For this reason, healthcare providers often individualize statin therapy for older adult patients according to their health status and medical history.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">In this article, we\u2019ll discuss which statins are commonly prescribed to older adults, what the research says about their effectiveness and safety in this age group, and what side effects to look out for. Understanding how these medications work in older adulthood can help you make more informed decisions with your healthcare provider.<\/span><\/p>\n<h2 id=\"why-older-adults-may-need-a-different-approach-to-statins\"><span style=\"font-weight: 400;\">Why older adults may need a different approach to statins<\/span><\/h2>\n<p><span style=\"font-weight: 400;\">Statins help prevent <\/span><a href=\"https:\/\/www.singlecare.com\/blog\/symptoms-of-clogged-arteries\/\"><span style=\"font-weight: 400;\">atherosclerosis<\/span><\/a><span style=\"font-weight: 400;\"> and heart disease. <\/span><span style=\"font-weight: 400;\">Kristen Fuller<\/span><span style=\"font-weight: 400;\">, MD, a family medicine physician in Mammoth Lakes, California, explains that lowering cholesterol helps \u201creduce the risk of plaque buildup in blood vessels, which can reduce high blood pressure, heart attacks, and strokes.\u201d<\/span><\/p>\n<p><span style=\"font-weight: 400;\">According to the <\/span><a href=\"https:\/\/www.nih.gov\/nih-style-guide\/age\"><span style=\"font-weight: 400;\">National Institutes of Health<\/span><\/a><span style=\"font-weight: 400;\">, older adults can be people aged 65 and older, but there\u2019s no specific age that defines older adults. While statins are widely prescribed for middle-aged adults, their use in people over 75 is less straightforward. The American Heart Association (AHA) <\/span><a href=\"https:\/\/www.ahajournals.org\/doi\/10.1161\/cir.0000000000000678\"><span style=\"font-weight: 400;\">recommends statin therapy in people ages 40 to 75<\/span><\/a><span style=\"font-weight: 400;\"> with low-density lipoprotein (LDL) cholesterol levels higher than 190 mg\/dL to prevent heart attacks and strokes. For people older than 75, the U.S. Preventive Services Task Force (USPSTF) has stated there <\/span><a href=\"https:\/\/www.uspreventiveservicestaskforce.org\/uspstf\/recommendation\/statin-use-in-adults-preventive-medication\"><span style=\"font-weight: 400;\">isn\u2019t enough evidence<\/span><\/a><span style=\"font-weight: 400;\"> to weigh the risks and benefits of starting statins in people older than 75 who have no history of heart attack or stroke.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">That doesn\u2019t mean statins aren\u2019t helpful for older adults, but it does mean they require a more nuanced approach. As people age, several factors can influence how statins work in the body:<\/span><\/p>\n<ul>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><b>Other health conditions: <\/b><a href=\"https:\/\/www.kidney.org\/kidney-topics\/aging-and-kidney-disease\"><span style=\"font-weight: 400;\">Being over the age of 60<\/span><\/a><span style=\"font-weight: 400;\"> is a risk factor for kidney disease, and older age is a risk factor for liver diseases like <\/span><a href=\"https:\/\/www.sciencedirect.com\/science\/article\/pii\/S0531556523002553\"><span style=\"font-weight: 400;\">non-alcoholic fatty liver disease (NAFLD) and drug-induced liver injury<\/span><\/a><span style=\"font-weight: 400;\">. Statins carry a risk of impacting kidney and liver disease, which can affect how the body processes medication and increase the risk of drug interactions.<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><b>Increased sensitivity to side effects:<\/b><span style=\"font-weight: 400;\"> Statins are known to cause muscle-related side effects like pain, weakness, and myopathy. These issues may affect older adults, who already face risks of <\/span><a href=\"https:\/\/www.mdpi.com\/1660-4601\/19\/2\/874\"><span style=\"font-weight: 400;\">age-related muscle loss (sarcopenia) and a higher risk of falls<\/span><\/a><span style=\"font-weight: 400;\">.<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><b>Cognitive and metabolic concerns: <\/b><span style=\"font-weight: 400;\">Warnings on statin labels state that these medications may be linked to confusion or memory issues. Statins can also slightly raise blood sugar levels, which may be a concern for those with prediabetes or diabetes.<\/span><\/li>\n<\/ul>\n<p><span style=\"font-weight: 400;\">Because of these risks, healthcare providers weigh the benefits of lowering cholesterol against a patient\u2019s overall health status. When discussing statin treatment with your healthcare provider, it\u2019s important to share any existing medical conditions and medications you\u2019re taking. This helps your provider make a personalized decision and continue evaluating whether the benefits of statin therapy outweigh the risks over time.<\/span><\/p>\n<h2 id=\"top-recommended-statins-for-older-adults\"><span style=\"font-weight: 400;\">Top recommended statins for older adults<\/span><\/h2>\n<p><span style=\"font-weight: 400;\">In general, statins reduce the risk of heart attack and stroke <\/span><a href=\"https:\/\/www.acc.org\/Latest-in-Cardiology\/Articles\/2020\/10\/01\/11\/39\/Statin-Therapy-in-Older-Adults-for-Primary-Prevention-of-Atherosclerotic-CV-Disease\"><span style=\"font-weight: 400;\">by 20%\u201330% in people ages 40 to 75<\/span><\/a><span style=\"font-weight: 400;\">.\u00a0 Because cardiovascular disease is a common cause of disability and death, this statistic is important.\u00a0 Current research doesn\u2019t point to any one statin as being safer or effective for older adults. Ultimately, healthcare providers will tailor treatment on an individual basis. \u201cThere is no single best statin for elderly, and statin choice will typically come down to provider preference and individual tolerance,\u201d says <\/span><span style=\"font-weight: 400;\">Austin Shuxiao<\/span><span style=\"font-weight: 400;\">, MD, internal medicine physician, founder, and medical director of PeachIV.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Despite the limited research comparing statins in older adults specifically, past clinical trials and studies in statin users highlight some points to consider.\u00a0<\/span><\/p>\n<h3 id=\"lipitor-atorvastatin\"><span style=\"font-weight: 400;\">Lipitor (atorvastatin)<\/span><\/h3>\n<p><span style=\"font-weight: 400;\">There isn\u2019t much research comparing the effects of <\/span><a href=\"https:\/\/www.singlecare.com\/prescription\/atorvastatin-calcium\/what-is\"><span style=\"font-weight: 400;\">atorvastatin<\/span><\/a> on <span style=\"font-weight: 400;\">older adults specifically, but there is some evidence for its benefits in this age group. In the <\/span><a href=\"https:\/\/www.acc.org\/Latest-in-Cardiology\/Clinical-Trials\/2010\/02\/23\/19\/22\/SAGE\"><span style=\"font-weight: 400;\">Study Assessing Goals in the Elderly (SAGE) trial <\/span><\/a><span style=\"font-weight: 400;\">\u00a0people ages 65 to 85 taking a high dose (80 mg) of atorvastatin had a <\/span><a href=\"https:\/\/www.mdpi.com\/1648-9144\/55\/11\/721\"><span style=\"font-weight: 400;\">lower rate of death by any cause<\/span><\/a><span style=\"font-weight: 400;\"> compared to people taking <a href=\"https:\/\/www.singlecare.com\/prescription\/pravastatin-sodium\/dosage\">40 mg pravastatin<\/a>, but there wasn\u2019t a significant decrease in their major cardiovascular events.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">In <\/span><a href=\"https:\/\/www.nejm.org\/doi\/full\/10.1056\/NEJMoa061894\"><span style=\"font-weight: 400;\">another study<\/span><\/a><span style=\"font-weight: 400;\">\u00a0on people who had a stroke previously, 80 mg of atorvastatin decreased the risk of recurrent stroke, but increased the risk of a specific type of stroke, hemorrhagic stroke.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">According to its <\/span><a href=\"https:\/\/dailymed.nlm.nih.gov\/dailymed\/drugInfo.cfm?setid=73cefb0b-1747-4fe0-b252-377f4ed94c6f\"><span style=\"font-weight: 400;\">drug label<\/span><\/a><span style=\"font-weight: 400;\">, there\u2019s no overall difference in the safety or effectiveness of atorvastatin in older adults compared to younger adults. However, atorvastatin in older adults over 65 increases the risk of myopathy and rhabdomyolysis (muscle breakdown).<\/span><\/p>\n<h3 id=\"crestor-rosuvastatin\"><span style=\"font-weight: 400;\">Crestor (rosuvastatin)<\/span><\/h3>\n<p><span style=\"font-weight: 400;\">In clinical trials, Crestor and its generic <\/span><a href=\"https:\/\/www.singlecare.com\/prescription\/rosuvastatin-calcium\/what-is\"><span style=\"font-weight: 400;\">rosuvastatin<\/span><\/a><span style=\"font-weight: 400;\"> have some evidence supporting their heart health benefits in people older than 70. In the Justification for the Use of Statin in Prevention: An Intervention Trial Evaluating Rosuvastatin (JUPITER) and Heart Outcomes Prevention Evaluation-3 (HOPE-3) <a href=\"https:\/\/www.nejm.org\/doi\/full\/10.1056\/NEJMoa0807646\">trials<\/a><\/span><span style=\"font-weight: 400;\">, rosuvastatin was found to lower the risk of non-fatal heart attacks and strokes and death due to cardiovascular issues by 25% in people older than 70 without a history of previous heart attack or stroke.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">According to its <\/span><a href=\"https:\/\/dailymed.nlm.nih.gov\/dailymed\/drugInfo.cfm?setid=6985e729-d963-4f32-89a1-727472b86daa\"><span style=\"font-weight: 400;\">drug label<\/span><\/a><span style=\"font-weight: 400;\">, there\u2019s no overall difference in the safety or effectiveness of atorvastatin in older adults compared to younger adults. The PROSPER trial found that 40 mg of pravastatin reduced the risk of nonfatal heart attack, cardiovascular death, and stroke by 15% compared to placebo.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">In the Antihypertensive and Lipid-Lowering Treatment to Prevent Heart Attack Trial (ALLHAT) <a href=\"https:\/\/jamanetwork.com\/journals\/jama\/fullarticle\/195627\">trial<\/a>, <\/span><span style=\"font-weight: 400;\">people older than 65 didn\u2019t have a significant improvement in their coronary artery disease (CAD) or rate of death compared to usual care. This trial also found a higher risk of death in people over 75 using statins, but other studies done since then show no change in the risk of death in this population.\u00a0<\/span><\/p>\n<p><a href=\"https:\/\/dailymed.nlm.nih.gov\/dailymed\/drugInfo.cfm?setid=6c011348-a236-47d3-bbef-3e0b956dd080\"><span style=\"font-weight: 400;\">Pravastatin\u2019s drug label<\/span><\/a><span style=\"font-weight: 400;\"> states no significant differences in safety or effectiveness between older adults and younger adults. But similar to other statins, adults 65 and older have an increased risk of myopathy and rhabdomyolysis.\u00a0<\/span><\/p>\n<h3 id=\"zocor-simvastatin\"><span style=\"font-weight: 400;\">Zocor (simvastatin)<\/span><\/h3>\n<p><span style=\"font-weight: 400;\">Zocor and its generic <\/span><a href=\"https:\/\/www.singlecare.com\/prescription\/simvastatin\/what-is\"><span style=\"font-weight: 400;\">simvastatin<\/span><\/a><span style=\"font-weight: 400;\"> have more of a beneficial effect in people younger than 65. In the <a href=\"https:\/\/www.thelancet.com\/journals\/lancet\/article\/PIIS0140-6736(02)09327-3\/abstract\">Heart Protection Study (HPS)<\/a><\/span><span style=\"font-weight: 400;\">, simvastatin decreased the risk of cardiovascular events by 24% in people younger than 65. In people 70 to 80, simvastatin still decreased the risk of cardiovascular events, 18% compared to placebo.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">People older than 65 <a href=\"https:\/\/www.singlecare.com\/prescription\/simvastatin\/dosage\">taking simvastatin<\/a> had a <\/span><a href=\"https:\/\/dailymed.nlm.nih.gov\/dailymed\/drugInfo.cfm?setid=5c1c694c-4b08-469e-b538-08e69df06146\"><span style=\"font-weight: 400;\">higher chance of myopathy<\/span><\/a><span style=\"font-weight: 400;\"> and rhabdomyolysis compared to people younger than 65.\u00a0<\/span><\/p>\n<h2 id=\"statin-research-in-older-adults\"><span style=\"font-weight: 400;\">Statin research in older adults<\/span><\/h2>\n<p><span style=\"font-weight: 400;\">It\u2019s worth noting that many of these statin trials were published more than 20 years ago. Though current research doesn\u2019t have a clear picture of the best statins for older adults, there is a major clinical trial called <\/span><a href=\"https:\/\/www.monash.edu\/medicine\/staree\/home\"><span style=\"font-weight: 400;\">Statins in Reducing Events in the Elderly (STAREE)<\/span><\/a><span style=\"font-weight: 400;\"> being conducted in Australia in people 70 and older. Older adults may be at higher risk for these effects, but pravastatin and <\/span><a href=\"https:\/\/www.singlecare.com\/prescription\/lescol-xl\"><span style=\"font-weight: 400;\">Lescol<\/span><\/a><span style=\"font-weight: 400;\"> (fluvastatin) are associated with fewer muscle-related side effects compared to other statins. These may include increases in hemoglobin A1c (HbA1c) and fasting blood glucose levels. This may be a concern for those with diabetes or prediabetes.<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Liver-related side effects may appear as elevated liver enzymes in blood work. If this occurs, providers may lower the dose, switch to pravastatin, or consider alternative treatments.<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Kidney-related issues may present as protein or blood in the urine, and there\u2019s an increased risk of kidney injury or even renal failure if rhabdomyolysis develops. Rosuvastatin and simvastatin are more likely to affect kidney function, while atorvastatin, fluvastatin, and pravastatin tend to have a lower risk.<\/span><\/li>\n<\/ul>\n<p><span style=\"font-weight: 400;\">Statins also carry the warning that they can affect cognitive function, such as memory loss and confusion.
