There are four tiers; the lower tiers mean patients have to pay less. So, where does atorvastatin fall?<\/span><\/p>\n<p><span style=\"font-weight: 400;\">\u201cFrom the mid-2010s to present, it has been a tier-1 medication on nearly all commercial insurance <\/span><a href=\"https:\/\/www.singlecare.com\/blog\/what-is-a-formulary\/\"><span style=\"font-weight: 400;\">formulary<\/span><\/a><span style=\"font-weight: 400;\"> lists, Medicare D plans, and state Medicaid formularies,\u201d says Dr. Brian Pacanowski, Pharm.D., director of experiential education and an assistant professor of clinical science in High Point University\u2019s Fred Wilson School of Pharmacy.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">However, that doesn\u2019t mean it\u2019s the cheapest statin. \u201cOccasionally, other statins will come in cheaper,\u201d says Dr. Yu-Ming Ni, MD, a board-certified cardiologist and lipidologist at MemorialCare Heart and Vascular Institute at Orange Coast Medical Center in Fountain Valley, California.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Patients can see where atorvastatin falls on their insurance plan\u2019s formularies. Here are some common ones:<\/span><\/p>\n<ul>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><a href=\"https:\/\/www.uhc.com\/medicare\/alphadog\/AAUT25HM0255400_000\"><span style=\"font-weight: 400;\">United Healthcare<\/span><\/a><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><a href=\"https:\/\/healthy.kaiserpermanente.org\/content\/dam\/kporg\/final\/documents\/formularies\/medicare\/2025\/comprehensive-formulary-wa.pdf\"><span style=\"font-weight: 400;\">Kaiser Permanente<\/span><\/a><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><a href=\"https:\/\/www.cigna.com\/static\/www-cigna-com\/docs\/medicare\/plans-services\/2025\/formulary-extra.pdf\"><span style=\"font-weight: 400;\">Cigna<\/span><\/a><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><a href=\"https:\/\/www.anthem.com\/pharmacy-information\/drug-list-formulary\"><span style=\"font-weight: 400;\">Anthem<\/span><\/a><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><a href=\"https:\/\/www.humana.com\/pharmacy\/medicare-drug-list\"><span style=\"font-weight: 400;\">Humana<\/span><\/a><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><a href=\"https:\/\/fm.formularynavigator.com\/FBO\/41\/2025_Aetna_Standard_Plan.pdf\"><span style=\"font-weight: 400;\">Aetna<\/span><\/a><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><a href=\"https:\/\/www.bluecrossnc.com\/content\/dam\/bcbsnc\/pdf\/members\/medicare\/prescription-drug-resources\/formulary-guides\/healthy-blue-medicare-25.pdf\"><span style=\"font-weight: 400;\">Blue Cross Blue Shield Healthy Blue + Medicare<\/span><\/a><\/li>\n<\/ul>\n<p><span style=\"font-weight: 400;\">While a medication\u2019s tier depends on the specific insurance plan, other common tier 1 drugs include the following generics: lisinopril, amlodipine, atenolol, benazepril, carvedilol, chlorthalidone, diltiazem, glipizide, quinapril, sertraline, alendronate, citalopram, clonidine, enalapril maleate, finasteride, fluoxetine, fosinopril, gemfibrozil, glimepiride, glyburide, labetalol, metformin hcl, nifedipine, terazosin, trandolapril, triamterene, pioglitazone, bupropion hcl, clopidogrel, irbesartan, telmisartan, latanoprost, olmesartan, and quetiapine.<\/span><\/p>\n<h2 id=\"factors-influencing-atorvastatin-s-tier-status\"><span style=\"font-weight: 400;\">Factors influencing atorvastatin\u2019s tier status<\/span><\/h2>\n<p><span style=\"font-weight: 400;\">When it comes to tier status, it matters whether the medication has a generic form. \u201cBecause most statins are available as generics, most will be on the tier-1 list of most commercial insurance plans, as well as Medicare Part D plans and state Medicaid plans,\u201d says Dr. Pacanowski.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Not all are, though. He mentions the exception of fluvastatin. \u201cDrug availability issues and a higher-than-average average wholesale price (AWP) have caused this medication to be removed from many tier-1 lists.\u201d<\/span><\/p>\n<p><span style=\"font-weight: 400;\">According to Dr. Pacanowski, other factors that affect the tier include whether the medication remains on the market (and atorvastatin has) and how well it works (pretty well). Regarding the latter, he points to a <\/span><a href=\"https:\/\/pmc.ncbi.nlm.nih.gov\/articles\/PMC10203693\/\"><span style=\"font-weight: 400;\">2023 study<\/span><\/a><span style=\"font-weight: 400;\"> that found atorvastatin was prescribed the most, followed by simvastatin.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Last but not least, according to Dr. Ni, the fact that other statins are available\u2014and are sometimes cheaper\u2014affects atorvastatin\u2019s tier status. <\/span><a href=\"https:\/\/www.singlecare.com\/prescription\/simvastatin\"><span style=\"font-weight: 400;\">Simvastatin<\/span><\/a><span style=\"font-weight: 400;\"> is one of those medications, while <\/span><a href=\"https:\/\/www.singlecare.com\/prescription\/rosuvastatin-calcium\"><span style=\"font-weight: 400;\">rosuvastatin<\/span><\/a><span style=\"font-weight: 400;\"> is not.<\/span><\/p>\n<h2 id=\"are-cholesterol-medications-covered-by-insurance\"><span style=\"font-weight: 400;\">Are cholesterol medications covered by insurance?<\/span><\/h2>\n<p><span style=\"font-weight: 400;\">Again, coverage isn\u2019t a one-size-fits-all answer\u2014at least to some degree. \u201cCoverage for these various statins depends on insurance to insurance,\u201d Dr. Ni says, \u201cbut usually, insurance plans will prefer other statins besides rosuvastatin, atorvastatin, and pitavastatin.\u201d<\/span><\/p>\n<p><span style=\"font-weight: 400;\">As mentioned earlier, insurance plans also look at the AWP, which explains why some other <\/span><a href=\"https:\/\/www.singlecare.com\/conditions\/high-cholesterol-symptoms\"><span style=\"font-weight: 400;\">high cholesterol<\/span><\/a><span style=\"font-weight: 400;\"> medications are on higher tiers. \u201cInjectable PC5K9 inhibitors, such as evolocumab (Repatha) and alirocumab (Praluent), are typically on tiers 3 or 4, requiring prior authorization in most cases due to approximate AWPs of $1400 monthly,\u201d Dr. Pacanowski says. \u201cIf the details of the patient\u2019s case meet the defined exceptions, the insurance plan will cover the medication, usually at the highest copay allowed by the plan.\u201d<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Examples of cholesterol medications include lovastatin, pravastatin, fenofibrate, and gemfibrozil.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Combination drugs usually have higher AWPs and therefore are on higher tiers, too, he adds, giving the example of Vytorin, which combines ezetimibe and simvastatin.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Most insurance companies will use the same methods to classify <\/span><a href=\"https:\/\/www.singlecare.com\/conditions\/hypertension-treatments-and-medications\"><span style=\"font-weight: 400;\">blood pressure medications<\/span><\/a><span style=\"font-weight: 400;\">, Dr. Pacanowski says. The main difference is that blood pressure medications can fall into different drug classes.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">\u201cThe drugs inside these classes may \u2018compete with each other\u2019 for tier status, but there is very rarely \u2018competition\u2019 between classes,\u201d Dr. Pacanowski explains. \u201cBeta blockers are compared to other beta blockers, diuretics to other diuretics, ACE inhibitors to other ACE inhibitors, etc.\u201d<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Some examples of blood pressure medications\u2014like calcium channel blockers, beta blockers, ACE inhibitors, and diuretics\u2014include hydrochlorothiazide, valsartan, furosemide, metoprolol succinate, metoprolol tartrate, propranolol HCl, losartan potassium, and ramipril.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">The good news is that the medications are often cheap or even free. \u201cI generally find most patients to be paying very little for their blood pressure and statin medications,\u201d Dr. Ni says. \u201cIn fact, depending on the insurance plan or prescription drug plan, patients will pay zero dollars for their medications, including atorvastatin.\u201d<\/span><\/p>\n<p><span style=\"font-weight: 400;\">According to Dr. Pacanowski, atorvastatin can be free for many reasons: the copay for generic drugs, or tier-1 medications, is $0, the patient reached their plan deductible, the patient reached their out-of-pocket maximum, or \u201cpreventative medications\u201d are free with the patient\u2019s plan.<\/span><\/p>\n<p><a href=\"https:\/\/www.singlecare.com\/blog\/atorvastatin-without-insurance\/\"><span style=\"font-weight: 400;\">Without insurance<\/span><\/a><span style=\"font-weight: 400;\">, the out-of-pocket cost for atorvastatin is around $1,500 per year. Here are the steps:\u00a0<\/span><\/p>\n<ol>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Go to a search engine (like Google), type in the name of your insurance company, and then type in \u201cformulary\u201d and the year.\u00a0<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">The right link will likely be one of the first. Click it.\u00a0<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">To find atorvastatin on the list, simultaneously press \u201cControl\u201d and \u201cf\u201d on your keyboard if you have a PC, or \u201cCommand\u201d and \u201cf\u201d if you have a Mac, and type in \u201catorvastatin.\u201d\u00a0<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">From there, you should be able to find atorvastatin listed, or not listed, as well as its tier status. The latter is usually to the right of the name.<\/span><\/li>\n<\/ol>\n<p><span style=\"font-weight: 400;\">People can also call the customer service number on the back of the ID card, Dr. Pacanowski says, or go to the insurance website, look for their \u201cpreferred drug list,\u201d and scroll to \u201ccholesterol medications\u201d or \u201ccholesterol-lowering agents.\u201d<\/span><\/p>\n<p><span style=\"font-weight: 400;\">When looking at various medications covered by insurance, it\u2019s important to consider that some forms of the same medication will be cheaper than others. \u201cAll insurance companies will have a list of tier pricing for their medications,\u201d Dr. Ni says. \u201cGeneric medications in general are going to be cheaper than branded medications, although sometimes the discount is not very much.\u201d<\/span><\/p>\n","protected":false},"excerpt":{"rendered":"<p>Atorvastatin, or atorvastatin calcium, is a prescription drug (not an over-the-counter medication) in the statin drug class.
