According to Dr. Goldberg, more than 75% of UTIs are caused by <\/span><i><span style=\"font-weight: 400;\">E. coli,<\/span><\/i><span style=\"font-weight: 400;\"> a type of bacteria that clindamycin is not effective at treating. Instead, clindamycin works better against other kinds of bacteria that are usually linked to skin, throat, or dental infections, not urinary tract infections.<\/span><span style=\"font-weight: 400;\"><br \/>\n<\/span><\/p>\n<p><span style=\"font-weight: 400;\">In addition, &#8220;clindamycin does not concentrate well in the urine, which limits its effectiveness for treating infections in the urinary tract,&#8221; says Reshma Kapadia Patel, a Pharm.D. in Dallas, Texas, and the founder of <\/span><a href=\"https:\/\/wisemedrx.com\/\"><span style=\"font-weight: 400;\">WiseMedRx<\/span><\/a><span style=\"font-weight: 400;\">. Therefore, healthcare professionals typically prescribe other antibiotics that are more effective against <\/span><i><span style=\"font-weight: 400;\">E.coli<\/span><\/i><span style=\"font-weight: 400;\">.<\/span><\/p>\n<h2 id=\"understanding-how-clindamycin-works\"><span style=\"font-weight: 400;\">Understanding how clindamycin works<\/span><\/h2>\n<p><a href=\"https:\/\/www.acs.org\/molecule-of-the-week\/archive\/c\/clindamycin.html\"><span style=\"font-weight: 400;\">Clindamycin works by preventing bacteria<\/span><\/a><span style=\"font-weight: 400;\"> from making the proteins they need to grow and multiply, helping the body fight off the infection. Depending on the infection site, the type of bacteria, and its concentration, <\/span><a href=\"https:\/\/www.ncbi.nlm.nih.gov\/books\/NBK519574\/\"><span style=\"font-weight: 400;\">clindamycin<\/span><\/a><span style=\"font-weight: 400;\"> either causes death of the bacteria (bactericidal) or slows the bacteria from growing and multiplying (bacteriostatic).<\/span><span style=\"font-weight: 400;\"><br \/>\n<\/span><\/p>\n<p><span style=\"font-weight: 400;\">\u201cWhen we talk about bacteria, they are often classified as gram-positive or gram-negative based on their cell structure. Gram-positive bacteria have a simpler outer layer, while gram-negative bacteria have an additional outer membrane that can make them harder for certain antibiotics to penetrate,\u201d Dr. Patel says. Gram-negative bacteria cause most UTIs, and since clindamycin is more effective against gram-positive bacteria, it&#8217;s often not the right match.\u00a0<\/span><\/p>\n<h2 id=\"why-is-clindamycin-rarely-the-first-choice-for-utis\"><span style=\"font-weight: 400;\">Why is clindamycin rarely the first choice for UTIs?<\/span><\/h2>\n<p><span style=\"font-weight: 400;\">There are three main reasons why clindamycin is rarely the first choice of medication for treating UTIs: the bacteria it targets, its poor urinary concentration, and its side effects.<\/span><\/p>\n<h3 id=\"effectiveness-against-gram-negative-bacteria\"><span style=\"font-weight: 400;\">Effectiveness against gram-negative bacteria<\/span><i><span style=\"font-weight: 400;\">\u00a0<\/span><\/i><\/h3>\n<p><span style=\"font-weight: 400;\">The bacteria that cause most UTIs (<\/span><i><span style=\"font-weight: 400;\">E. coli<\/span><\/i><span style=\"font-weight: 400;\">, <\/span><i><span style=\"font-weight: 400;\">Proteus<\/span><\/i><span style=\"font-weight: 400;\">, and <\/span><i><span style=\"font-weight: 400;\">Klebsiella<\/span><\/i><span style=\"font-weight: 400;\">) are gram-negative organisms, and clindamycin is not effective against them. Dr. Goldberg explains that healthcare providers often use first-line UTI antibiotics such as <\/span><a href=\"https:\/\/www.singlecare.com\/prescription\/nitrofurantoin\/what-is\"><span style=\"font-weight: 400;\">Macrobid <\/span><span style=\"font-weight: 400;\">(<\/span><span style=\"font-weight: 400;\">nitrofurantoin)<\/span><\/a><span style=\"font-weight: 400;\">, <\/span><a href=\"https:\/\/www.singlecare.com\/prescription\/sulfamethoxazole-trimethoprim\/what-is\"><span style=\"font-weight: 400;\">Bactrim (trimethoprim-sulfamethoxazole)<\/span><\/a><span style=\"font-weight: 400;\">, and fosfomycin to treat UTIs because they reliably kill these common bacteria.<\/span><\/p>\n<h3 id=\"the-localization-issue\"><span style=\"font-weight: 400;\">The localization issue<\/span><\/h3>\n<p><span style=\"font-weight: 400;\">&#8220;For an antibiotic to effectively treat a UTI, it needs to reach high concentrations in the urinary tract,&#8221; says Dr. Patel. Clindamycin is processed primarily by the liver and distributed throughout the body, but only a small amount is excreted into the urine in its active form.<\/span><span style=\"font-weight: 400;\"><br \/>\n<\/span><\/p>\n<p><span style=\"font-weight: 400;\">She explains that because clindamycin doesn&#8217;t concentrate well in the urine, it may not deliver enough medication to the site of infection to fully clear it. For a UTI, the antibiotic has to reach the urinary tract in sufficient amounts, and clindamycin simply doesn&#8217;t localize well there.\u00a0<\/span><\/p>\n<h3 id=\"significant-gut-side-effects\"><span style=\"font-weight: 400;\">Significant gut side effects<\/span><\/h3>\n<p><span style=\"font-weight: 400;\">Additionally, healthcare providers may also consider the side effects of clindamycin before prescribing it for UTI. Dr. Goldberg says taking clindamycin carries a well-known risk of disrupting the gut microbiome by killing many of the beneficial anaerobic bacteria that normally inhabit the intestines. Examples include:<\/span><\/p>\n<ul>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">If a urine culture identifies that an unusual gram-positive organism (such as certain Staphylococci) is involved in the urinary tract infection<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">If the test shows that the particular bacteria causing the infection are susceptible to clindamycin and resistant to standard UTI antibiotics<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">If a patient cannot tolerate first-line antibiotics due to allergies or side effects<\/span><\/li>\n<\/ul>\n<p><span style=\"font-weight: 400;\">Dr. Goldberg explains that there is no guideline from any major infectious disease society recommending clindamycin for UTI treatment, and that even in uncommon scenarios, other antibiotics would typically be preferred. The decision to prescribe clindamycin would be culture-guided, off-label, and made on a case-by-case basis.<\/span><\/p>\n<h2 id=\"the-risks-of-using-the-wrong-antibiotic\"><span style=\"font-weight: 400;\">The risks of using the wrong antibiotic<\/span><\/h2>\n<p><span style=\"font-weight: 400;\">The wrong use of antibiotics for treating UTIs or other infections can cause serious health risks:<\/span><\/p>\n<ul>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><b>Antibiotic resistance:<\/b><span style=\"font-weight: 400;\"> Antibiotic resistance occurs when <\/span><a href=\"https:\/\/www.cdc.gov\/antibiotic-use\/data-research\/facts-stats\/index.html\"><span style=\"font-weight: 400;\">microorganisms develop the<\/span><\/a><span style=\"font-weight: 400;\"> ability to survive and defeat the drugs designed to kill them. This can make it more difficult to treat infections affecting not only you but also others, and can cause extended hospital stays (for those hospitalized).<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><b>Secondary infection:<\/b><span style=\"font-weight: 400;\"> Using antibiotics incorrectly can increase the <\/span><a href=\"https:\/\/pmc.ncbi.nlm.nih.gov\/articles\/PMC10947062\/#:~:text=Inappropriate%20treatment%20and%20unnecessary%20antibiotic%20switching%20harms%20patients%20directly%20by,to%20higher%20health%20care%20costs.\"><span style=\"font-weight: 400;\">risk of recurrent infections<\/span><\/a><span style=\"font-weight: 400;\">, as the initial antibiotics you took may now make your body more susceptible to the disease.\u00a0<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><b>Prolonging infection:<\/b><span style=\"font-weight: 400;\"> When microorganisms become resistant to the antibiotics used to kill them, infections become harder to treat, prolonging illness.\u00a0<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><b>Adverse effects:<\/b><span style=\"font-weight: 400;\"> Using antibiotics that are not appropriate for you can also <\/span><a href=\"https:\/\/pmc.ncbi.nlm.nih.gov\/articles\/PMC4232501\/\"><span style=\"font-weight: 400;\">increase the risk<\/span><\/a><span style=\"font-weight: 400;\"> of both mild and serious side effects.
