At the same time, research shows that statins may raise blood sugar levels, which can affect people with diabetes or a risk of diabetes.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">This article explores the connection between statins and diabetes, including how statins may affect blood sugar, which statins may be preferred in people with diabetes, and what to discuss with your healthcare provider before starting or continuing treatment.<\/span><\/p>\n<h2 id=\"the-relationship-between-diabetes-and-cardiovascular-disease\"><span style=\"font-weight: 400;\">The relationship between diabetes and cardiovascular disease<\/span><\/h2>\n<p><span style=\"font-weight: 400;\">People with diabetes are more likely to develop heart disease, including high blood pressure, heart attacks, and heart failure. According to the Centers for Disease Control and Prevention (CDC), adults with Type 2 diabetes are nearly twice as likely to develop heart disease compared to those without diabetes. For people with Type 1 diabetes, the risk can be up to 10 times higher.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Managing blood sugar and cholesterol levels is important to <\/span><a href=\"https:\/\/www.cdc.gov\/heart-disease\/prevention\/index.html\"><span style=\"font-weight: 400;\">lower the risk of heart disease<\/span><\/a><span style=\"font-weight: 400;\">. That\u2019s why healthcare providers often prescribe statins to people with diabetes, especially those with additional cardiovascular risk factors. However, <\/span><a href=\"https:\/\/www.cdc.gov\/diabetes\/diabetes-complications\/statins-and-diabetes.html\"><span style=\"font-weight: 400;\">statins can slightly raise blood sugar levels<\/span><\/a><span style=\"font-weight: 400;\"> by affecting how the body responds to insulin, a concern for people already managing diabetes.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">In 2012, the Food and Drug Administration (FDA) announced that all statins should carry a warning about <\/span><a href=\"https:\/\/www.fda.gov\/drugs\/drug-safety-and-availability\/fda-drug-safety-communication-important-safety-label-changes-cholesterol-lowering-statin-drugs\"><span style=\"font-weight: 400;\">their potential to increase fasting blood sugar and hemoglobin A1c<\/span><\/a><span style=\"font-weight: 400;\"> (HbA1c) levels in people who take them. Clinical trials have found that statin use is associated with a <\/span><a href=\"https:\/\/www.frontiersin.org\/journals\/endocrinology\/articles\/10.3389\/fendo.2023.1239335\/full\"><span style=\"font-weight: 400;\">9%\u201312% increased risk of developing Type 2 diabetes<\/span><\/a><span style=\"font-weight: 400;\">. Some observational studies have reported even higher risks, particularly with high-intensity statin therapy.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Still, the cardiovascular benefits of statins often outweigh these risks. A meta-analysis found that statin therapy in people with diabetes <\/span><a href=\"https:\/\/www.sciencedirect.com\/science\/article\/pii\/S0939475324001650\"><span style=\"font-weight: 400;\">reduced their risk of major cardiovascular events<\/span><\/a><span style=\"font-weight: 400;\"> by 27%\u201330% compared to those not taking a statin. Starting statin therapy earlier may offer even greater protection. A 2025 study showed that people with diabetes who delayed statin therapy <\/span><a href=\"https:\/\/www.ahajournals.org\/doi\/10.1161\/JAHA.124.040464\"><span style=\"font-weight: 400;\">had significantly higher rates of heart disease<\/span><\/a><span style=\"font-weight: 400;\"> than those who began treatment when first prescribed.\u00a0<\/span><\/p>\n<h2 id=\"top-recommended-statins-for-people-with-diabetes\"><span style=\"font-weight: 400;\">Top recommended statins for people with diabetes<\/span><\/h2>\n<p><span style=\"font-weight: 400;\">There\u2019s no single best statin for people with diabetes, but healthcare providers typically choose one based on a person\u2019s medical history and overall risk of heart disease. The American Diabetes Association\u2019s (ADA\u2019s) clinical guidelines <\/span><a href=\"https:\/\/diabetesjournals.org\/care\/article\/47\/Supplement_1\/S179\/153957\/10-Cardiovascular-Disease-and-Risk-Management\"><span style=\"font-weight: 400;\">recommend statin therapy<\/span><\/a><span style=\"font-weight: 400;\"> for primary and secondary prevention of cardiovascular disease. That is, whether someone has ever had a heart attack or stroke before, starting statin therapy can lower their risk of heart disease and future cardiovascular events. This is especially important for preventing atherosclerosis, the hardening of arteries caused by high cholesterol, says <\/span><a href=\"https:\/\/www.diabetesteam.com\/writers\/6791512af76f05260bdd2a63\"><span style=\"font-weight: 400;\">Kristen Fuller<\/span><\/a><span style=\"font-weight: 400;\">, MD, a family medicine physician in Mammoth Lakes, California.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">The ADA guidelines suggest the following:<\/span><\/p>\n<ul>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Moderate-intensity statin therapy is recommended for people ages 40\u201370 with diabetes but no history of <\/span><a href=\"https:\/\/www.heart.org\/en\/professional\/quality-improvement\/ascvd\"><span style=\"font-weight: 400;\">atherosclerotic cardiovascular disease<\/span><\/a><span style=\"font-weight: 400;\"> (ASCVD), in combination with lifestyle changes.<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">High-intensity statin therapy is recommended for those at higher cardiovascular risk, with known ASCVD, or with a history of heart attack or stroke.<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">People 20\u201339 with ASCVD risk factors should consider starting statin therapy in addition to lifestyle changes.\u00a0<\/span><\/li>\n<\/ul>\n<table>\n<tbody>\n<tr>\n<td>\n<h3 id=\"about-statin-intensity\"><span style=\"font-weight: 400;\">About statin intensity\u00a0<\/span><\/h3>\n<p><a href=\"https:\/\/www.ncbi.nlm.nih.gov\/books\/NBK583664\/table\/ch1.tab1\/\"><span style=\"font-weight: 400;\">Statin intensity<\/span><\/a><span style=\"font-weight: 400;\"> refers to a specific type and strength of statin that is expected to lower \u201cbad cholesterol,\u201d or low-density lipoprotein (LDL) cholesterol, by a certain amount.\u00a0<\/span><\/p>\n<ul>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Low-intensity statins can lower LDL cholesterol by less than 30%<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Moderate-intensity statins lower LDL cholesterol by 30%\u201349%<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">High-intensity statins lower LDL cholesterol by 50% or trends have been observed with rosuvastatin, where higher doses may be more likely to raise HbA1c levels.<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><b>Zocor (simvastatin): <\/b><span style=\"font-weight: 400;\">People with prediabetes taking low- to moderate-intensity simvastatin had higher HbA1c levels compared to those on atorvastatin, pitavastatin, or placebo.<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><b>Livalo (pitavastatin): <\/b><span style=\"font-weight: 400;\">Those on moderate-intensity pitavastatin had the lowest HbA1c levels compared to those on high-intensity atorvastatin and moderate-intensity simvastatin.<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><b>Pravachol (pravastatin): <\/b><span style=\"font-weight: 400;\">Though there haven\u2019t been many recent studies on pravastatin\u2019s effect on blood sugar, one study published in 2001 found pravastatin therapy <\/span><a href=\"https:\/\/www.ahajournals.org\/doi\/10.1161\/01.CIR.103.3.357\"><span style=\"font-weight: 400;\">lowered the risk of developing diabetes by 30%<\/span><\/a><span style=\"font-weight: 400;\"> in people with normal fasting blood glucose.\u00a0<\/span><\/li>\n<\/ul>\n<h3 id=\"statins-in-people-with-type-2-diabetes\"><span style=\"font-weight: 400;\">Statins in people with Type 2 diabetes<\/span><\/h3>\n<p><span style=\"font-weight: 400;\">For people already diagnosed with Type 2 diabetes, statins are a key part of preventing major adverse cardiovascular events. However, research shows some differences in how statins may affect blood sugar control in this population:<\/span><\/p>\n<ul>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><b>Atorvastatin:<\/b><span style=\"font-weight: 400;\"> Some research has found that <\/span><a href=\"https:\/\/onlinelibrary.wiley.com\/doi\/10.1111\/jcpt.12690\"><span style=\"font-weight: 400;\">high-intensity atorvastatin may slightly raise HbA1c<\/span><\/a><span style=\"font-weight: 400;\"> levels compared to placebo or moderate-intensity dosing. High-intensity <a href=\"https:\/\/www.singlecare.com\/blog\/atorvastatin-without-insurance\/\">atorvastatin<\/a> was also associated with higher fasting glucose compared to placebo, moderate-intensity doses, and other statins such as rosuvastatin and simvastatin.<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><b>Simvastatin: <\/b><span style=\"font-weight: 400;\">Moderate-intensity simvastatin was shown to reduce HbA1c in people with Type 2 diabetes compared to high-intensity atorvastatin and rosuvastatin.<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><b>Pitavastatin: <\/b><span style=\"font-weight: 400;\">Moderate-intensity pitavastatin may lower HbA1c in people with Type 2 diabetes, potentially making it a preferred option for those concerned about glucose control.<\/span><\/li>\n<\/ul>\n<h3 id=\"statins-after-a-heart-attack\"><span style=\"font-weight: 400;\">Statins after a heart attack<\/span><\/h3>\n<p><span style=\"font-weight: 400;\">Secondary prevention is when someone starts a treatment to prevent major adverse cardiovascular events after they\u2019ve already had a stroke or heart attack. In people with diabetes who are using statins for secondary prevention, high-intensity statin therapy is recommended by the ADA to prevent a future event. However, high-intensity statins can impact someone\u2019s blood sugar control more.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">One study found <\/span><a href=\"https:\/\/www.nature.com\/articles\/s41598-024-67585-7\"><span style=\"font-weight: 400;\">no significant difference in new diabetes diagnoses between moderate- and high-intensity atorvastatin<\/span><\/a><span style=\"font-weight: 400;\"> in people with diabetes and a history of heart attack. Instead, they choose a statin based on a person\u2019s age, cardiovascular risk, medication history, and potential side effects. \u201cI personally don\u2019t have one statin I prefer for diabetes,\u201d says <\/span><span style=\"font-weight: 400;\">Austin <\/span><span style=\"font-weight: 400;\">Shuxiao<\/span><span style=\"font-weight: 400;\">, MD, an internal medicine physician and medical director of PeachIV in New York City.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">If someone experiences side effects such as muscle pain or elevated blood sugar, healthcare providers may adjust the dose or switch to a different statin with a better profile for that individual. \u201cI use other statins when patients present to me already on another statin, or if they have intolerable side effects,\u201d Dr. Shuxiao shares.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">In addition, the ADA and CDC recommend statin therapy along with lifestyle changes to support heart and metabolic health. Specific recommendations may include \u201cdaily exercise, eating a heart-healthy <\/span><a href=\"https:\/\/www.singlecare.com\/blog\/diabetes-diet\/\"><span style=\"font-weight: 400;\">diabetes diet<\/span><\/a><span style=\"font-weight: 400;\">, avoiding alcohol and smoking, and maintaining a healthy weight,\u201d Dr. Fuller says.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Ongoing communication with your healthcare provider is important in order to create an individualized treatment plan that works for you.
