In a <\/span><a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/9631989\/\"><span style=\"font-weight: 400;\">double-blind<\/span><span style=\"font-weight: 400;\"> study<\/span><\/a><span style=\"font-weight: 400;\">, 362 patients with mycologically confirmed <\/span><span style=\"font-weight: 400;\">onychomycosis<\/span><span style=\"font-weight: 400;\"> were randomized to medical treatment with 150, 300, or 450 milligrams (mg) of <\/span><span style=\"font-weight: 400;\">fluconazole<\/span><span style=\"font-weight: 400;\"> once a week, or <\/span><span style=\"font-weight: 400;\">placebo<\/span><span style=\"font-weight: 400;\"> once weekly for up to 12 months. At the study\u2019s conclusion, researchers found that 28% to 36% of participants receiving <\/span><span style=\"font-weight: 400;\">fluconazole<\/span><span style=\"font-weight: 400;\"> treatment achieved a <\/span><span style=\"font-weight: 400;\">clinical cure<\/span><span style=\"font-weight: 400;\"> (fully healed nail), compared with 3% of participants receiving the <\/span><span style=\"font-weight: 400;\">placebo<\/span><span style=\"font-weight: 400;\">. Also, there were mycological <\/span><span style=\"font-weight: 400;\">eradication<\/span><span style=\"font-weight: 400;\"> (complete elimination or destruction of fungi) rates of 47% to 62% at the end of therapy compared to 14% for <\/span><span style=\"font-weight: 400;\">placebo<\/span><span style=\"font-weight: 400;\">.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">While research supports <\/span><span style=\"font-weight: 400;\">fluconazole as an effective treatment for toenail fungus<\/span><span style=\"font-weight: 400;\">, it is typically not the first-line treatment healthcare providers prescribe. \u201cI have worked <\/span><span style=\"font-weight: 400;\">fluconazole<\/span><span style=\"font-weight: 400;\"> into <\/span><span style=\"font-weight: 400;\">nail fungus<\/span><span style=\"font-weight: 400;\"> strategies where long-term tolerability matters more than quick fixes, especially when topicals fail and systemic meds are the only route left,\u201d says Adrian Cole, MD, a board-certified <\/span><span style=\"font-weight: 400;\">dermatologist<\/span><span style=\"font-weight: 400;\"> and clinical consultant at <\/span><a href=\"http:\/\/www.injectco.com\/\"><span style=\"font-weight: 400;\">Injecto<\/span><\/a><span style=\"font-weight: 400;\"> in Dallas-Fort Worth, Texas. \u201cIt\u2019s not the first medication I reach for, but when it\u2019s chosen wisely, it delivers.\u201d<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Hannah Kopelman, DO, a physician dually trained in skin cancer and hair loss, specializing in <\/span><span style=\"font-weight: 400;\">dermatology<\/span><span style=\"font-weight: 400;\"> at <\/span><a href=\"https:\/\/dermondemand.com\/\"><span style=\"font-weight: 400;\">DermOnDemand<\/span><\/a><span style=\"font-weight: 400;\"> in New York, agrees: \u201cI\u2019ve used it in patients who either can\u2019t tolerate <\/span><span style=\"font-weight: 400;\">terbinafine<\/span><span style=\"font-weight: 400;\"> or who have liver enzyme elevations that make other <\/span><span style=\"font-weight: 400;\">oral antifungals<\/span><span style=\"font-weight: 400;\"> riskier. It\u2019s not a one-size-fits-all option, but in the right context, it works well.\u201d<\/span><\/p>\n<p><span style=\"font-weight: 400;\">In most cases, <\/span><a href=\"https:\/\/www.singlecare.com\/blog\/terbinafine-for-toenail-fungus\/\"><span style=\"font-weight: 400;\">terbinafine<\/span><\/a><span style=\"font-weight: 400;\"> and <\/span><a href=\"https:\/\/www.singlecare.com\/prescription\/itraconazole\"><span style=\"font-weight: 400;\">itraconazole<\/span><\/a><span style=\"font-weight: 400;\"> are typically the first-line treatments for <\/span><span style=\"font-weight: 400;\">toenail fungus<\/span><span style=\"font-weight: 400;\">. <\/span><span style=\"font-weight: 400;\">Terbinafine<\/span><span style=\"font-weight: 400;\"> (<\/span><span style=\"font-weight: 400;\">Lamisil<\/span><span style=\"font-weight: 400;\">, Terbinex) is a prescription <\/span><span style=\"font-weight: 400;\">antifungal medication<\/span><span style=\"font-weight: 400;\"> that treats skin, scalp, and nail infections.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">\u201cCompared to <\/span><span style=\"font-weight: 400;\">terbinafine<\/span><span style=\"font-weight: 400;\">, <\/span><span style=\"font-weight: 400;\">fluconazole<\/span><span style=\"font-weight: 400;\"> usually trails behind in speed,\u201d Dr. Cole says. \u201c<\/span><span style=\"font-weight: 400;\">Terbinafine<\/span><span style=\"font-weight: 400;\"> pushes faster clearance in the first eight to 12 weeks, but I have had more drop-offs due to lab spikes and gut upset. <\/span><span style=\"font-weight: 400;\">Fluconazole<\/span><span style=\"font-weight: 400;\"> is slower, steadier, and easier to commit to.\u201d<\/span><\/p>\n<p><span style=\"font-weight: 400;\">According to an <\/span><a href=\"https:\/\/jamanetwork.com\/journals\/jamadermatology\/fullarticle\/478735\"><span style=\"font-weight: 400;\">older study<\/span><\/a><span style=\"font-weight: 400;\">, a daily dosage of 250 mg of <\/span><span style=\"font-weight: 400;\">terbinafine<\/span><span style=\"font-weight: 400;\"> for 12 or 16 weeks clinically cured <\/span><span style=\"font-weight: 400;\">toenail fungus<\/span><span style=\"font-weight: 400;\"> in 46% of study participants without the need for a second medical treatment. Of those who received a second round of treatment, 88% were cured.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Itraconazole<\/span><span style=\"font-weight: 400;\"> (<\/span><span style=\"font-weight: 400;\">Sporanox<\/span><span style=\"font-weight: 400;\">, Onmel, <\/span><span style=\"font-weight: 400;\">Sporanox<\/span><span style=\"font-weight: 400;\"> Pulsepak) is also an <\/span><span style=\"font-weight: 400;\">antifungal medication<\/span><span style=\"font-weight: 400;\"> for <\/span><span style=\"font-weight: 400;\">toenail fungus<\/span><span style=\"font-weight: 400;\">. A <\/span><a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/23884486\/\"><span style=\"font-weight: 400;\">randomized, <\/span><span style=\"font-weight: 400;\">placebo<\/span><span style=\"font-weight: 400;\">-controlled trial<\/span><\/a><span style=\"font-weight: 400;\"> found that a daily dose of 100 mg of <\/span><span style=\"font-weight: 400;\">itraconazole<\/span><span style=\"font-weight: 400;\"> or 200 mg twice daily led to complete cures of <\/span><span style=\"font-weight: 400;\">toenail fungus<\/span><span style=\"font-weight: 400;\"> by the end of the 12-month medical intervention.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">\u201c<\/span><span style=\"font-weight: 400;\">Itraconazole<\/span><span style=\"font-weight: 400;\"> does better than <\/span><span style=\"font-weight: 400;\">fluconazole<\/span><span style=\"font-weight: 400;\"> in some cases, but has way more interactions,\u201d Dr. Cole says. \u201cSo, <\/span><span style=\"font-weight: 400;\">fluconazole<\/span><span style=\"font-weight: 400;\"> sits in a nice middle ground if you know how to use it.\u201d<\/span><\/p>\n<p><a href=\"https:\/\/www.singlecare.com\/prescription\/griseofulvin-microsize\"><span style=\"font-weight: 400;\">Griseofulvin<\/span><\/a><span style=\"font-weight: 400;\"> (Fulvicin, Grifulvin, Gris-PEG) is another amongst the many <\/span><span style=\"font-weight: 400;\">oral antifungal agents<\/span><span style=\"font-weight: 400;\"> that can treat <\/span><span style=\"font-weight: 400;\">dermatophyte onychomycosis<\/span><span style=\"font-weight: 400;\">, but it isn\u2019t as effective as <\/span><span style=\"font-weight: 400;\">once-weekly fluconazole<\/span><span style=\"font-weight: 400;\">.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">While it\u2019s not the top choice when it comes to oral medications, <\/span><span style=\"font-weight: 400;\">fluconazole<\/span><span style=\"font-weight: 400;\"> does have a few advantages that make it more effective than <\/span><span style=\"font-weight: 400;\">topical antifungals<\/span><span style=\"font-weight: 400;\">. \u201cCompared to <\/span><span style=\"font-weight: 400;\">topical treatments<\/span><span style=\"font-weight: 400;\"> like efinaconazole or ciclopirox, <\/span><span style=\"font-weight: 400;\">fluconazole<\/span><span style=\"font-weight: 400;\"> reaches deeper and is more systemic, which matters when the entire <\/span><span style=\"font-weight: 400;\">nail plate<\/span><span style=\"font-weight: 400;\"> is involved,\u201d Dr. Kopelman says.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Overall, <\/span><span style=\"font-weight: 400;\">fluconazole<\/span><span style=\"font-weight: 400;\"> can be an effective treatment for <\/span><span style=\"font-weight: 400;\">toenail fungus<\/span><span style=\"font-weight: 400;\">; however, it is typically not the primary treatment, as improvements may take several weeks to become noticeable. \u201c<\/span><span style=\"font-weight: 400;\">Fluconazole<\/span><span style=\"font-weight: 400;\"> works, but it does not work fast,\u201d Dr. Cole says. \u201cYou are treating an infection under layers of keratin, with poor blood flow and slow cellular turnover. You need something that lingers in the system long enough to stay active between doses, and <\/span><span style=\"font-weight: 400;\">fluconazole<\/span><span style=\"font-weight: 400;\"> does that. \u201c<\/span><\/p>\n<h2 id=\"how-long-do-you-take-fluconazole-for-toenail-fungus\"><span style=\"font-weight: 400;\">How long do you take <\/span><span style=\"font-weight: 400;\">fluconazole<\/span><span style=\"font-weight: 400;\"> for <\/span><span style=\"font-weight: 400;\">toenail fungus<\/span><span style=\"font-weight: 400;\">?<\/span><\/h2>\n<p><span style=\"font-weight: 400;\">Dr. Kopelman typically prescribes 150 to 300 mg of <\/span><span style=\"font-weight: 400;\">fluconazole<\/span><span style=\"font-weight: 400;\"> weekly for <\/span><span style=\"font-weight: 400;\">toenail fungus<\/span><span style=\"font-weight: 400;\">. \u201cMost patients need at least six months of treatment, sometimes up to nine or more depending on how quickly their nails grow,\u201d she explains. \u201cI always set expectations that the infected nail won\u2019t suddenly look better\u2014it\u2019s about growing out a new, clear nail from the base. You don\u2019t \u2018see\u2019 it working until a few months in.\u201d<\/span><\/p>\n<p><span style=\"font-weight: 400;\">\u00a0<\/span><span style=\"font-weight: 400;\">While <\/span><span style=\"font-weight: 400;\">fluconazole<\/span><span style=\"font-weight: 400;\"> is a slow-acting medication, several steps can be taken to expedite recovery, such as maintaining good foot hygiene. \u201cIf your socks are trapping sweat or your shoes are not aired, you are dragging the process,\u201d Dr. Cole explains. \u201cI tell patients to rotate between three pairs of shoes and change socks midday.
