That might have soured the public opinion of it, and that\u2019s understandable, but it\u2019s also important to know that fentanyl has several important, legitimate uses\u2014particularly when it comes to pain relief. Here are all the details on fentanyl\u2019s role in anesthesia and pain management.\u00a0\u00a0<\/span><\/p>\n<h2 id=\"how-fentanyl-works\"><span style=\"font-weight: 400;\">How fentanyl works<\/span><\/h2>\n<p><span style=\"font-weight: 400;\"><a href=\"https:\/\/www.singlecare.com\/prescription\/fentanyl\/what-is\">Fentanyl is an opioid analgesic<\/a>, just like tramadol, oxycodone, methadone, and codeine. That means its primary purpose is to reduce pain. It does this by attaching to opioid receptors, which are located on the parts of the brain that control pain and emotion. It blocks these receptors, interrupting signals to the brain that would trigger a pain response. However, it works a lot quicker than other opioids, and it has a short half-life, making it useful as a short-term intervention for procedures like surgeries.<\/span><\/p>\n<h2 id=\"is-fentanyl-dangerous\"><span style=\"font-weight: 400;\">Is fentanyl dangerous?<\/span><\/h2>\n<p><span style=\"font-weight: 400;\">Fentanyl is a powerful drug, and it can be very safe when it is used with the oversight of a healthcare professional. Medical professionals are extremely cautious and precise when prescribing fentanyl, and they actively monitor their patients for respiratory depression and hypoxia during fentanyl treatment.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">\u201cFentanyl is much more potent than other opioids, 100 times more than morphine. This means that doses are much smaller. We use micrograms rather than milligrams, and small amounts can have big effects,\u201d says <\/span><a href=\"https:\/\/www.uhhospitals.org\/doctors\/Marino-Ryan-1275948648?IncludeAllProviders=true&amp;view=faculty\"><span style=\"font-weight: 400;\">Ryan Marino, MD<\/span><\/a><span style=\"font-weight: 400;\">, Associate Professor at the Case Western Reserve University School of Medicine.<\/span><\/p>\n<p><a href=\"https:\/\/onlinelibrary.wiley.com\/doi\/abs\/10.1111\/aas.12662\"><span style=\"font-weight: 400;\">A study from <\/span><i><span style=\"font-weight: 400;\">Acta Anaesthesiologica Scandinavica<\/span><\/i><\/a><span style=\"font-weight: 400;\"> on intravenous fentanyl administered by ambulance personnel found that it was both safe and effective. Likewise, <\/span><a href=\"https:\/\/journals.lww.com\/anesthesia-analgesia\/fulltext\/2007\/11000\/The_Safety_and_Efficacy_of_Fentanyl_Iontophoretic.42.aspx\"><span style=\"font-weight: 400;\">an older study from <\/span><i><span style=\"font-weight: 400;\">Anesthesia &amp; Analgesia<\/span><\/i><\/a><span style=\"font-weight: 400;\"> found that it was both effective and well-tolerated when administered for post-surgical pain.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">But outside the hospital, where drugs aren\u2019t accurately measured or used as cautiously, it\u2019s a different story. \u201cWhen fentanyl is present in unregulated form\u2014like illicit fentanyl sold on the street\u2014it is impossible to know what the dose or amount is, and much easier for tiny changes to cause significant effects, including overdose,\u201d Dr. Marino says. \u201cThe difference is that pharmaceutical fentanyl comes in a known, labeled dose and is free of adulterants and impurities (or other drugs). Additionally, the use in healthcare is always in monitored settings to ensure that adverse effects don\u2019t happen or are treated promptly.\u201d<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Another significant issue with illegally manufactured fentanyl is that it\u2019s often found in counterfeit pills, which resemble other prescription drugs. <\/span><a href=\"https:\/\/nida.nih.gov\/publications\/drugfacts\/fentanyl\"><span style=\"font-weight: 400;\">Dealers may mix it with drugs<\/span><\/a><span style=\"font-weight: 400;\"> like cocaine, methamphetamine, or heroin because it\u2019s potent and cheap, delivering an intense high at a low cost. But that mixing is imprecise, skyrocketing the risk of overdose and death. In fact, in 2022, <\/span><a href=\"https:\/\/www.dea.gov\/alert\/dea-laboratory-testing-reveals-6-out-10-fentanyl-laced-fake-prescription-pills-now-contain\"><span style=\"font-weight: 400;\">the Drug Enforcement Administration found<\/span><\/a><span style=\"font-weight: 400;\"> that 60% of fentanyl-laced counterfeit prescription pills contained a lethal dose of the drug.<\/span><\/p>\n<h2 id=\"what-is-fentanyl-used-for\"><span style=\"font-weight: 400;\">What is fentanyl used for?<\/span><\/h2>\n<p><span style=\"font-weight: 400;\">All of fentanyl\u2019s uses revolve around pain management, but there are several different circumstances where it can help. Here are the most common uses of medical fentanyl.<\/span><\/p>\n<h3 id=\"anesthesia\"><span style=\"font-weight: 400;\">Anesthesia<\/span><\/h3>\n<p><span style=\"font-weight: 400;\">Given its quick action and exceptional pain relief, fentanyl works very well in anesthesiology. In fact, it has largely <\/span><a href=\"https:\/\/www.sciencedirect.com\/science\/article\/pii\/S0885392405000357\"><span style=\"font-weight: 400;\">replaced morphine as the go-to opioid anesthetic<\/span><\/a><span style=\"font-weight: 400;\">. \u201cAnesthesiologists use it every single day on almost every patient undergoing surgery,\u201d says Zachary Fisk, MD, MBA, founder of <\/span><a href=\"http:\/\/www.acutepaintherapies.com\/\"><span style=\"font-weight: 400;\">Acute Pain Therapies<\/span><\/a><span style=\"font-weight: 400;\">. \u201cIt is thought of by anesthesiologists almost like a primary \u2018currency,\u2019 where the anesthesiologists understand how fentanyl acts very intuitively, and then use that understanding as a baseline for their interpretation of other opioid analgesics.\u201d<\/span><\/p>\n<p><span style=\"font-weight: 400;\">For anesthesia, fentanyl is typically administered intravenously. The <\/span><a href=\"https:\/\/www.ncbi.nlm.nih.gov\/books\/NBK459275\/\"><span style=\"font-weight: 400;\">dosage depends on the patient\u2019s body weight and indication<\/span><\/a><span style=\"font-weight: 400;\">, but it\u2019s usually a single 20\u201350 mcg\/kg dose\u2014administered via IV\u2014for general anesthesia and 2\u201350 mcg\/kg as an anesthesia adjunct. For individuals undergoing a complicated procedure, healthcare providers may increase the dose to 150 mcg\/kg and administer it with a muscle relaxant. Sometimes it\u2019s also <\/span><a href=\"https:\/\/onlinelibrary.wiley.com\/doi\/10.1155\/2021\/9955488\"><span style=\"font-weight: 400;\">combined with propofol<\/span><\/a><span style=\"font-weight: 400;\"> for anesthesia.<\/span><\/p>\n<h3 id=\"postoperative-and-intraoperative-pain\"><span style=\"font-weight: 400;\">Postoperative and intraoperative pain<\/span><\/h3>\n<p><span style=\"font-weight: 400;\">Opioids are widely used as surgical pain medications, but fentanyl works slightly differently than drugs like morphine and Dilaudid. Specifically, \u201cIts onset of action is faster than those agents, with an onset time of one minute and a peak effect of five minutes. This makes fentanyl extremely useful for minimizing the body&#8217;s pain response to surgical stimuli and for quickly reducing pain during and immediately after surgery and in the recovery area,\u201d Dr. Fisk says. \u201cIt is very useful to add in slowly over the course of a procedure to achieve an optimal level of analgesia for the surgery itself; this is impossible with slower onset analgesics.\u201d<\/span><\/p>\n<p><span style=\"font-weight: 400;\">For those reasons, fentanyl has become the <\/span><a href=\"https:\/\/www.jpain.org\/article\/s1526-5900(14)00905-5\/fulltext\"><span style=\"font-weight: 400;\">most common drug for intravenous, intraoperative anesthesia<\/span><\/a><span style=\"font-weight: 400;\"> in the world. After surgery, the typical dosage is 50\u2013100 mcg every one to two hours as needed. Or, with an IV, it could be 0.5\u20131.5 mcg\/kg per hour as needed.<\/span><\/p>\n<h3 id=\"epidurals\"><span style=\"font-weight: 400;\">Epidurals<\/span><\/h3>\n<p><span style=\"font-weight: 400;\">Approximately <\/span><a href=\"https:\/\/www.yalemedicine.org\/news\/epidural-anesthesia\"><span style=\"font-weight: 400;\">70\u201375% of pregnant women use epidural analgesia<\/span><\/a><span style=\"font-weight: 400;\"> during childbirth, and fentanyl is a common drug for both analgesia and epidural anesthesia. <\/span><a href=\"https:\/\/www.ajogmfm.org\/article\/S2589-9333(24)00036-3\/fulltext\"><span style=\"font-weight: 400;\">A 2024 study from the <\/span><i><span style=\"font-weight: 400;\">American Journal of Obstetrics &amp; Gynecology<\/span><\/i><\/a> <span style=\"font-weight: 400;\">found that epidural fentanyl substantially decreased labor pain during vaginal delivery, and <\/span><a href=\"https:\/\/obgyn.onlinelibrary.wiley.com\/doi\/10.1111\/ajo.12970\"><span style=\"font-weight: 400;\">a separate study from 2019<\/span><\/a><span style=\"font-weight: 400;\"> found that using fentanyl as an epidural administration didn\u2019t have any effects on breastfeeding.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">\u201cFentanyl is safe during labor and safe in any supervised medical use. Some people may be more sensitive to the effects of drugs like fentanyl or may have more adverse effects than others, and it is possible to be allergic to fentanyl products,\u201d Dr. Marino says. \u201cBy targeting the nerves at the spine it is possible to get better pain control and fewer of the systemic side effects from intravenous administration, and usually at a lower dose as well.\u201d<\/span><\/p>\n<p><span style=\"font-weight: 400;\">As always, the ideal dosage depends on the situation, although <\/span><a href=\"https:\/\/associationofanaesthetists-publications.onlinelibrary.wiley.com\/doi\/10.1111\/anae.14000\"><span style=\"font-weight: 400;\">a 2017 study published in <\/span><i><span style=\"font-weight: 400;\">Anesthesia<\/span><\/i><\/a> <span style=\"font-weight: 400;\">concluded that 50 mcg and 100 mcg doses were more effective than 20 mcg. For labor analgesia, doctors sometimes opt to combine fentanyl with a local anesthetic like bupivacaine or ropivacaine. Fentanyl\/bupivacaine <\/span><a href=\"https:\/\/www.sciencedirect.com\/science\/article\/pii\/088539249290022A\"><span style=\"font-weight: 400;\">has shown effectiveness<\/span><\/a><span style=\"font-weight: 400;\">, although <\/span><a href=\"https:\/\/pmc.ncbi.nlm.nih.gov\/articles\/PMC4616487\/\"><span style=\"font-weight: 400;\">a 2015 study<\/span><\/a><span style=\"font-weight: 400;\"> found that fentanyl\/ropivacaine had a lower incidence of motor blocks.<\/span><\/p>\n<h3 id=\"acute-pain\"><span style=\"font-weight: 400;\">Acute pain<\/span><\/h3>\n<p><span style=\"font-weight: 400;\">In some cases, healthcare providers might use fentanyl off-label to treat \u201cbreakthrough pain,\u201d acute pain that occurs despite taking other opioids\u2014often in cancer patients who are undergoing treatment. <\/span><a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/27744534\/\"><span style=\"font-weight: 400;\">Studies have shown<\/span><\/a><span style=\"font-weight: 400;\"> that using fentanyl this way is both safe and effective (when it\u2019s administered by a healthcare provider). In addition to intramuscularly or intravenously, doctors may administer it as a nasal spray, lozenge, or buccal tablet. Intranasally, the typical <\/span><a href=\"https:\/\/www.singlecare.com\/prescription\/fentanyl\/dosage\"><span style=\"font-weight: 400;\">dose of fentanyl<\/span><\/a> <span style=\"font-weight: 400;\">is 1\u20132 mcg\/kg per hour as needed, with a <a href=\"https:\/\/www.singlecare.com\/prescription\/fentanyl\/dosage\">maximum dosage of 100 mcg<\/a>. Lozenge doses can be 200\u20131600 mcg, while buccal tablets are 100\u2013800 mcg.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Thanks to fentanyl\u2019s rapid onset, it\u2019s also effective for acute pain caused by trauma. <\/span><a href=\"https:\/\/journals.lww.com\/journaloftraumanursing\/abstract\/2013\/01000\/morphine_versus_fentanyl_for_pain_due_to_traumatic.4.aspx\"><span style=\"font-weight: 400;\">Research has confirmed<\/span><\/a><span style=\"font-weight: 400;\"> that it\u2019s just as effective as morphine but works even faster. <\/span><a href=\"https:\/\/pmc.ncbi.nlm.nih.gov\/articles\/PMC10179230\/\"><span style=\"font-weight: 400;\">One study from the <\/span><i><span style=\"font-weight: 400;\">Journal of Clinical Medicine<\/span><\/i><\/a> <span style=\"font-weight: 400;\">even said it\u2019s \u201cclearly superior to other opioids in initial resuscitation and treatment as it has minimal effects on hemodynamic status and does not cause central nervous system depression.\u201d<\/span><\/p>\n<h3 id=\"chronic-severe-pain\"><span style=\"font-weight: 400;\">Chronic severe pain<\/span><\/h3>\n<p><span style=\"font-weight: 400;\">Fentanyl is sometimes prescribed for chronic pain from cancer or other conditions. However, it\u2019s only administered via a transdermal patch. Since patients can use them at home, and it\u2019s so easy to overdose, these patches help the body absorb the drug at a steady, safer rate. They range from 12 mcg\/hr to 100 mcg\/hr, depending on the patient\u2019s condition, level of pain, and medical history. When using fentanyl, there is always a risk of respiratory depression, hypoxia, and death and this risk increases when the patient is not being monitored by a medical professional.\u00a0<\/span><\/p>\n<h2 id=\"fentanyl-side-effects-and-warnings\"><span style=\"font-weight: 400;\">Fentanyl side effects and warnings<\/span><\/h2>\n<p><span style=\"font-weight: 400;\">In a medical setting, administered by a healthcare provider, fentanyl is typically safe, but that doesn\u2019t mean it\u2019s a good option for everyone. It has certain contraindications and side effects that might cause serious complications.<\/span><\/p>\n<h3 id=\"contraindications\"><span style=\"font-weight: 400;\">Contraindications<\/span><\/h3>\n<p><span style=\"font-weight: 400;\">Considering fentanyl\u2019s reputation as an illicit drug, its risk of misuse likely won\u2019t come as a surprise. Like other opioids, it can cause dependence, which can lead to abuse. Yet because fentanyl is so much more potent than many other opioids, abuse can quickly turn into overdose. According to the <\/span><a href=\"https:\/\/drugabusestatistics.org\/fentanyl-abuse-statistics\/\"><span style=\"font-weight: 400;\">National Center for Drug Abuse Statistics<\/span><\/a><span style=\"font-weight: 400;\">, more than half of all overdose deaths in the United States involve fentanyl. That\u2019s why it\u2019s crucial for patients to tell their doctors about any past experience with substance use disorder or if there are others with substance abuse disorders who will have access to the medication.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Even when it\u2019s used in a clinical setting, fentanyl may cause respiratory depression, making breathing more difficult. That\u2019s why it\u2019s not prescribed for people with conditions that affect breathing, like asthma, COPD, obstructive sleep apnea, and obesity with hypoventilation. People who have liver failure, a known intolerance to fentanyl, or a hypersensitivity to drug delivery excipients like sodium chloride should also not take fentanyl, according to the National Institutes of Health.<\/span><\/p>\n<h3 id=\"side-effects\"><span style=\"font-weight: 400;\">Side effects<\/span><\/h3>\n<p><span style=\"font-weight: 400;\">Side effects are different for everyone, and they could be mild or nonexistent, but it\u2019s still good to know all the possibilities. Some of the most common <\/span><a href=\"https:\/\/www.singlecare.com\/blog\/fentanyl-side-effects\/\"><span style=\"font-weight: 400;\">fentanyl side effects<\/span><\/a><span style=\"font-weight: 400;\"> include gastrointestinal issues (such as stomach pain and nausea), sleep disturbances, feelings of malaise, skin problems, psychiatric effects (anxiety or depression), nervous system effects, loss of appetite, low heart rate, low blood pressure (hypotension), and shortness of breath.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">There are also more serious side effects, which include overdose, allergic reactions, respiratory depression, central nervous system depression, seizures, cardiac arrest, and delirium, among others.<\/span><\/p>\n<h2 id=\"bottom-line\"><span style=\"font-weight: 400;\">Bottom line\u00a0<\/span><\/h2>\n<p><span style=\"font-weight: 400;\">Fentanyl has earned a scary reputation as a medication that kills people because of its serious overdose risk, and while that\u2019s true on the street (where it\u2019s commonly abused) it\u2019s important to remember that the drug is both safe and extremely useful in controlled, observant medical settings. As a powerful opioid pain reliever, it\u2019s a versatile drug in a healthcare setting, effective for postoperative pain, anesthesia, epidural infusion, acute breakthrough pain, chronic pain, and more.
