It is approved by the U.S. Food and Drug Administration (<\/span>FDA<\/span>) to treat several yeast infections, such as vulvovaginal candidiasis, oral candidiasis (thrush), esophageal candidiasis, urinary tract infections (UTIs), and many more. <\/span>Healthcare<\/span> providers may also prescribe the <\/span>antifungal medication<\/span> off-label<\/span> to treat other <\/span>fungal infections<\/span>, such as coccidiomycosis (Valley Fever), blastomycosis, and histoplasmosis.<\/span><\/p>\n

According to researchers and <\/span>dermatologists<\/span>, while <\/span>fluconazole<\/span> can work for <\/span>toenail fungus<\/span> (<\/span>onychomycosis<\/span>), it\u2019s typically not the first-line treatment, as other medications are much more effective.<\/span><\/p>\n\t\t

Efficacy of <\/span>fluconazole<\/span> for <\/span>toenail fungus<\/span><\/h2>\n

Fluconazole<\/span> can be effective in treating certain <\/span>toenail fungi<\/span>, such as <\/span>distal subungual onychomycosis<\/span> and <\/span>dermatophyte onychomycosis<\/span>. In a <\/span>double-blind<\/span> study<\/span><\/a>, 362 patients with mycologically confirmed <\/span>onychomycosis<\/span> were randomized to medical treatment with 150, 300, or 450 milligrams (mg) of <\/span>fluconazole<\/span> once a week, or <\/span>placebo<\/span> once weekly for up to 12 months. At the study\u2019s conclusion, researchers found that 28% to 36% of participants receiving <\/span>fluconazole<\/span> treatment achieved a <\/span>clinical cure<\/span> (fully healed nail), compared with 3% of participants receiving the <\/span>placebo<\/span>. Also, there were mycological <\/span>eradication<\/span> (complete elimination or destruction of fungi) rates of 47% to 62% at the end of therapy compared to 14% for <\/span>placebo<\/span>.\u00a0<\/span><\/p>\n

While research supports <\/span>fluconazole as an effective treatment for toenail fungus<\/span>, it is typically not the first-line treatment healthcare providers prescribe. \u201cI have worked <\/span>fluconazole<\/span> into <\/span>nail fungus<\/span> strategies where long-term tolerability matters more than quick fixes, especially when topicals fail and systemic meds are the only route left,\u201d says Adrian Cole, MD, a board-certified <\/span>dermatologist<\/span> and clinical consultant at <\/span>Injecto<\/span><\/a> in Dallas-Fort Worth, Texas. \u201cIt\u2019s not the first medication I reach for, but when it\u2019s chosen wisely, it delivers.\u201d<\/span><\/p>\n

Hannah Kopelman, DO, a physician dually trained in skin cancer and hair loss, specializing in <\/span>dermatology<\/span> at <\/span>DermOnDemand<\/span><\/a> in New York, agrees: \u201cI\u2019ve used it in patients who either can\u2019t tolerate <\/span>terbinafine<\/span> or who have liver enzyme elevations that make other <\/span>oral antifungals<\/span> riskier. It\u2019s not a one-size-fits-all option, but in the right context, it works well.\u201d<\/span><\/p>\n

In most cases, <\/span>terbinafine<\/span><\/a> and <\/span>itraconazole<\/span><\/a> are typically the first-line treatments for <\/span>toenail fungus<\/span>. <\/span>Terbinafine<\/span> (<\/span>Lamisil<\/span>, Terbinex) is a prescription <\/span>antifungal medication<\/span> that treats skin, scalp, and nail infections.\u00a0<\/span><\/p>\n

\u201cCompared to <\/span>terbinafine<\/span>, <\/span>fluconazole<\/span> usually trails behind in speed,\u201d Dr. Cole says. \u201c<\/span>Terbinafine<\/span> pushes faster clearance in the first eight to 12 weeks, but I have had more drop-offs due to lab spikes and gut upset. <\/span>Fluconazole<\/span> is slower, steadier, and easier to commit to.\u201d<\/span><\/p>\n

According to an <\/span>older study<\/span><\/a>, a daily dosage of 250 mg of <\/span>terbinafine<\/span> for 12 or 16 weeks clinically cured <\/span>toenail fungus<\/span> in 46% of study participants without the need for a second medical treatment. Of those who received a second round of treatment, 88% were cured.<\/span><\/p>\n

Itraconazole<\/span> (<\/span>Sporanox<\/span>, Onmel, <\/span>Sporanox<\/span> Pulsepak) is also an <\/span>antifungal medication<\/span> for <\/span>toenail fungus<\/span>. A <\/span>randomized, <\/span>placebo<\/span>-controlled trial<\/span><\/a> found that a daily dose of 100 mg of <\/span>itraconazole<\/span> or 200 mg twice daily led to complete cures of <\/span>toenail fungus<\/span> by the end of the 12-month medical intervention.\u00a0<\/span><\/p>\n

\u201c<\/span>Itraconazole<\/span> does better than <\/span>fluconazole<\/span> in some cases, but has way more interactions,\u201d Dr. Cole says. \u201cSo, <\/span>fluconazole<\/span> sits in a nice middle ground if you know how to use it.\u201d<\/span><\/p>\n

Griseofulvin<\/span><\/a> (Fulvicin, Grifulvin, Gris-PEG) is another amongst the many <\/span>oral antifungal agents<\/span> that can treat <\/span>dermatophyte onychomycosis<\/span>, but it isn\u2019t as effective as <\/span>once-weekly fluconazole<\/span>.\u00a0<\/span><\/p>\n

While it\u2019s not the top choice when it comes to oral medications, <\/span>fluconazole<\/span> does have a few advantages that make it more effective than <\/span>topical antifungals<\/span>. \u201cCompared to <\/span>topical treatments<\/span> like efinaconazole or ciclopirox, <\/span>fluconazole<\/span> reaches deeper and is more systemic, which matters when the entire <\/span>nail plate<\/span> is involved,\u201d Dr. Kopelman says.<\/span><\/p>\n

Overall, <\/span>fluconazole<\/span> can be an effective treatment for <\/span>toenail fungus<\/span>; however, it is typically not the primary treatment, as improvements may take several weeks to become noticeable. \u201c<\/span>Fluconazole<\/span> works, but it does not work fast,\u201d Dr. Cole says. \u201cYou are treating an infection under layers of keratin, with poor blood flow and slow cellular turnover. You need something that lingers in the system long enough to stay active between doses, and <\/span>fluconazole<\/span> does that. \u201c<\/span><\/p>\n

How long do you take <\/span>fluconazole<\/span> for <\/span>toenail fungus<\/span>?<\/span><\/h2>\n

Dr. Kopelman typically prescribes 150 to 300 mg of <\/span>fluconazole<\/span> weekly for <\/span>toenail fungus<\/span>. \u201cMost patients need at least six months of treatment, sometimes up to nine or more depending on how quickly their nails grow,\u201d she explains. \u201cI always set expectations that the infected nail won\u2019t suddenly look better\u2014it\u2019s about growing out a new, clear nail from the base. You don\u2019t \u2018see\u2019 it working until a few months in.\u201d<\/span><\/p>\n

\u00a0<\/span>While <\/span>fluconazole<\/span> is a slow-acting medication, several steps can be taken to expedite recovery, such as maintaining good foot hygiene. \u201cIf your socks are trapping sweat or your shoes are not aired, you are dragging the process,\u201d Dr. Cole explains. \u201cI tell patients to rotate between three pairs of shoes and change socks midday. It\u2019s a simple fix that makes a huge difference.\u201d\u00a0<\/span><\/p>\n
