What is Otezla?

Otezla (apremilast) is a prescription medication approved for treating:

  • Plaque psoriasis in adults who are candidates for phototherapy or systemic therapy

  • Adults with active psoriatic arthritis

  • Oral ulcers associated with Behçet's Disease

Otezla is FDA-approved for use in adults only and is not approved for use in people under the age of 18 years.

Otezla is a PDE4 inhibitor that works inside inflammatory cells to reduce PDE4 activity. Decreased PDE4 activity is believed to help reduce the inflammation that occurs in people with psoriatic disease. Reducing inflammation in the body may help lessen certain symptoms, such as redness, swelling, and pain. 

Otezla is usually taken twice daily. Treatment with Otezla is usually started at a low dose and then increased gradually to reduce the risk of gastrointestinal side effects. Otezla should only be taken as prescribed by a healthcare provider. 

What are the side effects of Otezla?

The most common side effects of Otezla include headache, diarrhea, and nausea. However, side effects often go away on their own as the body adjusts to the medication. Other potential side effects include upper respiratory tract infections, fatigue, vomiting, decreased appetite, weight loss, insomnia, and back pain. 

Serious side effects of {{drug}} include allergic reactions. Seek immediate medical attention if you experience signs or symptoms of an allergic reaction, such as hives, itching, swelling of the face or throat, or trouble breathing. Other serious side effects may include severe vomiting and severe diarrhea.

Tell your doctor or healthcare provider if you have a history of depression before taking Otezla. The use of Otezla has been reported to cause worsened symptoms of depression, suicidal thoughts, or other changes in mood. You should also tell your healthcare provider about other medical conditions you may have, including kidney disease. 

Before taking Otezla, tell your healthcare provider about all the medications you take. Tell your healthcare provider if you take certain seizure medication, such as carbamazepine, phenobarbital, or phenytoin—or a drug called rifampin used to treat tuberculosis. These medications can decrease the effectiveness of Otezla. 

Tell your healthcare provider if you are pregnant, planning to become pregnant, or breastfeeding before taking Otezla. 

This is not a complete list of side effects associated with Otezla. Consult a healthcare provider for more information about other possible adverse effects, drug interactions, warnings, and precautions before starting treatment with Otezla.

Does Otezla cause weight loss?

Weight loss has been reported in clinical trials of Otezla. A decrease in weight of 5% to 10% was reported in some patients taking Otezla. Body weight may need to be monitored regularly while taking Otezla. Tell your healthcare provider if you experience unexpected weight loss while taking Otezla. 

How long does Otezla starter pack last?

Otezla is available in two-week and 28-day starter packs. The dosage of Otezla needs to be gradually increased to reduce the risk of gastrointestinal side effects, such as diarrhea, nausea, and vomiting. 

Your healthcare provider may give you a 14-day starter pack. Or, they may prescribe the 28-day starter pack through a specialty pharmacy.

The two-week and 28-day starter parks contain a titration pack plus enough 30 mg tablets for one or three additional weeks of treatment. The initial blister titration pack in each starter pack lasts a total of seven days. It contains the tablets needed to gradually increase the dosage of Otezla to 30 mg twice daily. This schedule helps reduce stomach problems. 

The titration schedule is as follows:

  • Day 1: 10 mg in the morning

  • Day 2: 10 mg in the morning and 10 mg in the evening

  • Day 3: 10 mg in the morning and 20 mg in the evening

  • Day 4: 20 mg in the morning and 20 mg in the evening

  • Day 5: 20 mg in the morning and 30 mg in the evening

  • Day 6 and onward: 30 mg twice daily

Is Otezla the same as Humira?

Otezla and Humira are not the same. Otezla is an oral medication that can treat psoriatic arthritis, plaque psoriasis, and oral ulcers associated with Behçet’s Disease. 

Humira is an injectable medicine. Like Otezla, it can treat psoriatic arthritis and plaque psoriasis but not oral ulcers associated with Behçet’s Disease. Humira also can treat many other conditions, such as rheumatoid arthritis, Crohn’s disease, uveitis, and others.
