Divigel is a brand-name topical estradiol gel prescribed to relieve menopause symptoms such as hot flashes. Estradiol is a female hormone. During menopause, estradiol levels fall, bringing on many of the symptoms of menopause. The gel is packaged in single-dose packets in a variety of dosage strengths. The gel in each packet is spread on the skin of the upper thigh once daily.

How to take Divigel

  • Follow all the prescriber’s instructions.

  • Please read the Patient Information sheet before using this medicine.

  • Divigel comes in single-dose packets. Use only one packet per day.

  • The gel is spread on the upper thigh. Use a different leg each day.

  • Do not allow anyone else to apply Divigel doses to your skin.

  • Do not apply Divigel to:

    • The face

    • Breast

    • In or around the vagina

    • Any skin that is irritated or has cuts or scrapes

  • Do not apply a dose while smoking or if you’re near flames or heat. The gel is flammable.

  • To apply a dose:

    • Wash and dry your hands.

    • Sit in a comfortable position.

    • Take out one packet and tear or cut off the top of the packet.

    • Squeeze out the contents of the packet onto the top of your thigh.

    • Gently spread the gel with your hand over an area as large as two palm prints.

    • Throw away the empty packet.

    • Wash your hands immediately. Do not touch your eyes until you’ve washed your hands.

    • Wait until the gel dries before putting clothing over the treated area.

    • Avoid fire, flames, or smoking until the gel has completely dried.

  • If you accidentally spill some of the Divigel in the packet while applying a dose, use the remainder. Do not use a second packet.

  • Store the Divigel packets in their original container at room temperature.

What to do if you miss a dose of Divigel

  • If a dose is missed, take the missed dose if the next dose is more than 12 hours away.

  • Skip the missed dose if the next dose is scheduled in less than 12 hours.

  • Take the next dose as scheduled.

  • Do not take two doses on the same day.

Divigel contraindications

  • This medicine is not right for everyone. 

  • Do not use it if you have had a severe allergic reaction to Divigel, are pregnant, or have:

    • Unusual vaginal bleeding not checked by a doctor

    • Breast cancer or a history of breast cancer 

    • Any other estrogen-dependent cancer such as uterine cancer

    • Active or past blood clot problems such as deep vein thrombosis (DVT), pulmonary embolism (PE), heart attack, or stroke

    • Liver impairment or disease

    • Protein C or protein S deficiency or any other problem that interferes with blood clotting

Divigel warnings

  • Tell any doctor or other healthcare provider who treats you that you are using this medicine. You may need to stop using this medicine several days before you have surgery, need certain types of medical tests, or need prolonged bedrest.

  • The prescriber will need to regularly check on your progress and monitor for side effects. Keep all appointments.

  • Make sure to tell the prescriber if you have:

    • Unusual vaginal bleeding

    • A history of cancer

    • Heart problems

    • Liver problems

    • Kidney problems

    • Thyroid problems

    • Asthma

    • Seizures

    • Diabetes

    • Migraine

    • Endometriosis

    • Lupus

    • A history of angioedema (swelling of the face and tongue)

    • High calcium

    • High blood pressure

    • High cholesterol

    • Porphyria

  • Estrogen therapy can cause serious problems including:

    • Blood clots

    • Heart attack

    • Stroke

    • Breast cancer

    • Endometrial cancer

    • Ovarian cancer

    • Gallbladder disease

    • Liver problems

    • Worsening of underactive thyroid

    • Vision problems

    • High blood pressure

    • High fats

    • Fluid retention

    • Worsening of angioedema

    • Increased risk of dementia

  • Smoking increases the risk of serious side effects.

  • To reduce the risk of serious side effects, have yearly:

    • Pelvic exams

    • Breast exams

    • Mammograms

  • If you think you are pregnant, immediately inform the prescriber. Divigel should not be used during pregnancy.

  • Do not breastfeed while using Divigel.

  • Do not allow other people, children, or pets to touch the skin where you applied the medicine. If this happens, wash the area that has touched your skin.

Divigel interactions

  • To avoid possible drug interactions, tell the prescriber about all the prescription drugs, over-the-counter medications, and supplements being taken, particularly:

    • Other female hormones

    • Thyroid hormones

    • Carbamazepine, clarithromycin, erythromycin, itraconazole, ketoconazole, phenobarbital, rifampin, ritonavir, St. John’s wort

  • Do not eat grapefruit or drink grapefruit juice while you are using this medicine.

  • Tell other healthcare providers you are using this medicine before they prescribe new medications.

Serious side effects

Immediately contact a doctor or get medical attention if you notice any signs or symptoms of a possible serious side effect including:

  • Unusual vaginal bleeding

  • Sudden or severe headache, numbness or weakness on one side of your body, problems with vision, speech, or walking

  • Severe pains in the legs or chest

  • New breast lumps, breast discharge

  • Swelling in your face, hands, ankles, or feet

  • Seizures

  • Abdominal pain, fatigue, nausea, yellow skin or eyes

  • Changes in vision or speech

  • Allergic reaction: Hives, rash, itching, swelling in the face, lips, or tongue, trouble breathing

Less serious side effects

  • Irregular vaginal bleeding or spotting

  • Breast tenderness

  • Upper respiratory tract infections

  • Vaginal yeast infections
