Most sinus infections are caused by viruses, meaning antibiotics like Zithromax wouldn\u2019t have an effect at all. Healthcare providers take care not to recommend antibiotics when it\u2019s not needed to prevent the rise of antibiotic-resistant infections. Still, Z-Paks may help in certain cases. We&#8217;ll explain when Z-Paks are prescribed, their cost, effectiveness, what clinical guidelines say, and alternatives that healthcare providers may consider.\u00a0<\/span><\/p>\n<h2 id=\"what-is-a-z-pak\"><span style=\"font-weight: 400;\">What is a Z-Pak?<\/span><\/h2>\n<p><a href=\"https:\/\/www.singlecare.com\/blog\/what-is-a-z-pak\/\"><span style=\"font-weight: 400;\">Z-Paks<\/span><\/a><span style=\"font-weight: 400;\"> are a prepackaged course of Zithromax, a brand-name prescription antibiotic used to treat certain types of bacterial infections. <\/span><a href=\"https:\/\/dailymed.nlm.nih.gov\/dailymed\/drugInfo.cfm?setid=db52b91e-79f7-4cc1-9564-f2eee8e31c45\"><span style=\"font-weight: 400;\">Z-Paks <\/span><\/a><span style=\"font-weight: 400;\">\u00a0may be taken as two 250 mg Zithromax tablets taken once daily for three days, or as two 250 mg tablets taken on the first day, then 250 mg once daily for the following four days. Azithromycin is particularly effective at treating <\/span><a href=\"https:\/\/www.merckmanuals.com\/home\/infections\/bacterial-infections-gram-negative-bacteria\/overview-of-gram-negative-bacteria\"><span style=\"font-weight: 400;\">gram-negative bacterial infections<\/span><\/a><span style=\"font-weight: 400;\"> caused by bacteria such as <\/span><i><span style=\"font-weight: 400;\">Escherichia coli <\/span><\/i><span style=\"font-weight: 400;\">(<\/span><i><span style=\"font-weight: 400;\">E. coli<\/span><\/i><span style=\"font-weight: 400;\">), <\/span><i><span style=\"font-weight: 400;\">Salmonella<\/span><\/i><span style=\"font-weight: 400;\">, and <\/span><i><span style=\"font-weight: 400;\">Pseudomonas<\/span><\/i><span style=\"font-weight: 400;\">. Azithromycin can kill certain gram-positive bacteria such as <\/span><i><span style=\"font-weight: 400;\">Streptococci<\/span><\/i><span style=\"font-weight: 400;\"> and <\/span><i><span style=\"font-weight: 400;\">Haemophilus influenzae<\/span><\/i><span style=\"font-weight: 400;\"> (<\/span><i><span style=\"font-weight: 400;\">H. influenzae<\/span><\/i><span style=\"font-weight: 400;\">), as well as atypical bacteria such as <\/span><i><span style=\"font-weight: 400;\">Chlamydia<\/span><\/i><span style=\"font-weight: 400;\"> and <\/span><i><span style=\"font-weight: 400;\">Mycoplasma<\/span><\/i><span style=\"font-weight: 400;\">.\u00a0<\/span><\/p>\n<h2 id=\"does-a-z-pak-work-for-sinus-infections\"><span style=\"font-weight: 400;\">Does a Z-Pak work for sinus infections?<\/span><\/h2>\n<p><span style=\"font-weight: 400;\">Most acute sinus infections (acute rhinosinusitis, infection lasting fewer than four weeks) are <\/span><a href=\"https:\/\/www.cdc.gov\/sinus-infection\/about\/index.html\"><span style=\"font-weight: 400;\">caused by viruses<\/span><\/a><span style=\"font-weight: 400;\">, not bacteria. Since Z-Paks are used for bacterial infections, it may be useful for a small percentage of sinus infections caused by certain bacteria.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Bacteria that may cause sinus infections include <\/span><i><span style=\"font-weight: 400;\">Streptococcus pneumoniae<\/span><\/i><span style=\"font-weight: 400;\"> (<\/span><i><span style=\"font-weight: 400;\">S. pneumoniae<\/span><\/i><span style=\"font-weight: 400;\">), <\/span><i><span style=\"font-weight: 400;\">H. influenzae<\/span><\/i><span style=\"font-weight: 400;\">, <\/span><i><span style=\"font-weight: 400;\">Moraxella catarrhalis<\/span><\/i><span style=\"font-weight: 400;\">, <\/span><i><span style=\"font-weight: 400;\">Streptococcus pyogenes<\/span><\/i><span style=\"font-weight: 400;\">, and <\/span><i><span style=\"font-weight: 400;\">Staphylococcus aureus<\/span><\/i><span style=\"font-weight: 400;\">. Other bacterial and fungal causes of sinus infections are less common. Even though sinus infections are the <\/span><a href=\"http:\/\/ncbi.nlm.nih.gov\/books\/NBK547701\/\"><span style=\"font-weight: 400;\">fifth most common reason<\/span><\/a><span style=\"font-weight: 400;\"> why antibiotics are prescribed, healthcare providers take care not to recommend antibiotics when not needed to prevent the rise of antibiotic-resistant infections.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Current guidelines from multiple organizations don\u2019t recommend azithromycin as the first-line treatment for bacterial sinusitis. The <\/span><a href=\"https:\/\/academic.oup.com\/cid\/article\/54\/8\/e72\/367144?login=false\"><span style=\"font-weight: 400;\">Infectious Diseases Society of America<\/span><\/a><span style=\"font-weight: 400;\"> (IDSA) recommends watchful waiting, followed by amoxicillin-clavulanate as the first-line antibiotic. Similarly, the <\/span><a href=\"https:\/\/aao-hnsfjournals.onlinelibrary.wiley.com\/doi\/10.1002\/ohn.1344\"><span style=\"font-weight: 400;\">American Academy of Otolaryngology\u2013Head and Neck Surgery Foundation<\/span><\/a><span style=\"font-weight: 400;\"> (AAO) and the <\/span><a href=\"https:\/\/www.cdc.gov\/antibiotic-use\/hcp\/clinical-care\/adult-outpatient.html\"><span style=\"font-weight: 400;\">Centers for Disease Control and Prevention<\/span><\/a><span style=\"font-weight: 400;\"> (CDC) recommend <\/span><a href=\"https:\/\/www.singlecare.com\/blog\/amoxicillin-for-sinus-infection\/\"><span style=\"font-weight: 400;\">amoxicillin<\/span><\/a><span style=\"font-weight: 400;\"> with or without clavulanate for first-line antibiotic therapy.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">It\u2019s worth noting that macrolide antibiotics such as azithromycin, found in Z-Paks, are explicitly not recommended as a first-line treatment because of the high prevalence of macrolide-resistant <\/span><i><span style=\"font-weight: 400;\">S. pneumoniae<\/span><\/i><span style=\"font-weight: 400;\"> infections in the U.S. According to the CDC, about 40% of <\/span><i><span style=\"font-weight: 400;\">S. pneumoniae<\/span><\/i><span style=\"font-weight: 400;\"> infections may be resistant to macrolide antibiotics. <\/span><a href=\"https:\/\/www.singlecare.com\/blog\/antibiotic-stewardship\/\"><span style=\"font-weight: 400;\">Bacterial resistance<\/span><\/a><span style=\"font-weight: 400;\"> is an increasing public health concern, which is why all the guidelines mentioned above recommend watchful waiting first before prescribing antibiotic therapy to people with a sinus infection.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">That being said, Z-Paks may still be effective for bacterial sinus infections caused by gram-negative bacteria or <\/span><i><span style=\"font-weight: 400;\">H. influenzae<\/span><\/i><span style=\"font-weight: 400;\">. Your healthcare provider may prescribe a Z-Pak for your sinus infection based on their clinical evaluation of your symptoms and medical history.\u00a0<\/span><\/p>\n<h3 id=\"bacterial-vs-viral-sinus-infections-how-to-tell-the-difference\"><span style=\"font-weight: 400;\">Bacterial vs. viral sinus infections: How to tell the difference<\/span><\/h3>\n<p><span style=\"font-weight: 400;\">According to the IDSA, viral causes of sinusitis account for 90\u201398% of cases, while only 2\u201310% of sinusitis cases are caused by bacteria. Viral upper respiratory infections (URIs) can give way to secondary bacterial infections in up to 2% of cases. Healthcare providers may suspect bacterial sinusitis when patients have severe symptoms, symptoms that continue for longer than 10 days without improvement, or symptoms that worsen after an initial improvement (e.g., in the case of a secondary infection). <\/span><\/p>\n<p><span style=\"font-weight: 400;\">\u201cViral sinusitis lasts less than 10 days, symptoms improve over time, and patients do not typically experience fevers,\u201d shares <\/span><a href=\"https:\/\/toneandtexturechicago.com\/dr-satara\/\"><span style=\"font-weight: 400;\">Satara Brown<\/span><\/a><span style=\"font-weight: 400;\">, MD, board-certified family medicine physician and cofounder of Tone &amp; Texture Aesthetics and Wellness in Chicago, Illinois. \u201cWith bacterial sinusitis, patients have symptoms longer than 10 days. As time passes, symptoms worsen, and patients usually have high fevers,\u201d she explains.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">If you suspect a bacterial sinus infection, see your healthcare provider for a full evaluation and appropriate treatment options. Even in the case of a suspected bacterial sinus infection, your healthcare provider may not prescribe antibiotics. Watchful waiting and supportive care remain the first-line therapies recommended by current clinical guidelines before antibiotic therapy.\u00a0<\/span><\/p>\n<h2 id=\"z-pak-sinus-infection-dosage\"><span style=\"font-weight: 400;\">Z-Pak sinus infection dosage\u00a0<\/span><\/h2>\n<p><span style=\"font-weight: 400;\">Though there\u2019s <\/span><a href=\"https:\/\/www.singlecare.com\/prescription\/zithromax\/dosage\"><span style=\"font-weight: 400;\">no recommended Z-Pak dosage for sinus infections<\/span><\/a><span style=\"font-weight: 400;\"> specifically, Z-Paks are typically recommended for three days or five days. If you take aluminum- or magnesium-containing antacids, wait at least <\/span><a href=\"https:\/\/www.singlecare.com\/blog\/azithromycin-interactions\/\"><span style=\"font-weight: 400;\">two hours before taking Zithromax to avoid drug interactions<\/span><\/a><span style=\"font-weight: 400;\">. Do not stop your medication early. If you don\u2019t feel better once you finish your Z-Pak, seek medical advice from your healthcare professional.
