Farxiga (dapagliflozin propanediol) and Jardiance (empagliflozin) are brand-name prescription medications in a class of drugs called SGLT2 inhibitors. They help the kidneys remove excess sugar from the body. Does insurance cover Farxiga?

Insurance coverage for Farxiga varies by plan and by what it’s prescribed for. Insurance coverage for FDA-approved indications, such as Type 2 diabetes management, is more likely than for off-label uses like weight loss.

If your plan covers Farxiga, your out-of-pocket cost depends on the medication’s tier, which is a number that determines how much you pay. The higher the tier (3, 4, or 5), the higher the coinsurance or copayment out-of-pocket. Many plans place brand-name Farxiga in Tier 2 or Tier 3. Generic dapagliflozin propanediol may be placed in a lower tier, making it the more affordable option under insurance.

AstraZeneca reports that the average person with commercial insurance pays $25.54 for a one-month supply of Farxiga.

Does Medicare cover Farxiga?

Medicare coverage for Farxiga varies by plan. Some plans cover the medication for FDA-approved indications, and others don’t cover it at all.

According to James J. Chao, MD, FACS, chief medical officer at VedaNu Wellness, the negotiated list price for Medicare patients is $178 for a 30-day supply of Farxiga as of 2026. This is part of Medicare price negotiation under the Inflation Reduction Act, a federal program designed to lower drug costs for Medicare beneficiaries.

If your plan requires coinsurance, you’ll pay a percentage of the list price. For example, 20% coinsurance would come to about $35.60 per month. Medicare coverage for Jardiance varies by plan, as do copays. Boehringer Ingelheim reports that out-of-pocket costs are less than $50 for 84% of Medicare beneficiaries.

The Medicare price of Jardiance has also been negotiated under the Inflation Reduction Act. According to Dr. Chao, the negotiated list price for Medicare beneficiaries is $197 for a 30-day supply as of 2026. “And with the new Part D out-of-pocket maximum, many Medicare beneficiaries will see additional savings on their prescription medications,” he says.

Medicaid coverage for Jardiance depends on where you live, as each state operates its own Medicaid program. Some states include Jardiance on their preferred drug list for FDA-approved indications, while others may require prior authorization.
