Malignant hyperthermia susceptibility is an inherited condition that affects one in 5,000 to 50,000. When certain types of inhaled anesthetics are administered to an MH-susceptible patient, skeletal muscles in the body rapidly release stored calcium. The muscles contract, causing rigidity, and release heat. The sudden contraction destroys muscle tissues. Body temperature can sore to 107 degrees or more. Both heart rate and breathing rapidly speed up and the blood becomes more acid. Left untreated, malignant hyperthermia can rapidly result in organ failure and death. Mortality statistics vary, but it is estimated that untreated malignant hyperthermia will kill between 7 and 8 out of ten patients.
The active ingredient in Dantrium, dantrolene sodium, is a muscle relaxant and the only known drug treatment. Dantrium prevents the release of calcium in muscle cells. Dantrium will be immediately administered as an intravenous infusion. In the days following, Dantrium will be administered as a capsule in four divided doses a day. With dantrolene treatment and other clinical treatments, the mortality rate for malignant hyperthermia is 5%.
Dantrium capsules may also be prescribed to patients who are known to be MH-susceptible for daily administration two to four days before general anesthesia. However, the standard procedure for MH-susceptible patients is to flush the anesthesia machine and use inhaled anesthetics that do not trigger malignant hyperthermia.
Dantrium is a muscle relaxant so the most common side effects are weakness, drowsiness, low energy, fatigue, and dizziness. Other common side effects include but are not limited to nausea, vomiting, stomach pain, and headache.
Dantrium is only used for a short time, so severe side effects are very rare. These include swelling in the lungs (pulmonary edema), blood clots, anemia, and heart failure.
Long-term use of Dantrium may cause liver damage, water on the lungs (pleural effusion), and seizures.
