Yet dropping overall low-density lipoprotein cholesterol (LDL) levels isn\u2019t the only benefit of statins. They can also help prevent kidney damage and cardiovascular disease in people with chronic kidney disease (CKD). That said, there are some considerations people with kidney disease should consider before starting statin therapy. Here\u2019s all the info you need about taking statins for patients with kidney disease.<\/span><\/p>\n<h2 id=\"how-do-statins-affect-people-with-kidney-disease\"><span style=\"font-weight: 400;\">How do statins affect people with kidney disease?<\/span><\/h2>\n<p><span style=\"font-weight: 400;\">There are two main reasons why statins are helpful for people with kidney disease: They reduce the risk of heart disease and may help prevent kidney damage.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">The <\/span><a href=\"https:\/\/www.ahajournals.org\/doi\/10.1161\/CIRCULATIONAHA.120.050686\"><span style=\"font-weight: 400;\">American Heart Association says<\/span><\/a><span style=\"font-weight: 400;\"> that half of people with stage 4 or 5 CKD also have cardiovascular disease. And <\/span><a href=\"https:\/\/pmc.ncbi.nlm.nih.gov\/articles\/PMC4119330\/\"><span style=\"font-weight: 400;\">studies show <\/span><\/a><span style=\"font-weight: 400;\">that the cardiovascular mortality rate is 58%\u201371% for people with CKD, compared to 22%\u201327.5% for people with normal renal function. So statins, which lower the risk of cardiovascular issues, are important for these patients.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Statins help prevent the buildup of LDL-C in the blood vessels, and that includes vessels feeding the kidneys. Also, <\/span><a href=\"https:\/\/pmc.ncbi.nlm.nih.gov\/articles\/PMC8901279\/\"><span style=\"font-weight: 400;\">a 2021 study<\/span><\/a><span style=\"font-weight: 400;\"> showed statin use helps slow kidney disease progression by reducing urinary albumin and protein excretion and increasing creatinine clearance. \u201cThey also provide additional benefits, such as reducing inflammation, offering antioxidant protection against cell damage, improving blood flow, and slowing the buildup of plaque and hardening of the arteries,\u201d says <\/span><a href=\"https:\/\/www.linkedin.com\/in\/raeeda-gheewala-m-d-52425b23\/\"><span style=\"font-weight: 400;\">Raeeda Gheewala, MD<\/span><\/a><span style=\"font-weight: 400;\">, founder and CEO of <\/span><a href=\"https:\/\/sportsnephrology.com\/\"><span style=\"font-weight: 400;\">Sports Nephrology<\/span><\/a><span style=\"font-weight: 400;\">.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">\u201cIn patients with end-stage kidney failure already on dialysis, evidence supporting the initiation of statins is less robust; however, continuing statin therapy is reasonable if it is well tolerated,\u201d Dr. Gheewala adds. \u201cAn individualized approach is essential, and this is where the art of medicine truly comes into play.\u201d<\/span><\/p>\n<p><span style=\"font-weight: 400;\">But there are downsides to statin use, too. While statins typically help protect the kidneys, <\/span><a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/26742473\/\"><span style=\"font-weight: 400;\">research from 2016<\/span><\/a><span style=\"font-weight: 400;\"> showed that long-term statin use might actually increase the risk of acute kidney injury for some people. Additionally, CKD patients who take statins may have <\/span><a href=\"https:\/\/pmc.ncbi.nlm.nih.gov\/articles\/PMC5874483\/\"><span style=\"font-weight: 400;\">an increased risk of myopathy<\/span><\/a><span style=\"font-weight: 400;\">\u2014including muscle pain and muscle breakdown. <\/span><a href=\"https:\/\/pmc.ncbi.nlm.nih.gov\/articles\/PMC4353943\/\"><span style=\"font-weight: 400;\">One study showed<\/span><\/a><span style=\"font-weight: 400;\"> an elevated risk of rhabdomyolysis, a particular type of myopathy where components of broken-down muscle are released into the bloodstream.\u00a0\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">This is why healthcare providers monitor kidney function and overall health routinely when you\u2019re taking a statin for CKD.\u00a0<\/span><\/p>\n<h2 id=\"top-recommended-statins-for-people-with-kidney-disease\"><span style=\"font-weight: 400;\">Top recommended statins for people with kidney disease<\/span><\/h2>\n<p><span style=\"font-weight: 400;\">There are many different statins on the market, which might leave you wondering which one is best for kidney disease. Ultimately, that\u2019s your healthcare provider\u2019s call to make, since they know your medical condition, history, other medicines, and overall health. But here are a few of the most commonly prescribed statins for people with kidney disease.\u00a0<\/span><\/p>\n<h3 id=\"lipitor-atorvastatin\"><span style=\"font-weight: 400;\">Lipitor (atorvastatin)<\/span><\/h3>\n<p><span style=\"font-weight: 400;\">\u201c\u200b\u200bAtorvastatin is often the first choice in my patients with mild-to-moderate CKD (stages 1\u20133) because it typically does not require dose adjustment in these stages and has extensive clinical trial data supporting its safety and efficacy,\u201d says Jason B. Carter, MD, a medical adviser at <\/span><a href=\"http:\/\/www.aeroflowurology.com\"><span style=\"font-weight: 400;\">Aeroflow Urology<\/span><\/a><span style=\"font-weight: 400;\">. \u201cAlso, its lipophilic nature allows for broad tissue distribution, contributing to its LDL-lowering effects.\u201d<\/span><\/p>\n<p><span style=\"font-weight: 400;\">A study from the <\/span><a href=\"https:\/\/www.ahajournals.org\/doi\/10.1161\/JAHA.118.010827\"><i><span style=\"font-weight: 400;\">Journal of the American Heart Association<\/span><\/i><\/a><span style=\"font-weight: 400;\"> found that atorvastatin both improved kidney function and decreased cardiovascular risk. It\u2019s also excreted through the liver, not the kidneys, so it avoids making the kidneys work harder. And dialysis doesn\u2019t remove it, so it doesn\u2019t require different doses for dialysis and non-dialysis patients. All that combined makes it a fairly safe and effective option for CKD patients.\u00a0<\/span><\/p>\n<h3 id=\"crestor-rosuvastatin\"><span style=\"font-weight: 400;\">Crestor (rosuvastatin)<\/span><\/h3>\n<p><span style=\"font-weight: 400;\">Like Lescol and Lipitor, <\/span><a href=\"https:\/\/www.sciencedirect.com\/science\/article\/pii\/S0929664624004741\"><span style=\"font-weight: 400;\">Crestor has shown efficacy<\/span><\/a><span style=\"font-weight: 400;\"> in reducing the risk of major adverse cardiovascular events (like a heart attack) for people with kidney disease. However, it also presents a higher risk of kidney-related side effects, including <\/span><a href=\"https:\/\/www.pharmacytimes.com\/view\/rosuvastatin-linked-to-signs-of-kidney-damage\"><span style=\"font-weight: 400;\">kidney damage at higher doses<\/span><\/a><span style=\"font-weight: 400;\">. Plus, people who have severe renal impairment might need a dose adjustment. \u201cIn patients with advanced CKD (stages 4-5), lower starting doses are recommended to minimize the risk of side effects such as statin-associated myopathy due to reduced renal clearance,\u201d Dr. Carter says.\u00a0<\/span><\/p>\n<h3 id=\"pravachol-pravastatin\"><span style=\"font-weight: 400;\">Pravachol (Pravastatin)<\/span><\/h3>\n<p><span style=\"font-weight: 400;\">\u201cPravastatin may be selected for patients at higher risk of drug\u2013drug interactions or those with a history of muscle-related adverse effects,\u201d Dr. Carter says. \u201cIt is more hydrophilic than other statins, which limits its penetration into non-hepatic tissues, including muscle.\u201d But while it\u2019s a good option for patients on multiple drugs, it\u2019s a lower intensity statin, so it might not lower LDL quite as much as Lipitor or Crestor.\u00a0<\/span><\/p>\n<h3 id=\"lescol-fluvastatin\"><span style=\"font-weight: 400;\">Lescol (fluvastatin)<\/span><\/h3>\n<p><span style=\"font-weight: 400;\">Lescol and Lipitor work the same way\u2014by inhibiting a liver enzyme that helps make cholesterol\u2014so it makes sense that both are effective for kidney disease. In fact, <\/span><a href=\"https:\/\/www.nature.com\/articles\/s41598-019-53064-x\"><span style=\"font-weight: 400;\">a 2019 study<\/span><\/a><span style=\"font-weight: 400;\"> found that fluvastatin might perform even better than atorvastatin in some metrics. It\u2019s also filtered out by the liver, not the kidneys, and it doesn\u2019t require dose adjustments based on the severity of the renal impairment. Any indication of kidney issues, like significant changes in urinary output, should also be addressed by a healthcare provider.\u00a0<\/span><\/p>\n<h2 id=\"special-considerations-for-people-with-ckd-taking-statins\"><span style=\"font-weight: 400;\">Special considerations for people with CKD taking statins<\/span><\/h2>\n<p><span style=\"font-weight: 400;\">Aside from side effects and other precautions, there are some considerations healthcare providers and patients will need to assess before prescribing statins.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Depending on which statin you\u2019re taking, dosage can be particularly important. Some statins will require <\/span><a href=\"https:\/\/www.ncbi.nlm.nih.gov\/books\/NBK305899\/table\/lipid_dyslipid-ckd.T.statin_dosing_in_ck\/\"><span style=\"font-weight: 400;\">different doses based on the stage of kidney disease<\/span><\/a><span style=\"font-weight: 400;\"> you\u2019re in, or whether you\u2019re on dialysis. Some statins aren\u2019t recommended at all for late-stage kidney disease, kidney failure, or dialysis patients.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">\u201c\u200b\u200bDose adjustments may be necessary based on CKD stage estimated by glomerular filtration rate (eGFR),\u201d Dr. Carter says. \u201cSince CKD patients may have a higher baseline risk of statin-induced myopathy, especially at higher doses or when interacting medications are present, I counsel my patients to promptly report muscle pain, weakness, or dark urine.\u201d<\/span><\/p>\n<p><span style=\"font-weight: 400;\">In the same vein, statins that are metabolized by the liver are preferable to those excreted through the kidneys. When the kidneys function inefficiently, they have more trouble completely eliminating renally excreted drugs. Since statins can cause muscle damage and affect blood sugar levels\u2014<\/span><a href=\"https:\/\/www.thelancet.com\/journals\/landia\/article\/PIIS2213-8587(24)00040-8\/fulltext\"><span style=\"font-weight: 400;\">potentially leading to new-onset diabetes mellitus<\/span><\/a><span style=\"font-weight: 400;\">\u2014healthcare providers will also likely want to monitor those levels regularly.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Most importantly, inform a healthcare professional about any changes in your health or side effects you experience during statin treatment.
