But the pharmacist role in sepsis starts long before a patient reaches a hospital. Sepsis begins with an infection, and you see patients every day who are picking up antibiotics, managing conditions that raise infection risk, or asking whether a symptom is serious.

This article shows how you can use those routine touchpoints to lower patients’ risk and recognize when a symptom needs emergency care instead of pharmacy advice.

What is sepsis?

Sepsis is the body’s extreme response to an infection in which the immune system damages healthy tissues. It can cause severe symptoms, such as confusion, low blood pressure, rapid breathing, seizures, and in some cases, tissue hypoperfusion (poor blow flow through the body’s tissues) or organ failure. Without prompt treatment, including urgent fluid resuscitation, sepsis can progress to septic shock, which has an in-hospital mortality rate of about 39%. Many patients with sepsis have recently used healthcare services or have a chronic disease that requires regular care, which means they often pass through your pharmacy first. Pharmacist involvement can be the difference between a managed infection and severe sepsis requiring a stay in the intensive care unit.

How pharmacists help stop infections before they become sepsis

Everyday pharmacist interventions are where infection risk often gets caught early. You can help in three ways:

Counsel on proper wound care: Explain how to clean and protect minor wounds, and describe signs of an infection, such as redness, swelling, or pus, that mean a patient should see a healthcare provider.

Support chronic condition management: Conditions such as diabetes and chronic obstructive pulmonary disease (COPD) raise infection risk when they are poorly controlled. Reinforce adherence and review how patients use their medications, including inhaler technique.

Recommend vaccines: Check patients’ flu, COVID-19, pneumococcal, and shingles vaccination status and offer vaccines at the visit when possible.

Why antibiotic adherence counseling matters sepsis prevention

Antibiotic adherence is one of the most practical sepsis prevention steps you can reinforce at the counter. Patients often feel better within a few days of starting antibiotics and may assume they no longer need the rest of the medication. But stopping early or skipping doses can let bacteria persist and multiply, which can make an infection harder to treat or more serious.

At pickup you can:

Confirm the dose, how often to take it, and for how long

Explain why the prescribed dose matters, even when symptoms improve, unless the prescriber changes the plan

Ask patients what they would do if they miss a dose, and walk them through it

Check for barriers such as cost, side effects, or a hard-to-follow schedule

Tell patients to contact their healthcare provider if symptoms don’t improve or get worse

If a patient tells you they stopped early or skipped doses, ask about their symptoms. Lingering or worsening symptoms need reevaluation by a healthcare provider, and fever, confusion, or shortness of breath can signal something more serious.

Antibiotic stewardship in the pharmacy

Antibiotic stewardship (also called antimicrobial stewardship) is the effort to improve how antibiotics are prescribed and used. It helps treat infections effectively and lowers the risk of antibiotic resistance. At the pharmacy counter, you can:

Verify the indication, dose, and duration against the patient’s diagnosis and kidney function

Contact the prescriber when therapy looks unnecessary or inappropriate

Screen for allergies, interactions, and duplicate therapy

Explain that antibiotics treat bacterial infections, not viral ones such as colds, and what to do if symptoms don’t improve

In the hospital, clinical pharmacists also help select an appropriate broad-spectrum antimicrobial therapy when sepsis is suspected, then narrow the therapy as blood cultures come back to improve sepsis care.

Reviewing patient medications to flag personal risk factors

Routine medication profile reviews can reveal risk factors that patients may not connect to infection. Look for:

Polypharmacy: Multiple medications raise the chance of interactions and side effects, especially for older adults. Flag duplicates and unnecessary therapy, and discuss opportunities to simplify the regimen with prescribers.

Immunosuppressive medications: Corticosteroids, chemotherapy, and many biologic and transplant medications lower the body’s ability to fight infection. Make sure these patients know which symptoms to report right away.

Drug interactions: Interactions can affect how well certain medications work, make antibiotics less effective at treating your infection, or increase the risk of side effects. Check every new antibiotic against the full profile, including over-the-counter products and supplements.

Also ask whether anything makes it hard for patients to take their medications as directed. Inconsistent use makes medications less effective at managing health conditions, which can raise the risk of complications such as infections.

Who faces the highest sepsis risk?

Anyone can get sepsis, but some patients have higher risk. You can support tailor patient care for each group in specific ways:

Older adults (ages 65 years and older): Review multiple-medication regimens for potential drug interactions and use the visit to catch issues early.

Infants (younger than 1 year): Verify weight-based doses of children’s medications, show caregivers how to give antibiotics, and answer questions about childhood vaccines. Remind caregivers that a fever in a newborn or young infant needs prompt medical care.

People with weakened immune systems: Recommending vaccines that lower infection risk and make sure patients know which symptoms to report right away.

People with chronic conditions, including diabetes or lung disease: Review medications, reinforce adherence, and check inhaler technique. Recommend vaccines such as flu and pneumococcal shots.

People who are pregnant or postpartum: Confirm that medications, including antibiotics, are safe to use during pregnancy and breastfeeding. Recommend vaccines and explain which symptoms, like fever or worsening pain, should prompt a call to a healthcare provider.

When to send a patient for emergency care

Severe sepsis is a medical emergency, and its symptoms can look like other illnesses. Only a healthcare provider can confirm it, so patients shouldn’t wait until they’re sure. When a patient has an infection, ask whether they have any combination of the following:

Confusion or disorientation

Shortness of breath

A high heart rate

Fever, shivering, or feeling very cold

Extreme pain or discomfort

Clammy or sweaty skin

If a patient reports these signs, or an infection that isn’t getting better or is getting worse, send them for emergency care right away. Tell them to call 911 or go to the nearest emergency department, and ask, “Could this infection be leading to sepsis?” Don’t advise waiting to see if symptoms improve.

The bottom line

Sepsis is a life-threatening medical condition that is triggered by an infection. Many infections that lead to sepsis can be prevented and early treatment lowers the risk. You can help by recommending vaccines, counseling patients on antibiotics, addressing drug interactions, counseling about proper wound care, supporting chronic condition management, and referring them for medical evaluation when a symptom calls for it.

Pharmacists may be the most overlooked resource in sepsis prevention.
