He explains that while the two do not interact directly, they have similar effects on the body. \u201cBoth caffeine and Sudafed are sympathomimetic agents and central nervous system stimulants,\u201d Dr. Cohen says. \u201cHence, both have similar cardiovascular and central nervous system (CNS) side effects.\u201d That means washing down a Sudafed with a sip of your morning coffee can exacerbate adverse reactions such as increases in <\/span><a href=\"https:\/\/www.singlecare.com\/blog\/ways-to-manage-high-blood-pressure-medication\/\"><span style=\"font-weight: 400;\">blood pressure<\/span><\/a><span style=\"font-weight: 400;\"> and heart rate, palpitations, headaches, dizziness, nervousness, excitability,\u00a0 tremors, and insomnia. At best, it can leave you feeling jittery and on edge. At worst, it can have a serious impact on your cardiovascular health.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">The two most common and potentially most concerning risks are increased blood pressure and increased heart rate, both of which result from activation of the sympathetic nervous system (aka the \u2018fight or flight response\u2019), explains <\/span><a href=\"https:\/\/www.binghamton.edu\/pharmacy-and-pharmaceutical-sciences\/academics\/departments\/pharmacy-practice\/profile.html?id=twarner1\"><span style=\"font-weight: 400;\">Taylor Clark, Pharm.D.<\/span><\/a><span style=\"font-weight: 400;\">, a clinical assistant professor at Binghamton University School of Pharmacy and Pharmaceutical Sciences. \u201cThe incidence of these side effects increases with higher doses and concomitant ingestion of stimulating substances,\u201d she adds. In other words, the more caffeine you consume while taking Sudafed, the worse these side effects can become.\u00a0<\/span><\/p>\n<h2 id=\"is-there-caffeine-in-sudafed\"><span style=\"font-weight: 400;\">Is there caffeine in Sudafed?<\/span><\/h2>\n<p><span style=\"font-weight: 400;\">None of the <\/span><a href=\"https:\/\/www.sudafed.com\/products\"><span style=\"font-weight: 400;\">Sudafed products<\/span><\/a><span style=\"font-weight: 400;\"> available in the United States contain caffeine. However, certain <\/span><a href=\"https:\/\/www.sudafed.co.uk\/products\"><span style=\"font-weight: 400;\">Sudafed products sold in other countries<\/span><\/a><span style=\"font-weight: 400;\"> may contain caffeine, so it\u2019s always important to check the label and the active ingredients before taking any medication.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">In the U.S., the primary active ingredient in Sudafed, which is sold behind the pharmacy counter (although it does not require a prescription), is <\/span><a href=\"https:\/\/www.singlecare.com\/prescription\/pseudoephedrine-hcl\"><span style=\"font-weight: 400;\">pseudoephedrine<\/span><\/a><span style=\"font-weight: 400;\">. (Over-the-counter Sudafed, or Sudafed PE, uses phenylephrine as the primary ingredient, which <\/span><a href=\"https:\/\/www.health.harvard.edu\/heart-health\/dont-let-decongestants-squeeze-your-heart\"><span style=\"font-weight: 400;\">research<\/span><\/a><span style=\"font-weight: 400;\"> shows is not as effective for treating nasal congestion.) \u201cPseudoephedrine is an alpha-1 agonist and reduces swelling in the nasal cavities\/passages and shrinks swollen mucous membranes, which can reduce sinus pressure and promote nasal and sinus drainage,\u201d Dr. Cohen says. \u201cSudafed treats congestion but has no effect on post-nasal drip or allergies.\u201d<\/span><\/p>\n<h2 id=\"the-green-light-yellow-light-red-light-approach-to-mixing\"><span style=\"font-weight: 400;\">The &#8220;green light, yellow light, red light&#8221; approach to mixing<\/span><\/h2>\n<p><span style=\"font-weight: 400;\">When thinking about whether or not it is safe to mix substances like caffeine with medications, it can be helpful to picture a traffic light. Green means go, or that it is generally safe for those people to mix the two substances. Yellow means proceed with caution, or that it may be safe in certain situations, but not always. And red, as we know, means stop\u2014that\u2019s a hard no to ever mixing the two.\u00a0<\/span><\/p>\n<h3 id=\"when-is-it-a-definite-no-red-light\"><span style=\"font-weight: 400;\">When is it a definite no? (red light)<\/span><\/h3>\n<p><span style=\"font-weight: 400;\">To get started in this category, Dr. Cohen says the following people should likely not take Sudafed at all, let alone combine it with caffeine:<\/span><b><\/b><\/p>\n<ul>\n<li aria-level=\"1\"><b>People with high blood pressure (hypertension): <\/b><span style=\"font-weight: 400;\">Combining the two may worsen symptoms.<\/span><\/li>\n<li aria-level=\"1\"><b>People with a history of stroke or cardiac arrhythmias: <\/b><span style=\"font-weight: 400;\">Combining the two may exacerbate heart disease.\u00a0<\/span><\/li>\n<li aria-level=\"1\"><b>People with diabetes: <\/b><span style=\"font-weight: 400;\">Dr. Clark says that sympathetic nervous system activation can also raise blood glucose, which can be dangerous for folks with <\/span><a href=\"https:\/\/www.singlecare.com\/blog\/reverse-diabetes\/\"><span style=\"font-weight: 400;\">diabetes<\/span><\/a><span style=\"font-weight: 400;\">.\u00a0<\/span><\/li>\n<li aria-level=\"1\"><b>People with thyroid disease, especially hyperthyroidism: <\/b><span style=\"font-weight: 400;\">Mixing Sudafed and caffeine can exacerbate cardiovascular side effects and, in rare cases, <\/span><a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/8238088\/\"><span style=\"font-weight: 400;\">induce thyroid storm<\/span><\/a><span style=\"font-weight: 400;\">, which is a life-threatening complication of hyperthyroidism.<\/span><\/li>\n<li aria-level=\"1\"><b>People who are pregnant: <\/b><span style=\"font-weight: 400;\">\u201cGiven smaller caffeine limits and preference for other medication classes for cough or cold in pregnancy, it would not be advisable to ingest these substances concomitantly,\u201d Dr. Clark says.<\/span><\/li>\n<li aria-level=\"1\"><b>People with kidney impairments: <\/b><span style=\"font-weight: 400;\">\u201cPseudoephedrine is eliminated largely via urine,\u201d Dr. Clark explains. \u201cThough the package labeling does not recommend dose adjustments for those with kidney impairment, this population is at an increased risk of accumulation, which may lead to increased risk of side effects. Co-ingestion with another stimulating agent, such as caffeine, would have additive adverse effects.\u201d<\/span><\/li>\n<li aria-level=\"1\"><b>People taking stimulant medications: <\/b><span style=\"font-weight: 400;\">This category of prescription drugs can include ADHD treatments such as Adderall or weight-loss drugs, such as phentermine. Mixing multiple substances with similar effects on the body increases the risk of dangerous drug interactions.<\/span><\/li>\n<\/ul>\n<h3 id=\"when-should-you-proceed-with-caution-yellow-light\"><span style=\"font-weight: 400;\">When should you proceed with caution? (yellow light)<\/span><\/h3>\n<p><span style=\"font-weight: 400;\">The following individuals should proceed with caution and consult a healthcare professional for medical advice before considering a combination of Sudafed and caffeine. (green light)<\/span><\/h3>\n<p><span style=\"font-weight: 400;\">\u201cHealthy patients with well-controlled blood pressure and no risk factors or history of cardiac conditions may be able to safely consume caffeine while taking pseudoephedrine,\u201d Dr. Clark says. \u201cAssuming treatment with the decongestant is temporary, they may be able to consume a caffeinated beverage, within the recommended daily limit.\u201d Indeed, it\u2019s important for even healthy people to avoid consuming too much of either substance at the same time and to listen to their bodies if they do. \u201cHigh doses of both Sudafed and caffeine can be toxic for normal patients,\u201d Dr. Cohen says. \u201cPatients may have different dose thresholds to the effects.\u201d<\/span><\/p>\n<h2 id=\"what-healthcare-providers-advise\"><span style=\"font-weight: 400;\">What healthcare providers advise<\/span><\/h2>\n<p><span style=\"font-weight: 400;\">In addition to the advice above about mixing Sudafed and caffeine, Dr. Clark notes that coffee, tea, soda, and energy drinks are not the only sources of caffeine that you need to be aware of. <\/span><a href=\"https:\/\/nutrition.ucdavis.edu\/outreach\/nutr-health-info-sheets\/pro-caffeine\"><span style=\"font-weight: 400;\">Coffee-flavored foods<\/span><\/a><span style=\"font-weight: 400;\"> like yogurt and ice cream often contain caffeine, as do certain types of hot chocolate and chocolate bars.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">\u201cMany over-the-counter (OTC) medications, such as Midol and Excedrin Migraine, also contain caffeine,\u201d Dr. Clark says. Always read the full label on any medication before taking it, whether alone or in combination with another, or consult a pharmacist or other healthcare provider if you\u2019re unsure.<\/span><\/p>\n<h2 id=\"practical-advice-for-managing-cold-symptoms\"><span style=\"font-weight: 400;\">Practical advice for managing cold symptoms<\/span><\/h2>\n<p><span style=\"font-weight: 400;\">\u201cWhen I was practicing primary care, Sudafed wasn&#8217;t a medication I commonly recommended due to its side effect profile,\u201d says<\/span><a href=\"https:\/\/www.coachsimranmd.com\/index.html\"><span style=\"font-weight: 400;\"> Simran Malhotra, MD<\/span><\/a><span style=\"font-weight: 400;\">, the founder of Wellness By LifestyleMD in Bethesda, Maryland. Instead, Dr. Malhotra recommends the following natural strategies for managing cold symptoms:<\/span><b><\/b><\/p>\n<ul>\n<li aria-level=\"1\"><b>Rest and hydrate. <\/b><span style=\"font-weight: 400;\">\u201cThese are the foundational keys to optimizing recovery from a cold,\u201d she says. In addition to water, Dr. Malhotra suggests sipping herbal teas. \u201cPeppermint tea may provide some relief from respiratory congestion and a sore throat, while chamomile contains anti-inflammatory compounds and may support sleep,\u201d she adds.<\/span><\/li>\n<li aria-level=\"1\"><b>Eat immune-boosting foods. <\/b><span style=\"font-weight: 400;\">These are mostly plant-based foods like citrus fruits and berries, and spices like turmeric and ginger. \u201cVitamin C, vitamin D, and zinc are three micronutrients that have the <\/span><a href=\"https:\/\/pmc.ncbi.nlm.nih.gov\/articles\/PMC7019735\/#sec6-nutrients-12-00236\"><span style=\"font-weight: 400;\">strongest evidence<\/span><\/a><span style=\"font-weight: 400;\"> for supporting the immune system and fighting off infection,\u201d Dr. Malhotra says.<\/span><\/li>\n<li aria-level=\"1\"><b>Try a nasal rinse. <\/b><span style=\"font-weight: 400;\">\u201cNasal lavage, such as with a neti pot, may ease congestion from a cold for some people,\u201d Dr. Malhotra says. \u201cThe most important thing to remember is to use lukewarm distilled or sterile water.\u201d<\/span><\/li>\n<\/ul>\n<p><span style=\"font-weight: 400;\">While most people recover from a cold at home, there are some red flag symptoms for which you should give your healthcare provider a call. \u201cIt is recommended to see your doctor if your symptoms are worsening or you experience difficulty breathing, chest pain, prolonged fevers, or can&#8217;t keep fluids down,\u201d Dr. Malhotra says. \u201cCertain populations, such as young children, people over 65, or those who are immunocompromised, should be extra cautious and seek attention sooner if needed.\u201d\u00a0<\/span><\/p>\n<h2 id=\"the-bottom-line\"><span style=\"font-weight: 400;\">The bottom line<\/span><\/h2>\n<p><span style=\"font-weight: 400;\">In general, healthcare providers advise against consuming caffeine while taking Sudafed. Since they are both central nervous system stimulants, combining them can exacerbate side effects, such as increased blood pressure and an elevated heart rate.
