{"id":6821,"date":"2019-11-13T10:00:06","date_gmt":"2019-11-13T15:00:06","guid":{"rendered":"https:\/\/www.singlecare.com\/blog\/?p=6821"},"modified":"2020-07-13T14:39:53","modified_gmt":"2020-07-13T18:39:53","slug":"in-flight-emergencies","status":"publish","type":"post","link":"https:\/\/www.singlecare.com\/blog\/in-flight-emergencies\/","title":{"rendered":"Are you prepared for an in-flight medical emergency?"},"content":{"rendered":"<p><span style=\"font-weight: 400;\">Allergic reactions and other medical emergencies can happen anywhere,\u00a0<\/span><a href=\"https:\/\/www.mdedge.com\/ccjm\/article\/139298\/cardiology\/there-doctor-board-flight-medical-emergencies\" target=\"_blank\" rel=\"nofollow noopener noreferrer\"><span style=\"font-weight: 400;\">including 31,000 feet in the air<\/span><\/a><span style=\"font-weight: 400;\">. In fact, one study estimated that\u00a0<\/span><a href=\"https:\/\/jamanetwork.com\/journals\/jama\/fullarticle\/2719313\" target=\"_blank\" rel=\"nofollow noopener noreferrer\"><span style=\"font-weight: 400;\">one out of every 604 flights<\/span><\/a><span style=\"font-weight: 400;\"> involves some sort of health problem. The most common ones? They involve fainting (32.7%), gastrointestinal issues (14.8%), respiratory (10.1%), and cardiovascular (7%) symptoms.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">And even if a doctor is on board who can help out if a passenger experiences anaphylaxis, a heart attack, or a seizure, the plane itself may or may not be carrying all the tools and supplies needed to take care of certain in-flight emergencies.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Earlier this year, for example, family physician and YouTube personality <\/span><a href=\"https:\/\/www.allergicliving.com\/2019\/07\/23\/lobby-for-in-flight-auto-injectors-gains-political-and-celebrity-muscle\/\" target=\"_blank\" rel=\"noopener noreferrer\"><span style=\"font-weight: 400;\">Dr. Mikhail Varshavski<\/span><\/a><span style=\"font-weight: 400;\"> (aka Dr. Mike) famously rescued a fellow passenger on a flight to Israel. The man, who had no history of allergies and therefore had no reason to carry his own <\/span><a href=\"https:\/\/www.singlecare.com\/blog\/epipen-instructions-information\/\"><span style=\"font-weight: 400;\">EpiPen<\/span><\/a><span style=\"font-weight: 400;\">, went into anaphylactic shock. The onboard medical kit contained epinephrine for heart attacks, but that dosage is too high to administer to a patient with anaphylaxis. Dr. Mike figured out a way to make it work, and the passenger made a full recovery.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">But the fact is many airlines are not currently required to carry EpiPens (or any epinephrine at all), thanks to a four-year exemption granted by the <\/span><a href=\"https:\/\/aes.faa.gov\/AES\/Exemption?ExemptionNumber=20708\" target=\"_blank\" rel=\"noopener noreferrer\"><span style=\"font-weight: 400;\">Federal Aviation Administration<\/span><\/a><span style=\"font-weight: 400;\"> to 50 specific carriers. The exemption also applies to three other medications: atropine, dextrose, and lidocaine. This exemption was created with the overall best interest of the public in mind due to frequent manufacturer shortages of drugs used in emergency situations. Most medications in aviation emergency supply kits end up expired and discarded, and ground-based emergency responders use the drugs much more frequently. The exemption was created to be able to divert the drugs in short supply to ground-based services first. This exemption expires in January 2020.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">While this doesn\u2019t mean your flight definitely <\/span><i><span style=\"font-weight: 400;\">won\u2019t<\/span><\/i><span style=\"font-weight: 400;\"> have these supplies; it means there is no <\/span><i><span style=\"font-weight: 400;\">guarantee<\/span><\/i><span style=\"font-weight: 400;\"> they will be on board.<\/span><\/p>\n<h2><span style=\"font-weight: 400;\">What medical supplies are airlines required to carry?<\/span><\/h2>\n<p><span style=\"font-weight: 400;\">According to the\u00a0<\/span><a href=\"https:\/\/www.ecfr.gov\/cgi-bin\/text-idx?SID=73a6cf5c23d08e1c4001b384fcfc1afb&amp;mc=true&amp;node=pt14.3.121&amp;rgn=div5#ap14.3.121.0000_0nbspnbspnbsp.a\" target=\"_blank\" rel=\"noopener noreferrer\"><span style=\"font-weight: 400;\">Code of Federal Regulations<\/span><\/a><span style=\"font-weight: 400;\">, all U.S.-based domestic and international flights must be equipped with the following items in preparation for in-flight emergencies:<\/span><span style=\"font-weight: 400;\">\u00a0<\/span><\/p>\n<ol>\n<li><span style=\"font-weight: 400;\">Between one and four first-aid kits, depending on the number of passenger seats<\/span><\/li>\n<li><span style=\"font-weight: 400;\">Lots of bandages, splints, and wound-care equipment<\/span><\/li>\n<li><span style=\"font-weight: 400;\">A stethoscope<\/span><\/li>\n<li><span style=\"font-weight: 400;\">CPR masks, a resuscitation device, and other respiratory equipment<\/span><\/li>\n<li><span style=\"font-weight: 400;\">An IV administration kit<\/span><\/li>\n<li><span style=\"font-weight: 400;\">Needles and syringes<\/span><\/li>\n<li><span style=\"font-weight: 400;\">Painkillers, antihistamines, and aspirin<\/span><\/li>\n<li><span style=\"font-weight: 400;\">A bronchodilator<\/span><\/li>\n<li><span style=\"font-weight: 400;\">Nitroglycerin<\/span><\/li>\n<li><span style=\"font-weight: 400;\">An automated external defibrillator<\/span><\/li>\n<\/ol>\n<p><span style=\"font-weight: 400;\">While these supplies are certainly useful and necessary, they won\u2019t cover every issue that could come up in flight. That means it\u2019s up to you, as a traveler, to be prepared, particularly if you have known issues (like food allergies), explains Norman Tomaka, a clinical consultant pharmacist in Melbourne, Florida, and spokesperson for the\u00a0<\/span><a href=\"http:\/\/www.pharmacist.com\/\" target=\"_blank\" rel=\"noopener noreferrer\"><span style=\"font-weight: 400;\">American Pharmacists Association<\/span><\/a><span style=\"font-weight: 400;\">. \u201cWe don\u2019t want [travelers] to have a false sense of security,\u201d Tomaka says.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Instead, expect the best but prepare for the worst, says <\/span><a href=\"https:\/\/www.asthmaandallergyassociates.net\/\" target=\"_blank\" rel=\"noopener noreferrer\"><span style=\"font-weight: 400;\">Sandra Gawchik, DO<\/span><\/a><span style=\"font-weight: 400;\">, co-director of the division of allergy and immunology at Crozer-Chester Medical Center in Chester, Pennsylvania. \u201cThe key is when you travel you want to make sure that you have your medicine with you,\u201d she says. \u201cYou don\u2019t want to have it in the overhead bin, you want to have it readily available.\u201d<\/span><\/p>\n<p><span style=\"font-weight: 400;\">This applies to rescue inhalers for asthma, EpiPens, and any other medication that you would need quick access to in an emergency situation, she says. She also recommends making sure your travel companions know where to find your medication and how to use it.<\/span><\/p>\n<h2><span style=\"font-weight: 400;\">What medical supplies should you pack?<\/span><\/h2>\n<p><span style=\"font-weight: 400;\">Depending on your condition, you should consider including some\u2014or all\u2014of these items in your carryon luggage.<\/span><\/p>\n<h3><span style=\"font-weight: 400;\">1.<\/span> <span style=\"font-weight: 400;\">EpiPen (or two)<\/span><\/h3>\n<p><span style=\"font-weight: 400;\">If you have severe allergies you probably already know that one is not enough\u2014<\/span><i><span style=\"font-weight: 400;\">two<\/span><\/i> <a href=\"https:\/\/www.singlecare.com\/prescription\/epipen-2-pak\"><span style=\"font-weight: 400;\">EpiPens<\/span><\/a><span style=\"font-weight: 400;\"> are essential, Dr. Gawchik says. That\u2019s because while one dose will relieve anaphylaxis, symptoms could return in three to eight hours. If they do, and you\u2019re still in transit, you\u2019ll need a second shot.<\/span><\/p>\n<h3><span style=\"font-weight: 400;\">2.<\/span> <span style=\"font-weight: 400;\">An antihistamine, like Benadryl<\/span><\/h3>\n<p><span style=\"font-weight: 400;\">Anaphylaxis isn\u2019t the only symptom of an allergy. In fact, hives and itching are much more common. A dose of <\/span><a href=\"https:\/\/www.singlecare.com\/prescription\/benadryl-allergy\"><span style=\"font-weight: 400;\">Benadryl<\/span><\/a><span style=\"font-weight: 400;\"> can help. The downside? It could make you sleepy (which might not be that big of a deal, particularly if you are on a red eye flight). Also, be sure to turn down the in-flight cocktail as <\/span><a href=\"https:\/\/www.singlecare.com\/blog\/benadryl-allergy-medicine-and-alcohol\/\"><span style=\"font-weight: 400;\">Benadryl and alcohol don\u2019t mix<\/span><\/a><span style=\"font-weight: 400;\">.<\/span><\/p>\n<h3><span style=\"font-weight: 400;\">3.<\/span> <span style=\"font-weight: 400;\">Anti-itch cream (hydrocortisone)<\/span><\/h3>\n<p><a href=\"https:\/\/www.singlecare.com\/prescription\/hydrocortisone-max-st\"><span style=\"font-weight: 400;\">Hydrocortisone<\/span><\/a><span style=\"font-weight: 400;\"> cream is fast-acting and should bring relief in the event that you experience a minor skin-related allergic reaction, Tomaka says.<\/span><\/p>\n<h3><span style=\"font-weight: 400;\">4.<\/span> <span style=\"font-weight: 400;\">OTC painkillers<\/span><\/h3>\n<p><span style=\"font-weight: 400;\">Nothing can make a flight drag on like a headache (well, aside from an overly chatty seatmate). Yes, <\/span><a href=\"https:\/\/www.singlecare.com\/prescription\/acetaminophen-extra-strength\"><span style=\"font-weight: 400;\">acetaminophen<\/span><\/a><span style=\"font-weight: 400;\"> is probably one of the medications on airplanes that you could get from the flight attendant\u2014but it is much easier to reach into your carryon bag for your headache remedy of choice.<\/span><\/p>\n<h3><span style=\"font-weight: 400;\">5.<\/span> <span style=\"font-weight: 400;\">Anti-diarrheal medication\u00a0<\/span><\/h3>\n<p><span style=\"font-weight: 400;\">If you start feeling ill from that bad hotel buffet an hour into your flight, you are going to wish you had some <\/span><a href=\"https:\/\/www.singlecare.com\/prescription\/imodium-multi-symptom-relief\"><span style=\"font-weight: 400;\">Imodium<\/span><\/a><span style=\"font-weight: 400;\"> at the ready. Remember: persistent GI problems can lead to dehydration, and dehydration can quickly become a bona fide medical emergency.<\/span><\/p>\n<h3><span style=\"font-weight: 400;\">6.<\/span> <span style=\"font-weight: 400;\">Blood sugar monitor<\/span><\/h3>\n<p><span style=\"font-weight: 400;\">It you have diabetes, be sure to carry whatever it is you normally use to keep your blood sugar in check, including your blood sugar monitor. A blood sugar crash on board a flight can be extremely dangerous, especially if your flight isn\u2019t carrying dextrose. You should avoid this possibility at all costs.<\/span><\/p>\n<h3><span style=\"font-weight: 400;\">7.<\/span> <span style=\"font-weight: 400;\">Copies of your prescriptions<\/span><\/h3>\n<p><span style=\"font-weight: 400;\">Finally, make sure all of your medicine on flights are labeled with the original manufacturer or pharmacy label, advises Tomaka. He urges travelers to bring copies of all prescriptions along with them and notes from your doctors. With EpiPens in particular, he suggests notifying the flight crew that you are carrying one so they can be prepared for the unlikely event that you would need to use it.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">When you\u2019ve packed the right items, you can relax and enjoy an in-flight movie, knowing that even if the worst happens, you\u2019re prepared for it!<\/span><\/p>\n","protected":false},"excerpt":{"rendered":"<p>Allergic reactions and other medical emergencies can happen anywhere,\u00a0including 31,000 feet in the air.
