{"id":7982,"date":"2020-02-05T07:00:02","date_gmt":"2020-02-05T12:00:02","guid":{"rendered":"https:\/\/www.singlecare.com\/blog\/?p=7982"},"modified":"2021-10-25T18:00:38","modified_gmt":"2021-10-25T22:00:38","slug":"daily-aspirin","status":"publish","type":"post","link":"https:\/\/www.singlecare.com\/blog\/daily-aspirin\/","title":{"rendered":"Should you take a daily aspirin?"},"content":{"rendered":"<p><span style=\"font-weight: 400;\">Taking a low-dose daily aspirin has long been recommended by cardiologists as an easy and effective way to help prevent cardiovascular events such as heart attacks and strokes, which are the leading killers in the United States. Aspirin has blood-thinning abilities and fights the formation of blood clots that can block blood flow in the arteries and lead to cardiovascular disease.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Two recently published studies, known as <\/span><a href=\"http:\/\/doi.org\/10.1056\/NEJMoa1804988\" target=\"_blank\" rel=\"noopener noreferrer\"><span style=\"font-weight: 400;\">ASCEND<\/span><\/a><span style=\"font-weight: 400;\"> and <\/span><a href=\"https:\/\/doi.org\/10.1016\/S0140-6736(18)31924-X\" target=\"_blank\" rel=\"noopener noreferrer\"><span style=\"font-weight: 400;\">ARRIVE<\/span><\/a><span style=\"font-weight: 400;\">, have called this recommendation into question, especially for the millions of healthy people who use aspirin as a way to reduce their risk of having a first-time heart attack or stroke. In light of these studies, the American Heart Association (AHA) and the American College of Cardiology (ACC) have recently revised their recommendations on daily aspirin use, noting that for many Americans, especially healthy older adults and those with bleeding risks, the dangers outweigh the benefits.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">\u201cThere\u2019s research showing more death and more bleeding with no reduction in cardiovascular events in healthy individuals taking a daily aspirin,\u201d says Erin Michos, MD, MHS, the associate director of preventive cardiology at Johns Hopkins University School of Medicine who also served on the writing committee for the <\/span><a href=\"https:\/\/www.ahajournals.org\/doi\/pdf\/10.1161\/CIR.0000000000000678\" target=\"_blank\" rel=\"noopener noreferrer\"><span style=\"font-weight: 400;\">2019 AHA\/ACC Guideline on the Primary Prevention of Cardiovascular Disease<\/span><\/a><span style=\"font-weight: 400;\">. \u201cIn people without a history of heart disease or without certain risk factors for it, daily aspirin use can do more harm than good.\u201d<\/span><\/p>\n<p><span style=\"font-weight: 400;\">On Oct. 13, 2021, a <\/span><a href=\"https:\/\/www.uspreventiveservicestaskforce.org\/uspstf\/sites\/default\/files\/file\/supporting_documents\/aspirin-cvd-prevention-final-rec-bulletin.pdf\"><span style=\"font-weight: 400;\">federal advisory panel<\/span><\/a><span style=\"font-weight: 400;\"> said people over the age of 60 should not start taking aspirin for heart disease because the risk of bleeding outweighs the benefits. People ages 40 to 59 should discuss it with their clinicians.<\/span><\/p>\n<h2><span style=\"font-weight: 400;\">Daily aspirin and heart disease<\/span><\/h2>\n<p><span style=\"font-weight: 400;\">Most <\/span><a href=\"https:\/\/www.heart.org\/-\/media\/data-import\/downloadables\/2\/4\/6\/pe-abh-what-is-a-heart-attack-ucm_300314.pdf?la=en&amp;hash=EAA08E35A3423AA8EF6EB41868F7E3BFB2FB6D79\"><span style=\"font-weight: 400;\">heart attacks<\/span><\/a><span style=\"font-weight: 400;\"> and strokes are the result of <\/span><a href=\"https:\/\/www.heart.org\/en\/health-topics\/heart-attack\/treatment-of-a-heart-attack\/aspirin-and-heart-disease\" target=\"_blank\" rel=\"noopener noreferrer\"><span style=\"font-weight: 400;\">blocked blood flow<\/span><\/a><span style=\"font-weight: 400;\">, according to the AHA. This occurs when plaque\u2014a fatty substance made up of cholesterol, cellular waste, calcium, and other products\u2014builds up on artery walls. Plaque narrows the arteries making it harder for blood to pass through. Ruptured plaques can also trigger the formation of blood clots that can lodge in arteries and clog them. When blood flow to the heart is affected, heart attacks can happen. If blood flow to the brain is restricted, a stroke can occur.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Aspirin is technically known as an antiplatelet medication. Platelets are tiny blood cells that help your blood clot. Aspirin thins blood and interferes with its clotting mechanism, making it less likely blood clots will form and clog arteries. While some studies have suggested that aspirin can lower blood pressure (having high blood pressure is a risk factor for cardiovascular disease), experts caution that the studies have not been well-conducted and the results have been conflicting.<\/span><\/p>\n<h2><span style=\"font-weight: 400;\">Benefits of daily aspirin use<\/span><\/h2>\n<p><a href=\"https:\/\/www.heart.org\/en\/news\/2019\/04\/04\/proactive-steps-can-reduce-chances-of-second-heart-attack\" target=\"_blank\" rel=\"noopener noreferrer\"><span style=\"font-weight: 400;\">One in five<\/span><\/a><span style=\"font-weight: 400;\"> people who have had one heart attack will be hospitalized with another one within five years, reports the AHA. Doctors recommend low-dose aspirin use in people with a history of heart disease because research\u2014some <\/span><a href=\"https:\/\/www.thelancet.com\/journals\/lancet\/article\/PIIS0140-6736(18)31924-X\/fulltext\" target=\"_blank\" rel=\"noopener noreferrer\"><span style=\"font-weight: 400;\">200 studies in over 200,000 people<\/span><\/a><span style=\"font-weight: 400;\">\u2014shows it reduces the chances of having a second cardiovascular event. One of the first studies pointing to a connection was printed in the <\/span><a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pubmed\/2899772\/\" target=\"_blank\" rel=\"noopener noreferrer\"><i><span style=\"font-weight: 400;\">Lancet<\/span><\/i><\/a><span style=\"font-weight: 400;\"> in 1988 and showed that one month of low-dose aspirin use started immediately after a heart attack or stroke could prevent <\/span><a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC4317158\/\" target=\"_blank\" rel=\"noopener noreferrer\"><span style=\"font-weight: 400;\">25 deaths in 1,000 patients and 10-15 nonfatal cardiovascular events<\/span><\/a><span style=\"font-weight: 400;\">.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">What are some other benefits of aspirin?<\/span><\/p>\n<ul>\n<li style=\"font-weight: 400;\"><span style=\"font-weight: 400;\">It appears to reduce the incidence of certain cancers, particularly colon cancer, but more research is needed.<\/span><\/li>\n<li style=\"font-weight: 400;\"><span style=\"font-weight: 400;\">It\u2019s widely available and fairly inexpensive.<\/span><\/li>\n<\/ul>\n<h2><span style=\"font-weight: 400;\">Risks of daily aspirin use<\/span><\/h2>\n<p><span style=\"font-weight: 400;\">Aspirin may seem like a harmless enough drug, but, in fact, its use carries side effects.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">According to research published in the journal <\/span><i><span style=\"font-weight: 400;\">PLoS One, <\/span><\/i><span style=\"font-weight: 400;\">the use of aspirin increases a person\u2019s risk of gastrointestinal bleeding by <\/span><a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC4973997\/\" target=\"_blank\" rel=\"noopener noreferrer\"><span style=\"font-weight: 400;\">40%<\/span><\/a><span style=\"font-weight: 400;\">. And while you might think some intestinal bleeding is preferable to a major heart attack, think again.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">\u201cThere are certainly good reasons to take aspirin, but it\u2019s not a completely benign drug,\u201d notes <a href=\"https:\/\/www.bidmc.org\/research\/research-by-department\/medicine\/general-medicine-research\/research-faculty\/christina-c-wee-md-mph\" target=\"_blank\" rel=\"noopener noreferrer\">Christina Wee, MD<\/a>, MPH, an assistant professor of medicine at Harvard Medical School and co-author of a recent <\/span><a href=\"https:\/\/annals.org\/aim\/article-abstract\/2738925\/prevalence-aspirin-use-primary-prevention-cardiovascular-disease-united-states-results\" target=\"_blank\" rel=\"noopener noreferrer\"><span style=\"font-weight: 400;\">article<\/span><\/a><span style=\"font-weight: 400;\"> published in the <\/span><span style=\"font-weight: 400;\">Annals of Internal Medicine<\/span><span style=\"font-weight: 400;\"> on the prevalence of aspirin use for the primary prevention of cardiovascular disease. \u201cIf you\u2019re middle aged or younger and healthy, a bleeding ulcer may be something you can recover from. But if you\u2019re frail or an older adult with underlying issues, that bleeding ulcer can cause you to lose a lot of blood, which could actually precipitate a heart attack seeing as your heart now has to pump much harder to get oxygen-carrying blood to your system.\u201d<\/span><\/p>\n<p><span style=\"font-weight: 400;\">\u201cGastrointestinal bleeding can be major,\u201d adds Dr. Michos. \u201cIt can lead to anemia, and it can put a lot of stress on your heart. Bleeding is not a trivial thing. It can cause a lot of morbidity and mortality.\u201d<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Because of this increased bleeding risk, the <\/span><a href=\"https:\/\/www.acc.org\/latest-in-cardiology\/ten-points-to-remember\/2019\/03\/07\/16\/00\/2019-acc-aha-guideline-on-primary-prevention-gl-prevention\" target=\"_blank\" rel=\"noopener noreferrer\"><span style=\"font-weight: 400;\">AHA\/ACC guidelines<\/span><\/a><span style=\"font-weight: 400;\"> on the primary prevention of cardiovascular disease have changed with respect to aspirin use. While it\u2019s still advised that those who\u2019ve already had a heart attack or stroke use a daily low-dose aspirin to prevent another cardiovascular event (ditto those who have stents or had bypass surgery) things are different for those who don\u2019t have heart disease. The new guidelines now advise against using aspirin to prevent a heart attack or stroke in certain groups who have a high risk of internal bleeding.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Higher risk patients include:<\/span><\/p>\n<ul>\n<li style=\"font-weight: 400;\"><span style=\"font-weight: 400;\">those who are 70 and over who are trying to prevent a first heart attack or stroke<\/span><\/li>\n<li style=\"font-weight: 400;\"><span style=\"font-weight: 400;\">those of any age who have conditions (such as ulcers) or take drugs (such as <\/span>anticoagulants or nonsteroidal anti-inflammatories, or NSAIDS, such as <a href=\"https:\/\/www.singlecare.com\/blog\/aspirin-vs-ibuprofen\/\">ibuprofen<\/a>) that can raise their bleeding risk.<\/li>\n<\/ul>\n<p><span style=\"font-weight: 400;\">What if you don\u2019t have diagnosed heart disease but you have some major risk factors for it\u2014for example, you smoke or have diabetes? Aspirin use may be appropriate, but you\u2019ll need your doctor\u2019s help in assessing and defining your actual risk. If you\u2019re advised to quit taking aspirin, don\u2019t worry about health risks. Stopping aspirin cold turkey shouldn\u2019t pose any dangers, says Dr. Michos.\u00a0<\/span><\/p>\n<h2><span style=\"font-weight: 400;\">How much aspirin should you take a day?<\/span><\/h2>\n<p><span style=\"font-weight: 400;\">Most of the studies surrounding aspirin and heart attack prevention focus on daily low-dose aspirin (sometimes referred to as \u201cbaby aspirin,\u201d which is a misnomer since babies should not generally take aspirin), defined as 75-100 mg per day. According to research published in the <\/span><a href=\"http:\/\/doi.org\/10.1136\/bmj.324.7329.71\" target=\"_blank\" rel=\"noopener noreferrer\"><span style=\"font-weight: 400;\">BMJ<\/span><\/a><span style=\"font-weight: 400;\">, this lower dose was found to be as effective as higher doses in preventing secondary heart attacks and strokes.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">But before you reach for a tablet, have a discussion with your healthcare provider. Your medical history may make you an inappropriate candidate for daily aspirin therapy, or your doctor may think you\u2019ll be better served by statins and other drugs that treat heart disease.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">\u201cUsing aspirin or not is something you need to talk about with your physician,\u201d cautions Dr. Michos. \u201cJust because a drug like aspirin is available over the counter does not mean it\u2019s safe or appropriate.
