What is Rituxan?

Rituxan is a FDA-approved brand-name prescription drug administered as an intravenous injection that treats a number of diseases involving B-lymphocytes, a type of white blood cell that produces antibodies. As an anti-cancer drug, Rituxan treats chronic lymphocytic leukemia and particular forms of non-Hodgkin's lymphomas. Rituxan also treats active rheumatoid arthritis, granulomatosis with polyangiitis, microscopic polyangiitis, and pemphigus vulgaris, all autoimmune diseases that attack healthy tissues in the body.

 Chronic lymphocytic leukemia and some types of non-Hodgkin's disease are cancers of B-cell lymphocytes. Cancerous B-cell lymphocytes grow uncontrollably. They are immature and defective and leave the patient vulnerable to infections.

Rheumatoid arthritis is a condition in which the immune system “misfires” and targets healthy joint tissue as if it were an infection. The immune system causes swelling, which damages the tissue and causes most of the symptoms of rheumatoid arthritis. Rituxan is always used in combination with steroids to treat rheumatoid arthritis to reduce the incidence and severity of infusion reactions.

Both granulomatosis with polyangiitis and microscopic polyangiitis are conditions where the immune system attacks blood vessels. The swelling closes off blood vessels, causing pain and potentially harming important organs such as the heart, liver, lungs, or nervous system.

Pemphigus vulgaris is a condition in which the immune system attacks proteins in the upper layers of the skin and mucous membranes. This causes blisters on the skin and in the mouth.

The active ingredient in Rituxan, rituximab, is an antibody that attaches to a protein on the surface of B-cell lymphocytes, called the CD20 antigen. Once attached, the antibody “paints a target” on the B-cell lymphocyte for other white blood cells to kill. This reduces cancerous B-cell lymphocytes for patients with B-cell cancers, but it also suppresses the immune system response in autoimmune diseases.

Rituxan is administered by a healthcare provider as an intravenous infusion. The infusion could last an hour or more. Each patient is premedicated with drugs to prevent infusion reactions and then closely monitored throughout the process for serious and potentially fatal allergic reactions. Rituxan is administered once weekly or biweekly for a total of anywhere from 4 to 12 doses depending on the underlying condition.

What are the side effects of Rituxan?

Common side effects of Rituxan include, but are not limited to, nausea, vomiting, diarrhea, indigestion, stomach pain, headache, fever, flushing, night sweats, muscle pain, joint pain, back pain, chest tightness, and dizziness.

Among the most serious side effects of Rituxan are severe and sometimes fatal infusion reactions. These are allergic reactions that could involve rash, dangerously low blood pressure, inability to breathe, respiratory failure, and heart attack. About 80% of infusion reactions happen during the first infusion. Patients will typically be given antihistamines and acetaminophen prior to infusion.

Rituxan can also cause a serious and sometimes fatal viral infection of the brain, multifocal leukoencephalopathy. Watch for symptoms such as memory problems, confusion, vision changes, problems with speech, problems walking, or weakness on one side of the body.

In cancer patients, Rituxan also may cause tumor lysis syndrome, a condition caused by the contents of dead cancer cells accumulating in the body. These cell contents radically change the balance of electrolytes in the body which can then lead to kidney problems and possible kidney failure. Symptoms are high blood potassium, low blood calcium, heart problems, and seizures.

Other severe side effects include heart problems, kidney problems, perforation of the bowels, and low white blood cell counts, making patients vulnerable to sometimes serious infections. Rituxan can reactivate hepatitis B infections, so patients will be screened for hepatitis B before receiving Rituxan.
