From wellness websites and doctor\u2019s office magazines to the Twitter feeds of your favorite celebs, the keto diet has been everywhere in recent years. There are an array of diets encompassing the \u201c<\/span><span style=\"font-weight: 400;\">ketogenic dietary therapies\u201d (KDTs). KDTs include the classic ketogenic diet, the modified Atkins diet, the low glycemic index treatment, and the medium-chain triglyceride diet.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Though the jury is largely still out on the benefits of keto, it\u2019s often touted as the answer to everyone\u2019s weight loss prayers. But <\/span><i><span style=\"font-weight: 400;\">does<\/span><\/i><span style=\"font-weight: 400;\"> the keto diet work for everyone? Unfortunately, nutrition experts say no. It can be restrictive, tough to sustain, and downright unhealthy for some people.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">If you\u2019re thinking of trying it, here\u2019s what you should know.<\/span><\/p>\n<h2 id=\"what-is-the-keto-diet\">What is the keto diet?<\/h2>\n<p><span style=\"font-weight: 400;\">The classic ketogenic diet is a high-fat, adequate-protein (like 1 gram\/kg), low-carbohydrate diet that produces metabolic changes associated with the starvation state<\/span><span style=\"font-weight: 400;\">. <\/span><span style=\"font-weight: 400;\">The ketogenic diet was developed in the 1920s as a treatment for epilepsy, according to registered dietitian Lainey Younkin, owner of <\/span><a href=\"https:\/\/www.laineyyounkin.com\/\" target=\"_blank\" rel=\"noopener\"><span style=\"font-weight: 400;\">Lainey Younkin Nutrition<\/span><\/a><span style=\"font-weight: 400;\">. Now, Younkin says, it\u2019s a fad diet based on putting your body into ketosis, a process of burning stored fat cells for energy.<\/span><\/p>\n<h2 id=\"does-keto-work\">Does keto work?<\/h2>\n<p><span style=\"font-weight: 400;\">Proponents of keto say the low-carbohydrate, high-fat approach can lead to significant amounts of weight loss. \u201cPeople do lose weight,\u201d agrees Younkin. \u201cBut can they keep that weight off and can they follow keto long-term? For most people, the answer to those questions is no.\u201d In other words, it\u2019s not necessarily the kind of permanent lifestyle change you\u2019re hoping to achieve.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Many health professionals worry about the extreme nature of the diet, which limits your carb intake to less than 50 net grams per day and may lead to nutritional deficits.<\/span><\/p>\n<h2 id=\"who-should-try-a-keto-diet\">Who should try a keto diet?<\/h2>\n<p><span style=\"font-weight: 400;\">So who <\/span><i><span style=\"font-weight: 400;\">is<\/span><\/i><span style=\"font-weight: 400;\"> a good candidate for keto? In short: people with seizures. While people with seizures should be on medication, the addition of maintaining a keto diet can help decrease their risk of a breakthrough seizure. (For more information on the connection between epilepsy and keto, this <\/span><a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC6361831\/\" target=\"_blank\" rel=\"noopener\"><span style=\"font-weight: 400;\">review<\/span><\/a><span style=\"font-weight: 400;\"> in <\/span><i><span style=\"font-weight: 400;\">Frontiers in Neuroscience<\/span><\/i><span style=\"font-weight: 400;\"> breaks down much of the relevant research.)\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">According to registered dietitian Danielle Schaub, culinary and nutrition manager for <\/span><a href=\"https:\/\/www.territoryfoods.com\/\" target=\"_blank\" rel=\"noopener\"><span style=\"font-weight: 400;\">Territory Foods<\/span><\/a><span style=\"font-weight: 400;\">, the diet can also help people who are looking to:<\/span><\/p>\n<ul>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">improve or reset their insulin sensitivity;<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">improve some health biomarkers, like blood pressure;\u00a0<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">or lose weight or body fat with a sedentary lifestyle.<\/span><\/li>\n<\/ul>\n<h2 id=\"who-shouldn-t-try-a-keto-diet\"><span style=\"font-weight: 400;\">Who shouldn\u2019t try a keto diet?<\/span><\/h2>\n<p><span style=\"font-weight: 400;\">On the flip side, there are several groups of people who should not do keto. You should stay away from the keto diet if you are:<\/span><\/p>\n<ul>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Looking to lose weight fast without having a plan on how to maintain the weight loss long-term<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Someone with a poor relationship with food and\/or disordered eating patterns<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">An elite athletes<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Have pancreatitis, liver failure, kidney disorder, or other fat metabolism disorders<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Diabetic and have ever been diagnosed with diabetic ketoacidosis.\u00a0<\/span><\/li>\n<\/ul>\n<p><span style=\"font-weight: 400;\">Like with most restrictive diets, children and pregnant or breastfeeding women should also avoid keto unless otherwise instructed by their physicians.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Additionally, ketogenic diets should absolutely be avoided if you have a rare genetic condition, such as porphyria, fatty acid beta-oxidation defects, or primary carnitine deficiency. Consult with your primary care provider before beginning any diet.<\/span><\/p>\n<h2 id=\"keto-diet-and-diabetes\">Keto diet and diabetes<\/h2>\n<p><span style=\"font-weight: 400;\">You may have noticed that people looking to improve their <\/span><a href=\"https:\/\/www.singlecare.com\/blog\/insulin-resistance\/\"><span style=\"font-weight: 400;\">insulin sensitivity<\/span><\/a><span style=\"font-weight: 400;\"> are often good candidates for the keto diet; typically, this refers to people who have diabetes, though not everyone with diabetes should try keto.<\/span><\/p>\n<p><b>RELATED: <\/b><a href=\"https:\/\/www.singlecare.com\/blog\/diabetes-diet\/\"><b>The best diet for diabetes<\/b><\/a><\/p>\n<p><span style=\"font-weight: 400;\">\u201cPatients with Type 1 diabetes must be very careful with the keto diet because the ketosis process could increase their risk of diabetic <\/span><a href=\"https:\/\/www.singlecare.com\/blog\/ketosis-vs-ketoacidosis\/\"><span style=\"font-weight: 400;\">ketoacidosis<\/span><\/a><span style=\"font-weight: 400;\">, a very dangerous state which can lead to hospitalization and death,\u201d says <\/span><a href=\"https:\/\/www.houstonmethodist.org\/doctor\/laila-tabatabai\/\" target=\"_blank\" rel=\"noopener\"><span style=\"font-weight: 400;\">Laila Tabatabai, MD<\/span><\/a><span style=\"font-weight: 400;\">, an endocrinologist at Houston Methodist.\u00a0<\/span><\/p>\n<p><a href=\"https:\/\/www.singlecare.com\/blog\/what-are-ketones\/\"><span style=\"font-weight: 400;\">Ketones<\/span><\/a><span style=\"font-weight: 400;\"> are produced by the liver as an alternative fuel when there\u2019s not enough sugar to be converted to energy. Both the keto diet and uncontrolled Type 1 diabetes can result in high ketone levels. For people without diabetes, ketones aren\u2019t usually dangerous. However, a buildup of ketones can be very dangerous for people with Type 1 diabetes, which can result in dehydration, acidic blood, coma, and even death.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Patients with Type 2 diabetes, though, could benefit more readily from the keto diet. Dr. Tabatabai says that the reduction in carbs and overall weight loss on keto means your body requires less insulin, which in turn helps stabilize blood glucose levels.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">But that still doesn\u2019t mean you should start keto ASAP if you have Type 2 diabetes. \u201cStudies have shown that short-term use of the keto diet can help patients with Type 2 diabetes lose weight [but we] do not yet have long-term data on its safety and efficacy,\u201d emphasizes Dr. Tabatabai.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">A <\/span><a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC6566854\/\" target=\"_blank\" rel=\"noopener\"><span style=\"font-weight: 400;\">research review<\/span><\/a><span style=\"font-weight: 400;\"> of the recent research into Type 2 diabetes and low-carb diets published in <\/span><i><span style=\"font-weight: 400;\">Nutrients<\/span><\/i><span style=\"font-weight: 400;\"> shows that most of the studies performed lasted several months (one, published in the <\/span><i><span style=\"font-weight: 400;\">American Journal of Clinical Nutrition<\/span><\/i><span style=\"font-weight: 400;\">, <\/span><a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pubmed\/26224300\/\" target=\"_blank\" rel=\"noopener\"><span style=\"font-weight: 400;\">lasted 52 weeks<\/span><\/a><span style=\"font-weight: 400;\">). Carbs aren\u2019t just lurking in refined pastas and breads; <\/span><a href=\"http:\/\/www.med.umich.edu\/1libr\/MEND\/CarbList.pdf\" target=\"_blank\" rel=\"noopener\"><span style=\"font-weight: 400;\">many healthy foods, such as fruits and veggies, contain carbs<\/span><\/a><span style=\"font-weight: 400;\"> (along with dozens of necessary vitamins and minerals).\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Here\u2019s an example of some popular keto-friendly foods:<\/span><\/p>\n<ul>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Meat and poultry<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Eggs<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Dairy products like cheese, Greek yogurt, cottage cheese, and whipping cream<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Low-carb vegetables like spinach, cauliflower, and mushrooms<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Avocados<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Nut butter<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Bacon, jerky, and sausage<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Nuts, seeds, and monounsaturated oils like olive oil<\/span><\/li>\n<\/ul>\n<p><span style=\"font-weight: 400;\">Keto is very popular, so there\u2019s no shortage of recipes and eating plans catered to the diet\u2019s specifications. Eating keto does require careful planning and preparation.<\/span><\/p>\n<h2 id=\"i-m-in-ketosis-but-not-losing-weight\">I\u2019m in ketosis but not losing weight<\/h2>\n<p><span style=\"font-weight: 400;\">As a reminder, the goal with keto is to put your body into a state of fat-burning ketosis by eliminating most carbs. Schaub says some people will reach ketosis after about one week, while others may take a little longer.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">\u201cOnce you are in ketosis, if you are consuming fewer calories than you are expending, you will burn body fat and lose weight,\u201d she explains.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">However, it\u2019s possible to be in a state of ketosis and <\/span><i><span style=\"font-weight: 400;\">not<\/span><\/i><span style=\"font-weight: 400;\"> lose weight. Schaub says that if you enter ketosis but continue eating enough food to cover your energy needs, your body won\u2019t start burning fat cells as a source of energy because it simply isn\u2019t necessary. You still need a calorie deficit to lose weight, ketosis or not.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">There\u2019s also another reason why you could find yourself not losing weight on keto or, worse, <\/span><i><span style=\"font-weight: 400;\">gaining<\/span><\/i><span style=\"font-weight: 400;\"> weight: eating too many fatty foods. Since the keto diet is high in fat and fat is very calorie-dense, Schaub says you will gain weight if you are in an overall calorie excess regardless of where those calories come from. (In other words, keto is not a free pass to chow down on fast-food burgers all day.)<\/span><\/p>\n<p><span style=\"font-weight: 400;\">There isn\u2019t a daily calorie limit for the keto diet, but <\/span><a href=\"https:\/\/www.hsph.harvard.edu\/nutritionsource\/healthy-weight\/diet-reviews\/ketogenic-diet\/\" target=\"_blank\" rel=\"noopener\"><span style=\"font-weight: 400;\">most sources suggest<\/span><\/a><span style=\"font-weight: 400;\"> 70% to 80% of total daily calories should come from fat, 10% to 20% from protein, and 5% to 10% from carbohydrates.<\/span><\/p>\n<p><b>RELATED: <\/b><a href=\"https:\/\/www.singlecare.com\/blog\/weight-loss-diet\/\"><b>The best diet for weight loss<\/b><\/a><\/p>\n<h2 id=\"the-bottom-line-keto-doesn-t-work-for-everyone\">The bottom line: keto doesn\u2019t work for everyone<\/h2>\n<p><span style=\"font-weight: 400;\">Keto can work for some people, and there are benefits to being in ketosis even if you\u2019re not dropping pounds; Schaub says it can improve insulin sensitivity and lower blood pressure. The short-term weight loss could be beneficial for preventing chronic conditions like obesity, diabetes, and <\/span><a href=\"https:\/\/health.clevelandclinic.org\/keto-diet-and-heart-disease\/\" target=\"_blank\" rel=\"noopener\"><span style=\"font-weight: 400;\">heart disease<\/span><\/a><span style=\"font-weight: 400;\">, but there\u2019s not a lot of research about keto yet to prove its claims. \u201cWe don&#8217;t know the effects of following keto long-term since most of the studies on the diet are short-term in the setting of treating epilepsy,\u201d explains Younkin.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Diet wisely.
