Though it can be used in both adults and children older than 1, the drug carries some strict warnings about its safety in some people already taking other medications. This combination makes Entresto the <\/span><a href=\"https:\/\/www.ncbi.nlm.nih.gov\/books\/NBK507904\/\"><span style=\"font-weight: 400;\">first drug of its class<\/span><\/a><span style=\"font-weight: 400;\">, called angiotensin receptor neprilysin inhibitors (ARNIs).\u00a0<\/span><\/p>\n<p><a href=\"https:\/\/www.linkedin.com\/in\/dr-stephanie-b-nielsen-5a1744a1\/\"><span style=\"font-weight: 400;\">Stephanie Nielsen<\/span><\/a><span style=\"font-weight: 400;\">, Pharm.D., practitioner at The DNA Company, explains how the drug works: \u201cSacubitril helps open up your blood vessels and remove excess fluid, making it easier for your heart to pump. <\/span><a href=\"https:\/\/www.singlecare.com\/prescription\/valsartan\/\"><span style=\"font-weight: 400;\">Valsartan<\/span><\/a><span style=\"font-weight: 400;\"> will relax your blood vessels and lower your blood pressure, reducing strain on your heart. Together, they help your heart move blood more efficiently, which helps you feel better and breathe easier.\u201d\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Common side effects of Entresto include low blood pressure, which may manifest as dizziness, lightheadedness, or fatigue. The drug also increases potassium levels in the blood and can cause the development of a cough or kidney failure.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Entresto is not for people who are pregnant, as it can interfere with fetal kidney function. This is a <\/span><a href=\"https:\/\/www.singlecare.com\/blog\/black-box-warning\/\"><span style=\"font-weight: 400;\">black box warning<\/span><\/a><span style=\"font-weight: 400;\"> on the drug, meaning it\u2019s a warning for very serious or life-threatening risks of the drug.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Lastly, Entresto is also cautioned in people who have kidney disease, as it can decrease kidney function.\u00a0<\/span><\/p>\n<h2 id=\"entresto-drug-interactions\"><span style=\"font-weight: 400;\">Entresto drug interactions<\/span><\/h2>\n<p><span style=\"font-weight: 400;\">Entresto\u2019s combination of drugs leads to some of its potential interactions with a few common classes of drugs.\u00a0<\/span><\/p>\n<h3 id=\"angiotensin-converting-enzyme-ace-inhibitors\"><span style=\"font-weight: 400;\">Angiotensin converting enzyme (ACE) inhibitors<\/span><\/h3>\n<p><a href=\"https:\/\/www.singlecare.com\/drug-classes\/ace-inhibitors\"><span style=\"font-weight: 400;\">ACE inhibitors<\/span><\/a><span style=\"font-weight: 400;\"> are typically prescribed for treating high blood pressure, and work on a pathway called the renin-angiotensin-aldosterone system to regulate blood pressure. Examples of ACE inhibitors include <\/span><a href=\"https:\/\/www.singlecare.com\/prescription\/lisinopril\/what-is\"><span style=\"font-weight: 400;\">lisinopril<\/span><\/a><span style=\"font-weight: 400;\">, <\/span><a href=\"https:\/\/www.singlecare.com\/prescription\/quinapril-hcl\"><span style=\"font-weight: 400;\">quinapril<\/span><\/a><span style=\"font-weight: 400;\">, and <\/span><a href=\"https:\/\/www.singlecare.com\/prescription\/lotensin\/what-is\"><span style=\"font-weight: 400;\">Lotensin<\/span><\/a><span style=\"font-weight: 400;\"> (benazepril). \u201cACE inhibitors such as lisinopril or enalapril are used to lower blood pressure, but taking them with Entresto can actually contribute to breathing problems or even swelling,\u201d Dr. Nielsen says.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">\u201cPatients should know that Entresto cannot be used within 36 hours of an ACE inhibitor, which can increase the likelihood of swelling or drops in blood pressure,\u201d says <\/span><a href=\"https:\/\/welzo.com\/pages\/dr-blen-tesfu\"><span style=\"font-weight: 400;\">Blen Tesfu<\/span><\/a><span style=\"font-weight: 400;\">, MD, physician and medical adviser at Welzo. This reaction is called <\/span><a href=\"https:\/\/www.pennmedicine.org\/conditions\/angioedema\"><span style=\"font-weight: 400;\">angioedema<\/span><\/a><span style=\"font-weight: 400;\">, and it can be life-threatening.\u00a0<\/span><\/p>\n<h3 id=\"angiotensin-ii-receptor-blockers-arbs\"><span style=\"font-weight: 400;\">Angiotensin II receptor blockers (ARBs)<\/span><\/h3>\n<p><span style=\"font-weight: 400;\">It\u2019s recommended not to use Entresto with another ARB drug since Entresto already has valsartan. Taking the two can increase the risk of side effects. Common side effects of ARBs include a stuffy nose, upper respiratory infections, cough, feeling dizzy, and <\/span><a href=\"https:\/\/www.ncbi.nlm.nih.gov\/books\/NBK537027\/\"><span style=\"font-weight: 400;\">low blood pressure<\/span><\/a><span style=\"font-weight: 400;\">. The most serious effects of combining the two medications include high potassium levels, very low blood pressure, and kidney damage.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Common ARBs include <\/span><a href=\"https:\/\/www.singlecare.com\/prescription\/losartan-potassium\/what-is\"><span style=\"font-weight: 400;\">losartan<\/span><\/a><span style=\"font-weight: 400;\"> (generic Cozaar), <\/span><a href=\"https:\/\/www.singlecare.com\/prescription\/diovan\/what-is\"><span style=\"font-weight: 400;\">Diovan<\/span><\/a><span style=\"font-weight: 400;\"> (valsartan), <\/span><a href=\"https:\/\/www.singlecare.com\/prescription\/candesartan-cilexetil\"><span style=\"font-weight: 400;\">candesartan<\/span><\/a><span style=\"font-weight: 400;\">, and <\/span><a href=\"https:\/\/www.singlecare.com\/prescription\/avapro\/what-is\"><span style=\"font-weight: 400;\">Avapro<\/span><\/a><span style=\"font-weight: 400;\"> (irbesartan).\u00a0<\/span><\/p>\n<h3 id=\"aliskiren\"><span style=\"font-weight: 400;\">Aliskiren<\/span><\/h3>\n<p><a href=\"https:\/\/www.singlecare.com\/prescription\/aliskiren-fumarate\/what-is\"><span style=\"font-weight: 400;\">Aliskiren<\/span><\/a><span style=\"font-weight: 400;\"> is the generic version of the brand-name drug <\/span><a href=\"https:\/\/www.singlecare.com\/prescription\/tekturna\"><span style=\"font-weight: 400;\">Tekturna<\/span><\/a><span style=\"font-weight: 400;\">. Both are drugs prescribed to <\/span><a href=\"https:\/\/dailymed.nlm.nih.gov\/dailymed\/lookup.cfm?setid=f3fd2503-25ef-4660-a29a-f62c491e28fb\"><span style=\"font-weight: 400;\">lower blood pressure<\/span><\/a><span style=\"font-weight: 400;\"> in adults and children 6 years and older. People with Type 2 diabetes or kidney problems taking aliskiren should not take Entresto at the same time.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Taking aliskiren and Entresto together can increase the risk of <\/span><a href=\"https:\/\/www.ema.europa.eu\/en\/documents\/product-information\/entresto-epar-product-information_en.pdf\"><span style=\"font-weight: 400;\">low blood pressure<\/span><\/a><span style=\"font-weight: 400;\">, high blood potassium, and decreased kidney function.\u00a0<\/span><\/p>\n<h3 id=\"potassium-sparing-diuretics\"><span style=\"font-weight: 400;\">Potassium-sparing diuretics<\/span><\/h3>\n<p><a href=\"https:\/\/www.singlecare.com\/drug-classes\/potassium-sparing-diuretics\"><span style=\"font-weight: 400;\">Potassium-sparing diuretics<\/span><\/a><span style=\"font-weight: 400;\"> are medications most often prescribed for <\/span><a href=\"https:\/\/www.singlecare.com\/conditions\/hypertension-treatments-and-medications\"><span style=\"font-weight: 400;\">high blood pressure<\/span><\/a><span style=\"font-weight: 400;\"> or heart failure. They\u2019re a type of water pill that removes excess fluid from your body. As the name suggests, potassium-sparing diuretics keep potassium ions in the blood in exchange for eliminating sodium. \u201cWhen combined with Entresto, they can actually cause irregular heartbeats by raising your potassium levels too high,\u201d Nielsen says.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">High potassium in the blood is called hyperkalemia. There aren\u2019t typically signs of early hyperkalemia, but at certain levels, <\/span><a href=\"https:\/\/www.singlecare.com\/conditions\/hyperkalemia-symptoms\"><span style=\"font-weight: 400;\">hyperkalemia can feel like<\/span><\/a><span style=\"font-weight: 400;\"> heart palpitations, muscle weakness, and tingling sensations. Seek medical attention if you\u2019re taking Entresto and you feel any of these symptoms. If untreated, hyperkalemia can cause paralysis, heart attacks, or death.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Examples of potassium-sparing diuretics include <\/span><a href=\"https:\/\/www.singlecare.com\/prescription\/spironolactone\/what-is\"><span style=\"font-weight: 400;\">spironolactone<\/span><\/a><span style=\"font-weight: 400;\">, <\/span><a href=\"https:\/\/www.singlecare.com\/prescription\/triamterene\/what-is\"><span style=\"font-weight: 400;\">triamterene<\/span><\/a><span style=\"font-weight: 400;\">, and <\/span><a href=\"https:\/\/www.singlecare.com\/prescription\/amiloride-hcl\/what-is\"><span style=\"font-weight: 400;\">amiloride<\/span><\/a><span style=\"font-weight: 400;\">.<\/span><\/p>\n<h3 id=\"nonsteroidal-anti-inflammatory-drugs-nsaids\"><span style=\"font-weight: 400;\">Nonsteroidal anti-inflammatory drugs (NSAIDs)\u00a0<\/span><\/h3>\n<p><span style=\"font-weight: 400;\">Taking nonsteroidal anti-inflammatory drugs (NSAIDs), including selective cyclooxygenase-2 (COX-2) inhibitor drugs with Entresto, can worsen kidney function. Older adults, people who are on diuretics, or who have poor kidney function, are especially at risk of this interaction. People taking these medications with Entresto may have their kidney function monitored by their healthcare providers.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Common NSAIDs include <\/span><a href=\"https:\/\/www.singlecare.com\/prescription\/advil\/what-is\"><span style=\"font-weight: 400;\">Advil<\/span><\/a><span style=\"font-weight: 400;\"> (ibuprofen) and <\/span><a href=\"https:\/\/www.singlecare.com\/prescription\/naprosyn\/what-is\"><span style=\"font-weight: 400;\">Naprosyn<\/span><\/a><span style=\"font-weight: 400;\"> (naproxen), often used for pain management. <\/span><a href=\"https:\/\/www.singlecare.com\/prescription\/celebrex\/what-is\"><span style=\"font-weight: 400;\">Celebrex<\/span><\/a><span style=\"font-weight: 400;\"> (celecoxib) is a selective COX-2 inhibitor used for pain and headaches.<\/span><\/p>\n<h3 id=\"lithium\"><span style=\"font-weight: 400;\">Lithium<\/span><\/h3>\n<p><a href=\"https:\/\/www.singlecare.com\/prescription\/lithium\/what-is\"><span style=\"font-weight: 400;\">Lithium<\/span><\/a><span style=\"font-weight: 400;\"> may be prescribed for people with bipolar disorder. Healthcare providers prescribe and monitor lithium very carefully to ensure patients get a safe and effective dose of the drug to control and prevent manic episodes in people with bipolar disorder. Having lithium levels too high or too low could mean an increased risk of <\/span><a href=\"https:\/\/www.singlecare.com\/blog\/lithium-side-effects\/\"><span style=\"font-weight: 400;\">side effects<\/span><\/a><span style=\"font-weight: 400;\"> or uncontrolled symptoms.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Entresto increases lithium concentrations in the blood. People taking Entresto with lithium have a risk of lithium toxicity, which could cause symptoms like tremors, difficulty walking, and altered consciousness. Healthcare providers may order regular blood tests to make sure you have safe lithium levels while taking Entresto.<\/span><\/p>\n<h2 id=\"other-entresto-interactions\"><span style=\"font-weight: 400;\">Other Entresto interactions<\/span><\/h2>\n<p><span style=\"font-weight: 400;\">How Entresto works can also affect someone\u2019s lifestyle and diet. Because of valsartan\u2019s tendency to increase potassium levels in the blood, patients taking Entresto should be most mindful of sources of potassium in their diet.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Look for excess potassium in potassium supplements, potassium-rich foods, and salt substitutes containing potassium salts. <\/span><a href=\"https:\/\/www.singlecare.com\/blog\/potassium-rich-foods\/\"><span style=\"font-weight: 400;\">Potassium-rich foods<\/span><\/a><span style=\"font-weight: 400;\"> include avocados, dried fruits, bananas, potatoes, <\/span><a href=\"https:\/\/www.dietaryguidelines.gov\/food-sources-potassium\"><span style=\"font-weight: 400;\">beets<\/span><\/a><span style=\"font-weight: 400;\">, acorn squash, spinach and other greens, almonds, and coconut water.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">People taking Entresto should also exercise caution with drinking alcohol. \u201cIf you enjoy an afternoon drink to wind down, you might want to reconsider it for a while since alcohol and CBD can make you feel dizzy or confused, especially while your body is getting used to Entresto,\u201d Dr. Nielsen says. Alcohol <\/span><a href=\"https:\/\/www.singlecare.com\/blog\/valsartan-side-effects\/\"><span style=\"font-weight: 400;\">lowers blood pressure along with valsartan<\/span><\/a><span style=\"font-weight: 400;\">, so people mixing the two may experience certain symptoms of low blood pressure. Be sure to share your full medication list, alcohol consumption habits, and your regular diet to be able to make a plan around changes or adjustments to your care.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">When you start taking Entresto, it\u2019s also important to follow up with your healthcare provider about any new or worsening symptoms to ensure you have the right dosage. If you\u2019re not sure whether a symptom is due to starting a new drug, it\u2019s best to check with your healthcare provider first. Dr. Tesfu recommends close monitoring by a medical professional to minimize risks and maximize the benefits of the drug.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Finally, contact the provider who prescribed Entresto to let them know about any new drugs you\u2019re prescribed so they can check for interactions. \u201cYou should always talk with your healthcare provider or pharmacist before starting anything new,\u201d Dr. Nielsen advises.<\/span><\/p>\n<h2 id=\"bottom-line\"><span style=\"font-weight: 400;\">Bottom line<\/span><\/h2>\n<p><span style=\"font-weight: 400;\">Since Entresto is a combination of two drugs, sacubitril and valsartan, it comes with the potential for interactions with either drug.
