For some individuals, these medications may be more tolerable, effective, and convenient than other weight loss drugs, such as <\/span><a href=\"http:\/\/www.singlecare.com\/prescription\/qsymia\"><span style=\"font-weight: 400;\">Qsymia<\/span><\/a><span style=\"font-weight: 400;\"> (phentermine-topiramate), <\/span><a href=\"https:\/\/www.singlecare.com\/prescription\/contrave\"><span style=\"font-weight: 400;\">Contrave<\/span><\/a><span style=\"font-weight: 400;\"> (naltrexone-bupropion), or <\/span><a href=\"https:\/\/www.singlecare.com\/prescription\/alli\"><span style=\"font-weight: 400;\">Alli<\/span><\/a><span style=\"font-weight: 400;\"> (orlistat).\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Despite the health benefits of weight loss and the efficacy of <\/span><a href=\"https:\/\/www.singlecare.com\/drug-classes\/glp-1-agonists\"><span style=\"font-weight: 400;\">GLP-1 agonists<\/span><\/a><span style=\"font-weight: 400;\"> like Wegovy, some health insurance plans don\u2019t cover them. Sometimes coverage is available for other FDA-approved uses, such as <\/span><a href=\"https:\/\/www.singlecare.com\/conditions\/sleep-apnea-treatment-and-medications\"><span style=\"font-weight: 400;\">Zepbound for obstructive sleep apnea<\/span><\/a><span style=\"font-weight: 400;\"> or <\/span><a href=\"https:\/\/www.singlecare.com\/blog\/news\/wegovy-heart-disease-fda-approval\/\"><span style=\"font-weight: 400;\">Wegovy for cardiovascular protection<\/span><\/a><span style=\"font-weight: 400;\">. Since coverage for weight loss medications is often subject to change, it\u2019s best to review your plan regularly.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">If you\u2019re interested in weight loss medications but don\u2019t want to pay out of pocket, this guide will help you understand your insurance options.\u00a0<\/span><\/p>\n<h2 id=\"what-insurance-plans-cover-weight-loss-medications\"><span style=\"font-weight: 400;\">What insurance plans cover weight loss medications?<\/span><\/h2>\n<p><span style=\"font-weight: 400;\">Insurance coverage for any medication varies depending on the plan. After reviewing more than a dozen formularies, we noticed the following trends:<\/span><\/p>\n<ul>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Employer-sponsored benefit plans are the most likely to cover weight loss drugs, with a <\/span><a href=\"https:\/\/www.healthaffairs.org\/content\/forefront\/annual-family-premiums-employer-coverage-rise-6-2025-nearing-27-000-workers-paying-6\"><span style=\"font-weight: 400;\">2025 study<\/span><\/a><span style=\"font-weight: 400;\"> showing that 19% of large firms and 43% of firms with 5,000 or more employees cover GLP-1 agonists.\u00a0<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Marketplace (Affordable Care Act) plans typically don\u2019t cover weight loss drugs.<\/span><\/li>\n<\/ul>\n<p><a href=\"https:\/\/arrcled.com\/our-contributors\/gail-clifford\/\"><span style=\"font-weight: 400;\">Gail Clifford<\/span><\/a><span style=\"font-weight: 400;\">, MD, an internal medicine hospitalist practicing in Orlando, Florida, says, \u201cOlder medications like <\/span><a href=\"https:\/\/www.singlecare.com\/blog\/phentermine-for-weight-loss\/\"><span style=\"font-weight: 400;\">phentermine<\/span><\/a><span style=\"font-weight: 400;\"> are often covered because they have been around a long time and are relatively inexpensive. Some plans will also cover Contrave or Qsymia, which are FDA-approved for weight loss and can be effective for the right person.\u201d She\u2019s also started to see better coverage for Wegovy.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">\u201cIn my experience, either all weight loss medications are covered under a specific insurance plan, or none are,\u201d says <\/span><a href=\"https:\/\/www.linkedin.com\/in\/jenbrownmd\/\"><span style=\"font-weight: 400;\">Jennifer Brown, MD<\/span><\/a><span style=\"font-weight: 400;\">, a board-certified physician specializing in obesity and family medicine. \u201cI haven\u2019t found that one class of weight loss medication is covered more often than another. Of course, we all want our patients on GLP-1s because they are so much more effective than the other alternatives.\u201d<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Most insurance plans do cover GLP-1 agonists\u2014just not for weight loss. You might look at a sample drug list and see that semaglutide is covered as <\/span><a href=\"https:\/\/www.singlecare.com\/prescription\/ozempic\"><span style=\"font-weight: 400;\">Ozempic<\/span><\/a><span style=\"font-weight: 400;\">, tirzepatide is covered as <\/span><a href=\"https:\/\/www.singlecare.com\/prescription\/mounjaro\"><span style=\"font-weight: 400;\">Mounjaro<\/span><\/a><span style=\"font-weight: 400;\">, and liraglutide is covered as <\/span><a href=\"https:\/\/www.singlecare.com\/prescription\/victoza\"><span style=\"font-weight: 400;\">Victoza<\/span><\/a><span style=\"font-weight: 400;\">. These are the brand names that are FDA-approved to treat Type 2 diabetes.\u00a0<\/span><\/p>\n<h3 id=\"why-do-insurance-companies-not-cover-weight-loss-drugs\"><span style=\"font-weight: 400;\">Why do insurance companies not cover weight loss drugs?<\/span><\/h3>\n<p><span style=\"font-weight: 400;\">So, why won\u2019t insurance companies pay for weight loss drugs when being overweight or having obesity is linked to many other health conditions? For starters, GLP-1 agonists can be pricey. Insurance companies like to keep costs low, and there are over 100 million adults who are overweight or have obesity in the U.S. who might want to use these products.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">\u201cLet\u2019s face it: GLP1s are the medications that doctors want to prescribe, and patients want to take,\u201d Dr. Brown says. \u201cThey are far superior to the other weight loss medications. Universal GLP-1 coverage for obesity treatment could bankrupt a lot of insurance plans.\u201d<\/span><\/p>\n<p><span style=\"font-weight: 400;\">However, inexpensive weight loss drugs like Alli (orlistat), <\/span><a href=\"https:\/\/www.singlecare.com\/prescription\/adipex-p\"><span style=\"font-weight: 400;\">Adipex-P<\/span><\/a><span style=\"font-weight: 400;\"> (phentermine), and Qsymia (phentermine-topiramate) are not always covered by insurance. This could be due to a controversial belief that obesity isn\u2019t a disease, despite the American Medical Association&#8217;s 2023 <\/span><a href=\"https:\/\/policysearch.ama-assn.org\/policyfinder\/detail\/obesity?uri=%2FAMADoc%2FHOD.xml-0-3858.xml\"><span style=\"font-weight: 400;\">statement<\/span><\/a><span style=\"font-weight: 400;\"> recognizing it as such.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">\u201cObesity is still thought of as a cosmetic issue that&#8217;s due to a lack of willpower by many. Even among other doctors, I still frequently hear, \u2018If only the patient would eat less or exercise more.\u2019 Of course, this outdated thinking is not the case at all,\u201d Dr. Brown says. \u201cObesity is a chronic metabolic disease that needs lifelong treatment.\u201d\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Dr. Clifford agrees. \u201cA big part of it comes down to outdated thinking. For a long time, obesity was not treated as a medical condition\u2014it was seen more as a personal choice or lifestyle issue. Even though we now know it is a complex, chronic disease with hormonal and genetic drivers, many insurance companies still exclude treatment, especially medications, from their coverage.\u201d\u00a0<\/span><\/p>\n<h3 id=\"does-medicare-cover-weight-loss-medication\"><span style=\"font-weight: 400;\">Does Medicare cover weight loss medication?<\/span><\/h3>\n<p><span style=\"font-weight: 400;\">Medicare doesn\u2019t currently cover weight loss medication. This puts it in the same boat as a handful of other medications Medicare never covers, including cosmetic hair loss treatments, fertility drugs, and erectile dysfunction treatments.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Medicare also doesn\u2019t cover medications for <\/span><a href=\"https:\/\/www.singlecare.com\/blog\/off-label-prescription-drugs\/\"><span style=\"font-weight: 400;\">off-label use<\/span><\/a><span style=\"font-weight: 400;\"> except when treating cancer.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Starting in 2026, Medicare will begin covering select GLP-1 medications for obesity under a new policy that expands access to these medications. Under the White House agreement, these drugs will be available to <\/span><a href=\"https:\/\/www.whitehouse.gov\/fact-sheets\/2025\/11\/fact-sheet-president-donald-j-trump-announces-major-developments-in-bringing-most-favored-nation-pricing-to-american-patients\/\"><span style=\"font-weight: 400;\">Medicare beneficiaries<\/span><\/a><span style=\"font-weight: 400;\"> at reduced costs, with monthly copays expected to be around $50.<\/span><\/p>\n<h3 id=\"does-medicaid-cover-weight-loss-medication\"><span style=\"font-weight: 400;\">Does Medicaid cover weight loss medication?<\/span><\/h3>\n<p><span style=\"font-weight: 400;\">Medicaid coverage for weight loss medications varies by state. As of October 2025, <\/span><a href=\"https:\/\/bpb-us-e1.wpmucdn.com\/sites.dartmouth.edu\/dist\/a\/2645\/files\/2025\/11\/Brief-1.-GLP1s-for-Weight-Management-FINAL.pdf\"><span style=\"font-weight: 400;\">13 Medicaid state plans<\/span><\/a><span style=\"font-weight: 400;\"> cover at least one GLP\u20111 receptor agonist. Coverage for other types of FDA\u2011approved weight loss drugs (like phentermine) is less frequently reported.<\/span><\/p>\n<h3 id=\"does-the-va-cover-weight-loss-medication\"><span style=\"font-weight: 400;\">Does the VA cover weight loss medication?<\/span><\/h3>\n<p><span style=\"font-weight: 400;\">Yes, the VA covers some weight loss medications, including Contrave, Xenical, and Qsymia.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">The VA considers <\/span><a href=\"https:\/\/www.va.gov\/formularyadvisor\/DOC_PDF\/CRE_Weight_Management_Medications_Clinical_Recommendations_Rev_Aug_2025.pdf\"><span style=\"font-weight: 400;\">GLP-1s for weight loss<\/span><\/a><span style=\"font-weight: 400;\"> \u201cnon-formulary\u201d medications, meaning healthcare providers must submit exception requests in order for the drug to be dispensed for the purpose of weight loss.<\/span><\/p>\n<h2 id=\"examples-of-insurance-plans-that-cover-weight-loss-medication\"><span style=\"font-weight: 400;\">Examples of insurance plans that cover weight loss medication<\/span><\/h2>\n<p><span style=\"font-weight: 400;\">We found several insurance plans that cover weight loss and obesity medications:<\/span><\/p>\n<ul>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">The <\/span><a href=\"https:\/\/www.fepblue.org\/-\/media\/FEPBlue-Sitecore-10-Media\/PDFs\/Brochures\/2025\/FEP2025AbbrevFormularyBook-R20-WCAG.pdf\"><span style=\"font-weight: 400;\">Blue Cross Blue Shield Federal Employee Program<\/span><\/a><span style=\"font-weight: 400;\"> covers name-brand <\/span><a href=\"https:\/\/www.singlecare.com\/blog\/wegovy-prior-authorization-criteria-blue-cross-blue-shield\/\"><span style=\"font-weight: 400;\">Wegovy<\/span><\/a><span style=\"font-weight: 400;\">, Saxenda, Contrave, Xenical, Qsymia, and Lomaira, and generic benzphetamine, diethylpropion, orlistat, phendimetrazine, and phentermine. Wegovy isn\u2019t covered in the Blue Focus plan.<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">The <\/span><a href=\"https:\/\/fm.formularynavigator.com\/FBO\/41\/2025_Aetna_Standard_Plan.pdf\"><span style=\"font-weight: 400;\">Aetna Standard Plan<\/span><\/a><span style=\"font-weight: 400;\"> covers name-brand Wegovy, Saxenda, Zepbound, and Qsymia, and generic diethylpropion. Adipex-P is available as a non-preferred brand, which means Aetna is willing to cover it if you try at least two of the other listed medications first and they don\u2019t work.\u00a0<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><a href=\"https:\/\/www.cigna.com\/individuals-families\/member-guide\/offered-by-employers-drug-list?indicator=nonIFP\"><span style=\"font-weight: 400;\">Cigna\u2019s employer-sponsored plans<\/span><\/a><span style=\"font-weight: 400;\"> cover name-brand Wegovy and Saxenda, plus generic benzphetamine, diethylpropion, phendimetrazine, and phentermine. Contrave, Qsymia, and Zepbound are non-preferred brands.<\/span><\/li>\n<\/ul>\n<p><span style=\"font-weight: 400;\">Coverage often requires <\/span><a href=\"https:\/\/www.singlecare.com\/blog\/prior-authorization\/\"><span style=\"font-weight: 400;\">prior authorization<\/span><\/a><span style=\"font-weight: 400;\">. Your healthcare provider may need to share your body mass index (BMI) and other documentation to show that you\u2019re following a <\/span><a href=\"https:\/\/www.singlecare.com\/blog\/weight-loss-diet\/\"><span style=\"font-weight: 400;\">weight loss diet<\/span><\/a><span style=\"font-weight: 400;\"> and exercise plan.\u00a0<\/span><\/p>\n<h2 id=\"common-weight-loss-medications-and-their-coverage-status\"><span style=\"font-weight: 400;\">Common weight loss medications and their coverage status<\/span><\/h2>\n<p><span style=\"font-weight: 400;\">Various medications are approved for weight loss. Generally, newer medications, such as GLP-1 agonists, are the least likely to be covered, while older drugs are commonly found on formularies.<\/span><\/p>\n<h3 id=\"glp-1-receptor-agonists\"><span style=\"font-weight: 400;\">GLP-1 receptor agonists<\/span><\/h3>\n<p><span style=\"font-weight: 400;\">Some plans may cover GLP-1s for weight loss, but they often require prior authorization. They\u2019ll generally base eligibility on whether you meet the BMI threshold, whether you\u2019ve tried other medications, and whether you\u2019re on an exercise and diet program. Although some plans may exclude GLP-1s for weight loss, they might approve them for diabetes or <\/span><a href=\"https:\/\/www.singlecare.com\/blog\/expanding-use-glp-1s\/\"><span style=\"font-weight: 400;\">sleep apnea<\/span><\/a><span style=\"font-weight: 400;\">.\u00a0<\/span><\/p>\n<h3 id=\"combination-drugs\"><span style=\"font-weight: 400;\">Combination drugs<\/span><\/h3>\n<p><span style=\"font-weight: 400;\">Medications like <\/span><a href=\"https:\/\/www.singlecare.com\/blog\/qsymia-vs-phentermine\/\"><span style=\"font-weight: 400;\">Qsymia<\/span><\/a><span style=\"font-weight: 400;\"> and Contrave are more likely to be on formularies since they\u2019re older and less expensive than GLP-1s. As with GLP-1s, coverage may also require prior authorization, a specific BMI, and consideration of other treatments first. Plans may also have limits on refills or require frequent follow-ups to continue coverage.<\/span><\/p>\n<h3 id=\"appetite-suppressants\"><span style=\"font-weight: 400;\">Appetite suppressants\u00a0<\/span><\/h3>\n<p><span style=\"font-weight: 400;\">Drugs like phentermine, diethylpropion, and benzphetamine may be covered by some plans as older medications. However, they\u2019re often only approved for short-term use, not chronic weight management. In many cases, plans may also need documentation that the drug is being used alongside a supervised weight-loss program.\u00a0<\/span><\/p>\n<h3 id=\"lipase-inhibitors\"><span style=\"font-weight: 400;\">Lipase inhibitors<\/span><\/h3>\n<p><span style=\"font-weight: 400;\">This drug class mainly <\/span><a href=\"https:\/\/www.singlecare.com\/drug-classes\/lipase-inhibitors\"><span style=\"font-weight: 400;\">refers to orlistat<\/span><\/a><span style=\"font-weight: 400;\">, known under the brand names Xenical and Alli. Plans may cover it since it\u2019s an older, less costly medication. Alli is the brand-name version that can be found over-the-counter (OTC) and is not usually covered by insurance. As an OTC drug, it usually has lower out-of-pocket costs than newer GLP-1 drugs.<\/span><\/p>\n<h2 id=\"what-to-do-if-insurance-won-t-cover-weight-loss-medication\"><span style=\"font-weight: 400;\">What to do if insurance won\u2019t cover weight loss medication<\/span><\/h2>\n<p><span style=\"font-weight: 400;\">If your insurance company won\u2019t cover your weight loss medication, your doctor can write an <\/span><a href=\"https:\/\/www.patientadvocate.org\/explore-our-resources\/insurance-denials-appeals\/things-to-include-in-your-appeal-letter\/\"><span style=\"font-weight: 400;\">appeal or exception letter<\/span><\/a><span style=\"font-weight: 400;\"> to explain why the drug is medically necessary. Such letters may also be required if your insurance company includes weight loss medications in its formulary but denies you coverage.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Prior authorization is often required before insurance companies will cover weight loss medications. Dr. Brown says the prior authorization process requires healthcare providers to answer pages of questions and submit medical records for review. For example, some insurance companies require patients to have already tried a specific diet and exercise routine for a certain period or to have tried a different type of weight loss medication first.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">If the insurance company denies your claim, your healthcare provider can send an appeal letter further explaining why the drug is medically necessary and why alternatives aren\u2019t appropriate.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">\u201cI have seen appeals succeed, especially when a healthcare provider explains why the medication is needed,\u201d Dr. Clifford says. \u201cFor example, if a patient has a high BMI and other health problems like diabetes or high blood pressure, and they have already tried diet and exercise without enough success, that makes a strong case.\u201d She says it\u2019s important to work with a healthcare provider who is willing to advocate for you and go through several rounds of appeals if needed.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Dr. Brown warns that, in her experience, the prior authorization and appeals process is rarely successful for weight loss drugs. Your final options are to pay out of pocket for the prescription medication or explore other savings options, which we\u2019ll discuss below.<\/span><\/p>\n<h3 id=\"what-to-do-if-insurance-stops-covering-weight-loss-medications\"><span style=\"font-weight: 400;\">What to do if insurance stops covering weight loss medications<\/span><\/h3>\n<p><span style=\"font-weight: 400;\">After the insurance company initially approves your weight loss prescription, your healthcare provider may need to submit updated medical records periodically to maintain approval. <\/span><a href=\"https:\/\/www.linkedin.com\/in\/michaela-robbins-dnp-72110919\/\"><span style=\"font-weight: 400;\">Michaela Robbins<\/span><\/a><span style=\"font-weight: 400;\">, a healthcare provider of nursing practice in New York City, says insurance companies may drop coverage for weight loss medications if they don\u2019t seem to be working, or if they\u2019ve worked and you\u2019re no longer considered to have overweight or obesity.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">This can be challenging because many people need to stay on weight loss medications to maintain a lower body weight, and some medications take weeks to work. If your insurance company stops covering your medication, contact your healthcare provider immediately so they can initiate the appeals process. Sometimes these cards can only be used once or must be used by people with insurance other than Medicare. Lilly, the manufacturer of Zepbound, offers a self-pay option that allows patients to purchase the medication directly from Lilly Direct at a discount. However, this offer only applies to vials of the drug, not the autoinjector pens.
