It\u2019s used to treat allergy symptoms like itching, sneezing, a runny nose, and watery eyes. Available both by prescription and over-the-counter for kids age 6 and older, plus adults, it&#8217;s often mistaken for a steroid nasal spray, but it works differently. Learn where the confusion comes from, how azelastine compares to steroid sprays like Flonase, and what you should know about using it for allergies.<\/span><\/p>\n<p><a href=\"https:\/\/www.singlecare.com\/resources\/allergies\"><b>Allergy medicine coupons &amp; resources<\/b><\/a><\/p>\n<h2 id=\"why-do-people-think-azelastine-is-a-steroid\"><span style=\"font-weight: 400;\">Why do people think azelastine is a steroid?<\/span><\/h2>\n<p><span style=\"font-weight: 400;\">We\u2019ve just established that azelastine is not a steroid, so why do people think it is? There are a few reasons. The first is because of who their neighbors are in the allergy treatment aisle. <\/span><a href=\"https:\/\/tcop.touro.edu\/pharmd\/faculty\/list\/fraidy-maltz.php\"><span style=\"font-weight: 400;\">Fraidy N. Maltz, Pharm.D.<\/span><\/a><span style=\"font-weight: 400;\">, an associate professor of pharmacy practice at Touro College of Pharmacy in New York City, explains that people might assume azelastine is in the same class as steroid nasal sprays like <\/span><a href=\"https:\/\/www.singlecare.com\/prescription\/fluticasone-propionate\"><span style=\"font-weight: 400;\">Flonase<\/span><\/a><span style=\"font-weight: 400;\"> (fluticasone), Rhinocort (budesonide), and <\/span><a href=\"https:\/\/www.singlecare.com\/prescription\/nasonex-24hr#coupons\"><span style=\"font-weight: 400;\">Nasonex<\/span><\/a><span style=\"font-weight: 400;\"> (mometasone) because they are all <\/span><a href=\"https:\/\/www.singlecare.com\/blog\/news\/astepro-azelastine-otc-switch\/\"><span style=\"font-weight: 400;\">available over the counter<\/span><\/a><span style=\"font-weight: 400;\"> and shelved together in the allergy section.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Likewise, both antihistamine and steroid nasal sprays were initially available only by prescription before OTC options became available in stores, further stoking confusion, says <\/span><a href=\"https:\/\/www.drkarawada.com\/about-dr-kara\"><span style=\"font-weight: 400;\">Kara Wada, MD<\/span><\/a><span style=\"font-weight: 400;\">, an allergist and immunologist and the founder of the Immune Confident Institute in Columbus, OH.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Finally, there&#8217;s the fact that azelastine is available in a combination prescription product with fluticasone, sold as <\/span><a href=\"https:\/\/www.singlecare.com\/prescription\/dymista\"><span style=\"font-weight: 400;\">Dymista (azelastine-fluticasone)<\/span><\/a><span style=\"font-weight: 400;\">. This medication contains azelastine and would be considered a steroid nasal spray, but that\u2019s because of the fluticasone, not the azelastine.\u00a0<\/span><\/p>\n<h2 id=\"azelastine-vs-flonase-which-works-better-for-allergies\"><span style=\"font-weight: 400;\">Azelastine vs. Flonase: Which works better for allergies?<\/span><\/h2>\n<p><span style=\"font-weight: 400;\">\u201cBoth sprays have their place in the treatment of nasal and eye allergy symptoms,\u201d Dr. Wada says. The one that\u2019s better for you may depend on your specific symptoms as well as their severity and how quickly you need relief. \u201cAzelastine may work better than steroid nasal sprays such as Flonase for eye symptoms, sneezing, itching, and runny nose,\u201d Dr. Maltz says. \u201cHowever, nasal steroids such as Flonase work better for congestion.\u201d Still, a <\/span><a href=\"https:\/\/pmc.ncbi.nlm.nih.gov\/articles\/PMC9702036\/\"><span style=\"font-weight: 400;\">study<\/span><\/a><span style=\"font-weight: 400;\"> that compared the two sprays head-to-head found that they had comparable efficacy in controlling symptoms in patients with nasal allergies.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">The bigger difference is in the time it takes them to work. \u201cAzelastine works more quickly, kicking in within 15 to 30 minutes of use,\u201d Dr. Wada says. While Flonase may begin working within a few hours, \u201cit can take about two weeks of daily use to reach its full potential,\u201d Dr. Wada adds.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">For these reasons, using both at the same time is not uncommon. \u201cI often recommend combining an intranasal antihistamine with an intranasal corticosteroid,\u201d Dr. Wada says. \u201c<\/span><a href=\"https:\/\/www.jacionline.org\/article\/S0091-6749(12)00272-2\/fulltext\"><span style=\"font-weight: 400;\">Double-blind, placebo-controlled trials<\/span><\/a><span style=\"font-weight: 400;\"> have demonstrated that the combination is more effective and has a faster onset of action than either spray separately.\u201d Combining the two treatments is generally recommended when your symptoms are not responding to a single medication on its own. The <\/span><a href=\"https:\/\/www.aaaai.org\/Aaaai\/media\/Media-Library-PDFs\/Allergist%20Resources\/Statements%20and%20Practice%20Parameters\/Rhinitis-2020-A-practice-parameter-update.pdf\"><span style=\"font-weight: 400;\">American Academy of Allergy, Asthma, and Immunology<\/span><\/a><span style=\"font-weight: 400;\"> considers it a first-line treatment for allergies, meaning it\u2019s among the <\/span><a href=\"https:\/\/www.singlecare.com\/blog\/nasal-spray-for-allergies\/\"><span style=\"font-weight: 400;\">best allergy nasal sprays<\/span><\/a><span style=\"font-weight: 400;\">. However, there are some notable drawbacks. Here are the ones our experts say you should know.<\/span><b><\/b><\/p>\n<ul>\n<li><b>Bitter taste. <\/b><span style=\"font-weight: 400;\">&#8220;Azelastine has a very bitter taste, which is hard for some consumers to tolerate,&#8221; Dr. Maltz says. &#8220;To minimize this, it is recommended to lean forward when administering the nasal spray to avoid dripping in the back of the throat.\u201d Also sniff gently while angling the bottle tip slightly towards the outside of the nostril when spraying the dose. These techniques serve to coat the sinuses with the spray and minimize the amount that passes through and into the throat. In addition, brand name Astepro includes sweeteners to largely mask the bitter taste.<\/span><\/li>\n<li><b>Drowsiness and fatigue. <\/b><span style=\"font-weight: 400;\">Drowsiness is reported in just over 10% of people using azelastine, Dr. Wada says. &#8220;This is unusual for a nasal spray and distinguishes azelastine from intranasal corticosteroids,\u201d she adds. \u201cPatients should be cautioned about driving or operating machinery and the possible additive effect with other sedating medications or substances.\u201d<\/span><\/li>\n<li><b>Frequent dosing. <\/b><span style=\"font-weight: 400;\">While a newer high strength 0.15% spray may be used once daily, some patients will require twice-daily dosing for best results. In contrast, most steroid nasal sprays only need to be dosed once a day, Dr. Wada says. Decongestant medications, such as <\/span><a href=\"https:\/\/www.singlecare.com\/prescription\/afrin-original\"><span style=\"font-weight: 400;\">Afrin<\/span><\/a><span style=\"font-weight: 400;\"> (oxymetazoline) and <\/span><a href=\"https:\/\/www.singlecare.com\/prescription\/sudafed-sinus-12hr-press-pain\"><span style=\"font-weight: 400;\">Sudafed<\/span><\/a><span style=\"font-weight: 400;\"> (pseudoephedrine), work by narrowing and reducing swelling in the blood vessels and tissues in the nose, according to the <\/span><a href=\"https:\/\/my.clevelandclinic.org\/health\/drugs\/24923-nasal-decongestant\"><span style=\"font-weight: 400;\">Cleveland Clinic<\/span><\/a><span style=\"font-weight: 400;\">. \u201cAzelastine works through a fundamentally different mechanism,\u201d Dr. Wada explains. \u201cIt blocks histamine-mediated inflammation and swelling, which clinical trials show significantly reduces a symptom complex that includes nasal congestion, rhinorrhea, post-nasal drip, and sneezing.\u201d<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Still, if you have severe congestion, Afrin will be more effective. \u201cIf someone has a lot of congestion, they may try a product like Afrin for short-term use along with the azelastine,\u201d Dr. Maltz says. The short-term use is key, though. \u201cAfrin carries the significant risk of rebound congestion with use beyond 3\u20135 days, and guidelines recommend limiting its use to short-term or episodic therapy,\u201d Dr. Wada says. \u201cAzelastine has no such rebound risk and is safe for long-term daily use.\u201d<\/span><\/p>\n<h2 id=\"what-should-you-not-take-with-azelastine\"><span style=\"font-weight: 400;\">What should you not take with azelastine?<\/span><\/h2>\n<p><span style=\"font-weight: 400;\">Dr. Wada says that one of the biggest advantages of azelastine is that it does not interact with many other medications. The most important ones to know about are those that can exacerbate the drowsiness caused by azelastine, which can be dangerous.
