Eliquis is a type of <\/span><a href=\"https:\/\/www.singlecare.com\/drug-classes\/factor-xa-inhibitors\"><span style=\"font-weight: 400;\">factor Xa inhibitor<\/span><\/a><span style=\"font-weight: 400;\"> and is approved by the Food and Drug Administration to reduce the risk of systemic embolism and support stroke prevention in adults with atrial fibrillation (AFib). It\u2019s also indicated to treat and reduce deep vein thrombosis (DVT) or pulmonary embolism (PE) and prevent DVT and PE after hip or knee replacement surgery.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Sometimes your dose of apixaban (Eliquis) needs to be decreased from the standard recommended dose of Eliquis, which ranges from 2.5 mg to 10 mg twice daily. Here, we\u2019ll go over everything you need to know about Eliquis dose reduction, including when it might be recommended, by how much your dose may be reduced, and the criteria healthcare providers use when they recommend an Eliquis dose reduction.<\/span><\/p>\n<h2 id=\"why-an-eliquis-dose-reduction-may-be-recommended\"><span style=\"font-weight: 400;\">Why an Eliquis dose reduction may be recommended<\/span><\/h2>\n<p><span style=\"font-weight: 400;\">A dose reduction for Eliquis is usually only recommended in certain limited circumstances and among people with certain risk factors. Most commonly, Eliquis reduction is recommended when a patient\u2019s kidneys are having trouble processing the medication. These patients also often have other health vulnerabilities, too.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">\u201cPatients who are elderly or have advanced kidney disease or low body weight may need a lower dose,\u201d explains Aeshita Dwivedi, MD, cardiologist at Northwell\u2019s <\/span><a href=\"https:\/\/lenoxhill.northwell.edu\/\"><span style=\"font-weight: 400;\">Lenox Hill Hospital<\/span><\/a><span style=\"font-weight: 400;\">. In some cases, other considerations may be taken into account. \u201cIf you are at higher risk for bleeding from gastrointestinal bleeding or high fall risk, it may warrant a reduced dose on a case-by-case basis,\u201d Dr. Dwivedi shares.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">According to Mohamed Dahodwala, MD, cardiologist at <\/span><a href=\"https:\/\/sahchicago.org\/\"><span style=\"font-weight: 400;\">Saint Anthony Hospital<\/span><\/a><span style=\"font-weight: 400;\"> in Chicago, sometimes doses of Eliquis may be reduced to prevent long-term effects of the medication. He says that some studies have shown benefits, such as a lower risk of bleeding, with smaller doses long-term.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">For instance, <\/span><a href=\"https:\/\/www.michiganmedicine.org\/health-lab\/reducing-dose-popular-blood-thinners-may-limit-risk-future-bleeding\"><span style=\"font-weight: 400;\">a study from the University of Michigan<\/span><\/a><span style=\"font-weight: 400;\"> found that reduced-dose apixaban (Eliquis) or <\/span><a href=\"https:\/\/www.singlecare.com\/prescription\/xarelto\"><span style=\"font-weight: 400;\">Xarelto<\/span><\/a><span style=\"font-weight: 400;\"> (rivaroxaban)\u2014<\/span><span style=\"font-weight: 400;\">another oral anticoagulant\u2014could potentially reduce a person\u2019s risk of major bleeding events or hospital visits. This was studied in patients who were treated for a blood clot and were taking blood thinners to prevent them from returning. Here\u2019s what to know.<\/span><\/p>\n<h3 id=\"people-taking-eliquis-for-nonvalvular-atrial-fibrillation-afib\"><span style=\"font-weight: 400;\">People taking Eliquis for nonvalvular atrial fibrillation (AFib)<\/span><\/h3>\n<p><span style=\"font-weight: 400;\">Sometimes Eliquis reduction is recommended for <\/span><a href=\"https:\/\/www.singlecare.com\/blog\/living-with-afib\/\"><span style=\"font-weight: 400;\">atrial fibrillation<\/span><\/a><span style=\"font-weight: 400;\"> patients who are taking Eliquis to reduce the risk of stroke and embolism. In these cases, \u201cthere are specific criteria why someone would need a reduced dose of Eliquis,\u201d explains <\/span><a href=\"https:\/\/medicine.yale.edu\/profile\/joyce-oen-hsiao\/\"><span style=\"font-weight: 400;\">Joyce Oen-Hsiao<\/span><\/a><span style=\"font-weight: 400;\">, MD<\/span><span style=\"font-weight: 400;\">, associate professor of medicine at Yale School of Medicine.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">According to Dr. Oen-Hsiao, patients must have at least two out of the following criteria to warrant a dose reduction:<\/span><\/p>\n<ul>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Are 80 years old or older<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Have a body weight less than or equal to 60 kg (132 pounds)<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Have a serum creatinine baseline level greater than or equal to 1.5 mg\/dl, indicating renal impairment or kidney disease<\/span><\/li>\n<\/ul>\n<h3 id=\"when-eliquis-is-taken-with-certain-medications\"><span style=\"font-weight: 400;\">When Eliquis is taken with certain medications<\/span><\/h3>\n<p><span style=\"font-weight: 400;\">According to <\/span><a href=\"https:\/\/dailymed.nlm.nih.gov\/dailymed\/drugInfo.cfm?setid=e9481622-7cc6-418a-acb6-c5450daae9b0\"><span style=\"font-weight: 400;\">DailyMed<\/span><\/a><span style=\"font-weight: 400;\">, there are some medications that may require you to reduce your Eliquis dose. For example, coadministration of Eliquis with combined P-glycoprotein (P-gp) and strong cytochrome P450 3A4 (CYP3A4) inhibitors requires you to reduce your daily dose of Eliquis by 50%. That means, if you previously took 5mg twice a day, you\u2019d reduce it to 2.5mg twice a day. People who take Eliquis 2.5 mg twice daily should not take these interacting medicines at all.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Examples of combined P-glycoprotein (P-gp) and strong cytochrome P450 3A4 (CYP3A4) inhibitors are <\/span><a href=\"https:\/\/www.singlecare.com\/prescription\/ketoconazole\"><span style=\"font-weight: 400;\">ketoconazole<\/span><\/a><span style=\"font-weight: 400;\">, <\/span><a href=\"https:\/\/www.singlecare.com\/prescription\/itraconazole\"><span style=\"font-weight: 400;\">itraconazole<\/span><\/a><span style=\"font-weight: 400;\">, and <\/span><a href=\"https:\/\/www.singlecare.com\/prescription\/ritonavir\"><span style=\"font-weight: 400;\">ritonavir<\/span><\/a><span style=\"font-weight: 400;\">. However, the manufacturer of Eliquis, <\/span><a href=\"https:\/\/www.eliquis.com\/assets\/buildeasy\/us-commercial\/eliquis-hcp\/en\/resources\/pdf\/Eliquis-Dosing-Guide-Desktop-Version.pdf\"><span style=\"font-weight: 400;\">Bristol Myers Squibb<\/span><\/a><span style=\"font-weight: 400;\">, says<\/span><span style=\"font-weight: 400;\"> that the antibiotic <\/span><a href=\"https:\/\/www.singlecare.com\/prescription\/clarithromycin\"><span style=\"font-weight: 400;\">clarithromycin<\/span><\/a><span style=\"font-weight: 400;\"> is safe to take with Eliquis and doesn\u2019t warrant an Eliquis dose reduction.<\/span><\/p>\n<h3 id=\"during-the-course-of-treatment-for-dvt-or-pe\"><span style=\"font-weight: 400;\">During the course of treatment for DVT or PE<\/span><\/h3>\n<p><span style=\"font-weight: 400;\">When you\u2019re being treated for venous thromboembolism, which includes <\/span><a href=\"https:\/\/www.singlecare.com\/conditions\/deep-vein-thrombosis-treatment-and-medications\"><span style=\"font-weight: 400;\">deep vein thrombosis (DVT)<\/span><\/a><span style=\"font-weight: 400;\"> or <\/span><a href=\"https:\/\/www.singlecare.com\/conditions\/pulmonary-embolism-symptoms\"><span style=\"font-weight: 400;\">pulmonary embolism (PE)<\/span><\/a><span style=\"font-weight: 400;\">, your Eliquis dose will be reduced over the course of treatment. According to Bristol Myers Squibb, people with venous thromboembolism will be prescribed 10 mg of Eliquis for their first seven days of treatment. After this, they will transition to 5 mg, taken twice daily.<\/span><\/p>\n<h2 id=\"what-is-a-reduced-dose-of-eliquis\"><span style=\"font-weight: 400;\">What is a reduced dose of Eliquis?<\/span><\/h2>\n<p><span style=\"font-weight: 400;\">Standard doses of Eliquis are between 2.5 mg and 10 mg, as a twice-daily dose. Dosing depends on which condition is being treated:<\/span><\/p>\n<ul>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><b>For stroke prevention or embolism reduction in people with nonvalvular atrial fibrillation:<\/b><span style=\"font-weight: 400;\"> 5 mg twice daily.<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><b>For the treatment of DVT or PE:<\/b><span style=\"font-weight: 400;\"> 10 mg twice daily for seven days; after this, 5 mg twice daily<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><b>For the risk reduction of recurrent DVT\/PE after initial therapy:<\/b><span style=\"font-weight: 400;\"> 2.5 mg twice daily, after at least six months of initial treatment<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><b>For the prevention of DVT or PE following hip replacement surgery:<\/b><span style=\"font-weight: 400;\"> 2.5 mg twice daily for 35 days<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><b>For the prevention of DVT or PE following knee replacement surgery: <\/b><span style=\"font-weight: 400;\">2.5 mg twice daily for 12 days<\/span><\/li>\n<\/ul>\n<p><span style=\"font-weight: 400;\">An Eliquis dose reduction depends on what your original treatment dose was, as well as why your dose needs to be reduced. Most commonly, Eliquis dose reduction is from 5mg to 2.5mg, twice daily. Whatever the case, when your dose is reduced, you will still take your medication twice per day, at the new dose prescribed by your healthcare provider.<\/span><\/p>\n<h2 id=\"risks-of-incorrect-eliquis-dosing\"><span style=\"font-weight: 400;\">Risks of incorrect Eliquis dosing<\/span><\/h2>\n<p><span style=\"font-weight: 400;\">You should never adjust your dose of Eliquis without your healthcare provider\u2019s guidance. That\u2019s because <\/span><a href=\"https:\/\/www.singlecare.com\/blog\/news\/medication-errors-statistics\/\"><span style=\"font-weight: 400;\">taking the wrong dosage<\/span><\/a><span style=\"font-weight: 400;\"> for your body can result in safety risks.
