https:\/\/www.thelancet.com\/journals\/lancet\/article\/PIIS0140-6736(25)00509-4\/abstract\"><span style=\"font-weight: 400;\">Phase 3 STRIDE trial<\/span><\/a><span style=\"font-weight: 400;\"> enrolled almost 800 adults with Type 2 diabetes and PAD and found that semaglutide 1 mg once weekly improved maximum walking distance by almost 13%\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Over placebo at 52 weeks. PAD could be a potentially new approved indication for GLP-1 medications, especially in patients with diabetes.<\/span><\/p>\n<h3><span style=\"font-weight: 400;\">Alcohol use disorder<\/span><\/h3>\n<p><span style=\"font-weight: 400;\">One of the more interesting potential uses of GLP-1s is addiction. Many patients on GLP-1s may report feeling less inclined to drink alcohol, which could be due to direct GLP-1 pathways in the brain. GLP-1 agonists are believed to work in the brain\u2019s reward system to diminish cravings.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">A <\/span><a href=\"https:\/\/jamanetwork.com\/journals\/jamapsychiatry\/fullarticle\/2829811\"><span style=\"font-weight: 400;\">Phase 2 randomized controlled trial<\/span><\/a><span style=\"font-weight: 400;\"> showed that low-dose semaglutide helped reduce both alcohol cravings and the amount of alcohol participants drank. In a <\/span><a href=\"https:\/\/www.thelancet.com\/journals\/lancet\/article\/PIIS0140-6736(26)00305-3\/fulltext\"><span style=\"font-weight: 400;\">2026 trial<\/span><\/a><span style=\"font-weight: 400;\">, adults with alcohol use disorder and obesity taking semaglutide clocked in fewer heavy drinking days than those on placebo. Trials are still investigating this potential use case, with a current Phase 3 trial ongoing in U.S. veterans.\u00a0<\/span><\/p>\n<h3><span style=\"font-weight: 400;\">Polycystic ovary syndrome (PCOS)<\/span><\/h3>\n<p><span style=\"font-weight: 400;\">Studies have shown that GLP-1 agonists may also help manage metabolic problems in PCOS. One <\/span><a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/39955537\/\"><span style=\"font-weight: 400;\">systematic review and meta-analysis<\/span><\/a><span style=\"font-weight: 400;\"> found that, when used alongside standard treatments, these medications can significantly reduce body weight, BMI, and waist circumference. They also improve insulin sensitivity in women with PCOS, as shown by lower insulin resistance index (HOMA-IR) scores and fasting blood glucose.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Different pharmacological options may be used together to target various aspects of the condition, including hyperandrogenism and dyslipidemia. For example, <\/span><a href=\"https:\/\/www.singlecare.com\/prescription\/orlistat\"><span style=\"font-weight: 400;\">orlistat<\/span><\/a><span style=\"font-weight: 400;\"> has been found to lower testosterone levels and raise HDL in PCOS when used alongside GLP-1 drugs. As research continues, incorporating GLP-1 receptor agonists could be a treatment strategy for women with PCOS.<\/span><\/p>\n<p><b>RELATED: <\/b><a href=\"https:\/\/www.singlecare.com\/blog\/wegovy-for-pcos\/\"><b>Wegovy for PCOS<\/b><\/a><\/p>\n<h3><span style=\"font-weight: 400;\">Migraine<\/span><\/h3>\n<p><span style=\"font-weight: 400;\">GLP-1 receptor agonists are being looked at for <\/span><a href=\"https:\/\/headachejournal.onlinelibrary.wiley.com\/doi\/10.1111\/head.14991\"><span style=\"font-weight: 400;\">migraine<\/span><\/a><span style=\"font-weight: 400;\">, particularly in patients with obesity. However, the evidence is preliminary so far, with a small pilot study finding that liraglutide reduced the number of monthly headache days. It didn\u2019t measure the severity of migraine days.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Some of the strongest evidence for headache-related benefits of GLP-1 drugs lies in studies with patients who have idiopathic intracranial hypertension. Effects on pain signaling and inflammation are currently based on <\/span><a href=\"https:\/\/www.sciencedirect.com\/science\/article\/abs\/pii\/S0304394023003567\"><span style=\"font-weight: 400;\">animal models<\/span><\/a><span style=\"font-weight: 400;\">. As research develops, GLP-1 receptor agonists could be a complementary option for people with migraine and coexisting obesity or metabolic dysfunction.<\/span><\/p>\n<h3><span style=\"font-weight: 400;\">Neurodegenerative conditions<\/span><\/h3>\n<p><span style=\"font-weight: 400;\">GLP-1 agonists have shown promise as <\/span><a href=\"https:\/\/pmc.ncbi.nlm.nih.gov\/articles\/PMC11579408\/\"><span style=\"font-weight: 400;\">neuroprotective agents<\/span><\/a><span style=\"font-weight: 400;\"> by improving memory and neuronal survival. However, the evidence is primarily based on animal studies. Recent studies in humans have shown disappointing results, with semaglutide <\/span><a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/41865758\/\"><span style=\"font-weight: 400;\">failing to slow the progression<\/span><\/a><span style=\"font-weight: 400;\"> of Alzheimer\u2019s disease. Still, GLP-1 medications are not being ruled out for potential benefits on cognition.\u00a0<\/span><\/p>\n<h2><span style=\"font-weight: 400;\">Keeping patients and healthcare providers up to date<\/span><\/h2>\n<p><span style=\"font-weight: 400;\">With digital health platforms making GLP-1 therapies more accessible, pharmacists are even more crucial for looking out for patients\u2019 health. They have the expertise to explain key details, like proper injection techniques, side effects, and new indications, to patients and healthcare providers.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">What started with diabetes and weight loss is now extending to several potential uses, and the FDA is welcoming these new applications for approval. Pharmacists are now even more important in helping patients make sense of GLP-1s, especially since these medications don\u2019t come without potential side effects.\u00a0<\/span><\/p>\n<h2><span style=\"font-weight: 400;\">Expert takeaway<\/span><\/h2>\n<p><span style=\"font-weight: 400;\">\u201cGLP-1 drugs are no longer just diabetes and weight-loss drugs, and the approved uses are growing fast.
