Entresto<\/span><\/a> (sacubitril and <\/span>valsartan<\/span>) is a first-in-class angiotensin receptor-eprilysin inhibitor (ARNi) approved by the U.S. Food and Drug Administration (<\/span>FDA<\/span>) to treat chronic <\/span>heart failure<\/span> in adults\u2014particularly in people with a left ventricular ejection fraction (LVEF) that\u2019s below normal. The <\/span>prescription drug<\/span> is also prescribed to children aged one year and older for symptomatic <\/span>heart failure<\/span> with systemic left ventricular systolic dysfunction. The cost of <\/span>Entresto<\/span> for <\/span>Medicare<\/span> patients averages about $25-$30 for a <\/span>30-day supply<\/span>.\u00a0<\/span><\/p>\n

Why is <\/span>Entresto<\/span> so expensive?<\/span><\/h2>\n

Like many <\/span>prescription medications<\/span> on the market, <\/span>Entresto<\/span> can be expensive without <\/span>insurance coverage<\/span>. The primary reason for the high price tag is that it\u2019s a relatively new brand-name drug. <\/span>Entresto<\/span>\u2014developed by <\/span>Novartis<\/span>\u2014was <\/span>FDA<\/span>-approved for public use in July 2015<\/span><\/a> and granted a patent that prevented other pharmaceutical companies from developing generic formulations until July 2025. Now, MSN Pharmaceuticals and Torrent have their <\/span>generic versions<\/span> of the brand-name heart medication.<\/span><\/p>\n

\u201cAll new medications that come out are expensive because of the patent that they can have when they first come out,\u201d explains Joyce Oen-Hsiao, MD, FACC, a board-certified cardiologist and associate professor of medicine (cardiovascular medicine) at Yale School of Medicine in New Haven, Connecticut. \u201cWhen a pharmaceutical company first develops a new medication to be used for a particular disease, it is initially only sold under the brand name because it is covered under patent protection. The pharmaceutical company has a patent on the medication to manufacture, market, and make a profit on the medication.\u201d<\/span><\/p>\n

Another reason for <\/span>Entresto<\/span>\u2019s high price tag is your <\/span>health insurance<\/span> provider. \u201cInsurance companies decide if they want to put the medications on their <\/span>formulary<\/span> and then also choose what tier the medication is covered at,\u201d Dr. Oen-Hsiao explains. \u201cGenerally, higher-tier medications will require a higher<\/span>co-payment or co-insurance<\/span> from the patient. If the patient has a <\/span>deductible<\/span> as part of their <\/span>insurance plan<\/span>, they may have to pay the entire cost of the medication at the beginning of the year.\u201d<\/span><\/p>\n

Does <\/span>Medicare<\/span> cover <\/span>Entresto<\/span>?<\/span><\/h2>\n

In most cases, <\/span>Medicare<\/span> Part D will cover <\/span>Entresto<\/span>. \u201cMost <\/span>Medicare Part D<\/span> and <\/span>Medicare Advantage<\/span> drug plans cover <\/span>Entresto<\/span>,\u201d says James Lyons, MD, a board-certified physician in hospitalist care, emergency medicine, psychiatry, and family medicine, and medical consultant for <\/span>Synergy Houses<\/span><\/a>. \u201cTypical annual <\/span>out-of-pocket cost<\/span> for beneficiaries averages around $300-$400, though this varies by plan and coverage stage. <\/span>Entresto<\/span> is not covered under Original <\/span>Medicare<\/span> Parts A and B unless the drug is administered in a hospital or physician\u2019s office.\u201d<\/span><\/p>\n

How much does <\/span>Entresto<\/span> cost with <\/span>Medicare<\/span>?<\/span><\/h2>\n

According to the Centers for <\/span>Medicare<\/span> & <\/span>Medicaid Services<\/span> (CMS), beneficiaries will have an <\/span>out-of-pocket cost<\/span> of $295 for <\/span>Entresto<\/span> in 2026. This price averages about $25 per month for a <\/span>30-day supply<\/span>. \u201c<\/span>Entresto<\/span> is one of the first drugs selected for <\/span>Medicare<\/span> price negotiation<\/span>, which may help lower <\/span>drug costs<\/span> in the coming years,\u201d Dr. Lyons says. \u201cWhile not a cure, it\u2019s a cornerstone therapy that can greatly improve quality of life when used as part of a comprehensive <\/span>heart-failure<\/span> treatment plan.\u201d<\/span><\/p>\n

Several factors can influence what you\u2019ll be paying out of pocket each month for your <\/span>Entresto<\/span> prescription. \u201c<\/span>Out-of-pocket cost<\/span> depends on formulary, <\/span>drug <\/span>tier, <\/span>deductible<\/span> status, pharmacy pricing, and eligibility for <\/span>copay<\/span> or patient-assistance programs,\u201d Dr. Lyons says.\u00a0<\/span><\/p>\n
