But what they cover and how much you have to pay out-of-pocket vary quite a bit.<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Medicaid is state-specific. Some states reliably cover both hypogonadism and gender-affirming therapy, but others do not.<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Some Medicare Part D plans cover TRT, but usually only when it\u2019s prescribed for low testosterone. Even then, you may need prior approval.<\/span><\/li>\n<\/ul>\n<h3 id=\"other-criteria-for-coverage\"><span style=\"font-weight: 400;\">Other criteria for coverage<\/span><\/h3>\n<p><span style=\"font-weight: 400;\">There may be other hurdles to overcome for your prescription to be covered.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">\u00a0<\/span><span style=\"font-weight: 400;\">For instance,\u00a0 prior authorization is often needed before your plan will cover testosterone. This is especially likely if you are asking for coverage for brand-name testosterone or less common formulations, Khoshaba says.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">\u00a0<\/span><span style=\"font-weight: 400;\">Lastly, some insurance plans will require step therapy before prescribing testosterone. That means you may need to try a more cost-effective form of treatment before other formulations are prescribed.\u00a0<\/span><\/p>\n<h2 id=\"what-do-insurers-look-for-proving-medical-necessity\"><span style=\"font-weight: 400;\">What do insurers look for? Proving medical necessity<\/span><\/h2>\n<p><span style=\"font-weight: 400;\">Whether or not your health insurance company covers testosterone also depends in part on your diagnosis.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">\u00a0<\/span><span style=\"font-weight: 400;\">Typically, health insurance companies will cover testosterone when it&#8217;s prescribed for <\/span><a href=\"https:\/\/www.singlecare.com\/conditions\/low-testosterone-symptoms\"><span style=\"font-weight: 400;\">low testosterone levels<\/span><\/a><span style=\"font-weight: 400;\"> (hypogonadism), says Dadhich. Some will require additional lab work, as well.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">\u00a0<\/span><span style=\"font-weight: 400;\">The process of proving to your insurance company that you qualify for testosterone can be tedious and discouraging, says <\/span><a href=\"https:\/\/www.biohackingcompanies.com\/united-states\/wellness-solutions\/international-pause-institute\"><span style=\"font-weight: 400;\">Betsy Greenleaf, DO<\/span><\/a><span style=\"font-weight: 400;\">, triple board-certified urogynecologist and founder of International Pause Institute. \u201cI have spent long, frustrating hours arguing with insurance companies that the patient&#8217;s testosterone is now normal because they are being treated,\u201d she shares. \u201cOccasionally, we would have to take people off hormones and then retest and then wait until it is low and then prescribe again.\u201d<\/span><\/p>\n<p><span style=\"font-weight: 400;\">\u00a0<\/span><span style=\"font-weight: 400;\">Coverage for other medical conditions can vary from one insurance company to another. \u201cSometimes there&#8217;s more paperwork or extra hoops to jump through when testosterone is prescribed for gender-affirming purposes,\u201d says Khoshaba.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">\u00a0<\/span><span style=\"font-weight: 400;\">And for women in menopause who are seeking testosterone, it\u2019s usually not covered, and typically, patients end up paying for it out of pocket, Greenleaf says.<\/span><\/p>\n<h2 id=\"how-does-the-method-impact-coverage\"><span style=\"font-weight: 400;\">How does the method impact coverage?<\/span><\/h2>\n<p><span style=\"font-weight: 400;\">Coverage can depend partly on the type of testosterone your healthcare provider prescribes, whether injections, gels, patches, or tablets. \u201cSome of the newer formulations of testosterone (oral therapies) have lower rates of coverage and often require that a previous form of therapy (gels or injectables) have failed or resulted in some sort of complication,\u201d Dadhich says.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">\u00a0<\/span><span style=\"font-weight: 400;\">Linda Khoshaba agrees that in her experience, not all forms of testosterone are equally covered by insurance. \u201cThe delivery method absolutely matters when it comes to what insurance will pay for,\u201d she says, noting that testosterone injections are typically the most affordable and more widely covered by insurance carriers.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">\u00a0<\/span><span style=\"font-weight: 400;\">\u201cGels and patches are more convenient for some people, but they tend to be more expensive and are sometimes only partially covered\u2014especially if they\u2019re brand-name,\u201d Khoshaba explains. \u201cTablets are less commonly prescribed and often not covered at all.\u201d<\/span><\/p>\n<h2 id=\"what-if-insurance-denies-coverage-for-testosterone\"><span style=\"font-weight: 400;\">What if insurance denies coverage for testosterone?<\/span><\/h2>\n<p><span style=\"font-weight: 400;\">If testosterone replacement is not on the drug formulary for your insurance plan, you and your physician will typically need to file a pre-authorization or <\/span><a href=\"https:\/\/www.singlecare.com\/blog\/prior-authorization\/\"><span style=\"font-weight: 400;\">prior authorization<\/span><\/a><span style=\"font-weight: 400;\"> request.
